Abstract

The plumbing installation process often harbors potential risks that can lead to workplace accidents. Considering the serious consequences of workplace accidents both in terms of health and economy, there are six types of workplace accidents in the plumbing installation process: foot crushed by a pipe, hand cut by a cutting tool blade, eyes exposed to welding smoke, burnt by hot welding slag, sparked by friction between iron and a cutting blade, and leg scratched by the edge of a pipe. This study aims to analyze workplace accident incidents during the plumbing installation process using the Fault Tree Analysis (FTA) method with the hope of identifying the root causes of the accidents and formulating recommendations to improve work safety. The FTA method allows for the development of a systematic model that illustrates the cause-and-effect relationships of system failures and their components. The primary advantage of using FTA is its ability to provide a comprehensive understanding of complex accident scenarios by breaking them down into simpler basic events. The novelty of this research lies in its application of FTA specifically to the plumbing installation process, which has not been extensively explored in previous studies. The results identified that the cause of eyes exposed to welding fumes led to 11 basic events, while the mocus analysis revealed 7 basic events with 11 failure combinations.

Abstract

CV. Permata Mitra Karya perusahaan yang bergerak di percetakan mulai dari stiker, baliho, benner, pamflet dan produk-produk percetakan lainya. Banyaknya aktivitas di perusahaan tersebut dilakukan secara manual yang dapat menyebabkan muskuloskeletal atau Musculoskeletal Disorder (MSDs) Permasalahan dalam penelitian ini adalah adanya keluhan karyawan mengenai sakit pada anggota tubuh, berdasarkan kuesioner Nordic body map yang disebarkan kepada karyawan di dapat bahwa keluhan yang paling dominan pada bagian tubuh yaitu bagian punggung karena pengangkatan beban yang berulang. Tujuan penelitian ini adalah untuk mengukur tingkat bahaya manual material handling flexone. Untuk menangani permasalahan tersebut penulis menggunakan metode National Institute of Occupational Safety and Health (NIOSH). Metode NIOSH di pakai untuk menilai postur kerja karyawan pada saat melakukan aktivitas memegang, memindahkan objek, mendorong dan lain-lain. Dari perhitungan menggunakan metode National Institute of Occupational Safety and Health (NIOSH) didapatkan Lifting Index  keenam pekerja CV. Permata Mitra Karya yaitu. Karyawan 1 dengan Lifting Index 3.35, Karyawan 2 dengan Lifting Index 3.17, Karyawan 3 dengan Lifting Index 3.45, Karyawan 4 dengan Lifting Index 3.08, Karyawan 5 dengan Lifting Index 3.25 dan Karyawan 6 dengan Lifting Index  3.08. berdasarkan nilai Lifting Index > 1 pengangkatan yang dilakukan karyawan CV. Permata Mitra Karya memiliki tinggkat risiko yang dapat menyababkan cidera tulang belakang (pekerjaan tidak aman).

Abstract

Grha Permata Ibu Hospital is one of the major hospitals in Depok with adequate facilities in its services. One of the main factor in hospital services is drug serviceat the pharmacy. Often the problema in drug service is the long waiting time from drug provision, therefore this research was conducted to identify the waste that occurs in drug services in outpatient pharmacies using the Lean Hospital method by making Value Stream Mapping (VSM) and Process Activity Mapping (PAM). This research  was also conducted using quantitative research method to see the optimization in the queuing system in drug services in outpatient pharmacies. Meanwhile the resulto f the observation and calculation of the queuing system in drug services at the pharmacy are that the speed of prescription arrival is 0.297 minutes, for the calculation of the speed of prscription sevice is 3.366 minute per patient, the expected waiting time in the system is 0.32 minutes per patient waiting on the queue is 0.029 minutes per patient. Base on the result of the percentage of Process Cycle Efficiency (PCE), it is known that the efficiency of activity time in drug services in outpatient pharmacies has increased 10.8% from the eclusive system.
