Abstract

Lansia dapat mengalami degenerasi di berbagai sistem organ, termasuk sistem pendengaran. Menurunnya fungsi koklea, yang dapat diukur dengan menggunakan ambang dengar hantaran tulang, merupakan salah satu penyebab gangguan pendengaran pada lansia. Penurunan fungsi koklea ini dapat menyebabkan penurunan kualitas hidup lansia. Tujuan penelitian ini adalah untuk mengetahui keterkaitan antara ambang dengar hantaran udara dan tulang dengan kualitas hidup lansia menggunakan kuesioner Hearing Handicap Inventory for the Elderly-Screening (HHIE-S). Penelitian ini diikuti oleh 28 responden (56 telinga). Hasil uji korelasi Spearman menunjukkan bahwa tidak terdapat keterkaitan secara bermakna antara rata-rata ambang dengar hantaran udara dan tulang dengan skor kualitas hidup lansia (p0,005). Namun demikian menurut beberapa penelitian menunjukkan hal yang berbeda, sehingga pada kasus penurunan pendengaran dengan berbagai tipe pada lansia sebaiknya perlu dievaluasi ambang dengar hantaran tulang dan kualitas hidupnya dengan kuesioner HHIE-S.

Abstract

Background: Type 2 Diabetes Mellitus (T2DM) is known to affect multiorgan systems, including auditory function, with metabolic dysregulation playing a central role. Leptin and ghrelin, as metabolic hormones, are involved in energy homeostasis and neuroprotection, and may be influenced by fasting behavior. Objectives: This study aimed to explore the associations between fasting practices, serum leptin and ghrelin levels, and hearing function in adults with T2DM.Methods: A cross-sectional study was conducted involving 86 adults with T2DM, categorized into fasting (n=46) and non-fasting (n=40) groups. Serum leptin and ghrelin levels were measured using ELISA after an overnight fast. Hearing thresholds were assessed with pure-tone audiometry at 500, 1000, 2000, and 4000 Hz. Hearing loss was defined as a mean threshold >25 dB. Data were analyzed using Mann–Whitney U tests, Chi-Square tests, and logistic regression.Results: The fasting group had significantly lower leptin (32.85 ± 31.03 ng/mL vs. 40.89 ± 23.40 ng/mL, p=0.012) and ghrelin levels (5.48 ± 3.37 ng/mL vs. 7.32 ± 3.32 ng/mL, p<0.001). Hearing thresholds were slightly lower in the fasting group (20.80 ± 8.10 dB vs. 23.32 ± 11.13 dB), though the difference was not statistically significant (p=0.192). However, the frequency of hearing loss was significantly lower in fasting participants (21.7%) than in non-fasting participants (37.5%, p=0.015). Logistic regression revealed that higher leptin levels increased the risk of hearing loss (OR = 1.023, 95% CI: 1.004 - 1.042, p = 0.015), whereas fasting status was protective (OR = 0.429, 95% CI: 0.197 - 0.935, p = 0.033). Ghrelin, age, and sex were not significant predictors.Conclusion: Fasting may be associated with more favorable metabolic profiles and a lower frequency of hearing loss in individuals with T2DM. These findings suggest a potential protective role of structured fasting practices in auditory health, warranting further longitudinal studies to explore causality and underlying mechanisms.

Abstract

Vestibular dysfunction is an important yet underrecognized adverse effect among patients with head and neck cancer (HNC) receiving platinum-based chemotherapy. Although cochlear toxicity is well documented, evidence specifically addressing vestibular impairment in this population remains limited and methodologically variable. The novelty of this study lies in presenting the first systematic review and meta-analysis exclusively examining vestibular dysfunction in adult HNC patients receiving chemotherapy, thus establishing a more focused and consolidated evidence base. To quantify the pooled incidence, clinical features, and diagnostic patterns of vestibular dysfunction in adults with HNC undergoing chemotherapy, and to identify chemotherapeutic agents strongly associated with vestibulotoxicity . Conducted in accordance with PRISMA 2020 guidelines and prospectively registered in PROSPERO (CRD420251080552), this review regularly searched Embase, Scopus, Web of Science, and the Cochrane Library for studies published between 2014 and 2024. Eligibility criteria included adult HNC patients treated with and reporting vestibular outcomes. Risk of bias was assessed using RoB 2.0 for randomized trials and the Newcastle–Ottawa Scale for observational studies. A random-effects model was applied for quantitative synthesis. Sixteen studies met the inclusion criteria, and contributed fourteen data to the meta-analysis. The pooled incidence of vestibular dysfunction was 24.7% (95% CI: 19.2–30.8). Dizziness and imbalance were the most frequently reported symptoms, while peripheral vestibular deficits were confirmed through vHIT , VEMP, and caloric testing. Cisplatin was consistently identified as the principal chemotherapeutic agent associated with vestibulotoxicity. These findings underscore the need for standardized vestibular assessment and routine monitoring, particularly within cisplatin-based treatment regimens.
