Abstract

Latar Belakang: Komunikasi SBAR merupakan alat komunikasi yang direkomendasikan oleh WHO, komunikasi SBAR  tidak hanya meningkatkan mutu pelayanan, tetapi juga dapat meningkatkan kualitas handover dan menekan angka medical error. Jika komunikasi dalam serah terima pasien tidak dilakukan, dapat menyebabkan beberapa masalah dan konsekuensi termasuk biaya perawatan yang lebih tinggi, penyedia layanan yang lebih besar dan ketidakpuasan pasien. Kekeliruan identifikasi pasien terjadi di hampir semua aspek atau tahapan diagnosis dan pengobatan sehingga diperlukan adanya ketepatan identifikasi pasien. Beban kerja perawat yang terlalu banyak dapat menurunkan produktifitas. Perilaku perawat yang tidak aman, lupa, kurangnya perhatian, kecerobohan, tidak teliti dan mengabaikan keselamatan pasien (patient safety) beresiko terjadinya kesalahan dan akan mengakibatkan cidera kepada pasien.Tujuan: Mengetahui hubungan komunikasi SBAR, Kedisiplinan dan Beban kerja Perawat terhadap Identifikasi Pasien Berbasis Patient Safety.Metode Penelitian : Metode Penelitian ini Kuantitatif korelasi dengan desain cross sectional.  sampel adalah perawat berjumlah 40 orang. Probability sampling. Data dianalisis menggunakan uji statistik Spearman rank.Hasil : Komunikasi SBAR terhadap identifikasi pasien memperoleh skor yang cukup baik sebesar 42,5%. Kedisiplinan  terhadap identifikasi pasien juga memperoleh skor yang cukup baik sebesar 52,5%, dan Beban kerja terhadap identifikasi pasien memperoleh skor tinggi sebesar 77,5, berdasarkan hasil uji statistik, tidak ada hubungan yang signifikan antara Komunikasi SBAR, Disiplin dan Beban Kerja terhadap identifikasi pasien di RS H. Boejasin Pelaihari dengan nilai r = 0,772, 0,427 dan 0,079 0,05. Simpulan : Tidak terdapat hubungan yang signifikan antara komunikasi SBAR, disiplin dan beban kerja terhadap identifikasi pasien. Kata kunci : Komunikasi, SBAR, Pasien, Keselamatan. Background : SBAR communication is a tool recommended by WHO, SBAR communication not only improves service quality, but can also improve handover quality and reduce medical error rates. If communication in patient handovers is not carried out, it can lead to several problems and consequences including higher costs of care, larger service providers and patient dissatisfaction. Errors in patient identification occur in almost all aspects or stages of diagnosis and treatment, so there is a need for accurate patient identification. Too much workload for nurses can reduce productivity. Unsafe behavior of nurses, forgetfulness, lack of attention, carelessness, carelessness and neglect of patient safety are at risk for errors and will result in injury to patients.Objective:  Analyzed the relationship between SBAR communication, Discipline and Nurse workload on Patient Safety-Based Patient Identification.Methods:  This research method is quantitative correlation with cross sectional design. The sample is 40 nurses. Probability sampling. Data were analyzed using Spearman rank statistical test.Results: SBAR communication on patient identification obtained a fairly good score of 42.5%. Discipline on patient identification also obtained a fairly good score of 52.5%, and Workload on patient identification obtained a high score of 77.5, based on the results of statistical tests, there was no significant relationship between SBAR Communication, Discipline and Workload on identification. patients at H. Boejasin Pelaihari Hospital with r values = 0.772, 0.427 and 0.079 0.05.Conclusion: There is no significant relationship between SBAR communication, discipline and workload on patient identification. Key Words: Communication, SBAR, Patient, Safety.

Abstract

This study aims to analyze learning difficulties in relation to students' levels of conceptual understanding—categorized as conceptually understood, misconceptions, and lack of understanding—among 11th-grade science students (Grade XI MIPA) in the Teupah Barat District, specifically concerning the topic of hydrocarbons. A Two-Tier Multiple Choice diagnostic test was employed as the primary instrument. The study utilized a qualitative descriptive method. The population comprised all Grade XI MIPA students from senior high schools in the Teupah Barat District. The sample included 40 students from SMA Negeri 1 Teupah Barat and SMA Negeri 2 Teupah Barat, selected using purposive sampling techniques. Data analysis was conducted using descriptive statistics to process and interpret the students' responses. The results indicate that the Two-Tier Multiple Choice diagnostic test was effective in identifying students' conceptual understanding and in uncovering alternative conceptions through dominant incorrect answer choices that diverged from scientifically accurate concepts.The findings reveal that among the participants, 64% demonstrated conceptual understanding, 23% exhibited misconceptions, and 13% showed a lack of understanding regarding the topic of hydrocarbons. These results highlight the diagnostic tool's utility in pinpointing areas where conceptual reinforcement is necessary to enhance students’ comprehension in chemistry education.
