Abstract

The correctness of the code is the conformity of the code of action diagnosis determined by the coding officer with the patient's medical record in accordance with ICD-9. Based on the results of observations, coders in coding still found inaccuracies in coding medical records, one of which was inaccuracy in coding medical procedures. This study aims to find out the relationship between the clarity and completeness of action writing and the accuracy of the coding of medical procedures at hospital x. The research was carried out from July to August 2022 quantitatively with a cross sectional approach. The research was conducted by observing medical records. Computerized data processing with univariate and bivariate data analysis. The results of the study found, from 86 medical records, it was clear that 82.6% of diagnoses were legible, 17.4% of diagnoses were illegible. Completeness of writing diagnostics 70.9% complete, 29.1% incomplete. Action coding accuracy 68.6% correct action coding, 31.4% incorrect. Pvalue = 0.000, which means (P ≤ 0.05). It was concluded that the clarity and completeness of writing can affect coding, where unclear writing such as using abbreviations and cursive makes it difficult for officers to read the diagnosis, so officers must first clarify to responsible party before generating the diagnostic code. In the future, it is hoped that the responsible medical staff will write down the diagnosis of the action clearly and completely, and the coder will do the coding carefully.Keywords: Clarity, Completeness, Code Accuracy, ICD-9 CM with great care.

Abstract

Perkembangan teknologi berdampak juga pada dunia kesehatan salah satunya yaitu penerapan RME (Rekam Medis Elktronik). Dunia pendidikan dimana kampus terkhusus prodi rekam medis dan informasi kesehatan diharapkan menerapkan pembelajaran untuk hal tersebut.. Tujuan dari kegiatan pengabdian kepada masyarakat adalah agar tersosialisasinya ke mahasiswa kebijakan Rekam Medis Elektronik (RME) serta mengaplikasikan penerapan aplikasi berbasis database access kodefikasi penyakit mental dan perilaku sebagai upaya penerapan RME di layanan kesehatan. Metode yang digunakan adalah metode ceramah,diskusi dan praktek. Kegiatan penyampaian materi dan latihan penerapan aplikasi dilakukan di STIKES Dharma Landbouw Padang pada bulan April 2022. Kegiatan dihadiri oleh dosen,mahasiswa pendamping dan mahasiswa tingkat 1 dan 2. Hasil dari kegiatan sosialisasi kebijakan RME dan praktek penerapan aplikasi berbasis database access kodefikasi penyakit mental dan perilaku adalah mahasiswa mengetahui tentang PMK No. 24 Tahun 2022 serta mampu menggunakan aplikasi berbasis database access kodefikasi penyakit mental dan perilaku sebagai upaya penerapan RME di layanan kesehatan.Kata Kunci: Rekam Medis Elektonik, Koding, Aplikasi, Database

Abstract

Technological developments also have an impact on the world of health, one of which is the implementation of RME (Electronic Medical Records). In the world of education, campuses, especially medical records and health information study programs, are expected to apply learning for this matter. The aim of this research is to obtain the effectiveness of database-based application design for access coding of mental illness and behavior as learning for STIKES Dharma Landbouw Padang students. The type of research used is Research and Development. The population was level III medical records students with a sample of 20. The research was conducted by observation and interviews using descriptive analysis techniques. The research results showed that a database access-based coding application design used Unified Modeling Language (UML) visual modeling, namely use case diagrams, class diagrams and activity diagrams. As well as using Microsoft Excel to create code tabulations, the design is attractive and easy to use. Research results from observations showed that the average coding time was 22.2 seconds with a code accuracy percentage of 100%. The design of a database-based coding application for access to mental and behavioral illnesses for student learning can be said to be efficient and effective when viewed in terms of time and code accuracy. It is hoped that this application can be developed in the laboratory for use in learning so as to support the implementation of RME in Yankes.Keywords : Databases; Codification; Mental; Behavior; Design

Abstract

The accuracy of the diagnosis code is the suitability of the diagnosis code determined by the coding officer with the patient's medical record in accordance with ICD-10 rules. The results of the observations showed that one of the problems was that 60% of doctors' writing was difficult for staff to read and 55% of patients' medical record resumes were still incomplete. The aim of this research was to find a relationship between the clarity of writing a diagnosis and the completeness of filling out a medical resume with the accuracy of the diagnosis code in the internal medicine ward of Hospital X in the city of Padang. The type of research used is quantitative with analytical descriptive methods. The population is all medical resumes of inpatients in the internal medicine ward, a sample of 96 medical resumes using saturated sampling techniques. The research was carried out by observation using the bivariate analysis technique chi-square test. Research results from observations showed that 56 (58.3%) wrote unclear diagnoses, 66 (66.6%) filled in incomplete medical resumes and 35 (36.4%) incorrect diagnosis codes, and. There is no significant relationship between the clarity of writing a diagnosis and the accuracy of the diagnosis code and there is no significant relationship between the completeness of filling out a medical resume and the accuracy of the diagnosis code. It is hoped that in the future officers will confirm again with the medical personnel concerned if there is an illegible writing of the diagnosis and incomplete filling in so that there are no errors in assigning the diagnosis code.

Abstract

Pelaksanaan pemusnahan masih terdapat kendala meliputi banyaknya dokumen rekam medis inaktif yang belum dimusnahkan, minimnya jumlah petugas di instalasi rekam medis terutama dibagian pemusnahan, kurangnya pengetahuan petugas terhadap pelaksanaan pemusnahan yang mengakibatkan tidak berjalannya SOP pelaksanaan pemusnahan secara efisien.Metode penelitian kualitatif dengan pendekatan Literature Review. Kriteria inklusi yang digunakan melihat google scholar dengan keyword pelaksanaan pemusnahan dan dokumen rekam medis inaktif dan menggunakan jurnal dari tahun 2015-2020 untuk mengetahui faktor-faktor yang mempengaruhi pelaksanaan pemusnahan dokumen rekam medis inaktif. Data dianalisa dengan compare (kesamaan), contras (ketidaksamaan), critize (pandangan), synthesize (perbandingan) dan summarize (ringkasan). Hasil dari 6 jurnal berdasarkan 5M (Man, Money, Mhetode, Mhacine, Material) diketahui bahwa terdapat pengetahuan petugas yang kurang baik, anggaran proses pemusnahan tidak ada, masih ada yang belum mempunyai prosedur tetap mengenai pelaksanaan pemusnahan, sarana dan prasarana seperti alat pencacahan belum tersedia, dan bahan yang digunakan dalam pemusnahan yaitu lembaran dokumen rekam medis yang tidak bernilai guna.Berdasarkan hasil penelitian tersebut dapat disimpulkan bahwa pelaksanaan pemusnahan belum berjalan sesuai SOP dan masih kurangnya petugas pemusnahan. Diharapkan kepada peneliti selanjutnya terhadap beberapa artikel terkait perlu penelitian lanjut tentang menganalisis pelaksanaan pemusnahan terutama pelaksanaan SOP, dan kurangnya SDM di bagian instalasi rekam medis yang sangat menjadi fakor banyaknya masalah di bagian pemusnahan

Abstract

This study aims to determine whether there is an influence between the variables of asset structure, profitability, liquidity, sales growth, and dividend policy on the capital structure of cigarette sub-sector companies listed on the IDX from 2019 to 2023. The data used in this study are secondary. The sampling technique used in the study is saturated sampling. The samples taken in this study were four cigarette sub-sector companies. The results of the study indicate that simultaneously asset structure, profitability, liquidity, sales growth and dividend policy affect the capital structure. Based on the results of the partial test, sales growth in this study has no significant effect on the capital structure. In contrast, the variables of asset structure, profitability, liquidity, and dividend policy have a significant effect on the capital structure.
