Abstract

Claim process proposed by a hospital, there were still returned claim documents verification of BPJS, in which medical treatment codes were not corrected.The problem was that in implementation of coding application could influence the health cost service.A goal of the research was to expose the problem of coding application in medical treatment through analysis process of input and output factors. A kind of the research was qualitative with case study approach. This research was conducted at Islamic Hospital of Siti Rahmah of Padang from February to July 2017.The research informants were about 6 persons taken with a technique of purpose sampling. The analysis of result data were from detailed interview sourced from coding data reduction to be transcription. Category of providing data, interpretation. The research result showed that  medical treatment disturbance of coding implementation was not clearness of doctor’s writing as icon of medical diagnose and incomplete resume sent. Computer coding system  was not maximal because of the new  medical record officers. Hospital officer added the strength in policy regulation  in human resources through increasing life skill such as training. The policy strength must be fixed regulation rewards and sanction for medical recorder not fulfilling the un accurate data. Besides, socialization from medical record unit about importance of medical diagnose treatment furthermore it relates with an implementation of SOP through regular watch.Pada proses pengajuan klaim di rumah sakit masih terdapat berkas yang dikembalikan verifikator BPJS, salah satu penyebab dikarenakan kode tindakan medis tidak tepat. Permasalahan pelaksanaan tindakan pengodean dan tindakan medis dapat mempengaruhi tarif pelayanan kesehatan. Tujuan penelitian adalah mengeksplorasi permasalahan pelaksanaan pengodean tindakan medis dengan menganalisis faktor input, proses dan output. Jenis penelitian adalah kualitatif dengan pendekatan studi kasus. Penelitian dilakukan di RSI Siti Rahmah Padang dari bulan Februari sampai Juli 2017. Informan peneltian ini berjumlah 6 orang diambil dengan teknik purposive sampling, Analisis data hasil wawancara mendalam dengan tahap transkripsi, reduksi, coding, kategori, penyajian dan interpretasi data. Hasil penelitian menunjukkan yang menghambat kelancaran pelaksanaan pengodean tindakan medis adalah ketidak jelasan tulisan dokter sebagai penegak diagnosa dan tindakan medis serta ketidak lengkapan pengisian resume. Pengodean disistem komputerisasi belum maksimal karena petugas koding masih baru. Sebaiknya rumah sakit melakukan penguatan input, dibidang SDM dengan meningkatkan keterampilan melalui pelatihan. Penguatan kebijakan melalui regulasi dan sanksi tegas bagi SDM yang tidak mengisi rekam medis dengan baik dan benar, serta perlu sosialisasi tentang pentingnya pengisian diagnosa tindakan medis sesuai kaidah ICD 9 CM dan Penguatan fungsi manajemen dalam pelaksanaan pengodean tindakan medis terutama berkaitan dengan SOP pelaksanaan melalui kegiatan pengawasan secara berkala.

Abstract

Promosi kesehatan merupakan upaya untuk meningkatkan kemampuan masyarakat melalui proses pembelajaran bersama masyarakat khususnya masyarakat pendidikan di sekolah. Perilaku kesehatan yang harus diajarkan secara dini adalah Perilaku Hidup Bersih dan Sehat (PHBS). Pada kenyataannya banyak orang tua yang tidak memahami tentang PHBS, sehingga perlunya sekolah atau lembaga pendidikan memberikan pengajaran tentang hal tersebut. Namun setelah dilakukan observasi dilapangan ditemukan masih banyak promosi kesehatan yang berkaitan dengan PHBS yang tidak disosialisasikan kepada anak sekolah terutama TK dan SD. Padahal anak-anak TK lebih sering melakukan aktivitas yang rentan terhadap kuman dan bakteri seperti: bermain, menulis dan mengambar sehingga ketika meraka pulang masih dalam keadaan kotor. Seharusnya, sebelum pulang anak-anak perlu diajarkan untuk bersih diri minimal dengan cara mencuci tangan dengan benar dan bersih. PKM ini ini bertujuan untuk memberikan edukasi dan demonstrasi tentang mencuci tangan yang benar bagi anak-anak TK agar mereka memiliki pengetahuan tentang kebersihan sejak awal. PKM ini dilakukan di TK Sabbihisma 2  dengan total peserta 20 orang anak-anak yang berada di kelas 0 besar. Penyuluhan disampaikan dengan metode ceramah dengan bantuan media audio visual. Edukasi dan demonstrasi berjalan lancar dan peserta dapat menerima materi yang diberikan serta aktif dan bersemangat selama mengikuti penyuluhan.

Abstract

Kartu Kunjungan Berobat pasien merupakan identitas atau pengenal pasien pada saat berobat ke tempat layanan kesehatan. Puskesmas Ulak Karang menerapkan family numbering diketahui dimana kartu kunjungan berobat pasien digunakan oleh semua anggota keluarga sehingga dapat memperlama waktu pelayanan pendaftaran pasien, Ini terjadi jika pasien tidak membawa kartu kunjungan berobat, selain itu kondisi nya yang rusak dan tulisannya yang luntur sehingga menyulitkan proses identifikasi. Tujuan pengabdian kepada masyarakat adalah bagaimana pentingnya membawa kartu kunjungan berobat agar pelayanan menjadi lebih bermutu. Pengabdian ini dilakukan di Puskesmas Ulak Karang Padang dimana yang menjadi audien adalah pasien yang datang berkunjung di waktu kami melakukan sosialisasi di puskesmas tersebut. Sosialisasi disampaikan melalui ceramah dengan batuan media audio visual, sehingga materi bisa di sampaikan dengan baik. Hasil kegiatan menunjukkan bahwa sosialisasi berjalan dengan lancar dan diharapkan peserta dapat menerima materi yang diberikan serta aktif dan bersemangat dalam penyuluhan. Kegiatan soasilisasi pentingnya kartu kunjungan/kartu identitas berobat merupakan indikator mutu pada pelayanan kesehatan.

Abstract

Keberhasilan pelayanan medis rumah sakit dapat dimulai dari kantor pendaftaran pasien rawat jalan, dan pasien yang datang ke rumah sakit hanya memiliki satu nomor rekam medis. Tujuan dari kegiatan pengabdian kepada masyarakat ini adalah untuk memberikan pemahaman kepada petugas kesehatan, dan juga agar sadar dalam pentingnya penomoran yang valid pada rekam medis. Metode pengabdian dengan melakukan penyuluhan, diskusi dan tanya jawab kepada petugas rekam medis. Hasil kegiatan memberikan peningkatan pengetahuan terbaru pada petugas dan kesadaran pemahaman akan akibat terjadinya penomoran ganda.

Abstract

Filling is where outpatient, inpatient and emergency medical records are kept in view of the fact that they are confidential and have a legitimate angle. The motivation behind this investigation is to describe medical records that are not properly placed, lack of storage rack facilities, workload of filling officers and describe the education level of medical record officers. The method used is a literature study by conducting a descriptive analysis by describing the facts that are then analyzed, looking for similarities (Compare), dissimilarities (contrast), views (critize), comparisons (syntheyze) and a summary (summarize) of several studies. This is because the tracer is not used on the storage rack, as well as the lack of facilities and infrastructure such as storage racks and very small storage space. There are officers who have double jobs and the level of education of officers. high school educated. Suggestions from this study are that it is advisable to carry out medical record storage using a tracer, must add medical record officers who are stored so that there is no double job officer and the hospital puts an officer with D3 medical record education in the storage room.

Abstract

REDUCING THE USE OF PLASTIC PACKAGING PRODUCTS SNACK FOOD FOR ELEMENTARY SCHOOL STUDENTS 06 ULAK KARANG UTARA PADANG. School-age children are the generation of the nation's administrators, when they are of developmental age that require nutritional intake to improve good nutritional status. School children generally spend one third of their time at school, thus causing children the opportunity to buy food in the school area. Based on previous research, it was found that almost all respondents (98.7%), students had the habit of buying snacks at school, with the highest frequency of snacks being 2 times a day. Often food products are sold in plastic packaging, where the plastic packaging for food products that does not comply with the Indonesian National Standard has harmful contents for the body. Children buy snack food products based solely on their preferences without thinking that the snack food products are served in packaging made of plastic-based products that can cause illness for themselves. Community service is carried out through socializing the use of plastic packaging products on snacks for elementary school students. Data collection was carried out through observation, document study and interviews, at Elementary School 06 Ulak Karang Utara Padang from March to July 2019. Respondents consisted of 3rd grade students and street food vendors selling at Ulak Karang Utara 06 Elementary School, Padang. The form of activity is the socialization of the types of plastics and the negative impact of using plastic products for body health by using posters and brochures and implementing plastic waste management with the concept of 3R, Reuse, Reduce, Recycle.

Abstract

Mobilisasi dini adalah pengembangan secara bertahap atau berangsur - angsurketahapan mobilisasi sebelumnya untuk mencegah terjdainya komplikasi. Pengetahuanresponden tentang mobilisasi dini post sectio caesarea diruang kebidanan RSUD dr.RasidinPadang masih rendah. Berdasarkan survei awal terhadap 10 orang pasien post sectio caesarea,6 (60%) orang yang tidak mengetahui pengertian mobilisasi dini post sectio caesarea.Tujuan dari penelitian ini adalah untuk melihat gambaran tingkat pengetahuan ibunifas tentang mobilisasi dini pada pasien post sectio caesarea di ruang kebidanan RSUD dr.Rasidin Padang. Jenis penelitian ini adalah deskriptif yaitu untuk menggambarakan tingkatpengetahuan Ibu Nifas Post Sectio Caesarea terhadap pelaksanaan mobilisasi dini, penelitiandilakukan di ruangan kebidanan RSUD dr. Rasidin Padang pada tanggal 12 November 2016s/d 10 Februari 2017.Populasi penelitian ini adalah seluruh ibu nifas post sectio caesarea di ruangankebidanan pada tahun 2016 sebanyak 230 orang, dengan sampel berjumlah 36 orang,pengumpulan data dengan cara membagikan kuisioner dengan analisis univariat.Hasilpenelitian didapatkan sebagian besar responden memiliki pengetahuan rendah tentangpengertian mobilisasi dini sebanyak 63,9%, 66,7% tentang tujuan mobilisasi dini, 75,0%tentang cara pelaksanaan mobilisasi dini, dan 77,8% tentang waktu pelaksanaan mobilisasidini.Berdasarkan penelitian yang telah dilakukan dapat disimpulkan bahwa banyak ibunifas post sectio caesarea memiliki pengetahuan rendah tentang pengertian, tujuan, cara danwaktu pelaksanaan mobilisasi dini. Disarankan kepada pimpinan RSUD dr. Rasidin Padangkhususnya kepada kepala ruangan kebidanan, untuk memberikan penyuluhan, leaflet danposter tentang pelaksanaan mobilisasi dini.

Abstract

Abstract: Coding is a classification of a patient's disease, the implementation of which there are still errors both in terms of writing a diagnosis that is unclear to and incomplete. Based on the initial survey conducted, it was found that 15 (75%) of the writing of the diagnosis were unclear, 12 (60%) of the writing of the diagnosis were incomplete, and 14 (70%) incorrect coding of the 20 medical record files. This study aims to determine the accuracy of the coding of injury cases based on ICD-10 at Ropanasuri Hospital, Padang. This research was conducted at the Hospital for Special Surgery Ropanasuri Padang,. The population was 74 medical record files, using the total sampling technique. This research is by means of observation, using a checklist table, the data is processed using a computerized and analyzed by univariate. The results showed that the clarity of writing the diagnosis of injury cases was not clear 49 (66.2%), the completeness of writing the diagnosis of injury cases was incomplete 51 (68.9%), the accuracy of coding the injury cases was not accurate 17 (22.9%). The results of the research on the implementation of coding, especially in injury cases, have been very good where the number of inaccuracies in the code of injury cases is small, but the clarity of writing the diagnosis and completeness of the diagnosis is still not good. Researchers suggest to doctors to improve diagnosis writing and coder to be more active and thorough in coding in order to produce good reporting so as to improve the quality of the hospital.Keywords: Accuracy of coding, Medical Records, ICD-10, Clarity, Completeness

Abstract

Implementation of e-puskesmas in Padang City health centers is expected to contribute to health centers in improving the effectiveness and efficiency of health services. The results of observations of e-puskesmas applications are not in accordance with the needs of health center staff (from registration to reporting). To assess the benefits of implementing an e-puskesmas one of the evaluation methods that can be used is an evaluation based on the user's perception using the PIECES method. This research is a qualitative case study approach. This research was conducted at the Padang City health center from February to May 2018. The informants in this study amounted to 10 people taken by purposive sampling technique, data analysis results of Focus Group Discastion (FGD) will be carried outprocess transcription, data reduction, coding, categories, presentation data and data interpretation. Aspects of Performance input of patient data and response time is faster, in the aspect of information there are still obstacles, namely the resulting report is still limited to LB1 reports, still constrained for data briging with P-care BPJS, on theaspect the economic use of e-puskesmas still does not provide benefits which is large in terms of reporting because there are still data input activities and reporting that must be done manually. In terms ofaspects control, the implementation of e-puskesmas still faces constraints in terms of data security because all officers can access the e-puskesmas with the same user and password. For theaspects of efficiency the e-puskesmas users are still constrained by the workload of recording and reporting that is still done manually to the City Health Office, theaspect service needs to be the development of the modules in the e-puskesmas such as the LB2 to LB 4 reporting menu module. It is necessary to develop a menu / module on the e-Puskesmas system, the need for a data security system by providing login access according to the user level and the need for communication with BPJS related to briging data.

Abstract

Based on the initial survey conducted by researchers, it was found that there were differences in perceptions between code verifiers and BPJS verifiers about medical action codes. In February, there were 15 disease codes with different perceptions, because the hospital had to follow the code based on the BPJS. The research objective was to analyze the perception of medical action codes by verifiers at Bunda Medical Center Hospital.This type of research is qualitative with a phenomonological approach. Research is conducted by in-depth interviews with 1 medical support person, 1 medical committee person, 1 code verifier, and 1 BPJS verifier, using a voice recorder and using interview guidelines. Data were analyzed using the Collaizzi method.The results showed that what hampered the implementation of the verification of medical action codes was that the doctor's writing was difficult to read by the verifier. There are differences in the perceptions of the verifier about the medical action code, where the verification of the medical action code is based on the number of resources used in the service, while the BPJS verifier thinks the medical action code is based on the level of severity or severity level. The conclusion in this study is that the educational qualifications between the BPJS verifier and the verification of medical action codes are not yet appropriate, then the BPJS verifier verifies the code based on the severity level it should be based on the most spent resources.
