Abstract

The success of the coping mechanism in patients with breast cancer depends not only on early detection but also an understanding of the mechanisms that cause it. Psychological impact experienced by each patient, especially breast cancer which underwent different treatment depending on the level of severity (stage). The differences before and after being given education can affect the coping mechanism of the individual itself. One of the conditions that can cause anxiety is the process of implementing chemotherapy treatment. The purpose of this study was to determine the increase in coping mechanisms in breast cancer patients at Grandmed Lubuk Pakam Hospital. Counselling was carried out to see an increase in coping mechanisms, namely by directly informing breast cancer patients about coping mechanisms with education, distributing leaflets and analyzing data before and after education. Data analysis was performed using the Paired Sample T-Test with a confidence level of 95% and a value of α = 0.05. The results obtained from this study have shown a change in the coping mechanism of the patient before and after being given education on the application of the coping mechanism with a p-value = 0.001 (α> 0.05). In conclusion, there is an increase in coping mechanisms in breast cancer patients who are carried out according to standards, for health institutions there is a need for supervision and counselling to the public about the importance of maintaining health, especially for breast cancer patients at Grandmed Lubuk Pakam Hospital.

Abstract

The purpose of this study was to determine the reduction in the risk of infection before and after open umbilical cord care for newborns at Grandmed Lubuk Pakam Hospital in 2020. This research method was quantitative with a pre-experimental research design in the form of an experimental design, namely one group pretest-posttest design, this research was conducted at Grandmed Lubuk Pakam Hospital. The results of this study are: most of the respondents before the open umbilical cord care was performed and after the open umbilical cord treatment was carried out in respondents with the risk of infection being no infection as many as 21 respondents or 70% and infection as many as 9 . By using the Wilcoxon test, the result is p-value = 0.000 with a value of = 0.05. Because the value of p = 0.000 less than 0.05, Ha is accepted, that is, there is an effect of open umbilical cord care on reducing the risk of infection in newborns at Grandmed Lubuk Pakam Hospital.

Abstract

Hypertension is a health disorder that causes an increase in blood pressure. Hypertension can cause serious complications in the form of stroke. These significant complications can be life threatening and even death. Stroke is a global health problem with more than 15 million people annually experiencing this condition worldwide. This study was conducted on 30 individuals who visited the Pagar Jati Community Health Center, using the accidental sampling method, to explore the contribution of community participation in increasing awareness about hypertension and promoting preventive behavior. Through outreach activities and public campaigns, community participation has succeeded in increasing understanding of the importance of regular health check-ups by checking blood pressure and consuming foods with balanced nutrition, including low salt, thereby preventing hypertension. Evaluating the effectiveness of the community service program using a questionnaire showed an increase from 70% at the pretest stage to 93% at the posttest stage. This reflects a significant increase in awareness, knowledge and preventive measures against complications of hypertension, such as stroke, in the community areas served by health community center (Puskesmas) Pagar Jati. Collaboration between health workers, local government, educational institutions and the community is the key to the success of this program. Although challenges remain such as limited access and inequality of information, community service has proven to be an effective tool in strengthening the community response to hypertension and advancing overall community health.

Abstract

Dementia is a syndrome of cognitive decline in which the repetitive processing of information from long-term memory occurs. When individuals reach the age of 60 years and over they are categorized as elderly, one of the many problems in the elderly that often arises is dementia/memory decline which will then have a negative impact on cognitive function. Community service is key in efforts to spread knowledge about dementia. This Community Service was carried out for the elderly at the Karang Anyar Community Health Center, Sidodadi Ramunia Lubuk Pakam Village, as many as 50 people. This activity will play a role in raising awareness of the elderly about the need to know knowledge about treating dementia by doing brain exercises that are practiced at the Karang Anyar Community Health Center. This activity is held through counseling and public campaigns, where PkM will be able to increase understanding of the benefits of doing brain exercises for the elderly. Evaluation of this activity is carried out through a review activity by asking the elderly questions regarding the meaning, signs and symptoms as well as treatment for dementia patients. The results of this PkM show that there has been a significant increase in knowledge and efforts to treat dementia at the Karang Anyar Community Health Center in Sidodadi Ramunia Village, Lubuk Pakam.

Abstract

Cardiovascular disease, also known as heart disease, generally refers to conditions involving narrowing or blockage of blood vessels that can lead to heart attack, chest pain (angina), or stroke. Other heart conditions that affect the muscles, valves, or rhythm of the heart are also considered forms of heart disease. Rest and sleep are a major part of physical and mental health. Every individual who is in the hospital will have poor quality sleep if the results are compared to people who sleep at home. This study aims to determine whether there is an effect of foot massage and progressive muscle relaxation on sleep quality in congestive heart failure (CHF) patients in the CVCU ward of Grandmed Lubuk Pakam Hospital. This research uses a semi-experimental (semi-trial) type of research with a one-group pre-trial study design, the intervention carried out using this method uses two groups. The first group consisted of CHF patients with decreased sleep quality and foot massage. The second group consisted of CHF patients with a gradual decrease in sleep quality and muscle relaxation. Based on the Wilcoxon test above, the test results with p-value are obtained:0.004 means p < 0.05 so it can be concluded that progressive foot and muscle massage has a significant effect on the sleep quality of congestive heart attack (CHF) patients at GrandMed Lubuk Pacam Hospital. It is recommended that patients who experience sleep disorders, especially CHF patients, massage their feet before going to bed.

Abstract

Patah tulang atau fraktur merupakan hilangnya kontinuitas tulang yang terjadi akibat cedera langsung ataupun tidak langsung. Salah satu perawatan patah tulang adalah dengan melakukan operasi. Ambulasi dini merupakan salah satu asuhan keperawatan pasca operasi fraktur. Untuk menghindari komplikasi setelah operasi, ambulasi dini sangat penting. Tujuan dari penelitian ini adalah untuk mengetahui faktor-faktor yang mempengaruhi kemampuan pasien untuk berjalan setelah menjalani operasi fraktur ekstremitas bawah. Jenis penelitian yang digunakan adalah penelitian kolerasi dengan metode pendekatan cross-sectional dengan jumlah responden sebanyak 28 orang dan pengumpulan data menggunakan lembar cheklis, lembar kuesioner, dan lembar observasi. Variabel bebas adalah hemoglobin, nyeri, pengetahuan, emosi, dan dukungan keluarga sedangkan variabel terikatnya adalah ambulasi dini pasien post operasi fraktur ekstremitas bawah. Uji yang digunakan dalam analisa multivariat adalah uji regresi logistik. Penelitian ini menunjukkan hemoglobin (p = 0,014), nyeri (p = 0,013), pengetahuan (p = 0,013), emosi (p = 0,488), dan dukungan kelurga (p = 0,009), dari hasil uji regresi logistik, untuk pasien yang telah menjalani operasi fraktur ekstremitas bawah, dua faktor yang paling signifikan adalah nyeri terhadap ambulasi dini, dengan nilai p=0,018, dan hemoglobin terhadap ambulasi dini, dengan nilai p=0,019. Kesimpulan: Ada dua faktor yang berpengaruh signifikan yaitu faktor hemoglobin dan faktor nyeri, dan disarankan kepada perawat agar terlebih dahulu melaksanakan pengkajian sebelum melakukan ambulasi dini.

Abstract

Background: Patients in the intensive care unit are individuals who are in critical condition due to failure of function of one or more organs, with hemodynamic instability. They require special care and intensive monitoring, in the hope that their condition can improve through appropriate medical action (Setiyawan, 2016). The incidence of stroke is increasing every year, there are 11 million stroke cases in the world, especially in low-income countries, especially in South Asian states and it causes 4 million deaths every year, around 30% of them in Southeastern countries. In addition, stroke is one of the leading causes of death and disability in adults in the United States. Every year, nearly 795,000 people in the country experience a stroke, either new or recurrent, and this causes approximately 130,000 deaths annually. (WHO, 2016). Research Objective: To understand the effect of level 1 progressive mobilization on changes in oxygen saturation in patients with decreased consciousness. Methodology: This research uses quantitative methods. The approach is quasi-experimental with a utilitarian design, where the intervention is applied to one group, then observations are made on the independent variables. Research Results and Discussion: Before progressive mobilization measures were carried out, the level 1 oxygen saturation value in patients with impaired consciousness at Grandmed Lubuk Pakam Hospital, of the 24 respondents, 2 respondents had oxygen saturation of 92% (8.3%), 4 respondents 93% (16, 7%), 7 respondents 94% (29.2%), 7 respondents 95% (29.2%), 3 respondents 96% (12.5%) and 7 respondents had oxygen saturation 98% (4.2%) . Conclusion: The average O2 saturation value in patients with decreased consciousness before doing level 1 progressive exercise was 93.33, whereas after it increased it was 95.67.

Abstract

Background: The main goal in treating diabetic ulcers is to achieve optimal wound healing. So far, many people assume that wounds can heal quickly due to the drying process. However, in fact, a balanced moist environment in the wound actually supports the growth of new cells. Therefore, a wound care method based on the principle of moisture, or known as moist wound healing, is the main key in modern dressing. Moist wound healing is maintaining a moist wound environment using a moisture-retaining dressing. Objective: The aim is to determine the effect of modern moist wound healing dressing on the wound healing process in diabetic ulcers in the inpatient room 4 in front of Grandmed Hospital Lubuk Pakam. Method: The research design used was a Quasy Experiment using a Pretest posttest One Group Design research design with 37 patients as respondents using the Slovin formula. Results: The average value of diabetic ulcer condition in patients before and after Modern Dressing with Moist Wound Healing technique was obtained, namely 601 with a Standard Deviation of 1,163, with the results of the simple Paired T-Test obtained ? 0.05) obtained P value 004 where P value 0.05, then there is an average difference. This means that there is a difference in wound condition in diabetic ulcer patients before and after modern dressing with moist wound healing technique. Conclusion: The modern dressing technique with the principle of moist wound healing aims to maintain wound moisture in the ulcer healing process. Increased exudate production is a natural part of the inflammatory phase in the wound healing process. Based on the results of the research observation, it shows that before Modern Dressing with Moist Wound Healing Technique was carried out, the Mean value was obtained on the sixth day of treatment of 40.30 with a Standard Deviation of 3.302. Meanwhile, after the Modern Dressing Moist Wound Healing technique was carried out, the Mean value obtained on the sixth day of treatment was 31.90 with a Standard Deviation of 2.601, so it is recommended that nurses use the Modern Dressing Moist Healing technique for the wound healing process when treating wounds.

Abstract

Acute Respiratory Infection is one of the main causes of death in children in developing countries. ISPA is a disease of the upper or lower respiratory tract that usually infects and can cause various diseases ranging from asymptomatic or mild to severe inflammation. Lethality depends on environmental factors and predisposing factors. Acute Respiratory Infection is still often a major health problem because it is the main cause of death and disease in the world. Upper respiratory tract infection is a cause of death and morbidity in infants and children in Indonesia. The types of antibiotics that are often consumed to treat acute respiratory infections are penicillins and cephalosporins, such as amoxicillin, ceftriaxone, and cefotaxime. The method used for this dedication is the open and community training method or the online lecture method. The purpose of this community service is to provide information about the use of antibiotics to ARI patients at Grandmed Hospital. The activity was carried out in the form of online discussions via zoom with several patients and people who suffer from ISPA. The results obtained in this study were more male patients with ISPA than women. This condition is associated with high physical activity in boys and can quickly worsen physical conditions, including the immune system. In this study, patients were given a questionnaire to ensure that the information conveyed by the presenting team was properly conveyed and understood. In this activity, patients must understand the evaluation of the use of antibiotics in ARI so that in the future patients who suffer from it will get clear information.

Abstract

Depression in adolescents is a significant mental health problem. One factor that can influence the incidence of depression in adolescents is family emotional support. This study aims to examine the effect of family emotional support on depression in adolescents. The purpose of this study was to analyze the effect of family emotional support on the incidence of depression in adolescents and to evaluate the level of emotional support provided by families to adolescents. This study used a cross-sectional design with questionnaires distributed to 202 adolescents aged 15-21 years. Data were analyzed descriptively and with statistical regression tests to assess the relationship between family emotional support and the incidence of depression. As many as 78.7% of adolescents felt comfortable with family emotional support, but 32.18% reported that their families often did not ask about their condition. Overall emotional support reached 67.3%. The study also found that female adolescents were more susceptible to depression although age, gender, and education factors did not show a significant relationship. Family emotional support plays an important role in reducing the impact of depression in adolescents. Although overall emotional support is good, many adolescents feel they are not getting enough attention from their families. Family emotional support has an effect on preventing depression in adolescents. Families need to pay more attention to the emotional needs of adolescents and improve counseling services to help parents understand their children's emotional conditions.
