Abstract

Background: Heart surgery or Coronary Artery Bypass Graft (CABG) is one of the interventions of choice for the management of patients with cardiovascular disease that aims to minimize symptoms and optimize heart function. CABG is not without complications, complications caused by surgical procedures include physiological changes. Conditions during hospitalization due to immobilization cause loss of strength and muscle mass, decreased functional capacity, and decreased physical condition, so early mobilization of patients in the postoperative period of heart surgery who are treated in the intensive care unit (ICU). Purpose: To determine the implementation of early mobilization in improving the physical function of patients after coronary artery bypass graft (CABG). Method: A systematic literature review study using the Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA) reference and literature searches originating from databases including; Published, ScienceDirect, and SAGE Journal. Then 6 literatures were found which were processed and presented in tabulated form. Results: Based on a review of the 6 included literatures, it showed a significant positive effect of early mobilization and no adverse events occurred during early mobilization after heart surgery. The benefits of early mobilization can prevent and reduce the risk of post-CABG surgery complications, accelerate the recovery of functional walking capacity and have a positive impact on the length of stay, thereby reducing patient care costs. Conclusion: Early mobilization can prevent and reduce post-CABG complications, accelerate the recovery of functional walking capacity, have a positive impact on the length of stay, and thus reduce the cost of care. Suggestion: Nurses should also assess potential barriers to early mobilization, such as patient resistance, lack of motivation, anxiety, and postoperative pain because these factors can negatively impact patient cooperation. In addition, early movement after CABG can be combined with breathing exercises and psychoeducation to improve recovery outcomes.   Keywords: Coronary Artery Bypass Graft (CABG); Early Mobilization; Physical Function.   Pendahuluan: Bedah jantung atau Coronary Artery Bypass Graft (CABG) adalah salah satu intervensi pilihan untuk tatalaksana pasien dengan penyakit kardiovaskular yang bertujuan meminimalkan gejala dan mengoptimalkan fungsi jantung. Tindakan CABG dilakukan tidak tanpa komplikasi, komplikasi yang diakibatkan oleh prosedur bedah salah satunya yaitu perubahan fisiologis. Kondisi selama rawat inap karena imobilisasi, menyebabkan kehilangan kekuatan dan massa otot, penurunan kapasitas fungsional, dan penurunan kondisi fisik, sehingga sangat diperlukan mobilisasi dini pasien dalam periode pasca operasi bedah jantung yang dirawat di unit perawatan intensif (ICU).                         Tujuan: Untuk mengetahui implementasi dari mobilisasi dini pada peningkatan fungsi fisik pasien post coronary artery bypass graft (CABG). Metode: Penelitian systematic literature review menggunakan acuan Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA) dan pencarian literatur berasal dari database antara lain;  Pubmed, ScienceDirect, dan SAGE Journal. Kemudian ditemukan 6 literatur yang diolah dan disajikan dalam bentuk tabulasi. Hasil: Berdasarkan review dari 6 literature yang disertakan menunjukkan efek positif yang signifikan dari mobilisasi dini dan tidak ada kejadian buruk yang terjadi selama mobilisasi dini pasca bedah jantung. Manfaat dari mobilisasi dini dapat mencegah dan mengurangi risiko komplikasi pasca operasi CABG, mempercepat pemulihan kapasitas berjalan fungsional, dan berdampak positif dalam lama perawatan, sehingga mengurangi biaya perawatan pasien. Simpulan: Mobilisasi dini dapat mencegah dan mengurangi risiko komplikasi pasca operasi CABG, mempercepat pemulihan kapasitas berjalan fungsional, berdampak positif dalam lama perawatan, sehingga mengurangi biaya perawatan. Saran: Perawat juga harus menilai potensi hambatan terhadap mobilisasi dini, seperti penolakan pasien, kurangnya motivasi, kecemasan, dan nyeri pasca operasi karena faktor-faktor ini dapat berdampak negatif pada kerja sama pasien. Selain itu, mobilisasi dini setelah CABG dapat dikombinasikan dengan latihan pernapasan dan psikoedukasi untuk meningkatkan hasil pemulihan.   Kata Kunci: Coronary Artery Bypass Graft (CABG); Early Mobilization; Fungsi Fisik.

Abstract

Anxiety is a common psychological issue experienced by patients undergoing hospitalization, particularly those diagnosed with tuberculosis (TB), due to prolonged treatment periods and fear of stigma. As much as health education has been proposed as a potential intervention, research conducted so far is limited and varies in study methodology, and thus it's difficult to make conclusions. Further, there is no synthesized evidence regarding the impact of formal health education on anxiety among hospitalized TB patients, particularly where there is cultural diversity. This study aims to review the effect of health education interventions on anxiety levels in  TB patients. A systematic review was conducted using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Literature was identified through PubMed, Scopus, and ProQuest databases.  Inclusion criteria: RCTs or quasi-experimental studies involving pulmonary TB patients, evaluating the effect of health education on anxiety compared to other psychological interventions. Fifteen articles met the eligibility criteria and were included for analysis. The reviews summarized between 2019 and 2024 consistently showed that health education significantly reduces anxiety in hospital-admitted TB patients. This review update encapsulates the up-to-date evidence, expanding on the earlier work by adding recent evidence, mentioning varied forms such as single sessions, multimedia gadgets, and culturally sensitive materials. The findings highlight the importance of integrating health education as part of comprehensive TB care to address psychological distress and improve treatment outcomes.

Abstract

Spiritual care for cancer pain is an intervention to address the spiritual needs of patients with cancer in managing pain. Despite numerous studies conducted in this area, the diversity of spiritual care interventions for cancer pain remains a challenge. Objective: This literature review aimed to identify spiritual care interventions for cancer patients in managing pain. Method: The database used Scopus, Pubmed, Proquest, Science Direct, and Google Scholar. The keywords used were “spiritual care” OR “spiritual therapies” OR “spiritual healing” OR “faith healing” OR “spiritual intervention” OR “religious intervention” AND “cancer pain” OR “cancer-related pain” OR “neoplasm-related pain” OR “oncology pain. The search was limited to articles published between 2011 and 2021, resulting in the identification of 6642 articles, which only 10 met the inclusion criteria. The selected articles underwent critical appraisal using JBI tools. Results: This literature review showed that the types of spiritual care interventions consist of yoga, psychoreligious intervention, mindfulness, music therapy, and therapeutic touch, and the primary outcomes of spiritual care are pain levels. Conclusions: This literature review showed the interventions were very diverse in all types of cancer. Therefore, a spiritual care intervention study on managing pain with a specific type of cancer is needed.

Abstract

The formation of a stoma impacts various aspects of a patient's life, including physical, psychosocial, and spiritual well-being. If these impacts are not properly managed, it could decline the patient's quality of life. This study aimed to explore the psychosocial experiences of colorectal cancer patients living with a stoma. Method: twenty participants were interviewed using semi-structured questions, and the data was analyzed using a phenomenological approach. the findings revealed three key themes: emotional reactions having a stoma, limited social interactions, and worries about performing religious practices. Based on these results, the study recommend the need for education for colorectal cancer patients with stomas, particularly psychosocial aspects, to improve adaptation and quality of life.

Abstract

Percutaneous Coronary Intervention (PCI) is currently indicated for patients experiencing acute coronary syndrome and individuals with chronic stable angina that is refractory to optimal medical therapy. Advancements in devices, stent design, adjunctive technology, and the development of more potent and effective antiplatelet therapies have contributed to improved outcomes. However, major periprocedural complications during PCI remain a concern, with an overall transulnar access site complication rate of 41%, a vasospasm incidence of 7%, and a hematoma occurrence of 29%. Cold compress application is widely used due to its physiological effects, including vasoconstriction, reduced tissue metabolism, increased blood viscosity, and local anesthetic properties. Cold compresses can decrease blood flow and capillary permeability by inducing arteriolar vasoconstriction, thereby reducing bleeding.Objective: This study aims to examine the implementation of cold compress therapy in preventing hematoma in post-Percutaneous Coronary Intervention (PCI) patients.Methods: This study employs a systematic literature review approach. Data synthesis was conducted by analyzing eight studies that met quality assessment standards and inclusion-exclusion criteria. The databases used for literature retrieval included Google Scholar, PubMed, ScienceDirect, EBSCO, and ProQuest. The findings were processed and presented in tabular form.Result: Cold compresses can help reduce hematoma and pain after a Percutaneous coronary Intervention (PCI), cold compresses can also help prevent hematomas. How cold compresses help cold compresses vasoconstrict blood vessels and cold compresses speed up blood clotting.Conclusion: The reviewed articles indicate that the application of an ice pack or cold compress is more effective in reducing pain and preventing hematoma caused by catheter removal in patients undergoing Percutaneous Coronary Intervention (PCI).

Abstract

Obesity is one of the main contributing factors to metabolic syndrome, which results in an increased risk of chronic diseases such as diabetes mellitus, cardiovascular disease, and other metabolic dysfunctions. Physical exercise has long been recognized as a potential nonpharmacological strategy in reducing metabolic risk factors, but the effectiveness of various types of exercise in managing metabolic syndrome still needs to be further reviewed. This study aims to evaluate the effectiveness of physical activity in managing components of metabolic syndrome in an obese population. Using the PRISMA method of literature searches through Pubmed, Ebsco, Scopus and Wiley databases by using specific word combinations to obtain suitable articles. Inclusion criteria included English language articles published in the last five years (2019-2024), randomized controlled trial type, and discussing physical exercise interventions in obese populations. Of the 893 articles retrieved, 7 were reviewed. Studies show that physical exercise significantly reduced risk components of metabolic syndrome such as blood sugar, insulin, HOMA-IR, visceral fat and improved VO 2 max and quality of life. However, long term success was strongly influenced by the level of adherence to the exercise program. Physical exercise in an effective and safe intervention in reducing the risk of metabolic syndrome in obese individuals.

Abstract

A systematic review of randomized clinical trials in cancer patients on the effectiveness of propolis mouthwash use in post-chemo and radiation cancer patients. Oral mucositis (OM) is a common and highly symptomatic complication of cancer therapy that affects function, quality of life. Chemotherapy and radiation are the most widely used interventions in cancer treatment. The prevalence of oral mucositis side effects among HNC patients is 93.9%. Research on the management of OM is still ongoing. This study aims to analyze a systematic review of the use of propolis in preventing and reducing the severity of oral mucositis in cancer patients undergoing chemo- and radiation therapy. Four databases through article searches tailored to the formulation of PICO questions consisting of ScienceDirect, Pubmed, ProQuest, Sage journals and EBSCO, A total of 2,252 articles were identified, and 8 articles were included in the final analysis. Of the ten articles selected, there were 6 articles that were suitable for oral mucositis treatment interventions that showed the effectiveness of using propolis mouthwash to treat ral mucositis with a Randomized Controlled Trial (RCT) design. The use of propolis mouthwash for oral mucositis can be done to overcome and prevent oral mucositis in cancer patients undergoing chemo and radiation therapy.

Abstract

Penyakit kanker dapat menimbulkan nyeri, kelemahan, masalah tidur, dan kelelahan, dan mempengaruhi keterbatasan fungsi yang akan berakibat ke kehidupan sehari-hari. Salah satu gejala yang paling umum dan sering dikeluhkan pada pasien kanker adalah nyeri. Penulisan ini bertujuan untuk mengetahui efek self care behavior untuk mengatasi nyeri pada pasien kanker. Penulis menggunakan beberapa database seperti Scopus, SpringerLink, Sage Journal. Clinical Key Nursing. untuk digunakan dalam pencarian artikel yang relevan. Terdapat total 8 artikel yang telah dianalisis dimana terdapat beberapa intervensi self care behavior yang digunakan untuk mengatasi nyeri pada pasien kanker antara lain spiritual, music terapi, olahraga, meditasi dan pijat. Berdasarkan dari beberapa jurnal yang telah dianalisis dapat disimpulkan bahwa pemberian intervensi self care behavior dapat mempengaruhi skala nyeri pada pasien kanker.

Abstract

Breast cancer is the second most prevalent type of cancer and one of the leading causes of mortality. Efforts to reduce mortality rates can be achieved by empowering patients to manage symptoms caused by cancer and the side effects of treatment therapies. This study aimed to evaluate the effectiveness of empowerment education in increasing self-efficacy in breast cancer patients. The study employed a systematic review of eight articles sourced from various databases, including ProQuest, SpringerLink, ScienceDirect, Sage Journal, Google Scholar, and PubMed. A total of 8,693 articles were retrieved from seven databases. After screening based on inclusion criteria, 638 articles were deemed eligible. Based on the selection of titles, abstracts, and full texts, 22 articles were chosen, and after duplicate elimination, 10 articles were analyzed using the Joanna Briggs Institute (JBI) quality assessment tool. The analysis results indicated that the empowerment approach is effective in improving self-efficacy and quality of life in breast cancer patients. By utilizing empowerment education, healthcare professionals can provide holistic support to patients, improve therapeutic outcomes, and reduce breast cancer mortality rates. This study recommended the integration of empowerment education as an essential component of breast cancer management in various healthcare facilities.

Abstract

The role of nurse medical surgical specialist is as caregiver, researcher, and innovator applied on nursing care of cancer patients that used the chronic illness trajectory model approach, application of evidence based nursing practise in spiritual care program on leukemia patients to reduce depression, and perform innovation group project on educational management for patients undergoing radioactive Iodine 131 in Radioactive Isolation Ward Dharmais Cancer Hospital. The trajectory concept is one of the nursing theories that assist patients and caregivers’ empowerment to provide appropriate care in the stage and phase of patient's illness and focuses on palliative care. Palliative care is one of the focus areas of nurse specialist in the field of cancer care that will improve the patient's independence and quality of life. The application of spiritual care program is non-invasive, cheap, safe, and practically in managing the patient's depression status. Education could improve patients' understanding of radiotherapy and its side effects to improve the quality of nursing care
