Abstract

Demam Berdarah Dengue (DBD) adalah penyakit infeksi yang masih menjadi masalah kesehatan yang serius karena bersifat endemis. Penyebaran di daerah tropis dan subtropis terutama di Asia Selatan-Timur, Asia Tenggara. Penelitian ini bertujuan untuk menjelaskan beberapa faktor yang berpengaruh terhadap kejadian DBD pada anak usia 6-12 tahun. Penelitian menggunakan desain penelitian campuran atau mixed methode yang mengkobinasikan bentuk kuantitatif dan kualitatif. Spesifikasi penelitian kuantitatif :observasional analitik desain case control study. Dilakukan kajian kualitatif dengan wawancara mendalam. Populasi studi adalah semua anak usia 6-12 tahun yang tinggal di Kota Semarang. Jumlah subyek 70 kasus dan 70 kontrol yang diambil secara consecutive sampling. Data di analisis secara bivariat dan multivariat dengan metode regresi logistik. Faktor-faktor  yang terbukti berpengaruh terhadap kejadian DBD pada anak usia 6-12 tahun; pendidikan ibu rendah (p= 0.004; OR  3.031; 95%CI 1.4281-6.434), kebiasaan tidak memakai obat anti nyamuk ( p= 0.001; OR 4.293; 95%CI 1.935-9.526), kebiasaan tidak memakai pakaian panjang (p= 0.013; OR 2.759; 95%CI 1.240-6.138) Sedangkan faktor yang terbukti tidak berpengaruh; kebiasaan tidur siang, kebiasaan menggantung pakaian dalam rumah, forum penyampaian informasi, rutinitas pemeriksaan jentik dan praktik PSN. Faktor-faktor yang terbukti berpengaruh terhadap kejadian DBD pada anak usia 6-12 tahun adalah pendidikan ibu rendah, kebiasaan tidak memakai obat anti nyamuk dan kebiasaan tidak memakai pakaian panjang memiliki probabilitas 85,3% untuk mengalami DBD. Kata kunci  : DBD, nyamuk, dengue.   VARIOUS FACTORS INFLUENCING THE DENGUE FEVER OCCURRENCES IN THE CHILDREN AGE 6 TO 12 YEARS OLD   ABSTRACT The Dengue Fever is an infectious disease that still becomes a serious health issue since it is endemic. The disease spreads in the tropical and subtropical areas particularly in the Southeast Asia. This research aims to explain the various factors influencing the occurrences of the dengue fever in the children age 6 to 12 years old. This research uses mixed method research design combined with both quantitative and qualitative methods. The quantitative research specification is the observational analytic case control design while the indept interview technique is used as the qualitative research method. The population of the study is all children age 6 to 12 years old who live in the Semarang City. The number of the subjects is 70 cases and 70 controls which are taken by consecutive sampling method. The data are analyzed using the bivariate and multivariate with the regression logistic method. The factors that has been proven to influence the dengue fever occurrences in the children 6 to 12 years old are the lack of the mother?s education ((p= 0.004; OR  3.031; 95%CI 1.4281-6.434), the habit of not using the insect repellent use (p= 0.001; OR 4.293; 95%CI 1.935-9.526) and the habit of not wearing long clothes (p= 0.013; OR 2.759; 95%CI 1.240-6.138). Meanwhile, the habit of taking nap, the habit of hanging clothes, the information delivery forum, the routine examination of the mosquitos? larva and the mosquitos? eradication are not shown as the influential factors causing the dengue fever. The factors recognized as the significant factors against the occurrences of the dengue fever in the children age 6 to 12 years old are the lack of the mother?s education, the habit of not using the insect repellent and the habit of not wearing long clothes have the probability 85,3% to experience the dengue fever. Abstract

Filariasis merupakan suatu penyakit yang masih menjadi masalah kesehatan di Indonesia. Kecamatan Pekalongan Selatan merupakan salah satu daerah endemis filariasis. Pada tahun 2014 - 2016 didapat 71 kasus filariasis yang mungkinkan oleh berbagai faktor lingkungan yang banyak terdapat rawa dan kolam dan digenangi air serta ditumbuhi oleh tanaman air. Faktor lain selain dari faktor lingkungan adalah faktor sosial, ekonomi dan perilaku masyarakat. Tujuan penelitian ini mengetahui faktor-faktor risiko lingkungan fisik (genangan air), lingkungan biologi (tanaman air, ikan predator), Lingkungan sosial ekonomi (pekerjaan, pendidikan dan penghasilan) dan faktor perilaku (kebiasasaan keluar malam hari, kebiasaan menggunakan obat anti nyamuk dan kebiasaan menggunakan baju pelindung diri dari gigitan nyamuk) yang berpengaruh terhadap kejadian filariasis. Jenis penelitian ini merupakan penelitian observasional dengan rancangan studi kasus kontrol. Kasus adalah penduduk yang menderita filariasis dan kontrol adalah penduduk yang  tidak menderita filariasis. Jumlah kasus dan kontrol adalah 80. Pengambilan data dilakukan melalui observasi dan wawancara. Analisis dilakukan secara bivariat dan multivariat dengan menggunakan regresi logistik. Analisis Multivariat menunjukkan bahwa dari 11 (sebelas) variabel yang dianalisis terdapat 1 variabel yang terbukti berpengaruh terhadap kejadian filariasis di Kecamatan Pekalongan Selatan, yaitu : Pemakaian Obat Anti Nyamuk (OR = 35,286, CI 95% = 7,390 ? 168,476). Penggunaan obat anti nyamuk merupakan faktor risiko yang paling dominan untuk terjadinya penularan filariasis. Masyarakat disarankan menggunakan kelambu atau anti nyamuk sewaktu tidur, memakai pelindung diri (baju dan celana panjang) waktu keluar rumah pada malam hari. Perlu adanya tindakan penyuluhan dan penyebarluasan informasi tentang filariasis dalam rangka meningkatkan pengetahuan masyarakat.     Kata kunci: filariasis, lingkungan, perilaku.   VARIOUS ENVIRONMENTAL AND BEHAVIOR FACTORS THAT INFLUENCE THE FILARIASIS EVENT   ABSTRACT Filariasis is a disease that is still a health problem in Indonesia. South Pekalongan District is one of the endemic areas of filariasis. By the year 2014 - 2016, it was found about 71 cases of filariasis. This is caused by many factors in the environment, such as swamp and pool that was flooded with water with many water plants. Other factor, that caused by are sosio economic and community behavior. The objective of this study was to determine physical factors (swamp / pool), Biological (water plants, fish / animal predators) of the environment. Socio ecomic factor (education, job and income), behaviour factor the habitat of (going outside at night, habit of using mosquito repellent and wearing clothes to protect from mosquitoes bite) that may give influence the filariasis cases in South Pekalongan District. This research was an observasional research with a case-control approach. Case in this study was filariasis cases and for control was people suffer from filariasis. Total sampler were 80 sample. Data was taken by observation and interview. Data collected was analyzed by using logistics regression. Multivariate analysis showed that from 11 variables, there are 1 variable were proved to be the risk factor of filariasis at South Pekalongan District, which are : respondent, habit of using mosquito repellent (OR = 35,286, CI 95% = 7,390 ? 168,476). The use of mosquito repellent is the most dominant risk factor for filariasis transmission. It is suggested that people sould use mosquito net or repellent when bed time, self protection dress when they go out at night.  It is necessary to perform health promotion and extend the information related to filariasis in order to improve people knowledge .    Keyword : Filariasis, Environmental, Behavioral.

Abstract

Obesity increases morbidity and mortality through its multiple efects on nearly every human system. Obesity has efect on the immune response which leads to susceptibility to infections. ß lactamases are bacterial enzymes that inactivate ß-lactam antibiotics by hydrolysis. One group of ß lactamases, extended-spectrum ß lactamases (ESBLs), have the ability to hydrolyse and cause resistance to various types of the newer ß-lactam antibiotics. The management of skin and soft tissue infection with polymicrobial infection and multi drug resistance can be challenging problem. We present a case of an obese patient with polymicrobial SSTI. bPatient showed clinical sign of sepsis with acute kidney injury. With fluid rescucitation, early antibiotic administration and surgical treatment, she gained clinical improvement. The key to successful management of patients with severe skin and soft tissue infection are early recognition and complete surgical debridement. Clinicians need to consider appropriate early empirical antibiotic combination therapy coverage or the use of combination therapy to treat SSTI

Abstract

Growing evidence suggests that microRNAs (miRNAs) play a pivotal role in viral infection. The objective of this study was to assess the association between the expression of miR-150, hsa-let-7e, and miR-146a on cytokine expression during dengue infection. Dengue virus (DENV) strain SJN-006, a serotype 2 DENV strain of the Cosmopolitan genotype, isolated in Bali, Indonesia, was used to infect peripheral blood mononuclear cells (PBMCs) isolated from healthy individuals. The relative gene expressions of miR-150, hsa-let-7e, and miR-146a as well as the gene expression of cytokines (IL-6, IL-8, IP-10, and MIP-1β) were determined using quantitative real time - polymerase chain reaction (qRT-PCR) at 6, 12 and 24 hours post infection (hpi). Correlations between the microRNAs and cytokines were analyzed by means of causality tests. Our data suggests that miR-150 and hsa-let-7e were significantly higher in infected-PBMCs after 12 hpi compared to the uninfected-PBMCs (p<0.05). The causality tests demonstrated that miR-150 and hsa-let-7e were negatively correlated with IL-8 expression, meanwhile miR-146a was the contrast. DENV infection was negatively and positively correlated with miR-150 and hsa-let-7e, respectively, after 24 hpi. In conclusion, our data demonstrates the vital role of miR-150, hsa-let-7e, and miR-146a in regulating IL-8 expression with possible different pathways.
