Abstract

Tindakan operasi atau pembedahan walaupun minor/mayor merupakan pengalaman yang sulit dan bisa menimbulkan kecemasan bagi hampir semua pasien dan keluarganya teknik relaksasi dapat dilakukan untuk mengintervensi kecemasan pre operasi. Keadaan relaksasi akan meningkatkan sekresi hormon endorfin dari dalam tubuh pasien. Pasien akan merasa nyaman dan tidak berfokus pada kecemasan akan operasi. Penelitian ini bertujuan untuk menganalisis penurunan gejala kecemasan pada pasien sesudah penggunaan teknik relaksasi dan menganalisis keefektifan penggunaan teknik relaksasi terhadapat penurunan tiap-tiap gejala kecemasan.  Penelitian ini merupakan penelitian merupakan penelitian pre eksperimental dengan desain One Gruop Pretest-Postes Design. Sampel dalam penelitian ini adalah pasien yang akan dilakukan operasi di ruang bedah RSUD Banyumas yang berjumlah 30 responden. Data dalam penelitian ini dianalisis dengan menggunakan pair t-test dan chi square test. Pasien pre operasi rata-rata mengalami 11 gejala kecemasan dari 20 gejala kecemasan, setelah dilakukan tehnik relaksasi turun menjadi 4 gejala kecemasan. Penggunaan teknik relaksasi secara signifikan menurunkan gejala kecemasan pada pasien pre operasi (p<0,05). Penggunaan teknik relaksasi secara signifikan berpengaruh menghilangkan gejala kecemasan  berupa perasaan khawatir, merasa tegang, gemetar, lesu, tidak bisa beristirahat dengan tenang, sering terbangun dimalam hari, sedih atau terlihat murung, nyeri pada otot, lemas, denyut nadi meningkat, berdebar-debar, rasa tertekan atau sempit di dada, nafas pendek atau sesak dan  kepala terasa berat (p<0,05). Penggunaan teknik relaksasi tidak dapat menghilangkan gejala kecemasan secara signifikan berupa gejala pucat, rasa mual atau muntah, sering buang air kecil, mudah berkeringat dan kepala pusing, tidak tenang serta gelisah (p>0,05).  Dari penelitian ini dapat disimpulkan bahwa Teknik relaksasi efektif menurunkan gejala kecemasan pada pasien pre operasiKata Kunci: Teknik relaksasi, Kecemasan

Abstract

In regional hospitals are still found nebulizer therapy using oxygen as a as a driving gas, but there is already a tool Jet nebulizer is intended specifically for producing steam therapy for asthma patients. This study aims to evaluate the effectiveness of oxygen as a driving gas for nebulisers steam therapy in asthmatic patients. This study is an observational analytic study. Popolation in this study was asthma patients who received nebulizer therapy by using oxygen as a driving gas, the number of samples in this study amounted to 30 respondents. The measured variables were Breath Pattern, Respiration Rate (RR), Breath Sound, Oxygen Saturation (SpO2). The results showed that, the nebulizer using oxygen as a driving gas is still effective against changes in breath patterns from fast and shallow (Tachypne) to become normal (Eupnea). Nebulizer using oxygen as a driving gas is still effective for lowering RR of asthma patients. Nebulizer using oxygen as a driving gas is still effective against changes in breath sound from rhonchi / wheezing to vesicular. Nebulizer using oxygen as a driving gas is still effective against the increase in blood spO2 in asthmatic patients. The conclusions of this study are nebulizer therapy by using oxygen as a driving gas, still effective a changes in breath sound from tachypne to eupnea , can increase SpO2 in blood and decrease RR, and change breath pattern from rhonchi / wheezing to vesicular. Abstract

Background: Treatment for distal hypospadias is a controversy among the experts. Some argue that it does not require any surgery if it does not interfere with the patient's urinary and sexuality. Purpose: This study aimed to determine whether patients with distal hypospadias need treated or untreated.Methods: The method used is a case study. Participants in this study is a 10-year-old male with distal hypospadias—location of the study in Purwokerto city, Banyumas district, Central Java. Participants were interviewed and physically examined by a doctor and nurse. The results of interviews and physical examinations are recorded and analyzed based on a literature review to determine the decision, treatment, or untreated. Results: A 10-year-old patient with hypospadias and the pediatric urologist do not give surgery advice, because the urethral opening is still close to the glans penis, and there are no symptoms of urinary dysfunction. Conclusion: Patients with distal hypospadias may be untreated as long as they do not interfere with urinary function

Abstract

Background: Some of regional hospitals still provide oxygen gas for nebulizer to therapies in astma patiens. The study was aimed to observe the effectiveness of jet nebulizer vs oxygen as a driving gas for nebulizer on respiratory parameters of asthma patients which is breath pattern, respiration rate (RR), breath sound, oxygen saturation (SpO2), peak expiratory flow rate (PEF}. Method: The research was an experimental with a combination design, pre-post test with control group and post test only with control group in RSUD dr. R. Goeteng Taroenadibrata Purbalingga consisted of 60 respondents selected by proportional stratified random sampling. Data analysis used was independent t-test and fisher’r exact test. Result: There was no difference in the respiratory pattern variables between jet nebulizer and oxygen as a driving gas nebulizers (p> 0.05). Abstract

Phlebitis is one of nosocomial infections that frequently occurred in hospitals. Phlebitis is mainly related with infusion and therapy. Many risk factors can cause phlebitis. The purpose of this study was to predict the prevalence of phlebitis in patients based on the causal factors in Banyumas Regional Hospital. The design of this study was an analytic survey of cohort approach, with a sample of 218 by using consecutive sampling. The study was conducted from November to December 2019 using checklist of observation sheets. The results demonstrates 5 phlebitis risk factors associated with the occurrence of phlebitis, namely the type of fluid (P = 0.011), nutritional status (P = 0.001), catheter size (P = 0.002), injection therapy (P = 0.027) and comorbidities (P = 0.003). The probability of 5 risk factors for the occurrence of phlebitis (88.28%) with nutritional status being the dominant factor (b = 3.928) with probability of (13.48%). To minimize the prevalence of phlebitis, the medical personnel may conduct initial phlebitis screening to determine accurate and appropriate preventive measures.

Abstract

The control of infection in diabetic foot ulcers is essential to prevent injuries from getting worse. Infection in diabetic foot ulcers is generally caused by bacteria existence in the wound. Antibiotics may not be orally able to exterminate bacteria rapidly in the wound area, so antibiotics are needed directly and topically to the wound to reduce the infection process. This case describes wound care intervention using gauze added by antibiotic metronidazole as a wound compress in a 46-year-old male patient with diabetic foot ulcers treated at Cempaka Ward Banyumas Regional Public Hospital. This case is interesting because the medical intervention given to patients is rarely implemented in clinical practice. The conclusion is that a metronidazole antibiotic used to compress wounds in patients with diabetic foot ulcers can reduce the wound infection process.

Abstract

Industri Kecil dan Menengah (IKM) merupakan salah satu bidang yang memberikan kontribusi yang signifikan dalam memacu pertumbuhan ekonomi Indonesia, terutama para generasi muda karena merasakan hal ini merupakan profesi yang cocok untuk dikerjakan. Meskipun sangat populer, ekonomi kreatif memiliki tantangan di masa pandemi Covid-19 kali ini banyak sekali IKM yang bergerak di bidang tersebut terpaksa gulung tikar karena tidak mampu bertahan karena resesi pandemi Covid. Penelitian ini bertujuan untuk mengeksplorasi peran IKM dalam meningkatkan kreativitas dan cara mengelola ekonomi kreatif selama masa pandemi Covid-19 dan Adaptasi Kebiasaan Baru (AKB) dengan dukungan metode survei. Hasil penelitian menunjukkan bahwa pengembangan IKM melalui penguatan branding dalam Ekonomi Kreatif; Promosi dan Pemasaran di digital; dan memetakan faktor-faktor internal maupun eksternal dengan penciptaan iklim usaha yang kondusif, memberikan bantuan permodalan, adanya perlindungan usaha dari pemerintah, diusahakan adanya pengembangan penguatan kemitraan, pengembangan promosi, dan pengembangan kerjasama yang setara.

Abstract

Background: Doing activities that burden the back, such as bending over for a long time and lifting weights in the wrong position, can cause injury to the spine, leaving back pain. Thoracic Lumbar Sacral Orthosis Brace (TLSO Brace) is a tool used to support the spine so that it can prevent such an injury. Objective:This research aims to design and pilot a new ergonomic TLSO Brace that can be used to improve posture, reduce back pain and prevent back pain.Methods: This research is divided into several stages, namely designing, manufacturing, and testing.Results: The results show that the level of ergonomics is mainly in the Good category. The use of the TLSO Brace was proven effective in reducing back pain intensity (p0.0001); the average pain was reduced after 14 minutes. The results also found that all respondents who used the brace at work felt no back pain. It can be seen that the lordosis back arch becomes upright after using it, meaning the tool also improves posture.Conclusions: The ergonomic TLSO Brace has been created and can improve body posture, reduce pain intensity and prevent back pain from recurring.

Abstract

Abstract The aim of this research is to analyze the influence of work motivation, work environment and leadership style on employee job satisfaction at the Paser Regency Environmental Service. The design in this research is quantitative descriptive research. The research population was all employees at the Paser Regency Environmental Service, totaling 156 people. The sampling technique in this research used a purposive sampling technique. The sample chosen for this research was all 82 employees with State Civil Apparatus status. In this research, the research instrument used was a questionnaire. The results of data analysis explain that the influence of the work motivation variable on the job satisfaction variable is 0.183, the influence of the work environment variable on the job satisfaction variable is 0.434, and the influence of the leadership style variable on the job satisfaction variable is 0.331. This shows that work motivation, work environment and leadership style have a positive and significant effect on employee job satisfaction. It is hoped that leaders can pay attention to employee incentives, make improvements to existing buildings, and build active communication with employees.

Abstract

Background : The number of diabetic patients in Indonesia is on the rise. In contrast to typical wounds in non DMT2 patients, chronic wounds in DMT2 patients heal more slowly, which makes it difficult to achieve complete primary wound healing. Thus, surgery is frequently required to achieve optimal healing. Patients' physical condition, age, comorbidities, and financial circumstances such as high medical costs frequently limiting patients from receiving comprehensive care, resulting alternative treatments are required to treat chronic wounds in DMT2 patients whom prefer conventional medications, addressing all circumstances. Honey, in addition to having fructose compounds, which has the benefit of increasing glucose homeostasis and insulin response, resulting in lower insulin and plasma glucose levels, also has been shown to contain anti-inflammatory and antimicrobial substances that aid in wound healing. Thus, it may be an alternative therapy for chronic wound in DMT2 patients. Methods : A Case-series studies of four DMT2 patients who were referred to Plastic and Reconstructive Outpatient ward with chronic wound were evaluated on a monthly basis following conservative treatment using Nusantara local honey-coated gauze. Result : Secondary wound healing, which can be assessed from the epithelialization process started from the peri-wound area has been obtained through monthly observations. Conclusion : Honey is used as an alternative therapy for patients with diabetic foot ulcers due to its anti-microbial and anti-inflammatory properties in the wound healing process. Furthermore, honey is considered less expensive and more affordable alternative for patients with co-morbidities that is impossible to operate, or with financial limitations.
