Abstract

Healthy and Clean Behavior (HCB) is an endeavor to promote individual’s consciousness to improve their health. HCB components include proper hand washing, adequate fruits and vegetables intake, adequate physical activity, and clean environment maintenance. Teachers have their own portion in HCB. They can directly transfer their knowledge about HCB to their students or they can become role models to the students in their action. This study aimed to assess whether teachers’ knowledge is sufficient and how it impacts their students’ behavior. This observational study used a cross-sectional design. A total of 38 students and 3 teachers from Klamono 60 Inpres Elementary School, Sorong Regency were involved in this study. Questionnaires on HCB knowledge and HCB practice were used to evaluate both the teachers and students. All teachers (n= 3) had a good HCB knowledge while 74% of students had poor attitude towards Healthy and Clean Behavior. The knowledge of Healthy and Clean Behavior of all teachers were good while HCB practices among students were mostly poor.

Abstract

Growing evidence suggests that microRNAs (miRNAs) play a pivotal role in viral infection. The objective of this study was to assess the association between the expression of miR-150, hsa-let-7e, and miR-146a on cytokine expression during dengue infection. Dengue virus (DENV) strain SJN-006, a serotype 2 DENV strain of the Cosmopolitan genotype, isolated in Bali, Indonesia, was used to infect peripheral blood mononuclear cells (PBMCs) isolated from healthy individuals. The relative gene expressions of miR-150, hsa-let-7e, and miR-146a as well as the gene expression of cytokines (IL-6, IL-8, IP-10, and MIP-1β) were determined using quantitative real time - polymerase chain reaction (qRT-PCR) at 6, 12 and 24 hours post infection (hpi). Correlations between the microRNAs and cytokines were analyzed by means of causality tests. Our data suggests that miR-150 and hsa-let-7e were significantly higher in infected-PBMCs after 12 hpi compared to the uninfected-PBMCs (p<0.05). The causality tests demonstrated that miR-150 and hsa-let-7e were negatively correlated with IL-8 expression, meanwhile miR-146a was the contrast. DENV infection was negatively and positively correlated with miR-150 and hsa-let-7e, respectively, after 24 hpi. In conclusion, our data demonstrates the vital role of miR-150, hsa-let-7e, and miR-146a in regulating IL-8 expression with possible different pathways.

Abstract

Severe Malaria is commonly found due to Plasmodium falciparum infection, which is causing high mortality in patient. A Severe case might be difficult to treat optimally in the remote settings area that could be due to lack in awareness of disease manifestation and inadequate diagnostic facilities. We report a case of Severe Malaria Falciparum infection, confirmed by blood smear examination of Plasmodium falciparum (Pf) parasites. Patient had received Artemicinin Based Combination Therapy (ACT), which consisted of dihydroartemisinin (DHA)-piperaquine and primaquine (PQ) with no clear clinical and microscopy improvement. The second line anti malaria consisted of quinine, doxycycline, and PQ was then initiated, showing clinical and microscopy responses.

Abstract

Mucormycosis is an opportunistic andfrequently fulminating fungal infection causedby members of the family Mucoraceae, orderMucorales and class Zygomycetes which highly invasive and high mortality. Mucormycosis occurs in diabetic patients with poor blood glucose control. We reported a case of rhinocerebral mucormycosis in 20-year-old female patient with type 1 diabetes. The patient was treated for diabetic ketoacidosis and swollen symptoms in the left eye and out secretions from the left nose containing blackish spots. The probable diagnosis of rhinocerebral mucormycosis is made based on the results of direct examination and culture of nasal and palate tissue biopcywhich result suitable for Mucor sp. She was given systemic antifungal therapy with amphotericin B and symptoms of swelling of the left eye and secretions of the left nose with patches of blackness improved.

Abstract

Penegakkan diagnosis infeksi pada seorang pasien tidak selamanya mudah untuk dilakukan. Upaya ini harus selalu dimulai dari melakukan anamnesis yang terarah, pemeriksaan fisik yang menyeluruh ditambah dengan pemeriksaan laboratorium yang sesuai dengan kecurigaan tempat terjadinya infeksi.

Abstract

Clinical symptoms during the treatment of uncomplicated P. falciparum malaria have been reported in several studies. Parasite density in the blood is associated with clinical symptoms, platelet, and bilirubin count. A 52-year-old male, weighing 52 kg, was given one capsule containing 325 mg bitter melon (Momordica charantia) extract for 3 days of treatment. Patients were followed up for 28 days, days 1,2,3,4,7,14,21,28, and peripheral blood microscopic examination and clinical symptoms were carried out. Measurement of platelets, total bilirubin, direct bilirubin D0, D14 and D28. Naranjo Scale, adverse events measurements were taken on D7, no signs of severe malaria, no comorbidities, no history of malaria, blood pressure 110/80 mmHg, peripheral blood parasite examination on D0 found P. falciparum parasites of 31,714/µL, D0 platelet value of 94x10 /mm3, D0 total bilirubin value of 1.3 mg% and D0 direct bilirubin value of 0.6 mµ/mL. On D1 parasite count was 2,400/µL, D2 was 372/µL, D3, D4, D7, D14, D21, D28 no asexual parasites were found again. Clinical symptoms that arose on D0 were nausea, no appetite, sweating, chills, sleep disturbance, dizziness, headache, and weakness with a body temperature of 37°C. After D3 of bitter melon extract treatment, no clinical symptoms were found again. Naranjo scale score based on nurse and pharmacist agreement was 6 (probable) at D7 for the adverse events of nausea and vomiting. There were no reported adverse events such as nausea, vomiting, diarrhea, abdominal pain, cough, sleep disturbance, dizziness, headache and weakness on D2 treatment. Bitter melon extract is effective for uncomplicated malaria P. falciparum, reduces clinical symptoms, and is safe.

Abstract

Introduction. One consequence of DM is the emergence of peripheral artery disease (PAD) and diabetic peripheral neuropathy (DPN). Diabetic peripheral neuropathy is a complication of diabetes that presents signs and symptoms of motor and sensory disturbances, while PAD is an atherosclerosis condition that gradually develops in the arterial vessels. This study aimed to determine correlation of arterial obstruction assessed using arteriography and the nerve impairment assessed using nerve conduction velocity (NCV) in DM patients with PAD and DPN. Methods. This is a cross-sectional study that takes secondary data from previous research conducted from July 2018 to June 2021 in Cipto Mangunkusumo hospital. Subjects were people with DM type 2 who had NPD and PAP who met the inclusion criteria and did not meet the exclusion criteria. Subjects underwent an arteriography examination to assess the stenosis in the peroneal artery, anterior tibial artery, and posterior tibial artery. Subjects also underwent nerve conduction velocity (NCV) examinations in the peroneal N. communis, N. peroneal superficialis, N. tibialis, and N. suralis. The correlation between the two variables was then tested using the Spearman correlation test. Results. The peroneal artery stenosis had a moderate negative correlation (r = - 0.420) with the sensory NCV of the superficial peroneal nerve which was statistically significant (p = 0.023). Meanwhile, the correlation of stenosis and NCV disorders in the peroneal nerve with the common peroneal nerve, anterior tibial nerve with tibialis nerve and posterior tibial nerve with sural nerve was not statistically significant. This can be explained by the possibility of collaterals appearing in people with PAD, the structure of vasculature on the nerves, and the small number of samples. Conclusions. There is a moderate negative correlation between the stenosis of the peroneal nerve and the sensory NCV of the peroneal superficialis. However, insignificant results were found in the correlation between peroneal artery with common peroneal nerve, anterior tibialis artery with tibial nerve, and posterior tibial artery with sural nerve.

Abstract

Susceptibility to infection and increasing antibiotic resistance put burn patients at risk of developing infections caused by multidrug-resistant organisms (MDRO). This condition can progress to sepsis, increasing morbidity and mortality. This retrospective cohort study employed the medical record data of patients treated at Public Hospital X in Jakarta, Indonesia, from January 2020 to June 2022. Of 160 subjects, most were aged <60 years (82.5%) and had comorbidities (16.88%). The most common cause of burns was fire (86.25%). The use of medical devices was 90.63%, with a 14-day median length of stay. The most common gram-negative MDRO pathogens were K. pneumoniae (29.91%), Enterobacter sp (22.32%), and Acinetobacter (20.54%); 45% of patients infected with MDRO died. The bivariate analysis found an increased risk of death due to MDRO infection in burn patients (RR 1.103; 95%CI 1.004-1.211, p-value = 0.046). After adjusting for role variables (age, comorbidities, total body surface area, use of medical devices, length of stay) and from multivariate analysis, the confounding variables for MDRO infection and mortality were length of stay and age. MDRO infection increases the mortality rate in burn patients. Mortality in burn patients due to MDRO infection is greater than non-MDRO.

Abstract

Indonesia continues to grapple with sanitation issues, contributing to high incidence of gastrointestinal infections. Timely and accurate diagnosis is crucial to minimize the use of empirical antibiotics and medical expenses. Microbial culture, the current gold standard for diagnosing infections, has certain limitations in terms of duration and accuracy. There is a novel multiplex PCR (mPCR)-based diagnostic approach for infections called syndromic testing that can identify up to 20 pathogens simultaneously within 1–2 hours. This evidence-based case report aims to evaluate its sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) in diagnosing gastrointestinal infections. Literature search was conducted on February 21, 2024, across several databases which were PubMed, Scopus, Wiley Online Library, and ProQuest. Two prospective cross-sectional studies met the inclusion and exclusion criteria which were then critically appraised. Both studies measured the diagnostic accuracy of syndromic testing for Salmonella and Shigella, yielding sensitivity of 75–95.2%, specificity of >98%, PPV up to 88.2%, and NPV of >99%, indicating its good accuracy. Syndromic testing presents as a promising alternative diagnostic modality for infections, addressing the limitations of culture-based methods.
