Abstract

This study aims to prove the application of evidence-based nursing hand-held fan therapy for dyspnea in lung cancer patients. The method used was pre-experimental, with a sample size of 10 lung cancer patients. The results of the study showed that there was a significant effect of the application of hand-held fan therapy on dyspnea, respiratory rate, and SpO2 in lung cancer patients (p<0.05). In conclusion, hand-held fan therapy can reduce shortness of breath, reduce respiratory rate, and increase oxygen saturation in lung cancer patients. Abstract

Introduction: This systematic review aims to determine the effectiveness of relaxation therapy in reducing anxiety in heart disease patients and knowing what relaxation techniques are more effective than other techniques. The method used in this review is a review of various data bases including ProQuest, ScienceDirect and Pubmed published between 2013-2023 with the search keywords Relaxation Technique, Anxiety, Heart Disease. The results showed that there were four articles that showed the effectiveness of relaxation therapy in reducing anxiety in patients with heart disease. Conclusion: Relaxation therapies such as Benson's Relaxation Technique, prayer therapy, TaiChi, Meditation can be used effectively in the management of anxiety in heart disease patients. Abstract

Background: Background: Ventilator-Associated Pneumonia (VAP) is a major complication in mechanically ventilated patients, often resulting from microaspiration due to inadequate endotracheal cuff pressure. Maintaining optimal cuff pressure can be achieved through continuous or intermittent monitoring. Aim : To compare the effectiveness of continuous versus intermittent cuff pressure monitoring in preventing VAP. Methods: A systematic review was conducted across PubMed, Scopus, Taylor dan Francis, and ScienceDirect. Articles were screened based on publication year, title, abstract, and relevance. Results: Of 550 identified studies, 7 met the inclusion criteria. Four studies reported greater VAP prevention efficacy with continuous monitoring (p < 0.05, p = 0.06, p = 0.109, p = 0.02; OR 0.45, 95% CI = 0.22–0.89). Six studies found no significant impact on secondary outcomes, including ICU length of stay, ventilation duration, and mortality. Conclusion: Continuous cuff pressure monitoring ensures more stable pressure regulation, potentially reducing VAP risk. Further studies are needed to assess its impact on clinical outcomes

Abstract

Background: The coronavirus disease (COVID-19) pandemic has posed significant challenges in critical care, particularly for patients with severe acute respiratory distress syndrome (ARDS), ventilator weaning failure, and septic shock requiring tracheostomy.  Case Presentation: This report presents the clinical course and nursing management of Mrs. X, a COVID-19 patient who presented to the emergency department with severe dyspnea, hypoxia (SpO2 69%), and tachypnea (30 breaths/min).  Despite initial noninvasive ventilation (NIV), her condition deteriorated, requiring endotracheal intubation and intensive care unit (ICU) admission.  She was diagnosed with moderate ARDS (P/F ratio: 146 mmHg) and ventilator weaning failure after 15 days, leading to tracheostomy. Results: Key nursing diagnoses included impaired spontaneous ventilation and impaired gas exchange. ICU nurses implemented comprehensive tracheostomy care, adhered to ventilator-associated pneumonia (VAP) prevention bundles, and performed continuous assessments to minimize infection risks. These interventions successfully prevented common tracheostomy-related complications such as ulcers and site infections. However, the patient developed septic shock and multiple organ dysfunction syndrome (MODS), ultimately resulting in death on the 27th day of admission.  Conclusion:  This case highlights the essential role of ICU nursing in managing complex COVID-19 patients with tracheostomy.  Effective tracheostomy care, stringent infection control, and timely nursing interventions are crucial to optimizing patient outcomes.  Nevertheless, the progression to septic shock underscores the challenges of managing critically ill patients with COVID-19.Keywords: COVID-19, nursing care, critical care, ventilator weaning, tracheostomy, ARDS

Abstract

Clinical judgment is a critical and multifaceted process essential for ICU nurses, integrating technical expertise and interpersonal skills to ensure safe, effective patient care. Clinical judgment, recognized as a core competency in intensive care nursing, requires further conceptual clarity. This study aimed to define and analyze the concept of clinical judgment in ICU nurses using Walker and Avant’s eight-step concept analysis method. A comprehensive literature search was conducted across databases including CINAHL, Medline, Scopus, ProQuest, SpringerLink, ClinicalKey Nursing, EMBASE, ScienceDirect, EBSCOhost, and Google Scholar, covering publications from 1994 to 2024. The analysis identified key attributes of clinical judgment: clinical knowledge, critical thinking, practical wisdom, data prioritization, intuition, scientific analysis, evidence use, and interaction. Antecedents included education, knowledge, experience, analytical ability, and the nursing process. Consequences of clinical judgment were accurate responses, reflective practice, safe nursing care, and improved patient safety. Decision-making was identified as the primary empirical referent. This concept analysis demonstrates that clinical judgment in ICU nurses is developed through a synthesis of critical thinking, clinical expertise, and situational experience, reinforcing its significance in the delivery of high-quality critical care.

Abstract

Dyspnoea is the most common and distressing symptom of lung disease, and it causes uncomfortable feelings. Dyspnoea occurs in patients with lung diseases such as lung cancer, COPD, asthma and tuberculosis. Positioning that can reduce shortness of breath in COPD patients is the tripod position and breathing exercises, namely lip breathing exercises, which can affect oxygen saturation. The aim of study is to determine the effectiveness of the application of forward leaning position and pursed lip breathing on dypsnea in patients with lung disease. Pursed lips breathing exercises and a tripod position significantly improved symptoms of shortness of breath and strengthened respiratory muscles. To determine the effectiveness of applying the forward leaning position and pursed lip breathing for dyspnea in patients with lung disease. This research is quantitative with a quasi-experimental design with a pretest-posttest design approach without a control group design. The inclusion criteria for adult patients ≥ 18 years, patients with lung disease: COPD, Asthma, Tuberculosis and Lung Cancer and moderate to severe dyspnea at rest as measured by the Modified Borg Dyspnea Scale (MBDS ≥3). The number of samples was 30 people. The results of the analysis using the Wilcoxon Test and the results obtained were that providing a forward leaning position (FLP) and Pursed Lips Breathing (PLB) breathing exercises in patients with lung disease could significantly reduce the degree of dyspnea. The application of a combination of forward-leaning positions and pursed lips breathing exercises can have an effect on reducing the degree of dyspnea in patients.

Abstract

Background: Stroke often causes physical impairments and risk of disability, making patients highly dependent on caregivers for physical and emotional support. The complex caregiving role frequently leads to physical, psychological, and social pressures, known as caregiver burden, which negatively impacts caregiver well-being and patient care. Purpose: To identify interventions provided to stroke caregivers to reduce caregiver burden. Method: A systematic review was conducted following PRISMA guidelines on randomized controlled trials (RCTs) published in English between 2014 and 2024 with full-text availability. Pilot and feasibility studies were excluded. Literature searches were performed through Science Direct, Medline (via EBSCOhost), Sage Journals, Taylor & Francis, and Scopus. Results: Eleven studies with a total of 1,229 stroke caregivers were included. Identified interventions comprised psychoeducation, counseling, coaching, education, and training, all designed to alleviate caregiver burden. Conclusion: Family-based interventions are effective in reducing caregiver burden. Their implementation can be initiated during the acute phase in the hospital and continued through follow-up approaches such as telenursing after discharge. This review emphasizes the potential of family-based interventions as a comprehensive and sustainable strategy to support stroke caregivers.   Keywords: Caregiver Burden; Family Intervention; Stroke Survivors.   Pendahuluan: Stroke sering menimbulkan gangguan fisik dan risiko kecacatan sehingga pasien mengalami sangat bergantung pada caregiver dalam pemenuhan kebutuhan fisik dan emosional. Peran yang kompleks kerap menimbulkan tekanan fisik, psikologis, dan sosial, yang dikenal sebagai caregiver burden. Hal ini akan berdampak pada kesejahteraan caregiver dan kualitas perawatan pasien. Tujuan: Untuk mengidentifikasi intervensi keluarga untuk menurunkan caregiver burden pada keluarga yang merawat penyintas stroke. Metode: Tinjauan literatur sistematis dilakukan sesuai panduan PRISMA terhadap artikel Randomized Controlled Trial (RCT) berbahasa Inggris, terbit antara tahun 2014-2024, dan tersedia teks lengkap. Artikel pilot atau feasibility study dikecualikan. Penelusuran menggunakan database Science Direct, Medline (via EBSCOhost), Sage Journals, Taylor & Francis, dan Scopus. Hasil: Sebanyak 11 penelitian dengan 1,229 partisipan (caregiver pasien stroke) diikutkan. Berbagai intervensi keluarga teridentifikasi, antara lain psikoedukasi, konseling, coaching, edukasi, dan pelatihan, yang bertujuan menurunkan beban caregiver. Simpulan: Intervensi keluarga terbukti efektif menurunkan beban caregiver. Pelaksanaannya dapat dimulai sejak fase akut di rumah sakit dan dilanjutkan dengan follow-up seperti telenursing setelah pasien pulang. Tinjauan ini menegaskan bahwa pendekatan intervensi keluarga berpotensi menjadi strategi komprehensif dalam mendukung caregiver penyintas stroke secara berkelanjutan.   Kata Kunci: Caregiver Burden; Intervensi Keluarga; Penyintas Stroke.
