Abstract

Selama pasien dirawat di ruang Intensive Cardiac Care Unit (ICCU), keluarga pasien mengalami kesulitan dan ketidakberdayaan. Bagi keluarga pasien, ruang rawat intensif merupakan tempat yang tidak menyenangkan.  Keluarga masuk dalam kondisi yang tidak terduga dan dibutuhkan ketepatan keluarga dalam mengambil keputusan guna keberlangsungan hidup terkait kondisi pasien yang membutuhkan penanganan.Lama perawatan pasien di ruang ICCU sangat beragam.  Lama rawat pasien ICCU berdampak langsung terhadap kualitas hidup pasien dan keluarga, risiko terjadinya di masa depan, dan besarnya pembiayaan dampak dari perawatan. Selama keluarga mendampingi perawatan pasien di ruang ICCU, keluarga akan mengalami berbagai reaksi emosional seperti kecemasan. Tujuan: Mengetahui hubungan lama hari rawat dengan tingkat kecemasan keluarga pasien di ruang ICCU. Metode: Korelasi dengan pendekatan cross sectional dengan sampel 42 responden di ruang ICCU, yang dipilih dengan menggunakan metode purposive sampling. Instrumen penelitian ini menggunakan kuesioner HARS.  Data dianalisis dengan uji korelasi Spearman. Hasil: Lama hari rawat pasien rata-rata 3,14 hari.  Kecemasan keluarga pasien ICCU paling banyak berada pada tingkat kecemasan berat (33,3%). Terdapat hubungan yang bermakna antara lama hari rawat dengan tingkat kecemasan keluarga pasien (p=0,0003) dengan keeratan hubungan yang sedang (r=0,532) di ruang ICCU. Diskusi: Semakin lama pasien dirawat di ruang ICCU maka semakin tinggi tingkat kecemasan yang dirasakan keluarga pasien karena kondisi pasien yang tidak stabil ataupun semakin parah. Kesimpulan: Keluarga pasien perlu diberikan informasi dan edukasi yang jelas terkait kondisi pasien dalam sehari atau saat ada perburukan kondisi.Kata kunci: kecemasan, keluarga, lama hari rawat, perawatan intensif, perawatan kritis.Correlation Between Length of Stay and Anxiety Levels of Patients’ Family in Intensive Cardiac Care Unit RoomABSTRACTWhile a patient is being treated in the Intensive Cardiac Care Unit (ICCU), the patient's family experiences difficulties and helplessness. For the patient's family, the intensive care unit is an unpleasant place. The family enters into unexpected conditions and is required to make proper decisions for survival related to patient conditions. The patient's length of stay in the ICCU room is very diverse. The length of stay of ICCU patients directly impacts patients' quality of life and their families, the risk of future incidence, and the extent of the treatment cost. While the family accompanies the patient treated in the ICCU room, the family will experience various emotional reactions such as anxiety. Objective: To reveal the correlation between length of stay and the anxiety levels of the patient’s family in the ICCU room. Methods: Correlation with cross-sectional approach with a sample of 42 respondents in the ICCU room, who were selected using a purposive sampling method. The research instrument used the HARS questionnaire. Data were analyzed using the Spearman correlation test. Results: The average length of the patient's stay was 3.14 days. Family anxiety of ICCU patients was mostly at the level of severe anxiety (33.3%). There was a significant correlation between length of stay and anxiety levels of the patient’s family (p=0.0003) with a moderate correlation coefficient (r=0.532) in the ICCU room. Discussion: The longer the patient is treated in the ICCU room, the higher the anxiety level felt by the patient’s family due to the patient's unstable or worsened condition. Conclusion: Clear information and education should be given to the patient's family regarding the patient's condition within one day or when the condition worsens.Keywords: anxiety, family, length of stay, intensive care, critical care

Abstract

Filtration is a method for controlling water pollution. This study aimed to engineer a filtration system using a combination of Melaleuca cajuputi (galam) wood charcoal and zeolite to process domestic wastewater. The research focused on domestic wastewater from Sungai Pinang Village, Samarinda, East Kalimantan, an area with significant water pollution issues. The samples were divided into three groups, each containing zeolite and galam wood charcoal in different ratios: Group 1 (1:1), Group 2 (1:2), and Group 3 (2:1). Each sample was exposed to contact times of 30, 60, and 90 minutes. Biological Oxygen Demand (BOD), Chemical Oxygen Demand (COD), Ammonia (NH3), and pH were measured for each treatment group at each contact time. Results showed that prolonged contact with the filtration method significantly reduced all parameters except pH (P<0.05). Each treatment combination did not significantly reduce all parameters except pH (P>0.05). Group 2 was found to be the most effective in improving water quality. In conclusion, this study demonstrates that the filtration method can reduce pollutant levels, with the combination of galam wood charcoal and zeolite in a 2:1 ratio being the most effective.

Abstract

Aims: Families of patients in intensive care units (ICUs) face significant psychological and emotional challenges, with diverse needs spanning assurance, information, proximity, support, and comfort. Understanding and addressing these needs is crucial for enhancing family well-being and satisfaction with ICU care. Objective: This study aims to synthesize existing evidence on the needs of ICU patients’ families, identify priority domains, and explore factors influencing these needs. Methods: A systematic literature review was conducted following PRISMA guidelines. Searches across PubMed, Scopus, CINAHL, Web of Science, and PsycINFO identified studies published between January 2015 and December 2024. Inclusion criteria focused on peer-reviewed articles addressing the emotional, informational, or practical needs of ICU patients’ families. Data extraction and quality assessment were performed using the Joanna Briggs Institute Critical Appraisal Tools. Thematic synthesis was applied for qualitative data, while quantitative findings were analyzed descriptively. Results: From 11 studies, assurance emerged as the most critical need across diverse populations, followed by information, proximity, support, and comfort. Sociodemographic factors such as age, education level, and gender, as well as contextual factors like cultural norms and length of ICU stay, significantly influenced family needs. Assurance and information were universally prioritized, while proximity and support varied based on regional and cultural contexts. Conclusion: ICU families’ needs are multidimensional and influenced by individual, clinical, and sociocultural factors. Assurance and information consistently rank as top priorities, highlighting the need for family-centered, culturally sensitive care strategies. Tailored interventions, effective communication, and policies facilitating family presence can address these needs and improve family satisfaction. Future research should explore real-time strategies and the role of technology in meeting family needs.

Abstract

Tingginya angka kematian di Ruang ICU menuntut tenaga kesehatan untuk mampu memberikan perawatan paliatif menjelang kematian agar mengantarkan pasien meninggal dalam kedamaian. Dalam prakteknya karena keterbatasan tenaga perawat ICU, beberapa tindakan perawatan menjelang ajal dibantu oleh keluarga pasien terutama pada aspek dukungan emosional, budaya, dan spiritual. Tujuan penelitian ini untuk mengeksplorasi perawatan menjelang ajal oleh keluarga menurut perspektif Budaya Banjar di Ruang ICU RSUD Ulin Banjarmasin. Penelitian ini kualitatif dengan pendekatan etnografi. Key informant adalah keluarga pasien ICU yang memenuhi kriteria inklusi sebanyak 11 orang. Teknik pengumpulan data adalah observasi, wawancara mendalam, dan Focus Group Discussion. Muncul empat tema yaitu jenis perawatan menjelang ajal meliputi melaksanakan ibadah di dekat pasien, melakukan perawatan kebersihan diri pasien, bertawakkal hasil akhir kepada Tuhan, pemberian makanan atau minuman yang dipercaya untuk kesembuhan pasien. Cara keluarga mendapatkan informasi mengenai perawatan menjelang ajal meliputi pendidikan dari keluarga sejak kecil, informasi dari tenaga kesehatan. Orang yang terlibat dalam perawatan menjelang ajal yaitu anggota keluarga, perawat, dokter. Keluarga pasien melakukan perannya dalam perawatan menjelang ajal dengan memasukkan unsur budaya pada pasien kritis di Ruang ICU RSUD Ulin Banjarmasin berkolaborasi dengan tenaga kesehatan. Tenaga kesehatan dapat memfasilitasi keluarga melakukan perannya dalam perawatan menjelang ajal.

Abstract

Sleep disturbance in critical patients maybe related with the disease suffered, environmental disturbance,interruption of treatment procedures, anxiety disorder and medication factors. Identification and treatment ofpatient sleep disorders is an important goal for nurses. Nurses must understand the nature of sleep, factors thatinfluence sleep and sleep habits of patients to get the need for sleep and rest. Know the factors related to thequality of sleep of patients in the ICCU Ulin Banjarmasin Hospital. Observational analytic with cross sectionaldesign and using purposive sampling technique with a sample of 33 respondents at ICCU Ulin BanjarmasinHospital. The instrument of this research was in the form of RCSQ questionnaire, STAI, symptoms of physicalillness, environmental disorders, disruption of treatment procedures and medical records. Based on theanalysis using the Fisher Exact test, there is a relationship between sleep quality and symptoms of physicalillness (p value=0,016), there is a relationship between sleep quality and environmental disorders (pvalue=0,004), there is a relationship between sleep quality and disruption of treatment procedures (pvalue=0,014), anxiety disorder (p value=0,002), there is a relationship between sleep quality and medicationfactors (p value=0,006). Sleep quality is a condition in which the sleep experienced by an individual producesfreshness when he wakes up from sleep. Awakening from sleep, especially at night in intensive care patients caninterfere with sleep quality and usually caused by several factors, namely symptoms of physical illness,environmental disturbance, interruption of treatment procedures, anxiety disorder and medication factors.

Abstract

The nursing process including nursing assessment has constraints in it’s application includingwithin the complex scope of IGD with high workload. The nursing philosophy views theindividual in a holistic way as a whole, which had biological, psychological, social, cultural andspiritual dimensions. Objective To determine the description of holistic implementation nursingassessment by nurses at IGD RSUD Ulin Banjarmasin. This study used Descriptive research on34 nurses in IGD room of Ulin Banjarmasin General Hospital taken in total sampling. The studywas conducted on 20-27 November 2017. Result of research got description of nursing holisticstudy showed result of biological aspect of applikative category 76%, psychological aspect ofapplikative category 91%, social aspect of applikative category 76%, spiritual aspect categorynot applicative 59% and culture aspect applicative and non applicative category with percentageof 50%. Nurses as health professionals provide health services, especially comprehensive nursingcare. It was suggested that nurses need to improve their ability to support the implementation ofholistic nursing assessment especially on the spiritual aspect.

Abstract

Family experience when making decisions in critical patients is not an easy thing. Foundmany problems experienced by the family when making decisions such as physiological,psychological, spiritual, social, and cultural issues. Those problems must be overcomeand the resulting decision is the best decision for the patient and the patient's family. Theaim of this research was to explore about family’s experiences in making decision incritical patients. The research design used qualitative research with descriptivephenomenology approach. This research have been done in ICU ofUlin Hospital on July20 to August 6, 2017. The number of informants had reached saturation that was 7informants family of critical patients in ICU. The techniques of data collection in thisstudy has been used in-depth interviews. Data analysis used thematic analysis of Braunand Clarke. The Result found that 6 themes are discussions with family-related to patientcare, information about the action to be taken, patient's cure a top priority, length ofpatient care, patient care funding, and family involvement in patient care. The Advice fornurses and health personnel are to provide all needs of services that must be given to thefamily before making decision.

Abstract

Pain assessment of unconsciousness patiens who unable to show their pain respon verbally is an important thing to learn. CPOT is an instrument for scoring the pain of patients who couldn’t doing verbal communication, and Wong Bekker is an easier and faster instrument for pain scale prediction. A sistematic pain assessment on critical unconsciousness patients will reduce long of stay and Healthcare-Associated Infections number. Objective for this research were knowing the effectivity of pain assessment used CPOT and Wong Bekker on critical unconsciousness patients in ICU of Ratu Zalecha Hospital Martapura. Analytic observational experimental by cross sectional on 31 responden in ICU of Ratu Zalecha Martapura, used purposive sampling. This experiment in September up October 2017. Based on the experiment that the CPOT was more effective (17.48) than Wong Bekker (12,54). P Value < 0,05 by mann whitney test. Pain assessment used CPOT was better because pain evaluating based on comprehensive behave signs and indicators. Hospitals may doing pain assessmenet used CPOT.

Abstract

This incidence rate of STEMI patients followed by risk factors that affect prognosis of STEMIpatients, there’s a need to acknowledge what risk factors that role in STEMI as prevention todecrease mortality incidence. The objectives was to describe risk factors of mortality incidence inSTEMI patients in RSUD Ulin Banjarmasin. This study used cross sectional with accidentalsampling method that showed through univariate analysis. The data taken from ECG sheet,patient and family interviewed, nurse and doctor declaration written on medical record thatbeing record directly by researcher on observational sheet in RSUD Ulin Banjarmasin sinceNovember - December 2015. The results17 patients having STEMI with mortality rate (11,8%)average age STEMI patients is 57 years old, dominated by males (88,2%) with risk factors arehistory of prior heart (35,5%), hypertension (29,4%), smoking (29,4%), stroke (5,9%), nomedical history (17,6%). The result of this study was avarege age of STEMI patients in RSUDUlin Banjarmasin was 57 years old and the most common in males with risk factors were historyof prior heart, hypertension, smoking, stroke and no medical history.
