Abstract

Background Chest physiotherapy and suctioning are routine methods for airway clearance in mechanically-ventilated children. However, chest physiotherapy has not been confirmed to affect ventilation parameters, such as tidal volume (TV), peak inflation pressure (PIP), peak inspiratory flow (PIF), and peak expiratory flow (PEF), as well as blood gas analysis (BGA) values in pediatric intensive care unit (PICU) patients.Objective To determine the efficacy of chest physiotherapy and suctioning vs. suctioning alone for improving the mechanical ventilation parameters and BGA.Methods This randomized, single-blind, clinical trial was conducted from November 2012 to June 2013 in the PICU at Haji Adam Malik Hospital, Medan. A total of 40 mechanically-ventilated pediatric patients were enrolled and divided into either the chest physiotherapy and suctioning group (24 subjects) or the suctioning alone group (16 subjects). Subjects underwent treatment, followed by monitoring of their ventilation parameters and blood gas analyses. Data were analyzed by independent t-test and Mann-Whitney test.Results Subjects comprised of 23 boys and 17 girls, with an age range of 1–204 months. After the respective treatments (chest physiotherapy and suctioning vs. suctioning alone), the ventilation parameters were as follows: median TV (60.0 vs. 56.5 mL, respectively; P=0.838), median PEF (10.4 vs. 10.8 I/s, respectively; P=0.838), median PIF (7.4 vs. 8.2 I/s, respectively; P=0.469), and mean PIP (17.3 vs. 15.6 cmH2O, respectively; P=0.23). The BGA values were: median pH (7.4 vs. 7.3, respectively; P=0.838), median pCO2 (38.4 vs. 36.2 mmHg, respectively; P=1.000), mean pO2 (136.6 vs. 139.2 mmHg, respectively; P=0.834), median HCO3 (20.4 vs. 22.7 mmol/L, respectively; P=0.594), median TCO2 (22.0 vs. 23.7 mmol/L, respectively; P=0.672), mean BE (-4.3 vs. -3.1 mmol/L, respectively; P=0.629), and median O2 saturation  (98.5 vs. 98.3 %, respectively; P=0.967).Conclusion In mechanically-ventilated children in the PICU, ventilation parameters and BGA values are not significantly different between subjects who received both chest physiotherapy and suctioning and those who received suctioning alone.

Abstract

Background The prevalence of short stature in thalassemia patients ranges from 39.3 to 65%. The cause of short stature is complex and still up for debate. In Indonesia, data on the prevalence and risk factors of short stature in adolescents with thalassemia have been limited. Objective To assess for the prevalence and risk factors of short stature in adolescents with beta-thalassemia major. Methods This cross-sectional study was done from February to March 2017 at the Thalassemia Clinic at Dr. Hasan Sadikin General Hospital, Bandung. The baseline characteristics data of 80 adolescents with thalassemia aged 10-14 years were recorded. Short stature was assessed by height-for-age, (Z-score <-2SD) based on the 2007 WHO Reference Growth Chart. Mid-upper arm circumference was scored according to age and sex and serum IGF-1 was measured by ELISA method. Data analyses used were Chi-square, Fisher’s, and Mann-Whitney tests. Logistic regression model was used to further analyze for risk factors of short stature. Results Subjects were 40 males and 40 females, 81.2% of whom had short stature. The mean serum IGF-1 level was 32.2 (SD 26.38) ng/mL. The IGF-1 cut-off point by ROC curve was ≤38.51 ng/mL, with sensitivity of 64.4% and specificity of 86.7%. The risk factors of short stature were IGF-1 level ≤38.51 ng/mL (PR 40.66; 95%CI 4.37 to 377.58; P<0.001) and low family income (PR 19.76; 95%CI: 1.152 to 256.08; P=0.022). Conclusion IGF-1 level may be useful as a predictor of short stature in adolescent beta-thalassemia major patients.

Abstract

Mechanical ventilation is a life-saving technology for patients who develop acute lung injury (ALI) and investigators have conducted numerous investigations to determine the optimal support method and has been used for several decades. The incidence of ALI is quoted to be between 17 and 34 per 100,000 patients for year. Mechanical ventilation is indicated in patients with severe hypoxemia despite oxygen therapy, severe hypercapnia, or excessive work of breathing. Fluid management with the goal to obtain zero fluid balance in ARDS patients without shock or renal failure significantly increases the number of days without mechanical ventilation. Liberal and conservative fluid strategies are therefore complementary and should ideally follow each other in time in the same patient whose hemodynamic state progressively stabilizes.Key words : constipation; functional; children; medical therapy

Abstract

Based on industrial processes that still use semi-automatic processes, this often causes health problems, such as musculoskeletal problems or disorders in workers due to semi-automatic production processes and heavy loads. The aim is to find out the relationship between workload and musculoskeletal complaints in Opak Village factory workers. Sukaraya Bakti, District. Pancur Batu, Kab. Deli Serdang. The research method carried out here is quantitative descriptive research. Where this research tries to explore how and why this health phenomenon occurs. This descriptive research was carried out using observation and interview methods. Observations and interviews were carried out on several behavioral factors, which were then linked to musculoskeletal complaints in opak makers. It can be concluded that there is a significant relationship between workload and complaints of musculoskeletal disorders among Opak Factory Workers in Sukaraya Bakti Village, Pancur Batu District, Deli Serdang Regency.

Abstract

The pervasive use of screen media among high school students has raised concerns about its impact on academic performance. While digital devices offer educational benefits, excessive use for entertainment may negatively affect learning outcomes. This study aimed to analyze the effect of screen media use on school performance among students at SMA Negeri 4 Binjai City. An observational analytical study with a cross-sectional design was conducted in July 2025 involving 240 students selected through proportional stratified random sampling. Data on screen media types, usage duration, and activities (learning, movie watching, gaming, social media) were collected using validated questionnaires, while academic achievement was measured using school report cards. The chi-square test was used for data analysis with a significance level of p < 0.05. The majority of respondents (61.3%) used screen media for extended durations (>2 hours daily). Smartphones were the most prevalent device (82.5%). Significant correlations were found between screen media usage and academic achievement, particularly for duration (p = 0.000), movie watching (p = 0.000), gaming (p = 0.000), and social media use (p = 0.000). Learning activities through screen media showed a positive correlation with better academic performance (p = 0.001). Screen media usage patterns significantly affect students' academic achievement. High-duration usage and entertainment-oriented activities are associated with lower school performance, while educational use correlates with better outcomes. These findings highlight the need for balanced screen time management and guided media usage in educational settings.

Abstract

Perkembangan teknologi digital menyebabkan peningkatan signifikan penggunaan media layar di kalangan remaja. Penggunaan gawai seperti smartphone, laptop, dan televisi dalam waktu lama, terutama pada malam hari, diduga mengganggu ritme sirkadian dan menurunkan kualitas tidur.. Penelitian ini bertujuan untuk mengetahui hubungan antara tingkat pemakaian media layar dengan pola tidur pada siswa dan siswi Madrasah Aliyah TPI Sawit Seberang Tahun 2025. Desain penelitian deskriptif analitik dengan pendekatan cross-sectional. Populasi seluruh siswa berjumlah 177 orang, sampel 123 responden dipilih dengan proportional random sampling. Instrumen menggunakan kuesioner pemakaian media layar dan Pittsburgh Sleep Quality Index (PSQI). Analisis data menggunakan uji Chi-Square (?=0,05) . Hasil penelitian menunjukkan bahwa sebagian besar responden memiliki pola tidur tidak baik (52,8%), dengan rata-rata durasi tidur 6,2 jam per malam. Sebagian besar siswa menggunakan media layar dengan intensitas sedang hingga tinggi (81,3%), terutama menggunakan smartphone pada malam hari. Hasil uji Chi-Square menunjukkan nilai p = 0,001, dan yang berarti terdapat hubungan positif yang sangat kuat dan signifikan antara pemakaian media layar dengan pola tidur siswa. Semakin tinggi intensitas penggunaan media layar, semakin buruk kualitas tidur yang dialami. Terdapat hubungan yang signifikan dan sangat kuat antara pemakaian media layar dengan pola tidur pada siswa Madrasah Aliyah TPI Sawit Seberang. Penggunaan media layar berlebihan, terutama pada malam hari, berkontribusi terhadap menurunnya kualitas dan durasi tidur siswa. Diperlukan edukasi mengenai sleep hygiene dan pembatasan waktu penggunaan media layar sebelum tidur untuk meningkatkan kualitas istirahat remaja.
