Abstract

Sepsis berat dan syok sepsis memiliki angka kematian tinggi. Resuscitation bundle 6 jam adalah panduan tata laksana sepsis berat dan syok sepsis yang terbukti menurunkan mortalitas. Kesesuaian penatalaksanaan sepsis berat dan syok sepsis dalam 6 jam pertama dengan prosedur standar resuscitation bundle diteliti. Penelitian observasional deskriptif ini dilaksanakan selama September?Desember 2015 terhadap penderita sepsis berat dan syok sepsis di SMF Ilmu Penyakit Dalam RSUP Dr. Hasan Sadikin Bandung. Data dikumpulkan adalah data umum penderita dan pelaksanaan prosedur 6 jam pertama sesuai standar: pemeriksaan laktat, kultur darah, pemberian antibiotik, resusitasi cairan, pemberian vasopresor, pengukuran CVP, dan pemeriksaan Scvo2/Svo2. Dari 80 subjek penelitian, 59 (74%) sepsis berat dan 21 (26%) syok sepsis. Pada sepsis berat dan syok sepsis, dalam 3 jam pertama pemeriksaan laktat dilakukan pada 2%  dan 14%, kultur darah sebelum antibiotik 17% dan 10%, pemberian antibiotik spektrum luas 32% dan 43%, serta  resusitasi cairan 30 mL/kgBB 2% dan 14%. Pada syok sepsis dalam 6 jam pertama pemberian vasopresor 62% dan pengukuran CVP 5%. Pemeriksaan Scvo2/Svo2 tidak dilakukan pada syok sepsis. Angka kematian sepsis berat 7% dan syok sepsis 19,1% dalam 6 jam pertama. Simpulan, kesesuaian penatalaksanaan 6 jam pertama penderita sepsis berat dan syok sepsis dengan prosedur standar masih kurang.Kata kunci: Resuscitation bundle 6 jam, sepsis berat, syok sepsis  Compliance to 6-Hour Resuscitation Bundle Standard in Severe Sepsis and Septic Shock ManagementSevere sepsis and septic shock have a high mortality rate. Six-hour resuscitation bundle is a management standard for severe sepsis and septic shock that has been proven to reduce mortality rate. Compliance to the 6-hour resuscitation bundle standard was studied. This was a descriptive observational study on severe sepsis and septic shock patients in the Internal Medicine Department of Hasan Sadikin General Hospital Bandung in the period of September?December 2015. Data collected were patient?s general data and the implementation of the 6-hour management standard procedures that include lactate measurement, blood culture, broad spectrum antibiotics, fluid resuscitation, vasopressor use, CVP measurement, and Scvo2/Svo2 measurement. Of 80 subjects, 59 (74%) had severe sepsis and 21 (26%) had septic shock. In the first 3 hours, for severe sepsis and septic shock patients, lactate was examined in 2% and 14% respectively. For blood culture before antibiotics, broad spectrum antibiotic provision, and 30 mL/kgWt fluid resuscitation, the values were 17% and 10%, 32% and 43%; and 2% and 14%, respectively. In septic shock, in the 6 hours, vasopressor was given to 62% and CVP was measured in CVP 5%. Scvo2/Svo2 was not measured. The mortality rates in the first 6 hours for severe sepsis and septic shock were 7% and 19%. Compliance to first 6-hour standard procedure for severe sepsis and septic shock is still inadequate. Key words: Septic shock, severe sepsis, six hours resuscitation bundle

Abstract

Malaria remains a significant public health issue in Indonesia, characterized by high rates of illness and death. Proper laboratory diagnosis is one of the keys to malaria prevention efforts. Malaria microscopists' proficiency, knowledge, attitudes, and skills require regular assessment to maintain diagnostic quality. This study examines the correlation between malaria microscopists' knowledge, attitudes, and competencies and the accuracy of laboratory diagnostic outcomes at the Jayapura City Health Center. This study used quantitative methods with descriptive and verification approaches. Data were collected through questionnaires and laboratory diagnosis results. The study sample was 13 malaria microscopists at the Jayapura City Health Center. The data were analyzed with SPSS software. The results showed that most respondents had a good level of knowledge (76.9%). Almost half of the respondents had a good attitude (46.2%). Most respondents had a sufficient level of skill (61.5%). Interestingly, although most respondents (76.9%) could diagnose malaria very accurately, this study did not find an effect of malaria microscopists' knowledge, attitude, and skills on the accuracy of diagnosis. The knowledge level of malaria microscopists at the Jayapura City Health Center is good, but their attitudes and skills still need to be improved. It is recommended that microscope Officers routinely attend training to improve their knowledge, attitudes and skills.

Abstract

The background of this research is that the performance of Paspampres soldiers has yet to be discovered, and the factors that affect the performance of Paspampres soldiers still need to be discovered. Basedoon the background,,thissstudy attempts to examine the impacttof Occupational Safety and Health and OrganizationallCulture partially and directly on Soldier Performance and indirect influence through the intervening variable of Organizational Commitment. A descriptive verificative explanatory design wasuused in this research. In theqquantitative phase, The instrument offresearch wasuused to perform a survey on 144 Soldier which was then descriptively examine. . The data analysis technique uses SEM-PLS to test the outer model, inner model, and hypothesis test. The result show that the Paspampres Soldier Organization's Commitment and OccupationallSafety anddHealth have a positive and significant relationship..Occupational Safety and Health also positively and significantly affects Paspampres Soldiers' Performance, while Organizational Commitment has no significant affect and has a negative impact on Paspampres Soldiers' Performance. Moreover, Occupational Safety and Health and OrganizationalcCommitment both positivelyyand significantly affect Paspampres Soldiers' Performance at the same time. Research uses organizational commitment as a variable on soldier performance and does not have a significant effect in this research. Several previous research have examined the effect of organizational commitment variables onpperformance, showing significant differences in the research results that support the theory and suggest for more research.

Abstract

Penelitian ini mempunyai tujuan untuk menganalisis pengaruh kedisiplinan dan kompetensi pegawai terhadap kualitas layanan klinik yang berimplikasi terhadap kepuasan pasien di klinik Ibnu sina Rapak Balikpapan. Sumber data pada penelitian ini didapat dari kuesioner (primer) yang disebar kepada responden. Populasi dari penelitian ini adalah semua pasien yang berobat di klinik Ibnu sina Balikpapan. Sampel penelitian berjumlah 100 orang. Metode analisis penelitian yang digunakan adalah uji validitas, uji reliabilitas, path analysis, uji t. Temuan penelitian menunjukkan bahwa Kedisiplinan dan kompetensi pegawai sangat mempengaruhi kualitas layanan. Faktor yang paling dominan berpengaruh adalah kompetensi pegawai dari penghitungan uji t. Berdasarkan Hasil Penelitian ini menunjukkan bahwa pengaruh kedisiplinan dan kompetensi pegawai terhadap kualitas layanan klinik berpengaruh positif terhadap kepuasan pasien di Klinik Ibnu Sina Rapak Balikpapan.

Abstract

Sistem Informasi Manajemen Rumah Sakit (SIMRS) Sistem yang sangat penting guna mewujudkan sistem informasi yang dikelola oleh rumah sakit. Manajemen data manual juga memiliki banyak kekurangan. Penerapan Simrs di Rumah Sakit Hamori di Semester 1 tahun 2024 telah menghasilkan banyak hambatan dan keluhan bahwa pengguna merasa bahwa sistem waktu henti terjadi selama jam praktik dokter. Tujuan dari penelitian ini adalah untuk menentukan kualitas kualitas, perawat dan kinerja bidan, dan untuk mengetahui SIMRS tentang kualitas layanan, perawat dan kualitas layanan bidan, simrs mengenai perawat dan kinerja bidan, kinerja perawat, dan langkah -langkah berdasarkan kualitas layanan. Metode penelitian ini menggunakan pendekatan kuantitatif total populasi 136 sampel 57 dan pemrosesan data menggunakan aplikasi SPSS. Hasil: penelitian hipotesis diterima dengan hasil signifikasi 0,000<0,05, serta variabel intervening yaitu kualitas pelayanan mampu sebagai mediator. Kesimpulan: SIMRS berpengaruh terhadap kinerja perawat dan bidan, dengan adanya SIMRS membantu perawat dan bidan dalam melayani pasien sehingga berpengaruh terhadap kualitas pelayanan di Rumah Sakit Hamori. Saran: Pimpinan rumah sakit perlu melakukan evaluasi terhadap penerapan SIMRS, memberikan pelatihan penggunaan SIMRS sehingga kualitas pelayanan semakin meningkat di Rumah Sakit Hamori.

Abstract

Patient satisfaction is not merely the ultimate goal of quality service, but also a bridge to patient loyalty. Patient loyalty is crucial for the long-term operational sustainability and growth of a hospital. This study aims to determine the influence of pharmaceutical service quality and hospital image on patient loyalty, mediated by patient satisfaction. The research was conducted at RSUD Ratu Aji Putri Botung, with a sample of 98 respondents selected using a purposive sampling technique. Data were collected through a survey utilizing a 1-5 Likert scale instrument. The statistical analysis employed descriptive statistics and hypothesis testing using Structural Equation Modeling - Partial Least Square (SEM PLS). The results of the SEM PLS analysis indicate that the model's goodness of fit is acceptable, as shown by an R-squared value above 50% (categorized as a medium to high influence), a high Q-squared predict value (>50%), a GoF index of 0.664 > 0.36 (categorized as high GoF), and PLS Predict and CVPAT indicating high predictive power. The outer model evaluation confirmed acceptable levels of validity and reliability. The study's findings reveal that pharmaceutical service quality and hospital image positively impact patient satisfaction, and subsequently, patient loyalty.

Abstract

Balanced scorecards are often used in various organizations, including hospitals, because they are able to measure performance in a comprehensive, consistent, measurable and balanced manner. Until now, performance measurement in the outpatient clinic of Andi Djemma Regional Hospital still focuses on financial aspects only. This research aims to analyze polyclinic performance using the balanced scorecard method through four main perspectives: finance, customers, internal business processes, and growth and learning. The research method applied is a quantitative descriptive approach. Performance data was collected from hospital secondary data for 2023 and previously, as well as primary data through questionnaires involving patients and employees. The results of the analysis show that performance from a financial and customer perspective is classified as "fair," the internal business process perspective is rated as "good," while the growth and learning perspective is also in the "fair" category. Overall, the performance of the outpatient clinic at RSUD Andi Djemma Masamba is in the "fair" category, with a score of 0.57 which indicates a medium level of performance.

Abstract

Maintaining toddlers’ health from an early age is very important for their growth and development. Achieving good health in toddlers requires support from mothers, who play a crucial role in children’s growth and development; therefore, the author is very interested in conducting this study. The purpose of this research is to determine the influence of mothers’ level of knowledge, education, and behavior on the health status of toddlers at the Rantau Panjang Community Health Center in Jambi. This is a quantitative study with a cross-sectional design. The study population comprised 322 mothers who had toddlers and visited the Rantau Panjang Community Health Center, Jambi, from January 2024 to August 2024. Sampling used the Slovin formula, yielding a sample of 80 respondents. This study employed univariate, bivariate, and multivariate analyses. The results showed that most mothers had a good level of knowledge, namely 60 (75%); the majority had higher education, 43 (53.8%); and most exhibited good behavior, 60 (75%). There was an effect of mothers’ level of knowledge on toddlers’ health status at the Rantau Panjang Community Health Center, Jambi, amounting to 44.8%. There was an effect of mothers’ education on toddlers’ health status amounting to 10.7%. There was an effect of mothers’ behavior on toddlers’ health status amounting to 65.7%. Simultaneously, there was an effect of mothers’ knowledge level, education, and behavior on toddlers’ health status at the Rantau Panjang Community Health Center, Jambi, amounting to 66.3%.
