Abstract

Medical practice in its implementation is expected to contain high ethics and morals for doctors who are at the core of the implementation. The current phenomenon is that many doctors commit deviations and abuse their authority which violates the Indonesian Medical Code of Ethics (KODEKI), which KODEKi should serve as the basis for practice in the world of medicine. The object of research in this research is general practitioners who work at private clinics in Bandung Regency. The method used is qualitative with a descriptive phenomenological study approach. Related to the application of KODEKI values ​​carried out by general practitioners at Regency Private Clinics, the results obtained are 1) the character of godliness or responsibility, namely doctors always adhere to God and make religion the basis for carrying out the responsibilities of the doctor's profession, 2) alturism, namely doctors always prioritizing the interests of patients above personal interests, 3) professional idealism, namely doctors reprimanding colleagues if they make medical mistakes while maintaining the good name of colleagues, 4) patient accountability, namely doctors respecting and listening to patients and not discriminating against patients, 5) integrity scientific, namely doctors always follow developments in medical science through seminars, workshops and further studies and 6) social integrity, namely doctors provide clear information and provide education to improve health. All general practitioner respondents who work in private clinics in Bandung Regency apply KODEKI values.

Abstract

Abstrak: Kegiatan pengabdian kepada masyarakat pelatihan bantuan hidup dasar (BHD) dan pemeriksaan tanda vital bertujuan untuk meningkatkan pengetahuan dan keterampilan warga kampus Universitas Jenderal Achmad Yani (Unjani) tentang BHD dan pemeriksaan tanda vital. Metode yang digunakan dalam program ini adalah ceramah, praktik/implementasi, bermain peran, simulasi, pendampingan, focus group discussion (FGD) dan evaluasi dengan dukungan teknologi digital dalam pembuatan materi pembelajaran. Pelatihan dihadiri oleh 67 peserta dari berbagai unit kerja di lingkungan kampus Unjani. Hasil evaluasi peserta menunjukkan peserta juga pada umumnya sangat setuju (69%) bahwa pelatihan BHD ini bermanfaat bagi peserta sendiri maupun bagi instansi. Demikian juga dengan peran pelatihan BHD ini dalam meningkatkan kepercayaan diri peserta dalam pemberian BHD, 63% peserta pelatihan menyatakan sangat setuju. Tetapi hanya 50% peserta yang sangat setuju untuk kemungkinan penerapan materi pelatihan dalam praktik, terdapat 13% peserta yang masih ragu-ragu dalam mempraktikkan hasil pelatihan. Hasil evaluasi terhadap materi pelatihan menunjukkan 56% peserta sangat setuju bahwa materi pelatihan yang diberikan sesuai dengan harapan. Pelatihan sudah cukup efektif untuk meningkatkan pengetahuan dan keterampilan peserta tentang BHD dan tanda vital, namun perlu dilakukan secara berkala. Kata kunci: bantuan hidup dasar, pelatihan, tanda vital.

Abstract

Diabetes melitus (DM) merupakan gangguan metabolik kronis yang ditandai oleh hiperglikemia, berisiko menyebabkan komplikasi seperti kerusakan ginjal. Salah satu alternatif terapi yang potensial untuk mengurangi dampak komplikasi ini adalah penggunaan suplemen probiotik. Penelitian ini bertujuan untuk mengetahui efek suplementasi soyghurt beku-kering dan metformin terhadap gambaran histopatologi ginjal tikus Wistar hiperglikemik. Penelitian dilakukan dengan desain eksperimental posttest only group design, menggunakan bahan biologik tersimpan (BBT) ginjal tikus Wistar yang telah diinduksi aloksan dan disimpan dalam formalin selama satu tahun. Subjek penelitian terdiri atas 15 ekor tikus yang dibagi menjadi lima kelompok: kontrol negatif (tanpa perlakuan), kontrol positif (induksi aloksan), perlakuan metformin, perlakuan soyghurt beku-kering, serta kombinasi metformin dan soyghurt beku-kering. Preparat histopatologi ginjal dianalisis menggunakan pewarnaan Hematoksilin-Eosin dan diamati di bawah mikroskop. Hasil pengamatan difokuskan pada derajat hipertrofi glomerulus dan kelainan pembuluh darah ginjal. Penelitian ini menunjukkan bahwa kombinasi suplementasi probiotik dan metformin lebih efektif dalam mencegah kerusakan struktural ginjal, ditandai dengan penurunan hipertrofi glomerulus, dibandingkan penggunaan tunggal obat metformin. Hasil ini berpotensi memberikan kontribusi terhadap pengembangan suplemen pendukung untuk diabetes melitus yaitu soyghurt beku-kering sebagai suplemen yang mampu menurunkan glukosa darah.

Abstract

The Medical Certificate of Cause of Death (SMPK) is an official letter that contains the identity of a person and the causes that caused the person to die. The cause of death listed on the SMPK can be used for epidemiology, prevention, and management of certain diseases. Data from the SMPK itself becomes the main data to determine the epidemiology of certain diseases. The format for filling out the medical certificate of the cause of death generally follows the standards issued by the Ministry of Health through BALITBANGKES based on international standards issued by the World Health Organization (WHO). This study aims to determine how the level of knowledge of general practitioners about the medical certificate of the cause of death and its filling in RSDGJ. The research method used in this study is descriptive observational, this research approach is a cross-sectional approach. The sample of this study was a sample that met the inclusion and exclusion criteria using consecutive sampling technique. The sample size in this study was in accordance with total sampling of 42 respondents who worked as general practitioners on duty at Gunung Jati Regional Hospital, Cirebon City. The majority of respondents in this study were female, namely 61.9% with the most age range of 26-35 years by 76.2%. The results of this study showed that most respondents showed a good level of knowledge as much as 66.7%, respondents with sufficient knowledge were 28.6%, and respondents with less knowledge were 4.8%. Based on these results it can be concluded that the level of knowledge of general practitioners at Gunung Jati Regional Hospital, Cirebon City about filling out the Cause of Death Medical Certificate is good.

Abstract

Background: Ethical dilemmas in handling cases of Exclusively Transmitted Infections (STIs) are often experienced by doctors due to their knowledge and attitudes in respecting the patient's right to autonomy. The objective of the research is to study the practitioners’ knowledge and attitudes concerning autonomy principles in facing STI patients. Methods: This cross-sectional analytical study involved a total sampling of 32 general practitioners at Dustira Hospital. A researcher-designed, validated, and reliability-tested questionnaire was used to measure doctors’ knowledge (independent variable) and attitudes (dependent variable) toward patient autonomy. Expert consultation and a pilot test at Cibabat Hospital were conducted to ensure instrument suitability. Data were analyzed using the Pearson correlation test, and ethical approval was obtained before the study.. Results: The results showed that 66% of general practitioners at Dustira Hospital had good knowledge of the principle of autonomy in managing patients with sexually transmitted infections (STIs). Doctors with over five years of experience demonstrated better knowledge (73%) compared to those with less experience (58.2%). Interestingly, among less experienced doctors who frequently treated STI patients, all had poor knowledge, whereas those who treated STI patients less often showed better knowledge (63%). Regarding attitudes, 53% of respondents had a good attitude toward autonomy-related ethical dilemmas. A greater proportion of doctors with more than five years of experience (60%) had good attitudes than those with less experience (47%). Notably, less experienced doctors who frequently managed STI cases showed entirely poor attitudes. Pearson correlation analysis indicated no significant relationship between knowledge and attitude (p = 0.425), with a very weak negative correlation (r = –0.146). Conclusion: General practitioners with more work experience tend to demonstrate better knowledge and attitudes regarding patient autonomy in STI cases. However, practitioners with less than five years of experience who frequently treat STI patients exhibit both poor knowledge and unfavorable attitudes. A tendency toward empirical, experience-based decision-making over rational, principle-based reasoning may influence this.

Abstract

A doctor's professionalism consists of clinical competence, communication skills, ethical and legal understanding, accountability, altruism, excellence, and humanism, of which the seven pillars of professionalism are applied at the professional level. This research aimed to describe patient perceptions of the professionalism of junior interns at Dustira Teaching Hospital. Data collection occurred in November-December 2022. This research utilized a quantitative approach, employing a descriptive research method with a cross-sectional design. The study results, which included patient characteristics such as age, sex, education level, and length of stay based on the seven pillars, showed the following outcomes: clinical ability (97.6%), communication skills (95.4%), understanding of ethics and law (92%), accountability (94.8%), altruism (90.8%), and excellence (94.4%) all fell within the very good category. In contrast, the aspect of humanism (66.2%) was only rated as quite good. Researchers suggest that institutions and junior interns should re-evaluate the learning and application of the professionalism among junior interns. While aspects of humanism are generally considered positive, there is still room for improvement. Abstract

The ability to move the lower extremities in individuals who have suffered from a stroke presents significant challenges to daily mobility and function. We conducted a systematic evaluation to compare the effectiveness of physical rehabilitation following a stroke. We searched six databases (PubMed, Science Direct, Cochrane Library, Springer, Portal Garuda, and Nature Portfolio) for publications published before November 2023. The researchers recruited stroke patients and used a validated scale to assess lower extremity strength and walking capacity in post-stroke patients. We used the Joanna Briggs Institute Critical Appraisal Checklist to evaluate the quality of the papers that met the inclusion criteria. The PRISMA checklist was used to report the study. We identified nine relevant articles that discussed various rehabilitation methods for stroke patients with lower extremity impairment. We examined the specificity of these results using a statistical parameter. The findings were as follow: Biofeedback cycling (P < 0.001); Pelvic Stability Training (P < 0.05); Exoskeleton for post-stroke recovery of ambulation (ExStRA) (P < 0.05); Aerobic Cycling (P < 0.001); Somatosensory Training (P < 0.001), Lower-extremity constraint-induced movement therapy (P < 0.001), Motor imagery with circuit class therapy (P < 0.001), Robotic gait training (P < 0.05), and Early and Intensive  (P < 0.001). Various rehabilitation methods discussed in the article demonstrate high effectiveness in improving lower extremity function. These methods include biofeedback cycling, aerobic cycling, somatosensory training, movement therapy, motor imagery combined with structured progressive circuit therapy classes, and early and intensive rehabilitation after ischemic stroke.
