Abstract

AbstractPenyalahgunaan NAPZA di Indonesia meningkat secara cepat. Meskipun berbagai upaya telah dilakukan untukmencegah penyalahgunaan NAPZA baik oleh pemerintah maupun masyarakat, namun angka penyalahgunaanNAPZA terus meningkat tajam. Jumlah pengguna NAPZA di Kota Kendari dari Januari sampai November tahun2016 sebanyak 138 orang, dan dari angka tersebut, 107 orang adalah kalangan pelajar atau anak sekolah.Penelitian bertujuan untuk mengetahui upaya pencegahan penyalahgunaan Napza melalui peningkatan kontrolsosial masyarakat di Kelurahan Kampung Salo Kota Kendari. Penelitian studi kasus digunakan untuk mengkajipermasalahan dengan pendekatan kualitatif, melalui Diskusi Kelompok Terarah (focus group discussion/FGD).FGD dilakukan di salah satu rumah warga dengan jumlah peserta sebanyak 12 orang, yaitu: lurah, ketua RW,ketua-ketua RT, tokoh-tokoh masyarakat dan perwakilan warga setempat, serta menggunakan menggunakaninteractive model of analysis untuk analisis data. Hasil penelitian diperoleh empat tema yaitu pertama;penyuluhan kesehatan mencakup sasaran dan tempat penyuluhan, kedua; dukungan sosial yang mencakuppenyediaan pelatihan keterampilan dan kegiatan positif seperti olah raga, ketiga; perhatian orang tua yangmencakup pengawasan orang tua dan keterbukaan orang tua, keempat; pengawasan masyarakat yangmencakup peran tokoh masyarakat dan peran masyarakat. Saran-saran penelitian adalah pendidikan kesehatansebaiknya diberikan secara berkesinambungan kepada individu, keluarga dan kelompok serta perlunyadukungan sosial melalui kegiatan positif untuk mencegah penyalahgunaan Napza. Masyarakat perlu membuatsystem pengawasan pencegahan dan penyalahgunaan Napza yang melibatkan pemerintah setempat, aparatkeamanan, pihak sekolah, tokoh masyarakat, keluarga serta kelompok masyarakat.Kata kunci: kontrol sosial, penyalahgunaan napza, upaya pencegahan

Abstract

Penyakit tidak menular diantaranya dipengaruhi oleh perubahan pola perilaku masyarakat, dan menjadi penyebab kematian terbanyak di dunia. Penyakit tidak menular seringkali diabaikan dan tidak terdeteksi lebih dini oleh penderitanya, sehingga menimbulkan komplikasi lebih lanjut. Pos Pembinaan Terpadu Penyakit Tidak Menular (Posbindu-PTM) dapat digunakan sebagai tempat awal deteksi dini faktor risiko masalah kesehatan, dan kader memiliki peran yang penting dalam penyelenggaraan Posbindu-PTM. Tujuan pengabdian masyarakat ini adalah untuk meningkatkan pengetahuan masyarakat tentang Penyakit tidak menular melalui penyuluhan, melakukan deteksi dini faktor risiko PTM, serta meningkatkan pemahaman kader Posbindu tentang Penyakit tidak menular melalui kegiatan penyegaran. Kegiatan ini dilakukan dengan metode penyuluhan massa dan dilanjutkan dengan deteksi dini factor risiko Penyakit tidak menular terhadap 25 warga Kelurahan Abeli Kecamatan Abeli Kota Kendari yang bertempat di aula kantor Kelurahan Abeli. Kegiatan dilakukan selama satu hari. Penyuluhan kesehatan yang diberikan terbukti meningkatkan pengetahuan, dimana dari 5 peserta yang memiliki pengetahaun yang baik sebelum penyuluhan meningkat menjadi 23 peserta yang memiliki pengetahun baik. Sebanyak 52% peserta memiliki kadar gula darah sewaktu >200mg/dl, 56% memiliki kadar kolesterol >200mg/dl, dan 56% termasuk dalam kategori hipertensi grade 1 dan grade 2. Hasil menunjukan sebagian besar peserta memiliki factor risiko PTM yang tinggi. Hasil penyegaran kader terbukti meningkatkan pengetahuan kader tentang Penyakit tidak menular dan penatalaksanaannya. Kegiatan edukasi, deteksi dini perlu terus dilakukan secara berkesinambungan karena terbukti meningkatkan pengetahuan serta kewaspadaan masyarakat akan penyakit tidak menular. Selain itu kegiatan penyegaran perlu dilakukan secara berkala agar pelayanan Posbindu menjadi lebih baik dan kader lebih termotivasi dalam memberikan pelayanan.

Abstract

ABSTRACT Diabetes Mellitus (DM) is a health problem that requires a multidisciplinary approach, including the important role of community health center nurses. Telecoaching conducted by nurses has proven effective in improving glycemic levels in DM patients. This community service program aimed to enhance the capacity of primary healthcare center (puskesmas) nurses in providing telecoaching to Type 2 Diabetes Mellitus (DM) patients in the coastal areas of Kendari City. The program was designed to address the limitations in nurses' knowledge and skills while increasing the intensity of remote patient support for DM. A total of 10 primary healthcare nurses participated in the training, which covered telecoaching techniques, motivational interviewing, and the use of the Diabetes Mellitus Patient Health Assistance Application (DMPHAA). Evaluations assessed improvements in nurses' knowledge and skills, as well as reductions in the fasting blood glucose levels of DM patients. The results showed a significant increase in nurses' knowledge of telecoaching by 40.30% and a 52.76% improvement in telecoaching skills after the intervention. In addition, there was a 20.61% reduction in the fasting blood glucose levels of DM patients, demonstrating the program's effectiveness in helping patients manage their diabetes. Statistical tests showed significant results for all three variables with p<0.05. This program demonstrated that telecoaching can be an effective solution for providing remote healthcare services, especially in areas with limited access. It is recommended that the sustainability of this program is integrated into the services of primary healthcare centers to improve diabetes management in remote regions. Keywords: Telecoaching, Diabetes Mellitus, Primary Healthcare Nurses, Blood Glucose, Community Service

Abstract

Background: Accessibility to healthcare services remains a significant challenge among low-income families in coastal regions such as Maginti Island, West Muna Regency. Several socioeconomic and environmental factors are believed to influence the utilization of available health services. This study aimed to identify the factors associated with the utilization of healthcare services among poor families in Maginti Island. Methods: A quantitative, cross-sectional study was conducted from May to July 2024. A total of 57 respondents were selected using simple random sampling. Data were collected using structured questionnaires and analyzed using univariate and bivariate methods (Chi-square test). Results: The study found significant associations between healthcare service utilization and three main factors: knowledge of health, availability of transportation, and household income level (p < 0.05). Families with better knowledge, easier access to transportation, and higher income levels were more likely to use healthcare services. Conclusion: Socioeconomic status, transportation access, and knowledge play crucial roles in determining healthcare utilization among the poor in Maginti Island. Strengthening health education and infrastructure is essential to improving healthcare access in coastal communities.

Abstract

Tele-counseling has gained prominence as an effective tool for managing chronic diseases, including type 2 diabetes, particularly in underserved regions. This study aimed to assess the effectiveness of tele-counseling in improving self-care behaviors and metabolic markers in type 2 diabetes patients residing in the coastal areas of Kendari City, Southeast Sulawesi, Indonesia. A quasi-experimental design was employed, involving 70 participants who were randomly assigned to two groups: an intervention group (n = 35), which received tele-counseling for six weeks, and a control group (n = 35), which continued standard care. The primary outcomes measured included self-care behavior, fasting blood glucose (FBG), low-density lipoprotein (LDL) cholesterol, body weight, and blood pressure, all assessed at baseline and again six weeks after the intervention. Statistical analyses, including paired t-tests, Wilcoxon tests for non-normally distributed data, and independent t-tests for between-group comparisons, were conducted. Results indicated that the intervention group experienced significant improvements in self-care behavior (p < 0.001), along with reductions in FBG (p < 0.001), LDL cholesterol (p < 0.001), systolic blood pressure (p = 0.001), diastolic blood pressure (p = 0.003), and body weight (p < 0.001). In contrast, the control group showed minimal changes. These findings suggest that tele-counseling is an effective intervention for enhancing self-care behaviors and improving key metabolic markers in type 2 diabetes patients, particularly in coastal areas with limited access to healthcare services. This study provides valuable insights into the application of telemedicine for chronic disease management in underserved regions. Further research is recommended to examine the long-term effects of tele-counseling on diabetes management and to explore its scalability in other remote and rural areas.

Abstract

Background: Insulin therapy is essential for managing type 2 diabetes in patients who do not achieve adequate glycemic control with oral medications. Despite its benefits, initiating insulin therapy presents numerous challenges due to psychological, physical, and systemic barriers. This study examines the critical role of community health nurses in facilitating the initiation and management of insulin therapy, addressing patient and healthcare provider barriers, and improving clinical outcomes. Methods: A comprehensive review of existing literature was conducted, focusing on the impact of community health nurses on diabetes management, particularly in insulin initiation and adherence. Results: : Community health nurses play a pivotal role in overcoming barriers to insulin therapy. They provide essential patient education, psychosocial support, and practical training in insulin administration. These efforts lead to improved glycemic control, higher rates of insulin adherence, and better patient satisfaction. Additionally, community health nurses facilitate collaborative care models, integrating with primary care teams to provide continuous and coordinated care. The involvement of community health nurses in diabetes management addresses both patient-related and systemic barriers. Their holistic approach includes personalized patient education, emotional support, and care coordination, significantly enhancing patient adherence and clinical outcomes. Conclusions: Community health nurses are integral to the successful initiation and management of insulin therapy in type 2 diabetes. Future research should focus on developing standardized training programs for community health nurses and exploring innovative care models that leverage their unique skills. Policy support is essential to recognize and expand the role of community health nurses in diabetes care.

Abstract

Purpose: The aim of this literature review was to get an overview of the methods and strategic interventions of foot care education in the prevention of diabetic foot ulcers (DFU).Methodology: The literature search focused on ‘foot care education in the prevention of diabetic foot ulcers’. A number of databases such as Springer Link, EBSCOhost (Academic Search Complete), ProQuest, and Science Direct were searched for relevant articles. Literature search using keywords such as “type 2 dm”, “education”, “prevention” and “diabetic foot ulcer.” All types of articles were included for the study, such as systematic reviews, randomized controlled trial, quasi-experiment, and literature review with the range 2010-2020. Only articles in English are included in this review. Results: Based on keywords, it was identified nine articles that focus on intervention of foot care education to prevent diabetic foot ulcers. The literature review summarized the discussion about the methods of providing education, duration and follow up, the teams involved, and the effectiveness of educational interventions. The objectives of providing educational interventions are individuals, families and groups. The method of providing education was carried out face-to-face in clinics, hospitals, and at home, by phone and through android applications such as youtube. The intervention of foot care education effectively improves foot care knowledge, self-efficacy of foot care, foot care behavior, glycemic control, reduce skin and nail problems, improve diabetic foot, decrease DFU incidence, and improve the quality of life.Applications/Originality/Value: This review can be a reference for nurses and other health professionals in choosing the best methods and strategies for foot care education tailored to existing resources.
