Abstract

One of the shortcomings of manual patient registration systems is the procedures that affect the efficiency of service care to patients. Therefore, researchers are interested in designing a computerized patient registration application using Microsoft Access software. The type of research used is Research and Development (R&D). This research was conducted at Tanjung Pati Community Health Center, with in-depth interviews with two registration officers. The research results show that the data elements needed to design the patient registration application system, sourced from the Patient Registration Book, will be developed by creating an application system using UML modeling. The application system undergoes Black Box Testing with the result that all components run well. The application system is efficient, with an average comparison of manual patient registration time to system patient registration of 420:180 seconds for new patients and 360:33 seconds for existing patients. The application system is efficient according to the results of interviews with informants. Therefore, the design of the computerized patient registration application system using Microsoft Access meets the needs of the community health center and is efficient in terms of patient registration time. It is recommended that the patient registration application be used continuously in the medical records unit at Tanjung Pati Community Health Center.

Abstract

The pending BPJS claim occurred due to the rejection of the file by the BPJS health verifier. Problems with pending claims are caused by errors in determining the main diagnosis, incomplete diagnosis writing and code accuracy. This research aims to describe the causes of pending claims, including from the diagnostic aspect, namely the accuracy of the main diagnosis and the completeness of writing diagnosis as well as aspects of the accuracy of the diagnosis code. The research is descriptive in nature and the population is 35 medical records pending inpatient claims for 2023, the sample was determined by total sampling. The research data was analyzed univariately to determine the frequency distribution of causes of pending claims. The results of research on 35 pending claims showed that the accuracy of determining the main diagnosis was 19(54.3%), the accuracy of writing an incomplete diagnosis was 31(88.6%), and the accuracy of the incorrect diagnosis code was 29(82.9%). It is hoped that the doctor who is responsible for determining and writing the diagnosis can pay attention to the correctness of the diagnosis in the medical record and the coder can determine the correct diagnosis code based on ICD-10 and ICD-9 CM.

Abstract

The incompleteness of filling out the inpatient Integrated Patient Progress Record (CPPT) form can affect the accuracy of disease diagnosis coding. Based on the results of the initial survey on 10 inpatient medical records, 7(70%) CPPT incompleteness and 4(40%) inaccuracy in coding disease diagnosis. The purpose of this study was to determine the relationship between the completeness of filling out the inpatient CPPT form with the accuracy of coding a disease diagnosis based on ICD-10 at Prof. Mental Hospital. HB. Saanin Padang. This research is descriptive correlational with quantitative. The research was carried out at the Prof. Mental Hospital. HB. Saanin Padang on June 13-17, 2022. The population in this study was medical records of inpatients in April 2022 as many as 223 medical records with a sample of 69 medical records and sampling using simple random sampling. Data was collected by observation and data processing techniques of editing, coding, tabulating, and cleaning. The analysis used is univariate analysis and bivariate analysis. The results of the study showed that 67(97.1%) incomplete inpatient CPPT forms were filled in, 5(7.2%). filling in the CPPT with the accuracy of coding the diagnosis of disease with value=1,000(P>0.05). The conclusion of the study that there is no significant relationship between the completeness of filling the CPPT with the accuracy of coding the diagnosis of disease, but the incompleteness of filling in the CPPT inpatients is still in the high category, it is recommended to Professional Care Providers (PPA) to fill out the CPPT completely. Keywords: CPPT, ICD-10, completeness, coding accuracy

Abstract

The implementation of the referral system is held with the aim of providing quality health services, so that the objectives of the service are achieved without having to incur expensive costs. The purpose of this study was to determine the relationship between patient knowledge and the validity period of a referral letter at the Ambacang Padang Health Center in 2022. This type of research was quantitative with a cross sectional research design. This research was carried out at the Ambacang Health Center in July 2022. The population in this study was 400 patients per month. While the sampling using the slovin formula amounted to 80 respondents. Data were analyzed by univariate and biavariate by using Chi-square test. The results showed that more than half (51.3%) of respondents had a low level of knowledge, more than half (67.5%) of respondents had an invalid reference letter. There is a relationship between knowledge and the validity period of the referral letter with a p-value of 0.000 (p<0.05). Based on the results of the study, it can be concluded that there is a relationship between knowledge and the validity period of the referral letter. Suggestion, Through the head of the Puskesmas in order to further increase the patient in taking medication with the community about the importance of paying attention to the referral letter that is still valid so that the community is no longer mistaken with the referral letter used and so that the community is more aware of the importance of a valid referral letter. Keywords: Knowledge, Validity Period, Reference Letter
