Abstract

Anxiety disorders are a common mental health problem that affects many adolescents and young adults and can negatively impact both short-term and long-term health. Functional gastrointestinal disorder (FGID) occurs in Indonesian adolescents with a prevalence of 40.85% (Ministry of Health, 2020) and is often affected by anxiety through the communication pathways between the brain and intestines. This interaction means that anxiety worsens digestion and digestive problems increase anxiety. It reflects the complex interplay between psychological, biological, and social factors. The purpose of this systematic review was to review the current literature on the relationship between anxiety disorders and gastrointestinal dysfunction in adolescents, including mechanisms, risk factors, and diagnostic and treatment approaches. This systematic review used the PRISMA-P 2020 (Preferred Reporting Items for Systemic Review and Meta-Analysis Protocols) protocols. The study was collected through a search of Pubmed, EBSCO, Zendy, and Proquest source data from 2014 to 2020. Keyword search uses the PICO-S (Population Intervention Compare Outcome-Study design) technique. It found 225 studies and 18 studies were included in this study. Gastrointestinal (GI) disorders are often closely related to anxiety through the brain-intestinal axis, where anxiety can affect GI functions such as intestinal motility, visceral sensitivity, and microbiota composition. This can lead to conditions such as irritable bowel syndrome (IBS), acid reflux, and other indigestion. In contrast, GI disorders can also frustrate anxiety, creating mutually reinforcing cycles. The treatment of gastrointestinal disorders and anxiety from the results of our research is psychological therapy, medical treatment and pharmacological treatment such as corticosteroids and aminosalylates. Anxiety is a serious mental illness that often manifests as physical symptoms, such as heart palpitations and indigestion. It is related to the brain axis, where chronic and anxiety can affect gut motility, visceral sensitivity, and the gut microbiome. According to research, the gut microbiota plays an important role in depression and anxiety, making it a potential target for treatment and recovery. Web-based therapies, such as cognitive behavioral therapy, are effective in treating gastrointestinal disorders and anxiety and also some medications such as corticosteroids.

Abstract

Breast cancer is the most common type of cancer affecting women and often requires surgical management. Surgical procedures frequently cause anxiety in patients, which can impact the healing process and the effectiveness of treatment. High levels of preoperative anxiety may negatively affect patients’ physiological and psychological conditions. This study aims to describe the level of anxiety in breast cancer patients before undergoing surgery at RSUP Dr. M. Djamil Padang. This research is a descriptive categorical study using purposive sampling. The sample consisted of 59 breast cancer patients scheduled for surgery at the Oncology Clinic & Inpatient Ward 1 Day Before Surgery at RSUP Dr. M. Djamil Padang, who met the inclusion and exclusion criteria. The instrument used was the APAIS (Amsterdam Preoperative Anxiety and Information Scale) questionnaire. The majority of patients scheduled for surgery at RSUP Dr. M. Djamil Padang were over 45 years old (76.3%), had completed senior high school (49.2%), were unemployed (71.2%), and most had never undergone surgery before (59.3%). Anxiety levels showed that 40.7% of patients experienced mild anxiety, 37.3% moderate anxiety, and 22% severe anxiety. No patients experienced very severe (panic-level) anxiety. Most breast cancer patients in the preoperative phase at RSUP Dr. M. Djamil Padang experienced mild to moderate anxiety. These findings highlight the importance of providing education and psychological support to patients prior to surgery.

Abstract

In Vitro Fertilization (IVF) is an assisted reproductive technology that offers a significant solution for legally married couples experiencing infertility. This study aims to examine the medical, ethical, legal, and religious dimensions of IVF, including the use of emerging technologies such as Polygenic Embryo Screening (PES) and the practice of surrogacy. Medically, IVF is effective in addressing various infertility factors, yet it carries risks such as Ovarian Hyperstimulation Syndrome (OHSS), preeclampsia, and metabolic disorders, along with long-term health implications for offspring. Ethically, IVF raises debates regarding the moral status of embryos, the adequacy of informed consent, and concerns about eugenics through embryo selection. Legally, the lack of global harmonization in regulating surrogacy and gamete donation creates challenges in protecting the legal rights of children and intended parents. From an Islamic perspective, IVF is permissible if performed within a valid marriage using the couple’s own gametes, with strict prohibition against third-party involvement to preserve lineage and marital integrity. This study highlights the need for ethical and culturally sensitive regulatory frameworks and recommends further research on long-term health impacts of IVF, the effectiveness of informed consent, and global policy development for PES and surrogacy practices.

Abstract

Organ transplantation is a vital medical solution that faces complex ethical dilemmas in the distribution of limited organs. This study aims to explore and analyze ethical considerations in the distribution of transplanted organs, focusing on the principles of justice and medical priority through a systematic literature review. The search methodology was conducted in the PubMed, ScienceDirect, and Google Scholar databases using the keywords "ethical considerations," "organ transplantation," "justice principles," and "medical priority" for articles published between 2015 and 2025. The principle of distributive justice in organ transplantation integrates multiple criteria that include medical urgency, likelihood of success, age, and healthy lifestyle, while rejecting discriminatory criteria such as economic status or gender. Medical priority influences transplant decisions through systems such as MELD, which emphasize medical urgency but create a dilemma between saving the most critical patients versus optimizing long-term success. Consensus across perspectives indicates that organ transplantation is ethically and religiously acceptable under certain conditions by fulfilling the principles of autonomy, beneficence, non-maleficence, and justice. The findings indicate the need for a framework that harmonizes principles of distributive justice with medical priorities within the context of local cultural and religious values. Practical implications include developing more comprehensive organ distribution guidelines, enhancing the capacity of healthcare professionals in ethical decision-making, and strengthening regulatory systems that accommodate multi-criteria complexity without compromising the principle of justice.

Abstract

Female circumcision or Female Genital Mutilation (FGM) remains a prevalent traditional practice in Indonesia despite having no medical benefits and potentially causing various health complications. This study aims to analyze the practice of female circumcision from three main perspectives: medical ethics, medical regulations, and Islamic law in Indonesia. The research methodology employed a systematic literature review of 20 selected articles from PubMed, ScienceDirect, and Google Scholar databases published between 2015-2025. The results indicate that female circumcision causes various negative impacts including bleeding, infections, psychological trauma, and obstetric complications. Regulatory-wise, Indonesia has revoked the permission for this practice through Ministry of Health Regulation No. 6/2014, but its implementation still faces challenges due to socio-cultural factors and diverse religious interpretations. From the Islamic law perspective, the study found no consensus among scholars regarding the obligation of female circumcision, and the practice is more driven by tradition than strong religious doctrines. This study concludes the need for a multidisciplinary approach integrating health, legal, and socio-cultural aspects to effectively address the issue of female circumcision.

Abstract

Anxiety disorders are one of the most common mental health problems that have a significant impact on an individual's quality of life and productivity. In addition to pharmacological therapy and psychotherapy, music therapy is beginning to be widely used as a non-pharmacological approach to reduce anxiety and improve emotional well-being. This study aimed to evaluate the effectiveness of music therapy as a mental health support intervention in patients with anxiety disorders through a systematic literature review. This study used a systematic literature review method with meta-synthesis, analyzing 21 qualitative and quantitative studies selected from PubMed, EBSCO, and Google Scholar databases (period 2015–2024). Inclusion criteria include studies with a population of patients with anxiety disorders, music therapy interventions, as well as reports of anxiety or quality of life outcomes. Of the 1,463 articles screened, 21 studies were included in the analysis. Results showed that music therapy, both actively (playing or singing) and receptive (listening), was consistently effective in lowering anxiety levels, improving sleep quality, mood, and improving social function in various population groups, including medical patients, college students, and the elderly. Meta-analyses showed high effectiveness of music therapy (effect size Hedges' g >1), with additional benefits in the form of increased motivation and emotional regulation. The combination of music therapy with other interventions such as pharmacotherapy, CBT, or mindfulness improves clinical outcomes. Music therapy is an effective, safe, easy, and affordable non-pharmacological approach to overcome anxiety disorders and improve mental health. The integration of music therapy in mental health services deserves to be considered as part of a comprehensive approach. Further research is needed for protocol optimization and exploration of neurophysiological mechanisms of music therapy in various populations.
