Abstract

Breast cancer originates from malignant cells in the breast glands, ducts, and supporting tissues. According to the National Breast Cancer Foundation, various risk factors contribute to breast cancer, including genetic factors, family history, age, breastfeeding status, early age at menarche, age at menopause, parity, use of hormonal contraception, smoking, alcohol consumption, frequency of physical activity, excess weight after menopause, and chest radiation exposure. Data from the International Agency for Research on Cancer (IARC) indicate that in Asia, common risk factors for breast cancer include alcohol consumption, radiation exposure, diet, nutrition, excess body weight, physical activity, genetic factors, as well as reproductive and hormonal factors. This study aims to identify the risk factors associated with breast cancer in women. Methods of this study is a systematic literature review with a deductive approach, following the Preferred Reporting Items for Systematic Review and Meta-Analysis (PRISMA) guidelines. The search strategy utilized the PICO framework (Population, Intervention, Comparison, and Outcome). Relevant journals were sourced from online databases such as PubMed, Google Scholar, and DOAJ, covering publications from 2019 to 2023. From the search of online databases like PubMed, Google Scholar, and DOAJ, 10 full-text journals met the inclusion criteria and focused on discussing the risk factors for breast cancer. Several risk factors are associated with breast cancer. Among these, the use of hormonal contraception and a family history of breast cancer have a significant relationship with the incidence of the disease.

Abstract

Introduction: Suicide is one of the causes of death that often occurs in individuals who have mental disorders. Suicide can start from depression that occurs in the sufferer. Spiritual aspects are often associated with preventing suicidal behavior. Several related studies have explained how spiritual support can reduce levels of depression in individuals. Method: This literature review aims to determine the role of spirituality in influencing the risk of suicidal behavior in adults. Writing this journal uses a literature study approach originating from a database, namely Google Scholar using keywords. Filtered 159,320 journals and managed to find 14 related journals. Using inclusion criteria which include literature sources taken from 2018 to 2023, using Indonesian, appropriateness of writing keywords, relevance of literature writing results to the discussion raised. Results: Mental health problems such as depression which cause the risk of suicide can be overcome with psychosocial and religious interventions in order to create good copying in solving individual problems. Several interventions provided to treat individuals with mental health problems who are at high risk of suicidal behavior include the Social Spiritual Group (SSG) which can be applied to health services (Forrester-Jones, Dietzfelbinger, Stedman, & Richmond, 2018). Psychosocial support also often helps reduce patients' depressive symptoms and possible risk of suicide, and has been shown to improve psychological well-being. Conclusion: Suicide attempts often occur, both among teenagers, adults and the elderly. This behavior is not only increasing sharply in Indonesia, but almost throughout the world. Before taking action on a cooperative patient, do not deny that there has been a suicide attempt. However, the client speaks slowly and finds it difficult to start a conversation. Clients also do not understand the coping patterns that can be applied when clients face a problem.

Abstract

Background: Hypertension is a major non-communicable disease and one of the leading causes of death worldwide, affecting approximately 22% of the global population. It is often called the "silent killer" because it develops asymptomatically and can cause fatal outcomes. Elderly individuals are particularly vulnerable due to physiological changes, including reduced vascular elasticity and decreased taste sensitivity, leading to excessive salt consumption in their diet. Objectives: This systematic review aims to evaluate the relationship between salt consumption patterns and the incidence of hypertension in the elderly population. Methods: A systematic review was conducted following PRISMA guidelines. Relevant studies were identified through Google Scholar and PubMed using the keywords "hypertension," "elderly," and "salt consumption" within a publication range of 2018 to 2023. Inclusion and exclusion criteria were applied to screen and select studies. A total of 12 studies meeting the criteria were analyzed. Results: Most studies reported a significant association between excessive salt consumption and hypertension among the elderly. For example, one study found that 56.7% of elderly respondents consumed salt beyond the normal range, with 54.9% of them suffering from hypertension (p=0.012). Another study reported that elderly individuals with high salt intake had a 5.704 times greater risk of developing hypertension compared to those with low salt intake (p=0.010). These findings highlight the significant impact of excessive salt intake on hypertension risk in older adults. Conclusions: Excessive salt consumption is a significant risk factor for hypertension in the elderly, driven partly by reduced taste sensitivity. These findings underscore the importance of dietary interventions and public health strategies to educate and promote low-salt diets among older adults to mitigate hypertension risk.

Abstract

Background: Microglandular adenosis (MGA) is a rare breast lesion that poses diagnostic challenges due to its resemblance to invasive carcinoma, particularly invasive tubular carcinoma (ITC). Atypical MGA is of clinical concern because of its potential for malignant transformation. Accurate diagnosis relies on histopathological examination and immunohistochemical (IHC) analysis. Case Presentation: A 34-year-old woman presented with a painless lump in her left breast. Intraoperative frozen section analysis revealed small glandular structures with histological features mimicking ITC. Definitive diagnosis required further evaluation. Immunohistochemical analysis demonstrated S100 positivity, consistent with glandular differentiation, and negative p63 staining, indicating the absence of a myoepithelial layer. These findings, in the absence of definitive stromal invasion, supported a diagnosis of atypical MGA. Complete surgical excision was performed to ensure negative margins and exclude associated malignancy. Discussion: This case highlights the diagnostic complexity of atypical MGA, particularly in young patients. Frozen section analysis alone often fails to distinguish MGA from invasive carcinoma due to overlapping histological features. IHC markers, such as S100 and p63, are critical for differentiation. S100 positivity confirms glandular origin, while p63 negativity indicates the lack of a myoepithelial layer, distinguishing MGA from benign proliferative lesions. Accurate diagnosis is essential to avoid overtreatment, such as unnecessary chemotherapy or radical surgery, while ensuring appropriate management to mitigate malignant potential. Conclusion: This report underscores the importance of combining frozen section and IHC findings for rare breast lesions like atypical MGA. Increased awareness and careful evaluation are essential to achieve timely and precise diagnosis, enabling optimal surgical management and long-term outcomes.

Abstract

Background: Glomus tumor is a rare mesenchymal neoplasm that accounts for less than 2% of all soft tissue tumors and 1.5% of extremity neoplasms. Malignant glomus tumors are exceptionally rare, comprising only 1-2.9% of all glomus tumors, and are often associated with significant diagnostic and therapeutic challenges. Case Presentation: We report a case of a 62-year-old male presenting with a painful lump on his left thumb, progressively enlarging over six months. Clinical examination revealed a 3x2x1 cm subungual mass with positive cold sensitivity and pin love tests. The patient underwent amputation, and histopathological analysis showed a proliferation of round-to-oval and spindle cells with nuclear atypia, frequent mitotic figures (>5/50 HPF), and atypical mitoses, consistent with malignant glomus tumor grade 2. Surgical margins and bone were free of tumor infiltration. Conclusion: This case highlights the importance of clinical-pathological correlation in diagnosing malignant glomus tumors, which remain a diagnostic challenge due to their rarity. Complete surgical excision with negative margins remains the gold standard for treatment, ensuring favorable outcomes and minimizing recurrence risk.

Abstract

Background: One of the most influencing factors in achieving a satisfying score in an academic record is from the student himself. It is in form of the readiness for them to learn independently. This situation is often called sdlr. It is the level of readiness or the ability to learn independently. The important role for the sdlr formation in oneself can be used to score the level of students success in achieving their academic records. Objective: The aim of this study to know the correlation between self-directed learning readiness and 3rd-semester students Medical Faculty of Baiturrahmah University’s achievement. Methods: This study was conducted in the third semester of Medical Faculty of Baiturrahmah University from March to September 2020. This study belongs to unpaired comparative analytic by using Cross-Sectional Study design. The population of the study is all of the third-semester students of Baiturrahmah University with 105 samples. The sample is taken randomly. The union varied data analysis shown in frequency distributed table and bivariate analysis uses a Kruskal-Wallis test with SPSS program. Results: There is correlation between self directed learning readiness with students score in the third-semester students of medical faculty of baiturrahmah university, it is obtained the result of p=0,001 shows the most sldr score is in the high level, acquired by 67 students (63.8%) and the most concern is from the very satisfying grade acquired by the students, 78 students (74,3%). So it can be assumed that there is a significant relationship between sldr and the students' academic achievement. Conclusions: There is a significant correlation between Self Directed Learning Readiness and the third semester of Medical Faculty of Baiturrahmah University academic achievement.

Abstract

The high incidence of DM causes diabetes to become a health problem. For this reason, other supportive therapies are needed such as the use of probiotics which are known to have antidiabetic effects, high antioxidants and as anti-inflammatory that can improve the structure and function of organs. This study aims to determine the effect of dadiah administration on the histopathological picture of liver hepatocyte cells in diabetic model mice. The research method was an experimental study of male white rats of the wistar strain of white rats, which were divided into control groups and treatment groups. The previous treatment group in Induction became a type 2 diabetes rat model by injecting Streptozotocin (STZ) 45 mg/kgBB intraperitoneally. Dadiah intervention and BAL isolates were administered for 2 months and were analyzed the histopathological picture of rat liver tissue after the intervention and compared with controls. The results of the study showed an improvement in the histopathological structure of the liver in DM rats that experienced damage to liver tissue hepatocyte cells by the administration of dadiah and probiotics (L. Fermentum) at both low dose and high dose (p-value 0.001). The conclusion that dadiah and probiotic dadiah have been shown to be pathologically anatomically beneficial to degenerative and necrotic liver hepatocyte cells in diabetic rat model, due to their antioxidant and anti-inflammatory activity.

Abstract

Migraine is a common neurological condition that significantly impacts the academic performance and quality of life of medical students. Anxiety disorders, often triggered by academic stress, are recognized as major contributors to the onset and exacerbation of migraine episodes. Comorbid mental health conditions such as anxiety and depression further increase migraine-related disability. This systematic review employed a qualitative meta-synthesis approach in accordance with the PRISMA 2020 protocol. Relevant articles published between 2019 and 2024 were retrieved from PubMed and EBSCO databases using keywords such as “anxiety disorder,” “migraine,” and “medical students.” The selected studies were screened based on defined inclusion criteria focusing on qualitative data and mental health correlations. Five eligible studies were included in the final analysis. All studies demonstrated a significant association between anxiety and migraine among medical students. Academic stress was identified as a key contributing factor, with female students showing higher susceptibility. The coexistence of anxiety and depression was associated with increased frequency, intensity, and disability due to migraine. There is a strong correlation between anxiety disorders and migraine among medical students. This comorbidity negatively affects both academic performance and mental health. Psychological interventions and stress management strategies are essential to improve students' quality of life and academic resilience.

Abstract

Background: Meningiomas are the second most common intracranial tumors. The 2016 WHO classification divides the histopathological types of meningiomas into benign (WHO grade I) and malignant (WHO grade II or III). Benign meningiomas are characterized by a well-defined mass adhering to the dura mater and compressing the brain, whereas malignant meningiomas, though rare, are aggressive. Age is a factor that could influence the histopathological type of meningiomas, with incidence increasing with age. Meningiomas commonly occur in middle and old age, peaking in the sixth decade. Age is also a prognostic factor in meningioma occurrences. Method: This was an observational analytic study with a retrospective cross-sectional design on the medical records of meningioma patients at the Anatomical Pathology Laboratory of Dr. M. Djamil Padang Hospital from May 2023 to January 2024. The study included a total of 72 patients. Results: The Chi-Square test showed a p value of 0.330 (p>0.05), indicating no significant relationship between age and the histopathological type of meningiomas. Most meningioma patients were ≥40 years old (76.4%) and female (87.5%). The predominant histopathological type was WHO grade I (88.9%), with tumors ≤5 cm in size (65.3%). Conclusions: There is no significant relationship between age and the histopathological type of meningiomas according to WHO criteria.
