Abstract

Glaukoma neovaskular merupakan glaukoma sekunder dengan prognosis visus relatif buruk. Hal ini terkait dengan kerusakan saraf mata progresif akibat peningkatan tekanan intraokular (TIO) disertai gangguan aliran darah segmen posterior mata (retina). Penyakit diawali dengan kondisi iskemia retina yang merupakan komplikasi penyakit lain, khususnya retinopati diabetes atau oklusi vena sentral retina. Kondisi iskemia akan memicu neovaskularisasi dan pembentukan membran fibrovaskular di segmen anterior mata, terutama iris dan sudut iridokornea yang pada akhirnya akan mengobstruksi aliran humor akuos mata. Hal terpenting adalah deteksi dini dan mencegah kejadian glaukoma neovaskular.Neovascular glaucoma is a type of secondary glaucoma generally associated with relatively poor visual prognosis. It is characterized by progressive damage of optical nerve caused by increase of intraocular pressure and disturbance of retinal blood flow. The early stage starts with retinal ischemia as a complication from other diseases such as diabetic retinopathy and central retinal vein occlusion. This ischemic condition will drive neovascularization and form a fibrovascular membrane over the anterior segments of the eye, especially iris and iridocorneal angle which can obstruct the aqueous humor. Early detection and prevention are important.

Abstract

Pandemi global Coronavirus Disease 2019 (COVID-19) memiliki gejala utama batuk, demam, kelelahan, sesak napas, dan gejala respiratori lainnya. Didapatkan kasus pasien COVID-19 yang memiliki keluhan okular seperti konjungtivitis. Angiotensin-converting enzyme 2 (ACE2), yang merupakan reseptor SARS-CoV-2 juga didapatkan pada sel epitel konjungtiva dan kornea manusia, sehingga memungkinkan transmisi melalui okular. Tatalaksana gejala okular pada COVID-19 hingga saat ini masih terus dikembangkan serta diuji coba.The Global Coronavirus Disease 2019 (COVID-19) pandemic has cough, fever, fatigue, shortness of breath, and other respiratory symptoms as the main symptoms. There were cases of COVID-19 patients with ocular complaints such as conjunctivitis. Angiotensin-converting enzyme 2 (ACE2) – SARS-CoV-2 receptor, is also found in human conjunctival and corneal epithelial cells, thus allowing ocular transmission. Treatment for ocular symptoms in COVID-19 are still being developed and tested.

Abstract

Pendahuluan: Neuromyelitis optica spectrum disorder (NMOSD) merupakan kondisi autoimun yang ditandai dengan inflamasi pada nervus optikus, saraf spinal, dan sistem saraf pusat. Meskipun NMOSD memiliki angka prevalensi yang relatif rendah, prognosis visual penyakit ini cukup buruk dan laju relapsnya tinggi. Tinjauan pustaka ini diharapkan dapat mendorong pembaca untuk bisa mendiagnosis secara lebih dini untuk menentukan tatalaksana serangan akut dan mencegah serangan berikutnya sesuai dengan kompetensi kedokteran. Metode: Artikel ini disusun berdasarkan kepustakaan yang diperoleh dari database Google Scholar, PubMed, dan ProQuest, menggunakan kata kunci “neuromyelitis optica spectrum disorder” dari publikasi tahun 2012 sampai dengan tahun 2022. Hasil: Penyebab NMOSD hingga saat ini masih tidak diketahui dengan pasti, namun diduga disebabkan oleh kondisi sistem imun humoral di dalam tubuh menyerang astrosit. Kriteria diagnosis NMOSD didasarkan pada konsensus yang dikeluarkan oleh International Panel for Neuromyelitis Optica Diagnosis. Myelitis akut, neuritis optik yang berat, muntah dan cegukan berulang merupakan gejala khas pada NMOSD. Lesi myelitis transversal ekstensif longitudinal merupakan penemuan khas pada magnetic resonance imaging (MRI). Tatalaksana NMOSD pada fase akut berupa kortikosteroid dosis tinggi, sedangkan pada fase pemeliharaan berupa imunosupresan. Kejadian relaps ditemukan pada 90% pasien dan komplikasi yang terjadi dapat berupa disabilitas visual, disabilitas motorik, hingga kematian. Simpulan: Diagnosis NMOSD ditegakkan berdasarkan penemuan klinis, pencitraan, dan laboratorium. Tatalaksana yang tepat dan adekuat berguna untuk mencegah disabilitas dan morbiditas di kemudian hari.

Abstract

Leprosy has a high incidence of physical complications that will impact directly the physiological, economic, and social status of patients. Indonesia has a high burden of the disease, and several leprosy settlements are still spread in the country. Neglasari Village, Sitanala, Tangerang serves as one of the places of residence of people who have had leprosy. A health service initiative known as Identifikasi Tanda-Tanda Mata, Ekstremitas, dan Kulit pada Kusta (KATAMATAKU) was launched as a collaboration of health services among multi-departments (ophthalmology, dermatovenereology, and medical rehabilitation). Sitanala has a relatively high incidence of people who have had leprosy with disabilities of the hand, foot, and eye. As a continuation of this health service and combined with the efforts to improve the welfare of leprosy patients, in November 2019, a multidisciplinary program titled KATAMATAKU Universitas Indonesia, was conducted. This program aimed to determine the demographic data regarding the health, psychological, social, and economic status of the leprosy population at Sitanala The project consisted of a collaborative anti-stigma program by the Faculty of Psychology, Public Health, Social and Political Sciences, Cultural Sciences, Administrative Sciences, and Vocational Educational Program; thematic health program, which supports the improvement of physical abilities and empowerment of former leprosy patients, by the Faculty of Medicine, Dentistry, Nursing, and Pharmacy; thematic economics program, which aims to increase the economic capacity of the leprosy community, by the Faculty of Economics and Business, Mathematics and Natural Sciences, and Engineering. This program enabled the construction of a multidimensional management model, in which every aspect plays important roles to improve the patients’ quality of life.
