Abstract

Background: Being the most commonly found sexually-transmitted infection (STI), anogenital warts are mainly caused by human papilloma virus (HPV) type 6 and 11. These infections are often recurrent, leading to loss of time and medical expenses. Various therapeutic modalities can be chosen singularly or in combination with varying results. Treatments with less responsive results usually occur when acquired comorbidities are present, such as HIV infection. Case Illustration: A 28-year-old homosexual male was reported to have a recurrent perianal wart for the duration of eight months. He was also diagnosed with AIDS and pulmonary tuberculosis. Initially, patient was treated with 25% podophyllin tincture, but there was no improvement. Patient was known to still practicing high-risk sexual behavior. After six treatments of TCA, the warts shrank gradually and no new warts had grown. Discussion: It was found that there is a strong relationship between high incidences of genital warts with reduced immune function due to by HIV infection. Patients infected with HIV will often experience recurrence and needs longer treatment. In this case ARV treatment is given and followed by increase in CD4 count, thereby increasing the effectiveness of the treatment of perianal warts. Conclusion: As recommended by clinical guideline in Indonesia, 90% TCA is proven to be effective in treating recurrent anogenital warts in this case. It is important to counsel the patient regarding the sexual behavior for him and his sexual partner, as it will affect the patient’s prognosis significantly.

Abstract

Background: Scabies is a neglected tropical disease in which people affected primarily presents with intense pruritus. The disease is often recurrent, especially in people living in close contacts, such as in a community; thus, it negatively impacts their quality of life (QoL).This study analyzes the impact of pruritus in scabies on the QoL of patients living in a public boarding school (PBS) in West Java, Indonesia. Methods: This study is a mixed-method study conducted on 39 subjects. TheQoL was examined using the Indonesian version of the dermatology life quality index (DLQI) questionnaire and in-depth interviews. Results: Most subjects were female, grade 9 students, had a duration of symptoms between six months to one year, had been infected more than twice, and with no family member affected by the same symptoms. We found that sex, duration of symptoms, disease frequency, and affected family members did not significantly affect subjects’ QoL. In-depth interviews revealed that pruritus affected social relationships, quality of learning, and sleeping. Conclusion: Holistic and comprehensive community management is needed to treat and prevent scabies transmission, as its symptoms negatively impact the QoL of students in PBS.

Abstract

Pioderma fasiale merupakan kondisi kelainan kulit yang jarang terjadi. Kelainan ini secara klasik ditandai dengan munculnya erupsi inflamatori berupa papul, pustul, nodul, bahkan kista pada wajah, terutama mengenai perempuan usia 15-46 tahun. Kondisi tersebut saat ini disebut sebagai rosasea fulminan, suatu varian rosasea yang berat dengan etiologi yang belum diketahui jelas sampai sekarang. Diduga terdapat hubungan antara pioderma fasiale dengan faktor imunologi, hormonal, dan vaskular. Dilaporkan satu kasus yang sepengetahuan penulis belum pernah dilaporkan di Indonesia. Perempuan usia 15 tahun dalam terapi lupus eritematosus sistemik, datang dengan erupsi pustul multipel di atas plak eritematosa disertai beberapa telangiektasis pada wajah. Kondisi ini awalnya dipikirkan sebagai suatu pioderma yang membaik dengan antibiotik sistemik  amoksisilin/asam klavulanat, namun mengalami rekurensi. Kultur dari pustul tidak didapatkan pertumbuhan isolat bakteri, dan biopsi jaringan menunjukkan sebukan sel polimorfonuklear di sekitar pembuluh darah, jaringan interstitial, dan folikel rambut. Pasien kemudian diterapi dengan kombinasi asam retinoat dan eritromisin topikal yang memberikan respons sangat baik dalam 3 minggu sejak dimulainya terapi.

Abstract

Yaws, a neglected tropical disease, is still become the health problem in several countries. World Health Organization has proposed 2030 goals for yaws eradication after the previous target has not been achieved. Indonesia is still one of the largest contributors to yaws cases in Southeast Asia. The variety of lesions and the development of atypical clinical features make the yaws diagnose as a challenge. WHO guideline is used as a reference to determine the clinical manifestations, divided into early stage (infectious) lesions to advanced (non-infectious) yaws lesions. Yaws clinical features should be mastered by health workers in Indonesia, considering that the establishment of suspected cases begin with a clinical examination then confirmed by serological test. Several recent clinical surveillance had not reported early-stage lesions as the most common clinical finding, but rather had scars which was previously removed from the modified WHO guidelines. Missed diagnosis causes continually transmission.

Abstract

Leprosy has a high incidence of physical complications that will impact directly the physiological, economic, and social status of patients. Indonesia has a high burden of the disease, and several leprosy settlements are still spread in the country. Neglasari Village, Sitanala, Tangerang serves as one of the places of residence of people who have had leprosy. A health service initiative known as Identifikasi Tanda-Tanda Mata, Ekstremitas, dan Kulit pada Kusta (KATAMATAKU) was launched as a collaboration of health services among multi-departments (ophthalmology, dermatovenereology, and medical rehabilitation). Sitanala has a relatively high incidence of people who have had leprosy with disabilities of the hand, foot, and eye. As a continuation of this health service and combined with the efforts to improve the welfare of leprosy patients, in November 2019, a multidisciplinary program titled KATAMATAKU Universitas Indonesia, was conducted. This program aimed to determine the demographic data regarding the health, psychological, social, and economic status of the leprosy population at Sitanala The project consisted of a collaborative anti-stigma program by the Faculty of Psychology, Public Health, Social and Political Sciences, Cultural Sciences, Administrative Sciences, and Vocational Educational Program; thematic health program, which supports the improvement of physical abilities and empowerment of former leprosy patients, by the Faculty of Medicine, Dentistry, Nursing, and Pharmacy; thematic economics program, which aims to increase the economic capacity of the leprosy community, by the Faculty of Economics and Business, Mathematics and Natural Sciences, and Engineering. This program enabled the construction of a multidimensional management model, in which every aspect plays important roles to improve the patients’ quality of life.
