Abstract

Indonesia masih merupakan negara endemis kusta terbanyak ketiga didunia. Tim KATAMATAKU telah melakukan program kesehatan pada penderita kusta dan komunitasnya selama tiga tahun terakhir. Sebagai generasi penerus bangsa, kesehatan mata anak di lingkungan tersebut perlu diperhatikan. Deteksi gangguan mata anak dan tatalaksananya harus sedini mungkin dilakukan. Pandemi Covid-19 di Indonesia berdampak pada banyak aspek. Pembelajaran dengan system daring/online membuat anak menatap layar digital lebih lama sedangkan kegiatan diluar rumah dibatasi, hal ini meningkatkan gangguan penglihatan. Pemeriksaan kesehatan mata anak di Indonesia masih jarang dilakukan secara rutin. Tujuan dari pengabdian masyarakat ini adalah untuk mendeteksi gangguan mata pada anak di lingkungan sosio-ekonomi rendah, komunitas kusta, Tangerang saat pandemi covid 19. Metode pelaksanaan kegiatan dengan penelusuran riwayat kesehatan, pemeriksaan ketajaman visual dengan chart dan autorefraktometri, segmen anterior, dan segmen posterior. Pemeriksaan dilaksanakan 12-13 Desember 2020 melibatkan total 103 anak dengan median usia 11 (4 - 18) tahun. Rasio laki-laki dan perempuan adalah 1: 1,2. Mayoritas anak yang terpapar digital screen time lebih dari 2 jam perhari sebesar (80/103, 77.7%) dengan (11/103, 10.7%) anak mengalami gangguan refraksi.  Waktu menatap layar gadget yang lama, seringnya melihat dalam jarak dekat dan kegiatan diluar ruangan yang kurang, dapat berpotensi meningkatkan progresivitas gangguan refraksi. Meskipun dengan banyak keterbatasan karena pandemi, screening kesehatan mata anak tetap perlu dilakukan dengan melakukan modifikasi kegiatan sesuai protokol kesehatan. Kegiatan ini diharapkan dapat meningkatkan pemahaman anak dan orang tua terkait pentingnya pemeriksaan mata pada anak secara rutin sejak dini dan dapat menjadi contoh pelaksanaan skrining kesehatan di masa pandemi. Kata kunci: COVID-19, daring, digital screen time, tajam penglihatan

Abstract

Background: To evaluate and compare the clinical outcome and safety of Cultivated Limbal Epithelial Transplantation (CLET) and Cultivated Oral Mucosal Epithelial Transplantation (COMET) in severe ocular surface disease.Methods: Retrospective literature review. Studies reporting CLET or COMET as treatment of ocular surface disorder were included in this review. All studies were level IV or higher published between 2002 and 2012. The outcomes evaluated in each study include transplantation success rate, improvement of visual acuity, and the safety covered post operative complications. No publication data restriction was used.Results: Eight articles were reviewed. Steven-Johnson Syndrome (SJS) was found to be the most common cause of ocular surface disease in all studies. None of these studies compared CLET and COMET directly. All studies achieved success rate and 3-years survival rate of more than 50%. Visual acuity improvement ranged between 43.5-67.8%, while the mean duration unitl epithelization ranged between 13.7 days to 3 months. The most common complication was persistent epithelial defect (PED).Conclusion: Both CLET and COMET offer a viable and safe alternative in ocular surface reconstruction, and thus may be considered as the management of the ocular surface disorder following SJS or chemical injury. Keywords: Cultivated limbal epithelium transplantation, cultivated oral mucosal epithelium transplantation, ocular surface disorder, ocular surface disease

Abstract

Purpose : To evaluate the efficacy and safety of cationic emulsion and Carboxymethyl-Cellulose (CMC) 0,5% eye drops in post-menopausal women with dry eyeMethods : A randomized, double-masked clinical trial was conducted in polyclinic Ophthalmology Department during March-April 2009. Forty-six post menopausal women with dry eye were included in the study. The subjects were randomized into two groups. Group 1 received cationic emulsion and group 2 received CMC 0,5%. Tear Film Break-Up Time (TFBUT), Inter-Blink Interval (IBI) and Ocular Protection Index (OPI) were evaluated before treatment, day 1, 7 and 14 after treatment. The dry eye symptom severity score was evaluated before treatment, day 7 and 14 after treatment with Ocular Surface Disease Index (OSDI) questionnaire Results : The cationic emulsion had significantly better OPI score and TFBUT value on day 1, 7 and 14 compared to CMC 0,5% group (p<0,001). There were no significant differences between groups on IBI value between groups on day 7 and 14 (p>0,05). The severity symptom score on cationic emulsion group was significantly lower compared to CMC 0,5% on day 14 (p<0,001). The subjective complaints after using both eye drops were visual disturbance, foreign body sensation, and itchy eyes. All complaints were only reported first follow-up (day 7). Conclusion : Cationic emulsion in this study had a clearly beneficial effect both in increasing OPI score and lowering subjective symptom score in the treatment of dry eye for post menopausal women.

Abstract

Introduction & Objectives : Essential blepharospasm is a neurological condition that is defined by uncontrolled and abnormal eyelid closure from a central cerebral disorder. Between 40%-85% of blepharospasm occurs in conjunction with dry eye symptoms. Botulinum neurotoxin (BoNT) injections are commonly used treatment for reducing spasms in blepharospasm. Furthermore, it is often suggested for dry eye therapy due to their ability to reduce lacrimal drainage. The purpose of this review is to assess the effectiveness of BoNT treatment for blepharospasm with dry eye symptoms. Methods : A literature research was conducted on online databases such as PubMed, Cochrane library and Google scholar. All studies of BoNT and dry eye with full-text journals published in English within the last 10 years were included. We assessed Tear Breakup Time (TBUT), Schirmer Test, and Blepharospasm Disability Index (BSDI). Results : 204 patients from 6 publications were analyzed. In five trials, the BoNT treatment statistically significant increased TBUT (p<0,5), with mean final TBUT 5,7 ±1.1 s. Furthermore, the Schirmer test also increased with the mean final score 6,29 ± 4,6 mm. The mean BSDI score dropped by 6,05 ± 3,7 in two studies. Conclusion : It is confirmed that BoNT injection is an effective treatment for essential blepharospasm with dry eye symptoms. Clinicians should be screened for dry eye in all blepharospasm patients prior to BoNT treatment.

Abstract

Abstract Introduction : Ocular syphilis is a sight-threatening condition caused by spirochete infection, Treponema pallidum, with varied clinical presentations. Although uncommon, human immunodeficiency virus (HIV) infection increases the risk of ocular syphilis. If left untreated, it may lead to blindness. The disease can be treated effectively with appropriate antibiotic therapy from a multidisciplinary department. Therefore, early investigation and timely management can prevent infection progression and save patients’ eyesight. Case Illustration : A 29-year-old man complained of blurry vision with visual acuity light perception in both eyes one month before admission. The patient underwent examination and laboratory workup and was later diagnosed with HIV and ocular syphilis. A multidisciplinary approach was carried out with dermato- venerology, neurology, and internal medicine departments to treat the ocular and systemic infection. The patient was medicated with the antibiotic regimens Benzathine Penicillin 7.2 million IU and Ceftriaxone 1 g for 14 days. Visual acuity was later examined three weeks with a significant result for best corrected visual acuity (BCVA) in decimal 0.8 for the right eye and 0.67 for the left eye. Discussion : The patient was in poor eyesight condition when initially examined. According to reports, syphilis could be cured if it was diagnosed early and completely treated. Therefore, an immediately established diagnosis helped the patient get suitable therapy thus improving visual acuity. Conclusion : Early investigation and proper treatment are critical to managing systemic and ocular infections of HIV and syphilis. A multidisciplinary approach is required to achieve optimal results.

Abstract

BACKGROUND Tuberculous uveitis (TU) is the second most common cause of intraocular inflammation in Indonesia. This study aimed to determine the clinical characteristics of patients with TU and the therapies used at Jakarta Eye Center (JEC) Eye Hospitals and Clinics. METHODS This retrospective descriptive study included 265 eyes of 189 patients diagnosed with TU at the JEC Eye Hospital and Clinics between January 2018 and June 2022. This study used secondary data collected by reviewing patient medical records, including clinical profiles and therapeutic characteristics recorded during treatment. Data were analyzed using SPSS software version 24 for Windows. RESULTS Most patients (68.3%) complained of blurry vision, and 99 (52.4%) underwent an interferon-gamma release assay. HIV infection was present in 8.5% of the patients, and 4.8% had diabetes mellitus. Among them, 27 (14.3%) had posterior uveitis, 34 (18.0%) had anterior uveitis, and 127 (67.2%) had panuveitis. The most common feature of inflammation was vitreous opacity. Visual acuity (VA) improved to 0.4 (1.11) after treatment. The most common complication was cataract formation (71 patients). In the group that received combination therapy with systemic steroids and anti-tuberculosis treatment (ATT), 58.7% had a better VA. CONCLUSIONS TU has several clinical manifestations. Treatment with systemic steroids, ATT, or a combination of both can improve VA.

Abstract

Background: In cases of alkali burn, blood serum eyedrops may supply protein and growth factors. A standard preparation protocol is necessary for consistency in epitheliotropic capacity. Objective: We aim to evaluate how post-alkali-burn administration of blood serum eyedrops from different preparation protocols results in different outcomes on rabbits. Methods: This randomized experimental study used three treatments: placebo, non-diluted serum, and 25% concentration serum. We analyzed the corneal haziness, neovascularization degree, histopathological examination of neovascularization scores, and alpha-smooth muscle actin (α-SMA) expression. Results: Twelve rabbits (24 eyes) were split evenly into three treatment groups. On day 14, the group receiving the 25% concentration serum had the least corneal haziness (p = 0.005). No adverse effect was observed on the treated eyes. Histopathologically, no significant difference in neovascularization scores and α-SMA expression was observed. Conclusions: Treatment using the 25% concentration serum resulted in less corneal haziness, smaller defect size, and greater healing rate. There was also no significant difference in histopathological outcomes among the three treatment groups.

Abstract

Introduction: The COVID-19 pandemic has forced many conventional -face-to-face health promotion events to be conducted in an adaptive manner to keep healthcare in promotion, prevention, and even curative measures delivered to healthcare providers and patients. The same applies to leprosy, where Indonesia is still ranked as the third largest country with leprosy burden. This study aims to give rise to newer methods of health promotion on leprosy during this COVID-19 pandemic. Methods: Online training and case presentation workshop in the form of a KATAMATAKU webinar [MOU1] and live streaming via YouTube were held in August 2020. There were 120 general practitioners, 30 nurses, and 35 healthcare providers who registered and joined this event.[MOU2]  Pre- and post-test [MOU3] were conducted to evaluate the participants' knowledge about the topics lectured during this training[MOU4] . Pre- and post-test were conducted using an online form, and all participants needed to answer several questions from each session. Participants were evaluated before the webinar began and after all sessions. We assessed the different scores from the pre- and post-test. Results: In the general practitioner group, median scores were 68 (27 – 95) and 82 (50 – 100) for pre- and post-test scores, respectively, with a mean difference between post and pre-test of 14.23+9.72. In nurses and other health care providers, the median for the pre-test score was 56 (22-89), and the post-test was 72 (39 – 100), with a mean difference between the post and pre-test of 18.93+10.27. Conclusion: In the pandemic situation, online seminars can be chosen as an effective method for health promotion to increase health care provider's knowledge. Pre- and post-test can be performed to evaluate participant's knowledge after the training. Regular training using online methods can be held in order to maintain participants' knowledge. We hope it can impact all health providers to continue their participation in the management of leprosy.

Abstract

Leprosy has a high incidence of physical complications that will impact directly the physiological, economic, and social status of patients. Indonesia has a high burden of the disease, and several leprosy settlements are still spread in the country. Neglasari Village, Sitanala, Tangerang serves as one of the places of residence of people who have had leprosy. A health service initiative known as Identifikasi Tanda-Tanda Mata, Ekstremitas, dan Kulit pada Kusta (KATAMATAKU) was launched as a collaboration of health services among multi-departments (ophthalmology, dermatovenereology, and medical rehabilitation). Sitanala has a relatively high incidence of people who have had leprosy with disabilities of the hand, foot, and eye. As a continuation of this health service and combined with the efforts to improve the welfare of leprosy patients, in November 2019, a multidisciplinary program titled KATAMATAKU Universitas Indonesia, was conducted. This program aimed to determine the demographic data regarding the health, psychological, social, and economic status of the leprosy population at Sitanala The project consisted of a collaborative anti-stigma program by the Faculty of Psychology, Public Health, Social and Political Sciences, Cultural Sciences, Administrative Sciences, and Vocational Educational Program; thematic health program, which supports the improvement of physical abilities and empowerment of former leprosy patients, by the Faculty of Medicine, Dentistry, Nursing, and Pharmacy; thematic economics program, which aims to increase the economic capacity of the leprosy community, by the Faculty of Economics and Business, Mathematics and Natural Sciences, and Engineering. This program enabled the construction of a multidimensional management model, in which every aspect plays important roles to improve the patients’ quality of life.
