Abstract

Insiden preeklampsia dan eklamsia dapat menyebabkan kematian pada ibu hamil, dan pada wanita dengan riwayat keluarga menderita preeklampsia lebih berisiko dibandingkan dengan wanita tanpa riwayat keluarga dengan preeklampsia. Penelitian terdahulu mengindikasikan bahwa gen Storkhead Box 1 (STOX1) merupakan salah satu penyebab preeklampsia. Penelitian ini bertujuan untuk menganalisis polimorfisme varian T-344C gen STOX1 dalam kromosom 10q22 sebagai kandidat biomarker preeklampsia. Penelitian dilaksanakan secara kuantitatif observasional dengan desain cross-sectional pada sampel darah wanita hamil dengan preeklampsia (kelompok kasus) dan darah wanita yang hamil normal (kelompok kontrol) di kamar bersalin SMF Obstetri dan Ginekologi RSUD Dr. Saiful Anwar Malang. Sampel darah vena diambil dari 25 wanita hamil dengan preeklampsia dan 25 wanita hamil normal yang memenuhi kriteria penelitian dan dilakukan ekstraksi DNA. Identifikasi gen STOX1 dilakukan dengan metode  PCR dengan forward primer adalah 5’-TGGGTGTGAA-3’ dan reversed primer 5’-TTGGAGCGTTTGATGAAACA-3’ (First Base, USA) dan analisis varian T-344C dengan RFLP menggunakan enzim HaeIII. Data dianalisis secara statistik dengan aplikasi SPSS. Amplifikasi gen STOX1 dengan PCR pada kelompok preeklamsia dan kontrol menunjukkan  produk sebesar 639 bp. Analisis polimorfisme varian T-334C pada kelompok normal memiliki frekuensi TT 64%, TC 28%, dan CC 8%. Sementara pada kelompok preklampsia frekuensi TT 72%, TC 28%)dan tidak ditemukan adanya polimorfisme CC yang bersifat homozigot. Tidak ditemukan hubungan yang signifikan antara polimorfisme varian T-334C gen STOX1 dengan preeklampsia (Uji Mann Whitney p = 0,668 > 0,05). Kesimpulanya, secara teknis pemeriksaan varian T-344C gen STOX1 dapat digunakan sebagai kandidat biomarker preeklamsia, namun secara spesifik tidak sensitif untuk menjelaskan adanya hubungan antara polimorfisme varian T-344C gen STOX1 dengan preeklampsia.

Abstract

Antenatal care (ANC) is planned to prepare for a safe and comfortable delivery. Assessment of the quality of care received by patients is used as a guide to determine the best and weakest dimensions of service quality. Service quality aspects are evaluated based on five quality service dimensions, including tangible, reliability, responsiveness, assurance, and empathy. The purpose of this study is to analyze the relationship between the five dimensions of ANC service quality and patient satisfaction and revisit interest. The method in this study uses a quantitative research design with a cross-sectional approach, with a sample size of 86 pregnant mothers. Sampling was done using accidental sampling. Data were analyzed using chi-square. The results show that the tangible dimension has the strongest influence on satisfaction (OR 104.50) and revisit interest (OR 41.08) of patients in the KIA clinic at Puskesmas Gondanglegi. The conclusion from this study is the existence of a relationship between the dimensions of ANC service quality, including tangible, reliability, responsiveness, assurance, and empathy, with patient satisfaction and revisit interest in the maternal-children clinic at Gondanglegi primary health center.

Abstract

Objective: To evaluate the effects of calcium on the Sacrouterine Ligament by evaluating the expression of calmodulin and bFGF/FGF-2.Method: A true experimental study was carried out by assessing the cell cultures of sacrouterine ligament from female patients who underwent hysterectomy. The cell cultures were divided into groups that were exposed to calcium at different concentrations of 100 nM, 300 nM, 500 nM, 700 nM, and control group without any exposure. The expression of calmodulin and bFGF/FGF-2 was subsequently analyzed using immunofluorescence and ELISA method.Results: This study showed that exposure to calcium significantly affected calmodulin expression (p-value <0.05). The concentration found to be the most effective to induce calmodulin expression was at 500 nM. Calsium also significantly affected the bFGF/FGF-2 expression (p-value <0.05) with the concentration found to be the most effective to induce bFGF/FGF-2 expression at 500 nM.Conclusion: This study suggested that calcium had a significant positive effect of increasing extracellular matrix expression. A further in vivo study needs to be conducted in order to enhance the evidence of the potential effects to become a preventive agent for pelvic organ prolapse. Calcium is widely available in tropical countries like Indonesia, so this preparation is considered very easy for Indonesian women to apply.Key words: Calcium, calmodulin, bFGF/FGF-2, Extracellular MatrixPengaruh Kalsium pada Ekspresi Calmodulin dan bFGF/FGF-2: Studi In Vitro pada Ligamentum SakrouterinaAbstrakTujuan: Mengevaluasi efek kalsium pada Ligamen Sakrouterina dengan evaluasi ekspresi calmodulin dan bFGF/FGF-2,Metode: Sebuah penelitian true eksperimental dilakukan dengan menilai kultur sel ligamentum sakrouterurina dari pasien wanita yang menjalani histerektomi. Kultur sel dibagi menjadi beberapa kelompok yang diberi paparan kalsium pada konsentrasi yang berbeda yaitu 100 nM, 300 nM, 500 nM, 700 nM, dan kontrol tanpa paparan. Ekspresi calmodulin dan bFGF/FGF-2 selanjutnya dianalisis menggunakan metode ELISA dan imunofluoresensi.Hasil: Penelitian ini menunjukkan bahwa paparan kalsium berpengaruh nyata terhadap ekspresi calmodulin (p-value <0,05). Konsentrasi yang paling efektif untuk menginduksi ekspresi calmodulin adalah pada 500 nM. Kalsium juga berpengaruh nyata terhadap ekspresi bFGF/FGF-2 (p-value <0,05), dengan konsentrasi yang paling efektif untuk menginduksi ekspresi bFGF/FGF-2 adalah pada 500 nM.Kesimpulan: Penelitian ini menunjukkan bahwa kalsium memiliki efek positif yang signifikan terhadap peningkatan ekspresi matriks ekstraseluler. Penelitian in vivo lebih lanjut perlu dilakukan untuk meningkatkan bukti potensi efek sebagai agen pencegahan prolaps organ panggul. Kalsium banyak tersedia di negara tropis seperti Indonesia, sehingga olahan ini dinilai sangat mudah diterapkan untuk wanita Indonesia.Kata kunci: Kalsium, Calmodulin, bFGF/FGF-2, Matriks Ekstraseluler

Abstract

Polycystic ovary syndrome (PCOS) and obesity are prevalent conditions that independently impair fertility and increase the risk of adverse pregnancy outcomes. When coexisting, they present compounded challenges for conception and pregnancy management. We report a case of a 27-year-old primigravida with PCOS and class II obesity who achieved spontaneous conception after three years of infertility and multiple failed assisted reproductive attempts. Preconception lifestyle modifications and medical management improved insulin sensitivity and restored ovulatory function. Despite being categorized as a high-risk pregnancy, the antenatal course was unremarkable, with appropriate fetal growth and no major maternal or neonatal complications. Vaginal delivery occurred at term, resulting in a healthy newborn. This case highlights the potential for favorable reproductive and obstetric outcomes in women with PCOS and obesity through early diagnosis, lifestyle intervention, and meticulous prenatal care. It underscores the importance of individualized and multidisciplinary management strategies in optimizing maternal and fetal health. Spontaneous conception and favorable pregnancy outcomes are achievable in women with PCOS and obesity through comprehensive care. Early intervention and tailored monitoring play a crucial role in mitigating risks associated with these conditions.
