Abstract

Latar Belakang: Kontrasepsi hormonal pil, suntik dan implan merupakan jenis kontrasepsi yang banyak diminati oleh pasangan usia subur di Indonesia. Ketiga jenis kontrasepsi ini mengandung hormon seks sintetis berupa esterogen dan progesteron yang dapat meningkatkan cairan sulkus gingiva dan memicu pelepasan sitokin TNF-? sehingga mengakibatkan gingivitis. Tujuan Penelitian: untuk mengetahui perbedaan kadar Tumor nekrosis alpha (TNF-?) cairan sulkus gingiva pada penderita gingivitis pengguna kontrasepsi pil, suntik dan implan. Metode Penelitian: Penelitian ini menggunakan desain penelitian observasional analytic dengan rancangan penelitian cross sectional. Subyek penelitian sebanyak 60 wanita dengan usia 25-28 tahun yang terbagi dalam 20 kelompok pil, 20 suntik dan 20 implan di kecamatan Kebonarum Klaten. Penelitian ini diawali dengan pengukuran gingival indeks kemudian pengambilan cairan sulkus gingiva menggunakan absorbent paper point lalu dilakukan penghitungan kadar TNF-? menggunakan metode ELISA. Hasil Penelitian: Hasil peneltian dengan uji statistic didapatkan tidak terdapat perbedaan yang bermakna (p>0,05). Meskipun demikian, rerata kadar TNF-? cairan sulkus gingiva penderita gingivitis sedang pengguna kontrasepsi implan lebih tinggi dibandingkan dengan kontrasepsi pil dan suntik. Kesimpulan: Berdasarkan penelitian ini dapat disimpulkan bahwa tidak terdapat perbedaan kadar TNF-? cairan sulkus gingiva penderita gingivitis pengguna kontrasepsi pil, suntik dan implan.

Abstract

The severity of periodontitis on patients with type 2 Diabetes Mellitus patients was strongly thought caused by decreasing of leukocytes function such as monoctyes and neutrophils. In our previous research it was found that calprotectin (MRP8/MRP14) level in leukocytes of periodontitis patients with type 2 DM was higher than periodontitis in non DM. The aim of this study was to determine calprotectin (MRP8/MRP14)mRNA expression in human monocytes of periodontitis patients with type 2 DM and without DM. Monocytes were isolated from the peripheral blood of periodontitis patients with uncontrolled type 2 DM, controlled type 2 DM, and non DM. The expression of total RNA calprotectin (MRP8 and MRP14) were detected by RTPCR using GAPDH as the innate control. It was observed that the value of MRP8/MRP14 mRNA expression DM patients were higher than non DM, and the highly significant increase expression (p<0.05) was on the uncontrolled type 2 DM. The basal level of MRP8/MRP14 expression increased in monocyte of periodontitis and type 2 DM patients compared with non diabetes subjects. It was suggested that high basal level MRP8/MRP14 has role in the regulation of severity periodontitis with diabetes mellitus.

Abstract

Latar belakang. Diabetes Melitus (DM) merupakan penyakit sistemik yang sangat terkait dengan meningkatnya keparahan penyakit periodontal. Pada penderita DM diduga kuat bahwa keparahan periodontitis disebabkan oleh gangguan fungsi leukosit. Sel neutrofil (PMN) diketahui berperan besar di dalam system pertahanan jaringan periodontal. Kontrol glikemik pada penderita DM dapat diketahui dengan pemeriksaan level HbA1c. Tujuan. Penelitian ini untuk mengetahui pengaruh level HbA1c pada penderita periodontitis diabetika terhadap fungsi fagositosis sel PMN. Bahan dan Cara. Sel PMN dari darah tepi 15 pasien periodontitis terdiri 5 periodontitis tana DM, 5 periodontitis diabetika terkontrol (HbA1c<7) dan 5 periodontitis diabetika tidak terkontrol (HbA1C>7) diuji aktivitas fagositosinya dengan partikel lateks. Fungsi fagositosis dihitung dengan rumus indeks fagositosis (IF). Hasil. Terdapat perbedaan significant IF sel PMN subyek periodontitis diabetika tidak terkontrol (HbA1c>7) dengan periodontitis non DM. IF tertinggi pada subyek periodontitis non DM dan tertinggi pada periodontitis diabetika tidak terkontrol (HbA1c>7). Kesimpulan. Level HbA1c berpengaruh terhadap fungsi fagositosis sel PMN yang dilihat dari indeks fagositosis (IF).  Background. Diabetes mellitus is a systemic condition that has long been associated with an increased and severity of periodontal disease. The severity of periodontitis in diabetic patients was thought caused by decreation of leukocytes function. Polymorphonuclear leukocytes (PMN) play a key role in the maintenance of gingival (HbA1c) level. Aims. The objective of this study was to analyse the influence of HbA1C level to fagositosis index on diabetic periodontitis patients. Material and Methods. PMN were taken from peripheral blood of 15 periodontitis patients consisted of 5 subject with uncontrolled DM (HbA1c>7), 5 subject with controlled DM (HbA1c<7), and 5 subject non DM as control. Phagocytosis function were determined using latex particle and coaunted by phagocytosis index (PI). Results. These analysis revealed a significant (p<0.05) different of PI between diabetic periodontitis who have HbA1c>7 and periodontitis non diabetic subjects. The higest PI was on diabetic periodontitis with HbA1c level >7 and the lowest PI was on non diabetic subjects. Conclusion. Level of HbA1c on diabetic periodontitis patient influence to phagocytosis function of PMN.

Abstract

Guide tissue regeneration (GTR) is the treatment of infrabony pockets for soft and hard tissue regeneration. Membrane is used as a barrier and prevents apical migration of the cells in epithelial tissues. Membrane fixation is one of the procedures in GTR treatment because resorbed membrane is less stable. Simple sling suture technique for membrane fixation has 1 anchorage, located in coronal flap, while periosteal vertical mattress suture technique has 2 anchorages in periosteum, making it more stable. This study aimed to study the differences in the effectiveness of membrane fixation using periosteal vertical mattress suture and simple sling suture techniques in terms of probing depth, relative attachment loss, and alveolar bone height in the treatment of infrabony pockets. The samples were divided into 2 groups. The first group was open flap debridement (OFD) with demineralized freeze dried bone allograft (DFDBA) application and membrane fixation with simple sling suture, while the second group was OFD with DFDBA application and membrane fixation with periosteal vertical mattress suture, observed on day 0, 30th day, and 90th day. The results of the study showed significant differences in the probing depth and relative attachment loss (except from the 30th day to the 90th day), and there were no significant differences in the alveolar bone height from the baseline to the 90th day, between the group of membrane fixation using simple sling suture and that of periosteal vertical mattress suture techniques on flap surgery. This study concluded that membrane fixation in the treatment of infrabony pocket with periosteal vertical mattress suture technique is more effective in reducing the probing depth and relative attachment loss, but there is no difference in increasing the alveolar bone height when compared to simple sling suture technique.

Abstract

Carbonated hydroxyapatite is ideal as a bone graft material because it has similar organic matters to the bone, excellent osteoconductive properties, and good biodegradation in the body. Hydroxyapatite contains the risk of being contaminated by bacteria called Porphyromonas gingivalis (P. gingivalis) in the oral cavity because it has no vascularization, therefore, facilitating adhesion of bacteria, and when applied in the oral cavity, it may cause an infection that then inhibits healing. Thus, it is necessary to use a material that has an antibacterial effect with low potential of causing resistance to treat the postsurgical infection properly. Propolis has antibacterial, antiviral, antifungal, antitumor, and immunomodulatory activities. Propolis contains a large number of flavonoids and phenols. The phenol compound in propolis is usually called caffeic acid phenethyl ester (CAPE), and it has a good antibacterial property. The study aims to evaluate the antibacterial effect of carbonated hydroxyapatite when immersed with different propolis concentrations of 2.5%, 5%, 7.5%, and 10% for 24 h and to measure the zone of inhibition against P. gingivalis. The Kruskal–Wallis test resulted in p = 0.00 (p < 0.05), indicating that there were significant differences among the test groups. The data processing was followed by Mann–Whitney U-test, and the results showed a significant difference in the group of carbonated hydroxyapatite-10 % propolis compared with the other groups. Inhibition zone of carbonated hydroxyapatite that immersed with propolis 10% showed the largest mean of diameters zone of inhibition.

Abstract

Periodontitis can lead to the destruction of the alveolar bone. The loss of the alveolar bone can be treated using carbonated hydroxyapatite (CHA) as a bone graft material. However, CHA is an alloplastic graft whose primary function is to act as a scaffold, but it is unable to stimulate the process of bone regeneration. Carbonated hydroxyapatite is an avascular synthetic material, which will increase the risk of bacterial adhesion on site that can lead to unsuccessful periodontal therapy. The incorporation of propolis into CHA is expected to add antibacterial capability into CHA. Besides its antibacterial property, propolis also has a bone regenerating effect. Mixing CHA with propolis needs to consider the process of loading the active ingredients into the carrier. The release of propolis is expected to occur gradually over a lengthy period. The purpose of this study was to analyze the loading and releasing assay for propolis incorporated with CHA. A propolis solution of 5%, 7.5%, and 10% was each incorporated into 10 mg of CHA. The loading percentage and releasing assay of propolis were measured. The absorbance reading was done at 289 nm using a UV-vis. It was shown that a 10% propolis solution had the highest loading percentage (32.08%), while the 5% propolis solution had the smallest loading percentage (10.63%). The propolis releasing profiles in all concentration groups were similar. The difference in propolis concentration incorporated with CHA affected the loading percentage but did not affect the propolis releasing assay.

Abstract

Background: Amelogenin is a major protein constituent of the developing enamel matrix that is critical for enamel formation. Mutations of amelogenin cause hypoplastic enamel phenotypes. Previous research found that infant of diabetic mother has higher risk for having enamel hypoplasia. Purpose: The aim of this study was to determine the effect of gestational diabetes mellitus on the expression of amelogenin in Wistar rats offspring tooth germ. Methods: Sixteen female Wistar rats, aged 2.5-3 months, body weight 150-200 g were used in this study, Wistar rats were mated and divided into two groups and treated on day 0 of pregnancy. Group A was DM group, consisting of 8 rats, induced by streptozotocin (STZ) injection 40 mg/kg BW. Group B was control group, consisting of 8 rats received citrate buffer injection. Thirty-two rat pups were decapitated on day 5. Immunohistochemical procedures were performed on molar tooth germ of the mandibular rat pups using antibody anti-AMELX to determine the expression of amelogenin. Examination carried out on the images using ImageJ software. All data were then statistically analyzed by Mann Whitney test. Results: There was no significant difference in the expression of amelogenin in the DM group and control group (p>0.05). Conclusion: Gestational diabetes mellitus did not affect the expression of amelogenin in rat offspring tooth germ. Further study is needed to examine the pattern of amelogenin expression with measurement of glucose levels of rat pups.Latar belakang: Amelogenin merupakan protein terbanyak pada matriks email yang berperan penting dalam pembentukan email. Mutasi pada amelogenin dapat menyebabkan email menjadi hipoplastik. Penelitian sebelumnya menunjukkan bahwa anak yang dilahirkan oleh ibu pengidap diabetes memiliki resiko lebih tinggi untuk mengalami hipoplasia email. Tujuan: Penelitian ini bertujuan untuk meneliti pengaruh diabetes mellitus gestasional terhadap ekspresi amelogenin pada benih gigi anak tikus Wistar. Metode: Enam belas ekor tikus Wistar betina, umur 2,5-3 bulan, berat badan 150-200 g digunakan dalam penelitian ini, dikawinkan kemudian dibagi menjadi dua kelompok dan diberi perlakuan pada kehamilan hari ke-0. Kelompok A merupakan kelompok diabetes mellitus, terdiri atas 8 ekor tikus diberi perlakuan diabetes mellitus dengan injeksi streptozotocin (STZ) 40 mg/kg BB. Kelompok B merupakan kelompok kontrol, terdiri atas 8 ekor tikus diberi injeksi buffer sitrat. Tiga puluh dua anak tikus yang lahir didekapitasi pada hari ke- 5. Dilakukan prosedur imunohistokimia pada benih gigi molar rahang bawah anak tikus menggunakan antibodi anti-AMELX untuk mengetahui ekspresi amelogenin. Pemeriksaan dilakukan pada hasil foto menggunakan software ImageJ. Hasil kemudian dianalisa menggunakan uji Mann Whitney. Hasil: Tidak terdapat perbedaan yang bemakna pada ekspresi amelogenin kelompok kontrol dan kelompok diabetes mellitus (p>0,05). Simpulan: Diabetes mellitus gestasional tidak mempengaruhi ekspresi amelogenin pada benih gigi anak tikus. Diperlukan penelitian lebih lanjut untuk mengetahui pola ekspresi amelogenin dengan pengukuran kadar glukosa darah anak tikus.

Abstract

Background: Calprotectin, a major cytosolic protein of leukocytes, is detected in neutrophils and monocytes/machrophages. This protein is known to be a marker for several inflammatory diseases including periodontitis. In type 2 diabetes mellitus patients, the severity of periodontitis was strongly thought to be caused by decreasing of leukocytes function such as neutrophils. Previous research found that the calprotectin level in serum of periodontitis patients with type 2 DM is higher than periodontits patients non DM. Purpose: The aim of this study was to determine calprotectin mRNA (MRP8/MRP14) expression in human neutrophils of periodontitis patients with type 2 diabetes mellitus. Methods: Neutrophils were isolated from the peripheral blood of periodontitis patients with uncontrolled type 2 DM, controlled type 2 DM, and non DM. The expression of calprotectin mRNA (MRP8 and MRP14) were detected by RTPCR. Result: The result showed that the value of mRNA calprotectin expression in DM patients were higher than non DM, and the highest expression was on the uncontrolled type 2 DM. Conclusion: The basal level of calprotectin mRNA MRP8/MRP14 expression increased in neutrophil of periodontitis patient with type 2 DM compared non diabetic subjects. It was suggested that high basal level of calprotectin mRNA has a role in the regulation of periodontitis severity with diabetes mellitus patients.

Abstract

Background: The root cementum demineralization is an important step in regenerative periodontal therapy to smear layer removal on the root surface. Smear layer on the root surface becomes a barrier of the new attachment between periodontal tissues with the root surface. The use of tetracycline capsules as root surface demineralizing agent cannot be applied directly on the root surface and solvents such as saline or sterile water are needed. Purpose: The aim of this study was to determine differences between sterile water and saline solvent for tetracycline HCl (tetra HCl) as a cementum demineralization. Method: In this study the specimens were divided into three groups: a control, tetra HCl dissolved in saline, and tetra HCl dissolved in sterile water. Application using burnishing method for 3 minutes. Samples were dehydrated with ethanol series of 30% to 100%. Results of the root demineralization observed by scanning electron microscopy (SEM). Statistical analysis was performed using the Kruskal-Wallis followed by a Mann-Whitney nonparametric test. Result: Upon statistical analysis showed that the sterile water as a solvent of tetra HCl is more effective in smear layer removal and collagen structure exposure in the cementum. Conclusion: Tetra HCl dissolved in sterile water was found to be the best root cementum demineralization agent.

Abstract

Patients with Type 2 Diabetes mellitus (T2DM) were more prone to suffer from periodontitis due to unique alteration in host immune response. Application of curcumin oral gel as adjunctive therapy was expected to improve periodontal condition due to its anti-inflammatory properties. The objective of this study was to evaluate the effect of curcumin oral gel on periodontal inflammation in patients with T2DM. Sixteen periodontal pockets (n=16) from patients with T2DM were administered with curcumin oral gel using blunt cannula after periodontal curettage. Gingival Index (GI) and Periodontal Pocket Probing Depth (PPD) were evaluated a month after application. GI was assessed using visible sign of inflammation parameters including swelling, redness and bleeding upon probing. PPD was assessed using UNC-15 periodontal probe. The results were analyzed by Wilcoxon test (two related samples). The results showed that GI before treatment decreased by 51.61% a month post application. There was statistically significant difference (Sig 0.003) between GI mean before and after curcumin application. PPD decreased by 42.81% and showed statistically significant difference (Sig 0.000) by Wilcoxon Test. It is concluded that application of curcumin oral gel post curettage could decrease periodontal inflammation in patients with T2DM.
