Abstract

Renal disease is a disorder that occurs in the kidneys. Such diseases caused by various factors, such as infections, tumours, congenital disorders, metabolic or degenerative diseases, and others. Chronic renal disease usually occurs slowly and is chronic. Patients suffering from chronic renal disease require renal replacement therapy (TPG) to maintain their life. The success of this chronic renal disease therapy depends on the patient's involvement in maintaining their diet and fluids. Fluid management experienced by hemodialysis patients is essential to decrease the risk of excess fluid and is a significant factor that can determine the success of hemodialysis therapy. This research was performed aiming to analyze the effect of implementing fluid management on the excess fluid in hemodialysis patients. This study employed a quasi-experimental design through pre-test and post-test methods using a control group. There were 90 respondents involved in this research as samples, who were divided into two groups, 45 respondents in the intervention group and another 45 respondents in the control groups. These samples were selected through purposive sampling. Furthermore, the research data were collected using fluid management guide instruments, fluid intake monitoring charts, and weight recording sheets. The results revealed that there was no effect of fluid management on IDWG values between the intervention group after fluid management and the control group of hemodialysis patients (t = -1.58; p = 0.118). The results of this study can recommend that the application of fluid management with family support can be an action to decrease excess fluid in hemodialysis patients.

Abstract

Stroke is a disease that most often causes disability in the form of paralysis of the limbs, speech disorders, thought processes, memory, and other forms of disability as impaired brain function. In general, people think that stroke is a disease that only occurs in middle age and old age. In fact, 10% of strokes occur at a relatively young age (less than 45 years). But now there is also a trend in patients under 45 years of age. According to WHO in 2012, 51% of deaths from stroke worldwide are caused by high blood pressure. Based on the results of the 2013 Riskesdas, the prevalence of stroke in Indonesia increases with age. The highest cases of stroke diagnosed by health workers were 75 years and over (43.1%) and the lowest were in the age group 15-24 years (0.2%). The prevalence of stroke based on gender is higher for males (7.1%) than for females (6.8%). Based on place of residence, the prevalence of stroke in urban areas is higher (8.2%) compared to rural areas (5.7%). This research was conducted in May 2017 which was descriptive in nature with the aim of getting an overview of the factors that influence the occurrence of stroke in early adulthood at the Imelda general hospital, Indonesian workers in Medan. This research technique used a sample method of 30 respondents. The results of this study showed that the most influential risk factors for the incidence of stroke in early adulthood were lifestyle, namely eating and drinking habits (89.1%), hypertension (84.3%), family history (84.3%) , obesity (83%), smoking (67.6%), and physical activity (30%). Eating and drinking habits have a high influence, because in their daily lives they consume the foods they like and what they want to eat without thinking that these foods are at risk of causing degenerative diseases. factors that influence the occurrence of stroke in early adulthood, so that the family's knowledge of stroke patients increases.

Abstract

Nurses believe that humans are complete bio-psycho-socio-cultural and spiritual beings who respond to changes that occur, among other things, due to health problems and irregularities in meeting needs. The nurse seeks to help meet the client's spiritual needs as part of the client's overall needs, including by facilitating the fulfillment of the client's spiritual needs, even though the nurse and client do not have the same spiritual or religious beliefs. This study aims to determine the relationship between implementing aspects of nurses' spirituality and fulfilling the spiritual needs of inpatients at Imelda Hospital in Medan. The type of research used is descriptive correlation with a cross sectional study design. The research was conducted in April 2019. The population in this study was all 350 patients treated in the inpatient ward at the Imelda Workers Indonesia Hospital. The sampling technique in this research is a quota sampling technique, so the number of samples in this research is 35 people. The results of the analysis of the relationship between the application of spiritual aspects of nurses and the fulfillment of patients' spiritual needs showed that there were 25 (71.4%) respondents whose spiritual needs were fulfilled by the application of spiritual aspects of nurses well. Statistical tests using chi square obtained a p value <0.05, meaning there is a relationship between aspects of spiritual application and the fulfillment of spiritual needs. It is hoped that the results of this research can be used as a reference for Imelda Hospital in improving health services, especially the implementation/nursing care in meeting the spiritual needs of patients

Abstract

Hemodialysis is a kidney function replacement therapy that is most often used to treat cases of chronic kidney disease. Most hemodialysis therapy procedures require 12-15 hours each week or on average the procedure is carried out 3 times a week where the implementation lasts 3-4 hours, so The length of hemodialysis therapy in patients with chronic kidney disease can cause limitations for the patient. The aim of this study was to determine the relationship between the length of time undergoing hemodialysis and the functional status of chronic kidney disease patients at the Imelda Workers Indonesia Hospital, Medan.
