Abstract

Background: Autism is a condition of developmental abnormalities in social, communication and behavior aspects. Polycystic ovary syndrome and maternal diabetes during pregnancy contribute more than 50% to the risk of ASD offspring. This study aims to analyze and estimate the influence of polycystic ovarian syndrome and maternal diabetes on autism in children.Subjects and Method: The meta-analysis was carried out according to the PRISMA flow chart and the PICO model. Q: Child, I: Mother with PCOS and maternal diabetes, C: T Mother without PCOS and maternal diabetes, O: Autism. Search for articles in this study through databases that include Google Scholar, Pubmed, ScienceDirect and Sage Journal. With keywords including: Polycystic Ovarian Syndrome” AND “Maternal Diabetes” OR “Gestational Diabetes Mellitus” AND “Autism” OR “Autism Spectrum Disorder” AND “Cohort”. A full paper article with an observational cohort study, the research subject was a child, the size of the relationship used was the adjusted odds ratio, the research outcome was autism. Analysis was performed with Revman 5.3.Results: There were 13 articles with cohort designs originating from America, Denmark, England, Sweden, Israel, China with a total of 4,641,483 research samples. A meta-analysis of 13 cohort studies concluded that children of pregnant women with PCOS had a 1.36 times greater risk of developing autism than those without PCOS, and the effect was statistically significant (aOR= 1.36; 95% CI= 1.24 to 1.49; p<0.001). In addition, pregnant women with maternal diabetes have a 1.24 times higher risk of having a child with autism than those without maternal diabetes, and this effect is statistically significant (aOR= 1.24; 95% CI= 1.08 to 1.43; p=0.002).Conclusion: Pregnant women who have PCOS and maternal diabetes increase the risk of autism in children. Abstract

Background: Preterm infants often experience weak sucking reflexes due to the immaturity of the oromotor neuromuscular system. This condition leads to delays in the development of oral feeding skills, increases the risk of aspiration, and prolongs hospitalization in the NICU. Oral stimulation is a non-invasive intervention aimed at facilitating the maturation of sucking–swallowing abilities in preterm infants. Recent studies have demonstrated the effectiveness of this intervention; however, a comprehensive summary of research findings is needed to strengthen clinical recommendations. Objectives: To determine the effect of oral stimulation on improving weak sucking reflexes in preterm infants. Methods: This study employed a systematic review design using a literature review approach that included five published studies from the past five years. Results: All five studies indicated that weak sucking reflexes in preterm infants are primarily caused by neuromuscular immaturity and poor suck–swallow–breathe coordination. Oral stimulation interventions—such as PIOMI, oral motor stimulation, and non-nutritive sucking—were shown to significantly improve sucking strength, sucking frequency, physiological stability, and the ability to transition to oral feeding. All studies reported accelerated achievement of full oral feeding, and most demonstrated a reduction in NICU length of stay. Conclusion: Oral stimulation has been consistently proven effective in improving the sucking reflex of preterm infants and accelerating readiness for oral feeding. This intervention is safe, easy to administer, and recommended as part of routine NICU care to support feeding maturation and enhance clinical outcomes. Abstract

Background: Autism spectrum disorder (ASD) affects social communication and reciprocal interaction abilities in children. Maternal diabetes mellitus during pregnancy, specifically gestational diabetes mellitus (GDM), represents a potentially modifiable risk factor associated with elevated ASD risk in offspring. The aim of this study was to assess the relationship between diabetes in mothers and the risk of autism spectrum disorder (ASD) in children through a systematic review and meta-analysis. Methods: This research is a systematic review and meta-analysis of nine cohort studies involving diverse populations to evaluate the association between mothers with diabetes during pregnancy and the risk of ASD in their offspring (2015–2024) sourced from PubMed, Google Scholar, and ScienceDirect. A random-effects model was used to calculate adjusted odds ratios (aOR). Results: Data from 3,908,133 children showed that maternal diabetes increases the ASD risk by 16% (aOR = 1.16, 95% CI: 1.11–1.22, p<0.001), with significant heterogeneity (I² = 79%).  Conclusion: This meta-analysis identified a statistically significant but modest association between maternal diabetes mellitus during pregnancy and ASD risk in offspring. The relationship reflects an observational association and cannot establish causality. These findings emphasize the clinical importance of preconception glycemic optimization, enhanced antenatal monitoring of maternal glucose control, and developmental surveillance of offspring born to diabetic mothers.

Abstract

Background: Standardisation of speech therapy services ensures consistent quality and reduces performance gaps between facilities within the Indonesian healthcare system. However, compliance remains inconsistently implemented across government hospitals, creating variations in service quality and patient outcomes. This study aimed to measure the compliance level of speech therapy services in two government hospitals with Minister of Health Regulation No. 81/2014. Methods: Descriptive quantitative study was conducted at two government hospitals in Central Java. Purposive sampling was used to select five speech therapists as informants. Data were collected through structured checklists aligned with 41 parameters from the regulation. Compliance was scored numerically (1-5 per parameter) and converted into percentages. Total scores were categorized into ratings (A=164-205; B=123-163; C=82-122; D=41-81; E=0-40). Results: Hospital A scored 146/205 (71.21%, Rating B), with deficiencies in screening tools (score: 2/5), documentation validation (2/5), and staff development programs (2/5). Hospital B scored 185/205 (90.24%, Rating A), demonstrating full compliance in interdisciplinary collaboration (5/5) and documentation (5/5). Conclusion: Compliance with national standards varies significantly between hospitals. Standardization of resources and staff training is essential to improve service quality.
