Abstract

The term Neuropsychiatric systemic lupus erythematosus (NPSLE) encompasses all neurologic and or psychologic symptoms arising in patients with SLE after other causes have been ruled out. Cerebral vasculitis as a manifestation of NPSLE is a rare case with a very broad spectrum of clinical presentation and severity. We report a case of a female with cerebral vasculitis presenting with diplopia as its early manifestation. Diagnosing cerebral vasculitis remains challenging given that biopsy as the gold standard is an invasive procedure and may not be available in many settings. A high suspicion of diagnosis requires clinicians’ judgement in reviewing combination of detailed history taking, physical examination, neuroimaging, and other available supporting modalities.

Abstract

Giant cell arteritis (GCA) is an autoimmune vasculitis involving large and medium arteries. GCA israrely associated with autoimmune diseases, such as Sjögren’s syndrome. Glucocorticoids (GC) arethe cornerstone of the treatment of GCA as they are very effective but are often given for 1 year toavoid relapses. Here we report a rare case of GCA overlapping with Sjögren’s syndrome along with themanagement.

Abstract

Background: CRP is normal or only slightly increased in active SLE. It is presumed that IFN-α mayinhibit the transcription process during CRP synthesis. There is also increasing in IFN-αgene expressionin active SLE. This study examined the correlation of plasma IFN-α with CRP and SLE activity.Methods:Forty SLE patients were included. SLAM and SLEDAI were used to measure SLE diseaseactivity. Laboratory tests were examined at dr.Soetomo Hospital Surabaya. CRP was measured usingimmunoturbidimetry. C3 and C4 were measured by radial immunodiffusion technique. IFN-αwasmeasured using ELISA.Results and conclusion:Twenty-six patients from the outpatient clinic and 14 from wards wereincluded from August 2019 to February 2020. The median age was 31.5 years old. The median SLAMscore was 8.5. Mean CRP was 5.19±2.69 mg/L. Median plasma IFN-α was 46.02 (16.43-177.96).Spearman correlation test revealed a moderate negative correlation between plasma IFN-α and CRPlevel (p=0.003; r=-0.455). A moderate positive correlation was showed between plasma IFN-α leveland SLAM score (p=0.001; r=0.568). No correlation found between CRP and SLAM. There was astrong correlation between complement levels with SLEDAI. Linear regression revealed a significantassociation of IFN-α and C3 (not C4) level with SLEDAI.

Abstract

Henoch schonlein purpura is a rare incidence case and almost similar to the clinical symptoms of autoimmunediseases such as systemic lupus erythematus in young adult woman. A 23-year old woman patient presentedwith reddish spots accompanied by a burning sensation and pain in both legs, vomiting profus and abdominalpain. She also complained mild fever, frequent mouth sores, hair loss, joint pain and stiffness, frequentfatigue in the last 1 month. Laboratory tests showed mild anemia, leukocytocis, proteinuria, erythrocytes andleukocytes microscopic in urinalysis, increased of serum IgA and LED. The skin biopsy showed neutrophilcell infiltration in the capillary vessel walls with necrotic debris. Methylprednisolone oral was given since shehospitalized and decreased gradually. This case hada good prognosis because the result of serum creatininewas normal insix months latereven though had moderate symptoms.

Abstract

Upper extremity deep vein thrombosis is less common than in the lower extremity site. Such conditions can appear primarily or secondary to other conditions such as thrombophilia. One of the unique forms of acquired autoimmune thrombophilia is antiphospholipid syndrome (APS). We describe a 25 years old female admitted with sudden swelling, redness, and pain in her left arm. Venous ultrasound confirmed the diagnosis of thrombosis in the left subclavian vein, left axillary vein, left proximal brachial vein, and left proximal basilic vein. The patient was known to have a spontaneous miscarriage in the second pregnancy at eight weeks of gestation. Screening for autoimmune and antibody phospholipid was done, and primary APS was confirmed. She has been treated with a subcutaneous injection of fondaparinux 2.5 mg for five days and oral rivaroxaban 15 mg twice daily for 21 days. But four months later, the patient came with a thrombus in the subclavian vein due to inadequate treatment, then long-term treatment with vitamin K antagonist warfarin proceeded.

Abstract

Systemic lupus erythematosus (SLE) has diverse clinical presentations, including lupus nephritis (LN), if LN manifest the mortality & morbidity will be elevated. The majority of SLE cases found in females, especially during puberty and childbearing age. A comprehensive understanding of the epidemiology, classification, diagnosis, and management of LN is essential for medical practitioners. Initially, the diagnostic process requires clinical examinations to find clinical manifestations. This process then followed by laboratory assessments to diagnose SLE and identify any indication related to kidney damage, commonly proteinuria. Other findings such as haematuria and leukocyturia may also present. Antinuclear antibodies (ANA) test and anti-dsDNA serves as primary diagnostic tool. In addition, kidney biopsy is the gold standard for LN due to its ability to confirm and measure the stadium for treatment guide. The therapy is specifically based on clinical and biopsy findings, categorized by World Health Organization (WHO) into five classes, namely normal, mesangial, focal and segmental proliferative, diffuse proliferative, and membranous. Accurate diagnosis in the initial stage and suitable therapy is crucial to significantly improve the prognosis of LN. In this review, we address some clinical manifestations related to LN and standardised diagnostic assessment tools, with an ultimate goal to improve the outcome of patients with lupus through an individual-specific management.

Abstract

Patients with autoimmune diseases, especially rheumatic diseases, have their own concerns regarding the coronavirus disease 2019 (COVID-19) vaccination. Their concerns are specifically related to the physicological response, side effects, and effectiveness of the COVID-19 vaccination. COVID-19 and autoimmune diseases share some similarities in their clinical manifestations, immune responses, and pathogenic mechanisms. The correlation between COVID-19 vaccination and autoimmune diseases can be attributed to epitope mimicry, where the antigen contained in the vaccine has a structural similarity to self-antigen. The purpose of this literature review was to discuss the various types of vaccines, the side effects of the COVID-19 vaccination, and the effect of autoimmune patients' medication on the administration of the COVID-19 vaccination. According to several articles obtained in this study, most autoimmune patients typically experienced mild to moderate side effects. It was not advised for these patients to stop treatment before receiving the COVID-19 vaccination, as doing so was unlikely to affect the vaccine's effectiveness or the patients' immune response. The condition of the autoimmune patients was what influenced the immune response mediated by antibodies. Therefore, the COVID-19 vaccination must be administered when the patients are in a stable condition. In conclusion, it was determined that there is no restriction preventing autoimmune patients from receiving the COVID-19 vaccination. However, it should be noted that autoimmune patients are not recommended to receive live vaccines.   Highlights: 1. This study reviewed the concerns of autoimmune patients over the side effects of the COVID-19 vaccination on their prescribed medication. 2. It is advisable for autoimmune patients to refrain from obtaining live-attenuated vaccines because of their potential effect on the immune response.

Abstract

Penelitian ini bertujuan untuk mengetahui pengaruh kepemilikan kelembagaan, ukuran dewan komisaris, ukuran perusahaan, dan ukuran komite audit terhadap pengungkapan islamic social reporting (ISR). Populasi dalam penelitian ini adalah perusahaan yang terdaftar di Jakarta Islamic index periode 2017-2021.  Metode  yang  digunakan  adalah  purposive  sampling sehingga  12 perusahaan  yang  menyajikan  laporan  keuangan  lengkap  sesuai  dengan  variabel  yang  diteliti  dan  di peroleh  sebanyak  60  sampel  perusahaan.  Teknik  analisis  yang  digunakan  adalah  analisis  regresi berganda. Hasil  penelitian  menunjukan  bahwa Kepemilikan Kelembagaan tidak berpengaruh terhadap pengungkapan Islamic Social Reporting (ISR), Ukuran Dewan Komisaris berpengaruh terhadap pengungkapan Islamic Social Reporting (ISR), Ukuran Perusahaan berpengaruh terhadap pengungkapan Islamic Social Reporting (ISR), dan Ukuran Komite Audit tidak berpengaruh terhadap pengungkapan Islamic Social Reporting (ISR).

Abstract

Introduction: Systemic lupus erythematosus (SLE) is a multi-system autoimmune disorder marked by pathogenic autoantibodies, resulting in considerable morbidity and mortality. Despite existing diverse treatment regimens, the need for more effective therapies persists. Recent advancements include monoclonal antibodies, such as belimumab, which can inhibit receptors tied to SLE’s pathogenesis. This meta-analysis aimed to evaluate the efficacy and safety of combining belimumab and standard therapy compared to placebo in SLE patients, utilizing as many indicators as possible to comprehensively assess the former's potential. Methods: This study adhered to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) and the Cochrane Handbook for Systematic Reviews of Interventions. A thorough literature search was performed across various databases, including PubMed, ScienceDirect, ProQuest, and grey literature sources such as MedRxiv and BioRxiv. The data underwent statistical analysis, with I²<50% indicating low heterogeneity and p<0.05 denoting statistical significance. Results: The literature search yielded seven records for analysis in this study. All the selected studies were multicenter, phase III/IV, randomized clinical trials published between 2011 and 2019. The selected studies' risk of bias was assessed using Cochrane’s Risk of Bias (RoB) 2 tool. The results indicated that belimumab and standard therapy significantly improved disease activity, reduced flare occurrences—particularly severe flares, lowered corticosteroid dosage, and enhanced key biomarkers compared to placebo. The safety profile was favorable, with significantly minimal side effects, infections, and mortality risks. Conclusion: Belimumab combined with standard therapy demonstrates promising efficacy and safety for SLE treatment, suggesting its potential for broader adoption in clinical practice.   Highlights: 1. This systematic review and meta-analysis provide a thorough and complete analysis of multiple indicators regarding the efficacy and safety of belimumab in the treatment of systemic lupus erythematosus (SLE), which have never been reviewed before. 2. The findings of this study may lead to broader acceptance and adoption of belimumab as the standard treatment for SLE.

Abstract

This study was conducted to determine the effect of the syllabus method assisted by poster media on the beginning reading skills of class I students of SDN 93 Palembang. The method used in this study is a quantitative experimental type with a One Group Pretest-Posttest Design. Data collection in this study was by means of a test question. The sample in this study was class I B as an experimental class consisting of 28 students. Data analysis in this study used the SPSS 25 application and the results showed that there was an effect of the syllabus method assisted by poster media on the beginning reading skills of class I students of SDN 93 Palembang as seen from the results of the sig output analysis (2-tailed) 0.000 <0.05 then H₀ was rejected and Hₐ was accepted. Thus, there is an effect of the syllabus method assisted by poster media on the beginning reading skills of class I students of SDN 93 Palembang.
