Abstract

Rheumatoid arthritis (RA) is an autoimmune and prominent inflammatory disorder that can affect wide of variety body systems, mainly joints. In Indonesia, the prevalence of RA is about 7.3% and mostly are women. The majority of women with RA are at childbearing and can be worsening throughout pregnancy and lactation. Conventional synthetic disease-modifying antirheumatic drugs (csDMARDs) are the most common used medicine in RA for pregnancy and lactation. This scoping review was conducted using publications obtained from PubMed, Embase, The Cochrane Library, POPLINE, and Google Scholar concerning the safety of csDMARD in rheumatoid arthritis during pregnancy and lactation from 2011 to 2021. Among csDMARDs reviewed, sulfasalazine, hydroxychloroquine, chloroquine, and cyclosporine are relatively safe for pregnant and lactating women. However, they should be used in caution by considering the risk and benefit as well as under clinical supervision.

Abstract

Hypertension in patients with end-stage chronic kidney disease undergoing hemodialysis is difficult to control leading to a significant risk factor for a cardiovascular event. The aimed of this review is to summarize the prevalence of uncontrolled blood pressure in dialysis patients. This review hopefully can serve as a valuable reference and source of insight for clinicians in their efforts to attain controlled blood pressure in dialysis patients. The relevant literatures was undertaken from PubMed, Scopus, and Google Scholar for studies addressing the factor associated with uncontrolled blood pressure in hemodialysis patients using antihypertensives from 2013 to 2023. Combinations of search terms were "uncontrolled" and "blood pressure" and "dialysis" and "antihypertensive”. Any study in English, including randomized control trials, cohort, case-control, and cross-sectional studies was included. Uncontrolled blood pressure is very common among hypertension in dialysis patients (28.7 - 78.33% of cases). Factors affecting blood pressure in dialysis patients include age, obesity, and comorbidity (diabetes and stage of chronic kidney disease). Although the relationship between gender, the number of antihypertensive, the number of dialysis, and the length of dialysis sessions with blood pressure varies, some studies have shown a positive association.

Abstract

Hypertension in end-stage chronic kidney disease (CKD) patients undergoing HD is difficult to control. The aim of the study was to determine the relationship between the number of antihypertensive medications taken and the use of dialyzed antihypertensive medications on uncontrolled blood pressure in CKD patients on hemodialysis. Consecutive sampling was employed in a retrospective cohort study. Data collection for the study took place in July 2023 at Temanggung Regional Hospital. The inclusion criteria in this study were CKD patients who undergoing routine hemodialysis twice a week, aged ≥ 18 years, and without or with comorbidities. Medical records and interviews with research participants were used to gather data. Forty-eight patients with clinical outcomes of uncontrolled predialysis blood pressure and four patients with regulated predialysis blood pressure made up the 52 research subjects. The number of antihypertensive drugs used is significantly associated with uncontrolled predialysis blood pressure in CKD patients with hemodialysis using antihypertensives (p-value 0,033). The use of dialyzed antihypertensive drugs is not associated with uncontrolled predialysis blood pressure in CKD patients on hemodialysis using antihypertensives (p-value 0,115).

Abstract

Pengetahuan tentang obat adalah keharusan bagi masyarakat, mengingat maraknya praktik pengobatan mandiri atau swamedikasi. Kurangnya pemahaman tentang obat berisiko menyebabkan kesalahan fatal, mulai dari cara penggunaan, penyimpanan, hingga pembuangan obat yang tidak sesuai dengan petunjuk standar. Kegiatan pengabdian kepada masyarakat ini bertujuan untuk mengenalkan masyarakat tentang penggunaan dan pengelolaan obat yang benar, khususnya melalui konsep DAGUSIBU (Dapatkan, Gunakan, Simpan, dan Buang). Kegiatan ini dilakukan bersamaan dengan pelaksanaan senam bersama yang bertujuan untuk meningkatkan dan memelihara kesehatan fisik dan mental. Peserta kegiatan yang hadir diberikan edukasi secara langsung oleh apoteker dengan metode ceramah dan menggunakan media informasi berupa leaflet. Leafleat ini memuat informasi tentang penggunaan obat menggunakan konsep DAGUSIBU. Partisipan adalah masyarakat umum yang beraktivitas di sekitar Lapangan Kalpataru. Sebanyak 30 partisipan hadir dan mengikuti kegiatan edukasi yang diberikan langsung oleh apoteker. Partisipan menunjukkan antusiasme tinggi dan mampu mempraktikkan cara identifikasi tanggal kedaluwarsa dan dosis dengan benar. Kegiatan pengabdian kepada masyarakat ini terlaksana melalui rangkaian senam dan edukasi obat menggunakan konsep DAGUSIBU. Masyarakat telah memperoleh informasi langsung mengenai praktik pengelolaan obat yang benar sesuai prinsip DAGUSIBU.
