Abstract

At the end of 2019, Wuhan, Hubei Province, reported the first case of a virus similar to pneumonia. The World Health Organization (WHO) reports this outbreak as the Corona virus (COVID-19). Infection with COVID-19 may be more common in people who have chronic kidney disease (CKD). Patients with chronic kidney failure may experience infectious complications due to a weakened immune system and immunosuppressive state. Similarly, they suffer from a chronic systemic inflammatory state, which can contribute to increased morbidity and mortality. Thus, the purpose of this study is to conduct a review of antiviral agents and to summarize recommended antiviral regimens in COVID-19 patients with chronic kidney disease. This article search used Mendeley, Google Scholar, ScienceDirect, and PubMed from the NCBI database in October 2020. The administration of drugs to patients with chronic kidney disease who have been exposed to COVID-19 must be seriously considered. Certain antivirals must be adjusted to avoid deteriorating the patient's kidney condition. Antiviral agents are medications used to treat and prevent COVID-19 infection. Numerous antiviral agents, including ramdesivir, ribavirin, hydroxychloroquine or chloroquine, and umifenovir, must be administered with caution and dose adjustments as necessary in CKD patients exposed to COVID-19.

Abstract

Abstrak: Pengaruh Pemberian Steroid Sebagai Terapi Adjuvan Terhadap Parameter Inflamasi Pasien Pneumonia Komunitas Berat Di Rumah Sakit “X” Kota Bandar Lampung. Parameter inflamasi nonspesifik seperti jumlah leukosit, jumlah neutrofil absolut, dan C-reactive protein (CRP) menunjukkan derajat reaksi inflamasi pada fase akut dan sering digunakan untuk menunjukkan infeksi bakteri. Salah satu terapi yang masih dipertimbangkan pada pasien Pneumonia yaitu Steroid. Sebagian studi menunjukkan bahwa pemberian kortikosteroid dosis sedang melalui intravena dapat menumpulkan respon sistemik sitokin proinflamatorik pada sepsis berat dan inflamasi paru pada pneumonia berat dan cidera paru akut. Penelitian ini bertujuan untuk mengetahui Pengaruh Pemberian Steroid Sebagai Terapi Adjuvan Terhadap Parameter Inflamasi Pasien Pneumonia Komunitas Berat Di Rumah Sakit “X” Kota Bandar Lampung. Peneliti menggunakan jenis penelitian deskriptif analitik. Penelitian ini dilaksanakan di Rumah Sakit “X” Bandar Lampung periode Juli- Desember 2022 dengan menggunakan data sekunder rekam medik. Variabel yang diteliti yaitu variabel independen berupa data terapi steroid atau tanpa terapi steroid, variabel dependen yaitu parameter laboratorium NLR dan CRP dan beberapa parameter klinis (demam, batuk, sesak) serta parameter vital antara lain saturasi oksigen dan Respiratory rate. Hasil penelitian pada kelompok steroid, menunjukan terdapat perbaikan yang signifikan pada parameter suhu (p=0,022) dan saturasi oksigen (p=0,006). Selain itu pada parameter inflamasi NLR, ditemukan perbaikan yang signifikan pada kelompok steroid (p=0,001). Kesimpulan: terdapat pengaruh pada pemberian steroid terhadap parameter suhu, saturasi oksigen dan NLR.

Abstract

Diabetes Mellitus (DM) is a chronic disease that occurs worldwide and continues to increase rapidly. The purpose of this study was to determine the activity and mechanism of action of the antioxidant compounds in cempaka kuning leaves as an antidiabetic using the in silico method. This study used 8 antioxidant compounds from cempaka kuning leaves, namely octadecadienoic acid, butanoic acid, oleic acid, camphorsulfonic acid, pimaric acid, phenol, andrographolide, and benzoic acid. The α-glucosidase enzymes used in this study were obtained from the Protein Data Bank website with the PDB ID codes 3A4A and 2QMJ. Ligand-macromolecule docking using AutoDockTools (ADT) and AutoDock Vina programs. The results showed that the antioxidant compounds in cempaka kuning leaves that had the lowest ΔGbind values were pimaric acid (-8.9 kcal/mol) and andrographolide (-8.6 kcal/mol) in PDB ID 3A4A. The docking results used PDB ID 2QMJ, the compound with the smallest ΔGbind was andrographolide (-7.4 kcal/mol). The results of visualization of the interaction of the ligand with the macromolecular amino acid residue PDB ID 3A4A showed that the residues Arg315, Arg442, and Tyr158 were found in the compound with the smallest ΔGbind. In the PDB ID 2QMJ macromolecule, Asp542 residue was found in all compounds with the smallest ΔGbind. Based on these results, the compounds pimaric acid and andrographolide have potential as antidiabetic drugs compared to other compounds with a mechanism of action of inhibiting the α-glucosidase enzyme.
