Abstract

Background: Perinatal mortality remains a major public health problem, particularly among women with high-risk pregnancies. Maternal, placental, infectious, and obstetric complications contribute significantly to stillbirth, early neonatal death, and other adverse neonatal outcomes. Objective: This study aimed to identify risk factors and complications associated with perinatal mortality in high-risk pregnancies. Methods: This study used a Systematic Literature Review (SLR) design following the PRISMA 2020 guidelines. The research question was developed using the PICOS framework, focusing on pregnant women with high-risk pregnancies, maternal and obstetric risk factors, and perinatal mortality outcomes. Literature searches were conducted in ProQuest, PubMed, SpringerLink, ScienceDirect, and Semantic Scholar between March and May 2026. Keywords included “high-risk pregnancy,” “perinatal mortality,” “stillbirth,” “early neonatal death,” “risk factors,” and “pregnancy complications,” combined using Boolean operators (“AND” and “OR”). Inclusion criteria were English-language full-text studies published between May 2025 and May 2026 using cohort, case-control, cross-sectional, prospective, or retrospective designs. Study selection followed the PRISMA flowchart through identification, screening, eligibility, and inclusion stages. From 21,401 identified articles, 14 studies met the eligibility criteria and were included in the review. Results: The reviewed studies showed that maternal COVID-19 infection, inherited thrombophilia, hypertensive disorders, placental dysfunction, maternal undernutrition, abnormal body mass index, advanced maternal age, and obstetric complications significantly increased the risk of stillbirth, preterm birth, fetal growth restriction, low birth weight, and neonatal death. Placental abnormalities and placental abruption were strongly associated with adverse neonatal outcomes. Specialized antenatal services, maternal risk stratification, thromboprophylaxis in selected patients, and planned birth at optimal gestational age were associated with improved maternal and neonatal outcomes. Conclusion: Perinatal mortality in high-risk pregnancies is influenced by multiple maternal and obstetric factors. Early identification, specialized antenatal care, and individualized management are essential to reduce preventable maternal and neonatal mortality.

Abstract

Garut Regency in 2021, women who experienced vaginal discharge were 318,976 or 29.73%. The high number of cases of vaginal discharge in women of childbearing age and the frequent occurrence of less serious treatment when vaginal discharge occurs are of particular concern among women. One way that can be done to treat vaginal discharge is to use a decoction of soursop leaves because they contain antiseptic substances that can kill germs, namely phenol, which has antiseptic properties 5 times more effective than ordinary phenol. Objective: To determine the effectiveness of soursop leaf decoction (Annona muricata L) on vaginal discharge in women of childbearing age in the Karangmulya Health Center Working Area, Garut Regency. Methodology: This study used a quasi-experimental pre-test-post-test with control design. The population in the study were all those who experienced leucorrhea with a score of >5 or 50 people. The sample in the study used total sampling which was divided into two groups, 25 respondents in the experimental group and 25 respondents in the control group. The instruments used were observation sheets and technical instructions about the use of soursop leaf decoction. Data analysis used the Wilcoxon and Mann Whitney tests. Results: The average score of vaginal discharge in the experimental group before the intervention was 10.32 and after the intervention was 6.84, while the average score in the control group in the first measurement was 10.36 and in the second measurement was 9.04. The bivariate results showed a p-value in the experimental group of 0.000, a p-value in the control group of 0.000 and a p-value comparison of the experimental and control groups of 0.000. Conclusion: Soursop leaf decoction has been proven effective in treating vaginal discharge in women of childbearing age, so there is a difference in vaginal discharge experienced by women of childbearing age after being given soursop leaf decoction between the experimental and control groups. Suggestion: It is hoped that women of childbearing age can handle or treat vaginal discharge independently by using soursop leaf decoction.

Abstract

Traditional postpartum care is still practiced in Cimanggu District. Based on 2024 data, 27 mothers experienced problems during the postpartum period caused by the use of bengkung, consumption of sambel peupeuh, application of campuh, and practicing the traditions of nyanda and sangsurkeun. Several factors influencing the practice of traditional care according to theories and previous studies include knowledge, attitude, culture, family support, and health worker support. Objective to analyze the factors influencing exclusive breastfeeding practices among mothers in Pancoran Mas District, Depok City, in 2025.  This research employed a qualitative approach to explore mothers’ experiences and perceptions of exclusive breastfeeding in Pancoran Mas District, Depok City. A total of 24 informants participated, consisting of 20 main informants (mothers with infants aged 6–12 months and family members) and 4 key informants (midwives and community health cadres). Participants were selected using purposive sampling. Data were collected through in-depth interviews guided by semi-structured interview protocols. All interviews were recorded, transcribed verbatim, and analyzed thematically using NVivo software to identify emerging themes related to perceptions, experiences, barriers, support systems, and workplace policies influencing exclusive breastfeeding. Trustworthiness of the findings was ensured through triangulation of sources, member checking, and peer debriefing. Most mothers perceived exclusive breastfeeding positively for maternal and infant health, yet concerns about physical changes, maternal fatigue, stress, cultural influences, and lack of workplace support emerged as barriers, while emotional, professional, and family support facilitated its practice. Exclusive breastfeeding is shaped by maternal perceptions, with support from family, health workers, and workplace policies playing a key role in sustaining the practice.

Abstract

Anxiety in third-trimester pregnant women is a common psychological challenge faced before childbirth and has the potential to interfere with the delivery process. The general objective of this study was to identify and develop the ALIZA counseling model for third-trimester pregnant women. This study employed a mixed-method design with an exploratory sequential approach divided into three stages. Stage I was an exploratory qualitative study conducted through in-depth interviews with 10 third-trimester pregnant women, 5 midwives, 5 psychologists, and 5 counselors, selected purposively. Stage II involved the development of the counseling and the preparation of the ALIZA counseling guide. Stage III was a quantitative study to test the model's effectiveness using a pretest–posttest design with control and intervention groups, each consisting of 20 participants (total n=40). The instruments used were the DASS-21 anxiety scale questionnaire, and the analysis employed descriptive tests, normality tests, and the Wilcoxon test. Qualitative findings indicated that third-trimester pregnant women experienced anxiety due to lack of information, negative experiences, environmental pressure, and limited support from their partners. The counseling components formulated of Assessment, Listening, Information, Zone of Comfort, and Action Plan. Empathic and educational counseling, using psychological approaches such as affirmations and relaxation techniques, was shown to increase self-confidence and preparedness for childbirth. Quantitative results showed a decrease in mean anxiety scores from 19.22 to 16.45 in the intervention group, and the Wilcoxon test yielded a Z value of -5.560 with p = 0.000, indicating a statistically significant reduction in anxiety. Multivariate linear regression analysis showed that all ALIZA components had a significant effect on anxiety, with R² = 0.695, meaning that 69.5% of the variance in anxiety could be explained simultaneously by these five components.

Abstract

Preparedness of pregnant women in recognizing danger signs during pregnancy is a crucial factor to prevent delays in the treatment of obstetric complications. The variation in results from previous research shows the need for in-depth analysis by combining sociodemographic, individual, social, and structural-systemic factors in one analytical model. This study aims to identify the related factors and determine the most dominant variables in influencing the preparedness of pregnant women. A quantitative study with a cross-sectional design was conducted on 121 pregnant women at the Dzakirah Clinic in South Tangerang using a total sampling technique. The research instrument has gone through a validity and reliability test process; all constructs are declared valid after one item on the anxiety variable is eliminated, and show adequate internal consistency (Cronbach's Alpha ≥ 0.70). Data analysis included univariate, bivariate (correlation test), and multivariate using multiple linear regression. Bivariate analysis showed that knowledge, family support information resources, cultural norms, healthy living behaviors, pregnancy app use, anxiety, information seeking, self-efficacy, social support, perception, attitude, and involvement in health programs had a significant relationship (p<0.05). Meanwhile, parity, income, and adherence to pregnancy screenings showed no statistically significant relationship. In the final regression model, the source of pregnancy information had the highest coefficient and became the most dominant variable in influencing preparedness. Preparedness of pregnant women is more influenced by access factors and information quality than demographic characteristics.

Abstract

This study examines the factors influencing employee performance at Puskesmas Cikulur, located in Lebak Regency, Indonesia. A quantitative approach was used, and a survey was conducted with 104 employees at Puskesmas to explore the impact of organisational culture, work discipline, and loyalty on employee performance. The findings show that 95.2% of employees reported a positive organisational culture, 96.2% indicated high work discipline, and 93.3% demonstrated high loyalty. Regression analysis revealed that organisational culture, work discipline, and loyalty had statistically significant positive effects on employee performance (p-values of 0.003, 0.002, and 0.004, respectively). However, competence and motivation did not significantly influence employee performance, with p-values of 0.329 and 0.465, respectively. These results suggest that enhancing organisational culture, improving work discipline, and fostering loyalty are key strategies for improving employee performance. This study provides important insights for healthcare management at Puskesmas, demonstrating that factors such as culture and discipline should be prioritised to improve overall employee performance, which is critical to delivering quality healthcare services in primary healthcare settings.
