Abstract

Diabetes Melitus (DM) merupakan penyakit kronik, oleh karena itu peran self-managementsangat penting dalam perawatan maupun pencegahan komplikasi akibat DM. Penelitian ini bertujuan untuk mengetahui gambaran perilaku self- management.pasien DM dari aspek diet, medikasi, olahraga, pemantauan kadar gula darah dan perawatan kaki. Sebanyak 94 responsden diambil secara acak. Perilaku self-managementdiukur menggunakan kuesioner yang dikembangkan oleh penulis, dengan skor Alpha Chronbach 0,930 dan korelasi inter-item 0,385–0,797 dimana nilai tersebut valid dan reliabel untuk uji instrumen. Hasil penelitian menunjukkan lebih dari setengah responsden (64,9%) melakukan lima aspek self-managementdengan baik. Hampir semua responsden (94,7%) melakukan medikasi dengan baik, lebih dari setengah responsden melakukan diet (69,1%), olahraga (61,7%) dan perawatan kaki (77,7%) dengan baik. Namun hanya 25,5% responsden yang melakukan pemantauan gula darah dengan baik. Hasil penelitian ini dapat menjadi acuan bagi perawat untuk memberikan penyuluhan mengenai pentingnya pemantauan pada gula darah.Kata kunci: Diabetes Melitus, perilaku, self-management AbstractDiabetes Mellitus (DM) is one of the serious health problems in Indonesia. Since DM is a chronic disease, patients’ role (self-management) is very important either for treatment or DM-related complications prevention. This study was purposed to determine the level of patients’ diabetes self-management behavior, including diet, medication, exercise, blood glucose monitoring and foot care. Ninety-four responsdents were recruited randomly by gave the informed consent to diabetes patients. The questionnaire was developed by the researcher, with Chronbach Alpha 0,930 and inter-item correlation 0,385 to 0,797, which mean the instrument was valid and reliable. The results showed that in five aspects, more than half of responsdents (64,9%) reported that they performed good self-management. Almost all responsdents (94,7%) reported good medication, more than half of responsdents did well in diet, exercise (61.7%) and foot care (77.7%). However, only 25.5% responsdents who performed monitoring blood glucose levels properly. It called be good level when it fill the score criteria of the answer.Therefore, it was recommended for nurses and the hospital to develop any program to improve patients’ diabetes self-management behaviors, especially in the monitoring blood glucose.Key words: Behavior, diabetes mellitus, self-management

Abstract

Penyakit diabetes melitus tipe 2 (DMT2) memerlukan pengontrolan untuk meminimalisir komplikasi melalui penerapan self-managementyang baik. Efektifitas penerapan self-managementdipengaruhi banyak faktor salah satunya dukungan keluarga. Keluarga merupakan lingkungan sosial yang paling dekat dengan pasien DM sehingga diharapkan dapat membantu, mengontrol dan membentuk perilaku pasien DM termasuk dalam hal ini perilaku self-management. Penelitian deskriptif kuantitatif dengan pendekatan potong lintang ini bertujuan untuk menggali dukungan keluarga dalam konteks pasien DM di Indonesia. Sebanyak 78 responden dilibatkan dalam penelitian ini dengan menggunakan metode concecutive sampling. Dukungan keluarga diukur menggunakan instrumen yang di modifikasi dari The Diabetes Social Support Questionnaire-Family Version (DSSQ-Family) dengan skor Alpha Cronbach 0,973 dan korelasi inter-item0.386-0.859. Data yang terkumpul dianalisis menggunakan analisis deskriptif, dan dukungan keluarga dikategorikan menjadi favorable(bila skor total individu > nilai mean kelompok 69,62). Hasil penelitian menunjukkan lebih dari setengah responden (55,1%) melaporkan dukungan keluarga favorable. Dari analisis domain dukungan keluarga, dimensi dukungan lingkungan sosial secara umum menunjukkan persentase terendah (48,71%) dibandingkan domain dukungan keluarga yang lainnya. Dengan demikian menjadi penting bagi perawat untuk meningkatkan keterlibatan keluarga dalam perawatan DMT2 serta meningkatkan aspek dukungan lingkungan sosial. The effectiveness of SMB is influenced some factors, one of them is family support. family is the closest social enviroment for patients of DM, thus it is hoped to help, control, and create patient of DM behaviour includes self management behaviour. This study was descrition quantitative with cross sectional approach purposed to determine the level of patients’ perceived of family support in Indonesia. Seventy-eight respondents were included for this study by using concecutive sampling methode. The questionnaire for family support was modified from The Diabetes Social Support Questionnaire-Family Version (DSSQ-Family) with Chronbach Alpha 0,973 and inter-itemcorrelation 0,386-0,859. The data collected were analyzed using descriptive analysis, where family support categorized into favorable (if individual score > the respondents mean score = 69.62) and oppositely. The results showed that more than half of respondents (55,1%) reported favorable family support. Regarding family support domains, social network support noted as the lowest percentage (48.71%). Therefore, it is important for nurses and other healthcare professional to improve family involvement in diabetes care especially improving network support aspect.Key words:Diabetes mellitus, family support, self-management behaviour

Abstract

Tuberculosis Multidrugs Resistance (TB-MDR) merupakan masalah serius di Indonesia. Selain memiliki risiko penularan yang tinggi, TB-MDR mempunyai banyak hambatan dalam pengobatan, baik lama pengobatan, jumlah obat yang banyak, dan efek samping yang buruk. DHal ini menjadi penting mengidentifikasi perilaku pencegahan penularan pada pasien TB-MDR beserta faktor yang melatarbelakanginya. Penelitian ini bertujuan untuk mengidentifikasi perilaku penderita TB-MDR dalam mencegah penularan beserta faktor yang melatarbelakanginya. Penelitian deskriptif korelasional ini melibatkan seluruh pasien TB-MDR yang sedang menjalani pengobatan fase intensif hingga November 2014 di Rumah Sakit Hasan Sadikin sebanyak 61 orang. Data karakteristik responden, perilaku dan faktor-faktor yang melatarbelakanginya dikumpulkan menggunakan kuesioner. Data yang terkumpul dianalisis secara deskriptif dan dihubungkan satu sama lain (independent t-test, one way annova, dan Pearson Correlational test). Hasil penelitian menunjukkan bahwa lebih dari setengah responden berpendidikan SMA (54,1%), berjenis kelamin laki-laki (60,6%), tipe MDR gagal pengobatan kategori 1 & 2 (60,7%), berusia < 44 tahun (68,9%), sebagian besar menikah (75,4%) dan berpenghasilan di bawah UMR (81,9%), serta mengeluhkan efek samping berupa mual (90,1%). Lebih dari setengah responden (57,4%) melaporkan perilaku pencegahan penularan yang baik. Perilaku pencegahan penularan ditemukan berhubungan secara bermakna dengan jenis kelamin (p = 0,01), perceived benefit (p = 0,02), cues to action (p = 0,00), dan self efficacy (p = 0,006). Akan tetapi, tidak ditemukan adanya hubungan yang bermakna antara perilaku pencegahan dengan data demografi (usia, satatus pernikahan, tingkat pendidikan, dan penghasilan) maupun tipe MDR (p>0,05). Hal ini menjadi penting bagi tenaga kesehatan untuk memperkuat faktor tersebut sebagai upaya meningkatkan perilaku pencegahan transmisi/penularan TB.Kata kunci: Faktor, pencegahan penularan, TB-MDR. Prevention Behaviors and Its’ Contributing Factors among Patients with Multi-drugs Resistance Tuberculosis (MDR-TB)AbstractMulti-drugs Resistance Tuberculosis (MDR-TB) is a serious health problem in Indonesia. Beside the risk of transmission, the treatment of MDR-TB encounters some obstacles namely lengthy medication, multiple drugs and adverse side effects. Therefore, it is important to identify patients’ prevention behaviors and its contributing factors. This study was aimed to identify MDR-TB patients’ prevention behaviors and its’ contributing factors. This descriptive correlational study involved all (61 patients) of MDR-TB patients who received intensive medication until November 2014 in Dr. Hasan Sadikin Hospital. Demographic and health characteristics data, as well as behaviors and its related factors were collected using questionnaires. Data were analyzed using descriptive analyses and correlational test (independent t-test, one way ANOVA, and Pearson correlation test). The results showed that more than half of respondents were male (60.6%), MDR with failed medication type 1 & 2 (60.7%), age less than 44 years old (68.9%), mostly married (75.4%), had income less than minimum standard (81.9%), and experienced nausea as the medication’s side effect (90.1%). Additionally, more than half of respondents (57.4%) reported good prevention behaviors. These behaviors significantly related to female gender (p = 0.01), perceived benefit (p = 0.02), cues to action (p = 0.000), and self-efficacy (p = 0.006). However, there was not any significant relationship between the prevention behaviors and demographic data (age, educational level, marital status, and income) or between the behaviors and patients’ medication categories (p > 0.05). Generally, MDR-TB patients in this study performed good preventive behaviors and it was related to their perceived benefit, cues to action, and self-efficacy. Therefore, it is important for healthcare professional to empower these identified factors in order to minimize the MDR-TB transmission.Key words: Factors, MDR-TB, prevention behaviors.

Abstract

Diabetic Gangrene Ulcers are chronic wounds with proinflammatory wound environment caused by elevated levels of  TNF-α, which tends to occur in delay in wound healing, and infection susceptibility. This research aims to determine the effect of wound care using Robusta coffee powder as an adjuvant  to the healing of diabetic ulcers gangrene.32 patients were included in this Quasi-experiment with Pretest-Posttest Control Group Design approach and the sampling was done by consecutive sampling method at random. The object was divided into 2 groups, namely the intervention group and the control group performed on patients undergoing treatment in the surgical and inpatient wards, Regional public hospital, Sekarwangi. The intervention group received wound treatment with Robusta coffee powder while the control group received conventional wound treatment for 2 weeks, during which also has been done assessment of wound scores with wound assessment Bates Jensen at the time of pretest and posttest.            Characteristics of respondents between the intervention group and the homogeneous control did not differ significantly. The difference of mean score of pretest and posttest score in two groups with t test showed there was significant difference with p value <0,05, whereas for difference of mean score between group of intervention and control with independent t test showed there is difference significantly.            There is influence of wound care using Robusta coffee powder as adjuvant to healing diabetic ulcers gangrene in Regional public hospital, Sekarwangi, Sukabumi. Therefore, it is important for the hospital to consider wound care using Robusta coffee powder on diabetic ulcer gangrene wounds as it can accelerate wound healing.  Keywords: Robusta cofee, Wound healing, Diabetic ulcers, Wound assessment Bates Jensen
