Abstract

Latar belakang: Salah satu permasalahan program Keluarga Berencana (KB) di Indonesia adalah tingginya angka kebutuhan ber-KB yang tidak terpenuhi (unmet need KB). Di Pulau Sumatera, terdapat disparitas angka unmet need KB antar wilayah, dimana sebagian provinsi memiliki angka unmet need KB tinggi dan sebagian lainnya rendah. Tujuan: Tujuan penelitian ini adalah untuk mendapatkan autokorelasi spasial berdasarkan angka unmet need KB dan menemukan wilayah hotspot unmet need KB di Pulau Sumatera tahun 2022. Metode: Penelitian ini merupakan penelitian kuantitatif dengan desain studi ekologi. Unit analisis dalam penelitian ini adalah kabupaten/kota, sedangkan ruang lingkup wilayah yang dianalisis meliputi kabupaten/kota yang berada di pulau induk Sumatera sebanyak 131 kabupaten/kota. Uji autokorelasi spasial dilakukan dengan menggunakan Indeks Moran dan Local Indicator of Spatial Autocorrelation (LISA). Hasil: Penelitian ini menunjukkan adanya autokorelasi spasial positif unmet need KB di Pulau Sumatera tahun 2022 (p value=0,001, I=0,646, E(I)=-0,008). Artinya, persentase unmet need KB pada satu wilayah kabupaten/kota berkaitan dengan wilayah sekitarnya, dengan pola mengelompok (clustered). Wilayah yang menjadi hotspot unmet need KB di Pulau Sumatera tahun 2022 berdasarkan pola kluster yang terbentuk adalah sebanyak 27 kabupaten/kota. Kesimpulan: Kejadian unmet need KB di Pulau Sumatera tahun 2022 membentuk pola mengelompok berdasarkan wilayah kabupaten/kota yang berdekatan dan tidak terjadi secara acak. Wilayah hotspot dapat diusulkan sebagai prioritas dalam melakukan intervensi penurunan unmet need KB di Pulau Sumatera.

Abstract

Introduction: South Jakarta City, within DKI Jakarta, bears a substantial burden of TB cases, yet case detection rates and treatment success remain low. Factors such as population density, gender disparity, socio-economic conditions, and healthcare accessibility influence TB transmission. The city's high population density presents challenges in controlling TB spread. Additionally, males and low-income families face higher TB risks. Objective: This study aims to analyze TB case distribution across 10 districts in South Jakarta, assess TB case clustering, and understand the spatial regression model of TB risk factors. Method: The methodology of this research utilizes a quantitative approach with an ecological study design and spatial methodology, secondary data from various sources, including the national TB control reporting application. Data processing involves GeoDa v1.22, QGIS v.3.32.3, SPSS v.22, and GWR v4.0 applications for spatial analysis. Result: Findings reveal that spatial autocorrelation tests using Moran's Index on TB cases in South Jakarta City are not statistically significant, indicating no spatial autocorrelation. The LISA test identifies Mampang Prapatan District as a cold spot in Quadrant III (Low-Low). GWR regression analysis highlights three spatially influential risk factor variables affecting TB cases: gender ratio, number of health centers, and population density. The first two variables affect all districts in South Jakarta City, whereas population density only impacts the Setiabudi District. Conclusion: In conclusion, Mampang Prapatan district in South Jakarta City exhibits low TB transmission risk, considering population density and other factors. Notably, three spatial risk factors affect TB transmission in South Jakarta City, warranting attention from the health department in TB elimination efforts.

Abstract

Tuberculosis (TB) remains a major public health problem in Indonesia and globally. TB is transmitted through airborne droplets inhaled from individuals with active TB. Transmission risk increases in densely populated areas, poor ventilation, and settings with high social interaction, such as Depok City. Persistently high TB prevalence reflects challenges in treatment success and community-based prevention. This study aims to analyze trends and spatial patterns of TB incidence in Depok City from 2022 to 2024 and their relationship with population density and hand washing behavior with soap (HWWS). A spatial descriptive study was conducted using secondary data on TB cases and HWWS from the Depok City Health Profile, and population density data from Depok City in Figures (2022–2024). TB cases were concentrated in districts with high population density, particularly Tapos, Sukmajaya, Pancoran Mas, and Cimanggis. HWWS showed no clear spatial association with TB incidence. Population density was identified as the main determinant of TB distribution in urban areas. Therefore, region-based TB control strategies supported by spatial analysis are needed to improve intervention prioritization and effectiveness.
