Abstract

Penelitian ini bertujuan untuk mengetahui strategi yang digunakan oleh guru PPKn kelas XI di SMA N 2 Karanganyar dalam membentuk karakter disiplin peserta didik. Metode yang digunakan dalam penelitian ini adalah deskriptif kualitatif, dengan teknik pengumpulan data melalui wawancara, observasi dan studi dokumentasi serta teknik pengambilan data menggunakan purposive sampling. Hasil penelitian menunjukan bahwa terdapat beberapa staregi yang digunakan oleh guru PPKn kelas XI di SMA N 2 Karanganyar, yaitu (1) strategi pembentukan karakter disiplin melalui pembelajaran kontekstual, bercerita dan diskusi, (2) melalui keteladanan, (3) habituasi (pembiasaan), (4) melalui kontrak belajar, dan (5) melalui pemberian nasehat. Dalam pelaksanaan strategi tersebut terdapat kendala yang ditemui berupa, (1) kendala internal, kendala yang berasal dari dalam diri peserta didik berkaitan dengan kurangnya minat dan kesadaran mengenai karakter disiplin, (2) kendala eksternal, kendala yang berasal dari lingkungan pertemanan yang memiliki pemahaman yang kurang mengenai karakter disiplin. Hasil penelitian ini sejalan dengan pendapat Lickona (2014:100) serta pendapat Ridwan Abdullah Sani dan Muhammad Kadri, Sani & Kadri (2016:128-154) mengenai startegi pembentukan karakter. Hasil penelitian dapat menjadi gambaran bagaimana strategi yang digunakan oleh guru PPKn dalam upaya pembentukan karakter disiplin pada peserta didik sebagai penerus bangsa. Kajian ini menggabaran bagaimana pembentukan karakter disiplin melalui strategi-strategi yang digunakan oleh guru PPKn serta kendala yang dihadapi saat proses pembentukan karakter disiplin untuk mengatasi perasalahan mengenai dekadensi moral yang sedang dialami bangsa Indonesia saat ini.

Abstract

This writing contains socialization regarding legal aid which is obtained free of charge which is socialized to the people of Tanjung Rambutan Village.This socialization aims to provide knowledge about the law and also get free legal aid to the underprivileged. The approach method used is a discussion approach with the community regarding legal aid. As for the results of this socialization, the recipient of legal aid is any person or group of poor people who cannot fulfill their basic rights properly and independently. The basic rights in question are the right to food, clothing, health services, education, work and work, and/or housing.

Abstract

Parathyroidectomy is the definitive treatment for primary hyperparathyroidism. Because of the hungry bone syndrome and prolonged hypocalcemia risk, we must follow up on a patient's parathyroid hormone, calcium, and vitamin D serum after parathyroidectomy. In this case report, we reported on a parathyroidectomy patient whom we followed for two years and who, interestingly, had elevated parathyroid hormone levels. A 35-year-old male patient diagnosed with a left parathyroid tumor underwent parathyroidectomy and isthmolobectomy. The patient was treated with calcium, vitamin D, and levothyroxine supplementation. We diagnosed the patient with hungry bone syndrome on the fourth day of post-parathyroidectomy. Then, we documented calcium, vitamin D, and PTH levels in the next two years. The calcium levels are 7.2 (June 2022), 8.2 (July 2022), 8.5 (September 2022), 7.8 (October 2022), 8.1 (June 2023), 9.7 (June 2024). The PTH levels are 244.2 (June 2022), 328.3 (July 2022), 306.5 (September 2022), 457.2 (October 2022), 163.3 (June 2023), 34.4 (June 2024). The Vitamin D levels are 34.4 (July 2022), 13.4 (March 2023), 35.2 (September 2023), 50.4 (April 2024). We increased the dose of calcium and vitamin D supplementation. The patient is in good condition and has reached a normal level of these laboratory parameters in the second year post-parathyroidectomy. PTH, calcium, and vitamin D serum are needed for follow-up in patients after parathyroidectomy. Normalizing calcium and vitamin D serum is essential to maintaining a normal PTH level. Normal PTH, calcium, and vitamin D serum are the cure indications in this patient.

Abstract

Pituitary macroadenomas can suppress pituitary hormone secretion, one of which is the gonadotropin hormone, causing hypogonadism. The condition of hypogonadism can increase the risk of NAFLD, which can then become liver cirrhosis. A 30-year-old man came to the hospital with decreased consciousness and hematemesis melena. From physical examination, laboratory, and pituitary MRI, the patient was diagnosed with pituitary macroadenoma, hypogonadotropic hypogonadism, primary hypothyroidism, acute adrenal insufficiency, and liver cirrhosis. The patient was given hydrocortisone therapy, correction of electrolyte levels, hypoglycemia protocol, levothyroxine 100 mg/day, and management of hematemesis melena. After that, Sustanon 250 mg was given every two weeks intramuscularly. The patient went home in good condition and was planned for neurosurgery consultation for transsphenoidal resection. After the injection of Sustanon, the patient experienced increased penile length and testicular volume. The condition of hypogonadism in patients with pituitary macroadenoma can be a risk factor for NAFLD, which can then progress to liver cirrhosis. NAFLD and liver cirrhosis also can cause hypogonadism in men by several mechanisms. The patient has been well-managed and experienced clinical improvement.

Abstract

It is so important to train communication in early childhood. Often children are silent and lazy to do an activity will result in communication skills in children not growing well. In fact, communication skills play an important role in a child's life. Children's good communication skills come from activities that children usually do in their world, namely through playing. However, it is very regrettable if the world for children's play is very sporadic, even difficult to produce. The limited playground for children causes children to be more interested or choose to play inside the house by playing gadgets, watching television although this has several positive impacts, but it makes children no longer interested in playing outside such as playing chase, cats, sobyong and others so that children do not socialize with their peers which can hinder the development of early childhood communication. Therefore, the development of early childhood communication is very important to be developed from an early age. Playing can train children's communication, not only that, but also some aspects of development children.

Abstract

With an emphasis on the balance between fulfilling needs and achieving benefits (maslahah), this study examines consumer behavior theory from an Islamic economic perspective. According to traditional economics, the goal of maximizing utility, or personal satisfaction, typically drives consumer behavior. However, according to the principle of maqasid al-shariah, consumption behavior in Islamic economics is evaluated not only based on material satisfaction but also on moral, spiritual, and social qualities. According to Islam, consumption must uphold the five main objectives of sharia: wealth, religion, life, intellect, and progeny. To ensure consumption remains within the boundaries of halal (permissible), thayyib (good), and not excessive (israf), consumption values such as justice, cleanliness, simplicity, charity, and morality serve as guidelines. The three degrees of human needs dharuriyat (primary), hajiyat (secondary), and tahsiniyat (tertiary) that are aimed at attaining success in this life as well as the next are also explained in this research from an Islamic standpoint. Islamic consumer behavior theory offers an option that strikes a balance between the interests of people, society, and the environment in a contemporary setting marked by a consumerist and materialistic culture. Islamic economics promotes fair, sustainable, and community-focused consumerism through a methodology based on spiritual values, ethics, and social consciousness.

Abstract

This study aims to analyze the social quality of the population in Medan Estate Village through three main indicators: life expectancy, employment conditions (salary), and education level. A quantitative approach was used, utilizing demographic and socioeconomic data obtained from relevant agencies and field surveys. The analysis results indicate that life expectancy in Medan Estate Village is in the fairly good category, supported by relatively adequate access to healthcare. However, job quality and income levels remain diverse, with the majority of residents working in the informal sector, which offers salaries below the regional average. The population's education level tends to increase, although intergenerational gaps remain. Overall, this study confirms that the social quality of the community in Medan Estate Village is influenced by the relationship between health conditions, economic capacity, and educational attainment. These findings are expected to inform the formulation of more targeted social development policies.

Abstract

Background: The classical paradigm of diabetic kidney disease (DKD) assumes a synchronous, linear trajectory where increasing albuminuria predicts the decline of glomerular filtration rate (GFR). However, emerging epidemiology suggests these markers may dissociate in advanced disease stages, particularly under modern renoprotective pharmacotherapy. We aimed to investigate this uncoupling phenomenon by evaluating the correlation between urine albumin creatinine ratio (UACR) and estimated GFR (eGFR) in a specific cohort of advanced DKD patients in Indonesia. Methods: We conducted a cross-sectional analytic study from January to November 2025 at Dr. M. Djamil General Hospital Padang, a tertiary referral center. The study population comprised 30 patients with established DKD, predominantly in CKD Stages 3b and 4. The primary outcome was the Spearman rank correlation (r) between UACR and eGFR, reported with 95% Confidence Intervals (CI). An exploratory sub-analysis compared trends in patients receiving SGLT2 inhibitors (n=12) versus standard care (n=18). Results: The cohort was elderly (mean age 61.93 years) with critical renal reserve depletion (median eGFR 32.50 mL/min/1.73 m²). Median UACR was 403.90 mg/g, yet exhibited massive heterogeneity (IQR: 170.82–1779.27). Spearman analysis revealed a complete lack of linear correlation between albuminuria and filtration function (r = 0.041; 95% CI: -0.322 to 0.395; p = 0.830). While SGLT2 inhibitor users (n=12) demonstrated numerically lower median UACR than non-users (n=18), the dissociation from eGFR persisted in both subgroups. Conclusion: We demonstrate a distinct dissociation between albuminuria severity and filtration function in advanced DKD. This uncoupling suggests that in late-stage nephropathy, structural glomerulosclerosis and tubulointerstitial fibrosis progress independently of permeability changes. Consequently, albuminuria cannot serve as a sole surrogate for disease progression in this phenotype, supporting a dual-biomarker strategy where UACR and eGFR are monitored as independent risk factors.

Abstract

Background: Graves’ disease is an autoimmune cause of hyperthyroidism marked by characteristic symptoms and abnormalities in TSH and FT4 levels. Management consists of an initial treatment phase followed by long-term maintenance, and variability in immune responsiveness may influence therapeutic outcomes. This study aimed to describe clinical manifestations and thyroid hormone profiles during initial and maintenance therapy among patients treated at Dr. M. Djamil General Hospital, Padang, from 2017 to 2022. Method: A descriptive cross-sectional design with total sampling was applied. Forty patients diagnosed with Graves’ disease were included. Clinical and laboratory variables were extracted from medical records, and univariate analysis was performed using SPSS v26. Results: The majority of patients were women aged 40–60 years. Palpitations were the most frequent symptom in both treatment phases, followed by excessive sweating, tremor, and exophthalmos. Treatment duration exceeded 24 months for the majority. Mean TSH increased from 0.11±0.17 µIU/mL in the initial phase to 0.62±1.03 µIU/mL during maintenance, while mean FT4 decreased from 47.09±32.79 pmol/L to 21.73±13.10 pmol/L. Discussion: The observed rise in TSH and decline in FT4 indicate biochemical improvement consistent with antithyroid drug responsiveness. The reduction in symptom frequency parallels hormonal stabilization, supporting the clinical utility of long-term therapy. The extended duration of treatment aligns with standard recommendations that emphasize sustained disease control to minimize the risk of relapse. Conclusion: Transition from initial to maintenance therapy was associated with symptomatic improvement and progressive normalization of thyroid function, emphasizing the importance of prolonged and well-supervised treatment in Graves’ disease.
