Abstract

Abstrak: Masalah kesehatan pada usia lanjut ditandai penurunan fungsi organ, adanya penyakit kronis seperti hipertensi dan diabetes mellitus. Tujuan dari kegiatan ini adalah untuk mengetahui peningkatan kualitas hidup pasien, tercapainya outcome terapi pasien yang terlihat dari terkontrolnya data tekanan darah, gula darah pasien lansia dengan kolaborasi bersama tenaga kesehatan di Puskesmas Babat yang memiliki keahlian khusus untuk diimplementasikan melalui interprofessional collaboration. Mitra pemberdayaan ini adalah tim tenaga kesehatan puskesmas babat dengan sasaran kegiatanya adalah di posyandu lansia desa Kuripan dan desa sumurgenuk. Peserta kegiatan berjumlah 40 pasien lansia. Pelaksanaan Kegiatan ini di bulan Juli-September yang terbagi menjadi beberapa bagian sosialisasi kepatuhan minum obat, pelatihan penggunaan website ‘pharmcare’ untuk kader posyandu lansia dan pasien, pemberian kemasan obat khusus, konseling edukasi obat untuk pasien lansia dan monitoring kepatuhan minum obat sebelum dan sesudah penggunaan website ‘pharmcare’. Adanya peningkatan kepatuhan sejumlah 47,50% kepatuhan tinggi,40,00% kepatuhan sedang, dan 12,50% kepatuhan rendah menjadi indikator keberhasilan program bahwasanya intervensi yang dilakukan oleh tim pengabdi berupa pemberian sistem layanan kesehatan yang dapat diakses dimanapun, pemberian kemasan obat praktis untuk mempermudah aturan pakai obatnya dalm hal waktu minum,pembeda waktu minum obat juga dimanfaatkan dengan baik oleh kader maupun peserta posyandu lansia.Abstract: Health problems in old age are characterized by decreased organ function, the presence of chronic diseases such as hypertension and diabetes mellitus. The aim of this activity is to determine the improvement in the patient's quality of life, achieving patient therapy outcomes which can be seen from the controlled blood pressure and blood sugar data of elderly patients by collaborating with health workers at the Babat Community Health Center who have special expertise to be implemented through interprofessional collaboration. This empowerment partner is a team of health workers at the Babat Community Health Center with the target of their activities being at the posyandu for the elderly in Kuripan village and Subergenuk village. The activity participants numbered 40 elderly patients. Implementation of this activity in July-September is divided into several sections: socializing medication adherence, training on using the 'pharmcare' website for elderly posyandu cadres and patients, providing special medication packaging, drug education counseling for elderly patients and monitoring medication adherence before and after use of the 'pharmcare' website. An increase in compliance of 47.50% high compliance, 40.00% moderate compliance, and 12.50% low compliance is an indicator of the success of the program because the intervention carried out by the service team is in the form of providing a health service system that can be accessed anywhere, providing practical drug packaging. To simplify the rules for using medicine in terms of when to take medicine, differentiating times for taking medicine are also well utilized by elderly posyandu cadres and participants.

Abstract

Coronary Heart Disease (CHD) is a cardiovascular disease that can caused by the presence of coronary artery plaque blockage. Clopidogrel is one of antiplatelet therapy in CHD patients. CYP2C19 *2 polymorphisms can reduce the effectiveness of clopidogrel active metabolites. This research was conducted at RSUD Sidoarjo East Java from November to December 2017continued monitoring of cardiovascular events to March 2018. The research method used is a method Polymerase Chain Reaction (PCR) and the Light Transmittance Aggregometry (LTA) method for platelet aggregation measurement. From 25 sample inclusion,there are wild type alleles (bp values ​​around 120 bp; 1 patient), homozygous alleles (bp values ​​around 169 bp; 18 patients) and allele heterozygotes (bp values ​​around 120bp, 169bp; 6 patients). The most common type of polymorphism is allele homozygotes. There is one patient hyperaggregation and cardiovascular events after being monitored for 3 months. Analysis relationship between CYP2C19*2 genetic polymorphisms with platelet aggregation and cardiovascular events, using the kruskal wallis test. The result is no significant relationship between CYP2C19*2 polymorphisms with platelet aggregation (p=0.512), as well as cardiovascular events did not make any difference which is also significant (p=0.426). The results of the association of platelet aggregation with cardiovascular events showed significant differences (p=0.027).
