Abstract

Cholinesterase examination related to liver function and carried out by kinetic photometric method with two type reagent addition techniques, namely sample start and substrat start. Both techniques are related to incubation time during examination which affects waiting time for laboratory service result. This study aims to determine comparison of serum cholinesterase activity measured by sample start and substrat start techniques. The research was conducted in May 2024 at Clinical Chemistry Laboratory of Bandung Polytechnic Health. The research using Paired Sample T-test of 30 serum samples measured cholinesterase activity at 405 nm wavelength photometer using sample start and substrat start techniques so that 60 research data were obtained. The result of Paired Sample T-test showed that serum cholinesterase activity with sample start technique had lower trend value than serum cholinesterase activity with substrat start technique and there was no clinical effect with an analytical difference in cholinesterase activity measured by both techniques indicated by the Sig value 0.000 and TE% < TEa% that 4.72% < 8.7%. Therefore, the sample start and substrat start techniques in photometric method of serum cholinesterase activity can be applied for daily testing because the difference in value of two techniques has no clinical effect.

Abstract

Active smokers are people who smoke cigarettes directly, while passive smokers are people who do not inhale cigarettes directly, but inhale cigarette smoke emitted from people who are smoking. One of the toxic heavy metals found in cigarettes is arsenic. Tobacco is the main component of cigarettes that contain many chemicals, including arsenic. Arsenic can be found in tobacco due to several factors, namely soil characteristics, water, phosphate fertilizer, environmental factors, and types of pesticides. This study aims to determine the differences in blood arsenic levels in active and passive smokers using the Inductively Coupled Plasma - Mass Spectrometry (ICP - MS) method. The number of samples in this study was 30 samples; 15 blood samples of active smokers and 15 blood samples of passive smokers. The examination of arsenic levels was carried out at the West Java Provincial Health Laboratory. Based on the data, it was found that in the active smoker group, the average arsenic level was 0.53 μg/dL. While in passive smokers, the average arsenic level was 0.49 μg/dL. Then the Independent Sample T-Test test was conducted to determine the difference between the two groups of variables and the results were obtained p>0.05, based on these results there was no significant difference between arsenic levels in the blood of active smokers and passive smokers.

Abstract

Background: Schizophrenia is a mental disorder affecting thoughts, emotions, and behavior. Antipsychotic therapy is the main treatment; however, it may cause neurological side effects, including Neuroleptic Malignant Syndrome (NMS). Creatine Phosphokinase (CPK) is commonly used as a biomarker for detecting NMS. Objective: To evaluate the relationship between the duration of antipsychotic therapy and CPK enzyme activity and to determine the cut-off value of therapy duration associated with increased CPK levels in patients with schizophrenia. Methods: This cross-sectional study used secondary data from 43 schizophrenia patients treated at the West Java Provincial Mental Hospital between January and September 2024. Samples were selected using purposive sampling. Inclusion criteria were patients receiving single or combination antipsychotic therapy, while patients with heart disease or muscle disorders were excluded. Spearman correlation analysis was used to assess the relationship between therapy duration and CPK activity, and Receiver Operating Characteristic (ROC) analysis was performed to determine the cut-off value. Results: No significant relationship was found between the duration of antipsychotic therapy and CPK enzyme activity (p = 0.311). ROC analysis identified a therapy duration cut-off at 19–20 days, although its predictive value was weak (Asymp. Sig = 0.237). Conclusion: There was no significant relationship between the duration of antipsychotic therapy and CPK enzyme activity in patients with schizophrenia. Although a therapy duration cut-off of 19–20 days was identified, further studies with larger samples are needed.
