Abstract

Objective: This study aimed to determine the functional role of the urine leukocyte count on type of UTI. Material & Methods: This is a systematic review study with searches for articles from 2010-2020 through the PubMed, ProQuest, and EBSCO databases. The exclusion criteria in this study are patients using catheters (CAUTI), UTI patients in the Intensive Care Unit (ICU), and pediatric or infant patients. There were 1158 studies found and 3 studies synthesized. Results: There are 1130 patients studied. Age range from 18 to 89.1 years. Three studies showed that there was an association between the number of urinary leukocytes and the type of UTI (Tommaso Cai et al., p < 0.0001; and Alexander R. Levine et al., p < 0.001) and one study had no relationship. Conclusion: There is a relationship between the number of urinary leukocytes with the type of UTI. An increase in the number of urinary leukocytes above the normal limit is the sign of body’s immune response to eliminate uropathogens. The innate immune characteristic factors and the body’s immune response have an important role in causing symptoms in UTI patients.

Abstract

Objective: To obtain the association between the erythrocyte indices and the stages of CKD. Material & Methods: This is a systematic review study conducted by searching for articles through PubMed, Proquest, and Google Scholar. Publication year are from 2010 to 2020. Results: There are 10 articles analyzed. There were 3 articles that showed a significant association and 2 articles that showed no significant association between erythrocyte indices and stages of CKD. Other article that used anemia morphology to represent the erythrocyte indices showed the normocytic normochromic as the largest percentage. 4 out of 5 articles with CKD patients who were known to be undergoing hemodialysis showed a significant difference in erythrocyte indices. Conclusion: There is association between the erythrocyte indices and stages of CKD.

Abstract

Background: Health ministry is one of ministries in Indonesia who needs high fund every year. It is used for national health, starting from the availability of health infrastructure to the availability of medicine in health primary care and hospital. In Indonesia, there is a myth that joint pain is gout arthritis. In Indonesia, 71% people who have joint pain buy pain killer without going to get an examination by doctor first. Therefore, a research  about the relationship between knowledge about gout arthritis and NSAID and Allopurinol consumption in Rumah Susun Penjaringan. Objective : Knowing if there is association between knowledge about gout arthritis and NSAID and Allopurinol consumption. Methods : This research used cross sectional study and involved 68 people in Rumah Susun Penjaringan. Result : There is no significant relationship between knowledge about gour arthritis with NSAID (p = 0.234) and Allopurinol (0.666) consumption in Rumah Susun Penjaringan. Conclusion : There is no relationship between knowledge about gout arthritis with NSAID and Allopurinol consumption in Rumah Susun Penjaringan.

Abstract

Pendahuluan: Hipertensi dalam kehamilan merupakan masalah kesehatan utama pada wanita hamil dan bayi. Insidensi hipertensi dalam kehamilan terus terjadi peningkatan meskipun telah meningkatnya kualitas perawatan prenatal. Akan tetapi, patofisiologi terjadinya hipertensi dalam kehamilan belum diketahui pasti dan merupakan penyakit yang multifaktorial. Sampai saat ini belum ada cara untuk mencegah hipertensi dalam kehamilan. Tujuan: Makalah ini bertujuan untuk meningkatkan kewaspadaan terhadap kondisi hipertensi dalam kehamilan, tata laksana, dan komplikasinya. Metode: Makalah ini disusun menggunakan metode tinjauan narasi sebagai bagian dari studi pustaka. Simpulan: Diagnosis dini dan tata laksana yang bijak merupakan kunci dalam menurunkan risiko komplikasi hipertensi baik pada ibu maupun bayi.

Abstract

Pendahuluan: Sindrom Gitelman merupakan kondisi tubulopati kehilangan garam yang ditandai dengan alkalosis metabolik dengan hipokalemia, hipomagnesemia, dan hipokalsiuria. Kasus sindrom Gitelman pada kehamilan jarang dipublikasikan hingga saat ini. Laporan Kasus: Seorang wanita berusia 23 tahun pada kehamilan ketiga dengan usia gestasi 21 minggu datang dengan kelemahan pada kedua tungkai dan kekakuan pada kedua tangan, kemudian terdiagnosis dengan sindrom Gitelman (SG). Pasien memiliki riwayat emesis gravidarum dan defek septum atrium (DSA). Pada pemeriksaan neurologis didapatkan penurunan kekuatan motorik pada kedua tungkai. Pemeriksaan elektrolit menunjukkan adanya hiponatremia, hipokalemia, hipokalsemia, dan hipomagnesemia, serta terdapat peningkatan kadar natrium, kalium, dan klorida pada urin. Ekokardiografi menunjukkan adanya defek septum atrium sekundum dengan left-to-right shunt. Selama perawatan, pasien diberikan natrium, kalium, kalsium, dan magnesium secara intravena yang kemudian dilanjutkan secara oral. Pasien kemudian melahirkan bayi yang sehat dan tidak ditemukan komplikasi selama dan sesudah persalinan. Dua bulan setelah persalinan, pasien kontrol ke poliklinik penyakit dalam dan kondisinya stabil dengan dosis suplementasi kalium yang diturunkan. Diskusi: Pasien dengan SG mengalami gangguan dalam kon-servasi kadar kalium dan magnesium. Kondisi ini dapat diperburuk dengan perubahan fisiologis yang terjadi selama kehamilan, meliputi ekspansi volume serta meningkatnya aliran darah ke ginjal dan laju filtrasi glomerulus, yang berkontribusi terhadap hipo-kalemia. Selain itu, terdapat peningkatan kebutuhan kalium dan magnesium pada populasi ibu hamil. Efek protektif yang menurun juga semakin memperburuk penurunan kadar kalium dan magnesium. Simpulan: Diagnosis dan tatalaksana yang baik dapat membantu ibu hamil dengan SG dan DSA menjalani persalinan dengan lancar dan melahirkan bayi yang sehat.

Abstract

Chronic inflammatory condition in chronic kidney disease (CKD) patients is associated with increased risk of cardiovascular morbidity and mortality. Gut dysbiosis is assumed as one of leading factors to the chronic inflammatory condition. The relationship between the kidney and the gastrointestinal, knowns as the gut-kidney axis, has a role in production and accumulation of uremic toxins derived from gut microbial fermentation of protein, and translocation of endotoxins and microbial from gut lumen into bloodstream due to alterations of intestinal epithelial barrier in CKD patients. Probiotics supplementation is one of the optional theraphy to restore the gut dysbiosis in CKD patients. Recent studies found that probiotics supplementation in CKD patients decreased uremic toxins and pro-inflammatory cytokines production, and delayed CKD progression. The improvement of this chronic inflammatory condition is expected to decrease cardiovascular disease risk in CKD patients. This review aims to describe the importance of gut-kidney axis in CKD patients, particularly in gut dysbiosis, and the role of probiotics in progression of CKD.

Abstract

This review aimed to investigate the interplay between dietary components and epigenetic modulation in the pathogenesis of hypertension. A comprehensive literature search encompassing all published primary and secondary research dating up to March 2024 was carried out on several electronic databases, including MEDLINE, EBSCO-Host, Science Direct, ProQuest, and Google Scholar. Individual genomes and dietary intake exhibit a bidirectional relationship, influencing the hypertension risk. Unhealthy dietary patterns can compromise DNA integrity through DNA methylation and histone acetylation, ultimately affecting both systolic and diastolic blood pressure. Dietary macronutrient composition (carbohydrates, lipids, and proteins) significantly alters the expression of specific microRNAs (miRNAs) known to regulate endothelial function and blood pressure homeostasis. Moreover, micronutrients (vitamin A, D, E, Zinc, Iodine, and Sodium) can exert epigenetic effects on blood pressure via receptor interactions, potentially modifying cardiovascular disease risk. Dietary imbalances in macro and micronutrients can epigenetically influence hypertension development. Addressing these deficiencies through targeted interventions may offer a complementary approach to hypertension treatment.
