Abstract

Pendahuluan: Sindrom Gitelman merupakan kondisi tubulopati kehilangan garam yang ditandai dengan alkalosis metabolik dengan hipokalemia, hipomagnesemia, dan hipokalsiuria. Kasus sindrom Gitelman pada kehamilan jarang dipublikasikan hingga saat ini. Laporan Kasus: Seorang wanita berusia 23 tahun pada kehamilan ketiga dengan usia gestasi 21 minggu datang dengan kelemahan pada kedua tungkai dan kekakuan pada kedua tangan, kemudian terdiagnosis dengan sindrom Gitelman (SG). Pasien memiliki riwayat emesis gravidarum dan defek septum atrium (DSA). Pada pemeriksaan neurologis didapatkan penurunan kekuatan motorik pada kedua tungkai. Pemeriksaan elektrolit menunjukkan adanya hiponatremia, hipokalemia, hipokalsemia, dan hipomagnesemia, serta terdapat peningkatan kadar natrium, kalium, dan klorida pada urin. Ekokardiografi menunjukkan adanya defek septum atrium sekundum dengan left-to-right shunt. Selama perawatan, pasien diberikan natrium, kalium, kalsium, dan magnesium secara intravena yang kemudian dilanjutkan secara oral. Pasien kemudian melahirkan bayi yang sehat dan tidak ditemukan komplikasi selama dan sesudah persalinan. Dua bulan setelah persalinan, pasien kontrol ke poliklinik penyakit dalam dan kondisinya stabil dengan dosis suplementasi kalium yang diturunkan. Diskusi: Pasien dengan SG mengalami gangguan dalam kon-servasi kadar kalium dan magnesium. Kondisi ini dapat diperburuk dengan perubahan fisiologis yang terjadi selama kehamilan, meliputi ekspansi volume serta meningkatnya aliran darah ke ginjal dan laju filtrasi glomerulus, yang berkontribusi terhadap hipo-kalemia. Selain itu, terdapat peningkatan kebutuhan kalium dan magnesium pada populasi ibu hamil. Efek protektif yang menurun juga semakin memperburuk penurunan kadar kalium dan magnesium. Simpulan: Diagnosis dan tatalaksana yang baik dapat membantu ibu hamil dengan SG dan DSA menjalani persalinan dengan lancar dan melahirkan bayi yang sehat.

Abstract

Septic arthritis is considered as a medical emergency which can lead to significant morbidity and cause substantial mortality, especially if the diagnosis is delayed. Prolonged use of immunosuppressive and cytotoxic medications as therapy for systemic lupus erythematosus (SLE) causing patient susceptible to secondary infection. However, septic arthritis due to Salmonella sp. is very rare,  makes this is an important extraintestinal manifestation especially in immunosuppressed patients. We presented a case of 25 y.o. female diagnosed with SLE 3 m.o. earlier presented with fever and arthritis on her left genu for 1 wk duration. Genu ultrasonography showed synovitis genu sinistra with fluid volume of 1-2 cc on recessus lateral genu sinistra. The patient was further analysis on her synovial fluid was conducted, the gram stained smear of the fluid showed >25 leucocytes, low power field, and Salmonella sp. was isolated from her synovial fluid analysis. The patient was given intravenous ciprofloxacin and discharged home well. Septic arthritis should always be considered in any patients on long-term immunosuppression state who present with acutely swollen joints. It considered as an important medical emergency with high mortality and morbidity. Hence, prompt recognition, joint aspiration with administration of systemic antibiotics and appropriate surgical intervention plays a pivotal role to minimize morbidity and mortality.

Abstract

Background: Situation of COVID-19 pandemic has become a global health problem and has caused public health anxiety, especially for medical students. Anxiety disorder experienced by a person can be treated by pharmacological or non-pharmacological therapy. One of many procedures of non-pharmacological treatments that can be done is by intervening in western classical instrumental music. According to existing studies, music interventions are known to have the effect of providing calm, elevating mood, and making a person feel more relaxed. This study aimed to determine the effectiveness of receptive western classical instrumental music intervention in overcoming anxiety disorders in preclinical medical students of FKIK Atma Jaya University during online classroom learning during the COVID-19 pandemic.Methods: This study was an experimental study composed of a single group with a research design of pre-test and post-test done by 78 medical students from FKIK UnikaAtma Jaya Batch 2021. The measuring instrument used was the GAD-7 questionnaire. The data analysis performed was univariate, bivariate, and multivariate analysis.Results: There were 78 respondents, with the majority aged 17-18 years old. The majority gender was female (61.54%). Most of the respondents were in the upper socioeconomic class (84.62%). The 78.21% of students live with their parents. Bivariate analysis showed a decrease in student anxiety before and after the music intervention. Multivariate analysis showed a significant relationship between anxiety level and place of residence.Conclusion: Western instrumental classical music can reduce anxiety in medical students of FKIK UnikaAtma Jaya Batch of 2021 during online learning.

Abstract

Latar belakang: Tuberkulosis (TB) merupakan salah satu masalah kesehatan masyarakat yang penting di dunia, sepertiga penduduk dunia telah terinfeksi kuman TB. Dalam upaya menanggulangi penyakit TB, dibutuhkan diagnosis yang dapat ditegakkan melalui pemeriksaan bakteriologi, radiologi, dan pemeriksaan penunjang lain, seperti pemeriksaan nilai laju endap darah (LED). Infeksi TB berpengaruh terhadap peningkatan nilai LED dan pembentukan lesi radiologi pada TB Paru. Tujuan: untuk mengetahui korelasi nilai LED dengan gambaran luas lesi radiologi toraks pada pasien TB Paru di RS Atma Jaya Jakarta. Metode: Desain dalam penelitian ini adalah korelatif analitik dengan pendekatan cross-sectional. Sampel penelitian ini yaitu pasien TB paru dewasa baru aktif yang melakukan pengobatan di RS Atma Jaya sebanyak 68 pasien yang diambil dengan teknik consecutive sampling. Pengolahan data menggunakan metode korelasi Pearson dengan interval kepercayaan 95%, dilanjutkan dengan metode analisis regresi linear. Hasil: Penelitian menunjukan hubungan yang bermakna antara nilai laju endap darah (LED) dengan luas lesi radiologi toraks dengan koefisien korelasi 0,452  (p = 0,0001), artinya mempunyai kekuatan korelasi sedang. Hasil dari faktor perancu usia (p=0,906), jenis kelamin (p=0,887), dan gula darah sewaktu (p=0,056) tidak bermakna terhadap hubungan nilai LED dengan luas lesi radiologi toraks. Kesimpulan: Penelitian ini menunjukkan adanya hubungan bermakna antara nilai LED dengan luas lesi radiologi toraks pada pasien TB paru di RS Atma Jaya.

Abstract

Introduction: World Health Organization (WHO) estimated the number of DM patients in Indonesia in year 2030 could reach up to 21,3 million people. This study was conducted to understand the correlation between HbA1c levels and albuminuria in DMT2 patients.Methods: Data included age, sex, levels of HbA1c, blood pressure, proteinuria, smoking habit, usage of ACE-Inhibitor and/or Angiotensin Receptor Blocker, and duration of treatment that was collected from the medical records of patients DMT2 in Atma Jaya Hospital according to certain inclusion and exclusion criteria using the cross-sectional study with the Spearman correlation method with 95% confidence interval (á = 0.05).Result: There’s a correlation between HbA1c levels and albuminuria (p less than 0,0001) with a quite strong correlation (r=0,439). There is also a relation between age (p less than 0.05), while blood pressure, smoking, and ACE-Inhobitor or Angiotensin Receptor Blocker consumption are not related significantly. Conclusion : This study showed a correlation between HbA1c levels and albuminuria in Type 2 DM patient in Atma Jaya Hospital.

Abstract

Introduction: Tuberculosis (TB) is an important health problem in developing countries. This infection results neutrophilia and lymphopenia. The neutrophil to lymphocyte ratio (NLR) is an easy, inexpensive, and sensitive examination. The NLR result in tuberculosis patients showed higher compare to normal population, with cut off greater than 2.16. The aim of this study was to know the difference of NLR result in adult TB patients before and after the intensive phase treatment. Methods: We collected all new cases adult TB patients who came to out-patient department in Atma Jaya Hospital, without any HIV infection, chronic kidney disease, diabetes mellitus, chronic obstructive pulmonary disease, and bacterial pneumonia. Baseline characteristic, leukocyte count, neutrophil and lymphocyte count, also NLR were collected before and after intensive phase treatment, then were analyzed using SPSS. Results: There were 44 participants included in this study, 23 (52.3%) of them were male, with median age were 37 years old. Eighty eight percent of the subject had pulmonary tuberculosis. The NLR result before treatment (2.77) was higher compare with the NLR result (2) after intensive phase treatment (p less than 0.001). Conclusion: There was a statistically significant decrease in NLR result after intensive phase treatment of adult TB patients.

Abstract

Anemia is commonly found in patients with chronic diseases and causing increased in morbidity and mortality. The prevalence of anemia of chronic disease is the second highest after iron deficiency anemia. Anemia of chronic disease oftenly occurs with iron deficiency anemia therefore both of them showed low in serum iron level. However, we should differentiate these two kinds of anemia in order to give the specific treatment. Some iron parameters have to be extracted to differentiate them. The main management for anemia of chronic disease is to treat the underlying causes.
