Abstract

Objective: Cataract is the leading cause of blindness in the world with increasing trend both in prevalence and incidence. Surgery is the only definitive treatment for cataract but often unaffordable for society in developing countries. The incidence of cataract could be prevented by understanding its risk factors and formulate health programs according to them. The purpose of this study was to assess the prevalence and risk factors of senile cataract among 50 years old or older Balinese population. Method: The population-based cross-sectional study was conducted from September 2015 until July 2016 in Bali province. The variable’s data were obtained through direct examination and interview. Poisson regression analysis was used to assess the definitive correlation between risk factors and cataract. A p-value <0.05 or value which falls within 95% confidence interval were considered significant. Result: 550 subjects were enrolled during the study. The number of cataracts found in this study was 231 (42%). According to the analyses, cataract was significantly associated with older age (PR: 3.05; p=0.001), low educational level (PR: 2.18; p=0.04), diabetes (PR: 2.96; p=0.001), and the rate of ultraviolet exposure (PR: 1.92; p=0,002). Conclusion: The prevalence of cataract among 50 years old or older in Balinese population was 42%. Older age, low education, diabetes mellitus, and ultraviolet exposure were found to be significant and independent risk factors for cataract in 50 years old or older Balinese population.Keywords: Cataract, Prevalence, Risk factor.

Abstract

Introduction: Ocular syphilis is a rare manifestation of syphilis. Ocular syphilis manifestations vary and can occur at all stages of the disease. Syphilis can affect all eye structures, the main complaint is blurred vision, accompanied by non-specific symptoms such as pain in the eye, and does not respond to steroids. Case: The male patient, 54 years old, presents with complaints of both eyes slowly getting worse with a history of sexual multi-partners. Visual acuity in both eyes were light perception bad projection, ophthalmological examination showed cloudy lens, cloudy vitreous, cell (+) flare (+) right eye optic nerve, hyperemia, and left eye optic nerve, pale at temporal. VDRL results: reactive> 1: 512 and TPHA: reactive> 1: 5120. The patient was diagnosed with late latent syphilis and neurosyphilis observation from the Dermatology and Venereology Department and received benzathine penicillin injection therapy. From the Neurology Department the patient was diagnosed with neuro-syphilis and was consulted to VCT department with pre-HAART Stage IV HIV infection (WHO). After receiving penicillin therapy and topical steroids, the patient was experienced improvement in visual acuity. Conclusion: In HIV patients without antiretroviral therapy, Treponema pallidum infection is more common and has manifestations such as ocular syphilis and neurosyphilis. Treatment of ocular syphilis using topical steroids to reduce inflammation and penicillin as the main antibiotic.

Abstract

Astenopia merupakan gejala subjektif yang timbul akibat penggunaan mata dalam aktivitas fisik jarak dekat. Upaya pembatasan penyebaran COVID-19 melalui perkuliahan daring menimbulkan peningkatan penerapan digitalisasi pada mahasiswa. Digitalisasi mempengaruhi fisiologis mata, dimana banyak di antara mahasiswa mengeluhkan kelelahan mata selama perkuliahan daring yang mewajibkan penerapan aktivitas fisik jarak dekat dalam waktu yang lama. Durasi aktivitas fisik jarak dekat yang berlebih menimbulkan ketidaknyamanan dan gerakan mata yang sering serta berisiko timbulnya astenopia. Penelitian ini untuk mengetahui hubungan durasi aktivitas fisik jarak dekat sebagai faktor risiko timbulnya astenopia Mahasiswa Fakultas Kedokteran Universitas Udayana yang menerapkan desain studi observasional dengan pendekatan analitik cross-sectional yang melibatkan 653 mahasiswa Pendidikan Dokter angkatan 2018-2020 Universitas Udayana. Pengambilan sampel sesuai pertimbangkan, yaitu purposive sampling. Bahan penelitian sesuai dengan data primer berupa kuesioner VFI (Visual Fatigue Index). Dari 653 mahasiswa Pendidikan Dokter angkatan 2018-2020 Universitas Udayana yang terlibat sebagai responden ditemukan sebanyak 465 mahasiswa (71,2%) mengalami astenopia. Rata-rata durasi aktivitas fisik jarak dekat yang yaitu pada durasi ? 4 jam. Hasil penelitian menunjukkan bahwa tiga aktivitas fisik jarak dekat yang diteliti meliputi, membaca, penggunaan smartphone, dan laptop atau komputer menunjukkan p value <0,05 dan nilai risk estimate PR>1, serta selang kepercayaan tidak melewati 1. Dalam penelitian ini ditemukan hubungan yang signifikan antara durasi aktivitas fisik jarak dekat dengan astenopia mahasiswa. Dalam penelitian ini disimpulkan bahwa terdapat hubungan bermakna antara durasi aktivitas fisik jarak dekat dengan astenopia pada mahasiswa Pendidikan Dokter angkatan 2018-2020 Universitas Udayana. Abstract

Mata merupakan salah satu organ yang digunakan manusia untuk berinteraksi dengan sesama manusia. Apabila terjadi penurunan visual, maka salah satu faktor resiko yang muncul adalah strabismus. Strabismus umum terjadi pada anak-anak dan menjadi salah satu faktor umum penyebab ambliopia. Oleh karena itu penulis ingin mengetahui apa karakteristik ambliopia pada anak strabismus di RSUP Prof. Dr. I.G.N.G. Ngoerah Denpasar pada tahun 2021. Desain penelitian ini adalah deskriptif dengan pendekatan cross sectional. Pengumpulan sampel dilakukan dengan total sampling dengan melihat rekam medis di RSUP Prof. Dr. I.G.N.G. Ngoerah periode Januari 2021 sampai dengan Desember 2021. Terdapat 18 pasien yang didiagnosis strabismus dengan adanya penurunan visus. Jumlah laki-laki 11 (61,1%) dan perempuan 7 (38,7). Sebagian besar pasien berada pada rentang usia 6-11 dan 12-17 tahun (n=8, 44,4%). Sebagian besar pasien berdomisili di daerah Badung (n=5, 83,3%). Terdapat 15 pasien (83,3%) tidak dijelaskan mengenai adanya riwayat keluarga dalam lembar rekam medis. Sebagian besar pasien memiliki arah deviasi keluar atau eksotropia (n=10, 55,6%). Sebagai kesimpulan, terdapat 18 kasus strabismus dengan ambliopia. Sebagian besar adalah laki-laki dengan kelompok usia 6-11 dan 12-17 tahun, memiliki arah deviasi eksotropia, berdomisili di Badung, dan tidak dijelaskan adanya riwayat keluarga pada rekam medis

Abstract

Background: Retinoblastoma (RB) is a common malignancy that affects children and is fatal if left untreated. This case report aims to raise awareness the importance of starting therapy immediately in RB patients. Case presentation: A girl aged 1 year 9 months, came with complaints that her left eye (LE) appeared white for 2 weeks. Anterior segment examination was normal. On funduscopic examination, optic nerve was difficult to evaluate, covered by a mass, multiple mass scars on the retina, a size of > 6 mm at the posterior pole, and vitreous seeding. Ultrasound examination and CT scan showed an intraocular mass with calcification. The patient was diagnosed with group D intraocular LE RB and was planned for enucleation but was postponed due to cost constraints. The patient came back after 1 year and was diagnosed with extraocular LE RB stage IVB. Conclusion: Early diagnosis without adequate therapy leads to tumor spread and reduces patient survival.

Abstract

Background: Systemic lupus erythematosus (SLE) is a chronic autoimmune disease known for its varied clinical presentations, often affecting multiple organ systems. Ocular involvement is common in SLE, but posterior segment manifestations like papilledema are rare, occurring in only about 1% of SLE patients. This case report details a unique instance of bilateral papilledema and macular edema as the primary presentation of suspected SLE in a young female patient. Case presentation: A 24-year-old female presented with a one-week history of sudden-onset blurry vision in both eyes. Ophthalmological examination revealed reduced visual acuity (6/45 in both eyes), bilateral optic disc swelling, and macular edema. Further investigations, including Optical Coherence Tomography (OCT) and Magnetic Resonance Imaging (MRI), confirmed macular edema and optic nerve sheath distention. A positive Antinuclear Antibody (ANA) test suggested an autoimmune etiology. Lumbar puncture results were normal, ruling out idiopathic intracranial hypertension. The patient was diagnosed with bilateral papilledema and macular edema, with suspected underlying SLE. Prompt treatment with high-dose corticosteroids and acetazolamide led to significant clinical improvement. Conclusion: This case highlights the rarity of bilateral papilledema and macular edema as initial presenting features of suspected SLE. It emphasizes the importance of thorough ophthalmological examination and relevant investigations in patients with sudden vision loss. Early recognition of such rare presentations is crucial to prevent potentially sight-threatening complications. This case underscores the need for a multidisciplinary approach for accurate diagnosis and management of complex presentations of systemic autoimmune diseases.
