Abstract

Background: Patient safety is one of the most important dimensions in quality of healthcare. Nurses are the key in care delivery, their compliance toward Patient Safety policy cannot be overlooked. Studies evaluating determinants of nurse’s compliance to Patient Safety policy is still lacking. Aims: To evaluate nurse’s attitude toward Patient Safety policy using Health Belief Model (HBM). Methods: An analytical cross-sectional study was carried on in Bali Royal Hospital, Indonesia, on December 2016. An anonymous, self-administered questionnaire was used to collect the data on socio demographics, knowledge, and attitude toward Patient Safety policy. Attitudes are specifically assessed using HBM. Results: A total of 124 nurses are included in this study. Mean age of the study population was 29.7±6.7 years. Mean score for knowledge about Patient Safety policy was 12.68±3.40 from 18.0- point scale. Nurses perceived a low risk of non-compliance (mean perceived risk 8.07±1.89) and a low barrier to comply with Patient Safety policy (mean perceived barrier 8.66±1.78). Knowledge and attitudes toward Patient Safety policy did not differ by sex and educational background but it did differ according to unit of workplace. Age and length of work correlated positively to knowledge (Spearman’s r=0.439, p-value 0.000; and r=0.400, p-value <0.001, respectively). A positive and moderate correlation was observed between knowledge vs. perceived severity (r= 0.394, p-value <0.001) and knowledge vs. perceived benefit (r=0.422, p-value < 0.001). There was a strong and positive correlation between perceived severity and perceived benefit (r= 0.725, p-value <0.001). Conclusion: Nurse’s attitudes toward Patient Safety policy were generally good and correlate positively with knowledge. The use of HBM provides an increased understanding of how individual perceptions can be influenced to improve nurse’s engagement in promoting safer health care.

Abstract

Backgrounds: Hospital waste management had become an international attention. One of the first meeting by Solid Hazardous Waste was related to domestic waste and medical waste. The previous study showed that nationally hospital solid waste expectedly reach 376.089 ton per day and the volume of wastewater was 48.985,70 ton per day. Aims: The purpose of this study is to examine the literatures on analysis of the cost effectiveness and efficiency hospital Wastewater Treatment Plant. Methods: This systematic review was based on Protocol Prisma. Literature searches were conducted using the electronic databases Google Scholar (2011 to present), SpringerLink, and Proquest.Over a total of 3.834 potential articles, 9 met all inclusion criteria. Results: Cost problems were founded due to there was no good financial planning, maintenance costs, and the wastewater quality supervision. The budget of wastewater treatment only focuses to the cost of damage, but only enough for one time laboratory examination. Moreover, we also highlighted another factors affected to effectiveness and efficiency where there was no regular training for sanitarian, and as the results, sanitarian cannot determine the best effective method, finally the budgets were only for authoritarian system. This paper also reviews methods and applications to reduce the cost in electrical, chemical, and maintenance, by new wastewater treatment alternatives. If compared the two techniques, extended aeration and sequencing batch reactors, evidently, sequencing batch reactor had lower cost than extended aeration. Conclusions: The cost effectiveness and efficiency of wastewater management can be achieved by good management systems, good knowledge about waste, and different methods result different efficiency too.

Abstract

Background: Nosocomial infections or known as Hospital Acquired Infections (HAIs) is a worldwide problem. VAP is nosocomial pneumonia which occurs more than 48 hours after installation of mechanical ventilation, either through an endotracheal tube or tracheostomy. The patient in ICU have high risk on VAP infection and increasing morbidity, mortality and also prolonged length of hospital stay Methods: This research is an observational study evaluating conditions before and after a specific standard operating procedure being implemented in reducing VAP infection in ICU EKA Hospital Pekanbaru. Results: It’s obtained from medical record data of 339 patients hospitalized at ICU of Eka Hospital Pekanbaru ranging from 2016 to 2017. There is significant decrease number of VAP infections in 2017 by 60%. The reduction occurred due to the application of a routine VAP Bundle covering 100% for all hospitalized ICU patients. Conclusions: Strict monitoring and supervision are required to maintain VAP bundle and reduce the incidence of VAP infection in ICU of Eka Hospital Pekanbaru.

Abstract

Medication Errors are the 4th leading cause of death in USA; to decrease medication errors, Automated Dispensing System had been used in USA. In Indonesia, most pharmaceutical departments still use manual system in drugs dispensing. This study analyze the effect of ADM (Automated Dispensing Machine) to medication errors, particularly dispensing errors in hospital pharmacy. Method: Systematic Review with PRISMA method retrieved from online database such as Scopus, Tripdatabase, Springerlink, and Google Scholar using keywords “automated dispensing machine” AND “medication errors” studied in the last ten years. Results and Discussion: All eleven studies show beneficial effects in ADM implementation, decreasing dispensing errors, both at outpatient and inpatient ward. Five studies suggest that ADM should be integrated with ePrescription, barcoding system, and hospital information system for optimal result. Automatic filling system also decrease dispensing error. ADM also gives beneficial effect in reducing pharmacy staffs’ workload, drug waiting time, drug inventory cost, and employee costs. ADM systems have some weaknesses : difficult to apply at emergency services, need trained staffs, and high costs. Conclusion ADM gives beneficial effect in reducing medication error. For better results, ADM should be integrated with hospital information system.

Abstract

The increase in non-communicable diseases such as hypertension, stroke, diabetes mellitus, joint disease, lack of physical activity, injury, and disability in line with the increasing need for physiotherapy services in health facilities, including in Public Health Centers. The role of the physiotherapist in the Public Health Centers is to carry out activities in the form of promotive and preventive without prejudice to curative and rehabilitative. The purpose of this study is to analyze physiotherapy services at the Public Health Centers using five levels of prevention measures, namely are health promotion, specific protection, early diagnosis and prompt treatment, disability limitation, and rehabilitation efforts at 6 PHC in DKI Jakarta. This study uses qualitative research methods through a phenomenological approach. The results of this study obtained a depth-overview of the efforts of health services that the provision of medical services is preferred over promotive and preventive efforts. Physiotherapy services at the PHC have been regulated in the Decree of the PHC’s Head by referring to Permenkes 75 (2014). Public health activities in collaboration with the Physiotherapy Services are only carried out by 3 PHC, and some of them only conduct individual services. This study recommends that adding one physiotherapist to be able to help physiotherapy services outside the building PHC and physiotherapy organizations can create public health training to support physiotherapy competencies.
