Abstract

  Coronary heart disease (CHD) is caused by obstruction of coronary blood flow due to endothelial dysfunction triggered by various genetic and non-genetic risk factors such as hyperlipidemia, hypertension, hyperglycemia and obesity. Endothelial cell activation due to hyperglycaemia in diabetes mellitus induces production of pro-inflammatory factors that damage the cell membrane triggering the formation of membrane particles called microparticles. Endothe-lial microparticles contain proteins including endothelial nitric oxide synthase (eNOS) which plays a role in the production of nitric oxide (NO). To determine the role of microparticles in the occurrence of coro-nary heart disease in diabetes mellitus due to endothelial dysfunction, a study was conducted by comparing the levels of eNOS and NO in DM patients who had CHD with DM patients who had no CHD. Blood samples from 20 DM patients who had CHD and 20 DM patients who had no CHD of the outpatients in Cardiology Department and Inter-nal Medicine department of regional public hospital were included in this study. All patients were fulfilled inclusion and exclusion criteria and diagnosed by the appropriate specialist. The eNOS and NO lev-els were measured using the ELISA method. The results of this study show that eNOS levels in the group of DM patients who had CHD (21,292±12,415 ng/ml) were significantly lower (p <0.05) than those in the group of DM patients who had no CHD (29,721±11,952 ng/ml). Nitric oxide levels in DM patients who had CHD (0,053±0,021 nmol/ μl) were not statistically different to the levels in DM patients who had no CHD (0,047±0,032 nmol/μl). From the results of this study we concluded that endothelial microparticle protein eNOS plays a role in the occurrence of CHD due to the complications of diabetes mellitus

Abstract

Rendahnya pemahaman mengenai pentingnya aktivitas fisik pada anak usia sekolah dasar dapat berdampak pada pembentukan gaya hidup sedentari sejak dini. Kegiatan pengabdian masyarakat ini bertujuan meningkatkan pengetahuan siswa melalui edukasi kedokteran olahraga di SDIT Adzkia Padang. Intervensi dilakukan melalui penyuluhan terstruktur dan demonstrasi aktivitas fisik, dengan evaluasi penilaian awal dan penilaian akhir. Tingkat pengetahuan siswa dievaluasi sebelum dan sesudah intervensi menggunakan tes tertulis dan dianalisis secara deskriptif untuk mengidentifikasi tingkat pengetahuan siswa. Hasil kegiatan menunjukkan adanya peningkatan tingkat pengetahuan setelah edukasi, yang ditandai dengan meningkatnya proporsi nilai pada kategori tinggi serta tidak ditemukannya lagi nilai pada kategori rendah. Temuan ini menunjukkan bahwa edukasi kedokteran olahraga yang terstruktur dan interaktif efektif dalam meningkatkan pemahaman siswa mengenai aktivitas fisik. Oleh karena itu, kegiatan ini dapat digunakan sebagai pendekatan promotif berbasis sekolah untuk mendukung pembentukan perilaku hidup aktif pada anak usia dini.

Abstract

Background Coronary heart disease is one of the causes of morbidity and mortality worldwide, especially in Indonesia. Coronary heart disease is closely associated with dyslipidemia, particularly increased low-density lipoprotein (LDL), which plays an important role in the formation of atherosclerosis. High-intensity statin therapy is recommended for patients with coronary heart disease after percutaneous coronary intervention (PCI) to optimally reduce LDL and reduce the incidence of major adverse cardiovascular events (MACE) due to its pleiotropic effects. Objective To review the scientific evidence regarding the effectiveness, safety, and clinical outcomes of the use of high-intensity statins in patients with coronary heart disease after PCI based on the current literature. Methods The literature was obtained from the PubMed, Scopus, and Google Scholar databases published within the last ten years. The included studies consisted of randomized clinical trials, retrospective and prospective cohort studies, observational studies, systematic reviews and meta-analyses, and narrative reviews. The evaluated outcomes included LDL target achievement, major adverse cardiovascular events (MACE), and safety profiles including myopathy, hepatotoxicity, diabetes mellitus, and other adverse effects. Results Most studies showed that high-dose statins effectively reduced LDL levels and provided clinical benefits in patients with cardiovascular disease. A comparison of atorvastatin 40 mg and 80 mg showed similar effectiveness in reducing LDL, with a low incidence of adverse effects and still tolerable. In patients after PCI, atorvastatin 80 mg and rosuvastatin 40 mg had comparable effectiveness, although atorvastatin showed a better safety profile and cost efficiency. Several studies reported that the combination of moderate-dose statins with ezetimibe resulted in higher achievement of LDL targets compared with high-dose statin monotherapy. In addition, the combination was associated with lower risks of muscle-related adverse effects, increased liver enzymes, and diabetes mellitus. Nevertheless, most high-risk patients still do not achieve the recommended LDL target, so treatment intensification strategies are needed, including the addition of ezetimibe or PCSK9 inhibitors.

Abstract

Background: Acute Decompensated Heart Failure (ADHF), often caused by fluid retention and elevated left ventricular filling pressure, is the most common form of acute heart failure, accounting for 50-70% of cases. It carries high morbidity and mortality, with in-hospital death up to 11% and one-year post-discharge mortality reaching 36%. Rehospitalization rates range from 22-30% within 1-3 months to 65% within one year. In Indonesia, the 2013 prevalence of physician-diagnosed heart failure was 0.13%, affecting approximately 230,000 people. Methods: This prospective observational study was conducted on ADHF patients admitted to the Cardiology Department of Dr. M. Djamil General Hospital, Padang, during July-August 2025. Blood and N-Terminal pro-B-type Natriuretic Peptide (NT-proBNP) assessments were obtained at admission and upon discharge. Participants were divided into two categories based on their estimated Plasma Volume Variation (ePVV) results (<0% and ≥0%). The Mann-Whitney test was used to compare NT-proBNP variations between these groups. Results: A total of 33 participants were included, the majority being male, with pneumonia as the most prevalent comorbidity and ischemic heart disease as the primary underlying cause. Patients with ePVV <0% showed a median change in NT-proBNP of 21.06%, whereas those with ePVV ≥0% showed a median change of 9.58%. The Mann-Whitney test indicated that this difference between groups was statistically significant (p = 0.012). Conclusion: The observed variation in NT-proBNP levels across ePVV categories suggests that ePVV may be a valuable non-invasive marker for assessing congestion status in ADHF. Combining ePVV measurements with NT-proBNP monitoring could enhance clinical assessment and management strategies for patients with heart failure.
