Abstract

Abstrak Tuberkulosis Disseminata didefinisikan sebagai penyebaran dua atau lebih pada organ tubuh yang disebabkan oleh penyebaran lymphohematogenous Mycobacterium tuberculosis. Dilaporkan pasien wanita usia 24 tahun dengan keluhan utama sesak nafas, serta adanya ulkus pada aksila, leher dan lengan. Ulkus ini diawali dengan munculnya benjolan yang kemudian pecah dan bernanah. Biopsi kulit didapatkan gambaran tuberkulosis kulit. Pemeriksaan pencitraan dikombinasikan dengan gambaran histopatologi, kecurigaan klinis dan perbaikan dengan terapi OAT diperlukan untuk mengkonfirmasi diagnosis, terutama dalam kasus tuberkulosis ekstrapulmoner. Kejadian tuberkulosis diseminata jarang, biasanya menyerang pasien dengan gangguan kekebalan tubuh. Pada pasien ini telah dilakukan berbagai pemeriksaan untuk menelusuri kondisi imunodefisiensi tapi tidak ditemukan. Gambaran lesi paru miliar pada rontgen thorak sering ditemukan pada pasien tuberkulosis diseminata dan gambaran ekstrapulmoner yang bervariasi. Diagnosis pasti tuberkulosis ekstrapulmoner sangat sulit, bergantung pada temuan histologis dan/atau bakteriologis dari hasil biopsi jaringan.

Abstract

AbstrakPemerintah berupaya menekan laju pertumbuhan Indonesia dengan melaksanakan program Keluarga Berencana (KB) yang tercantum dalam Rencana Pembangunan Jangka Menengah (RPJM) tahun 2004-2009 adalah meningkatkan penggunaan kontrasepsi jangka panjang. Saat ini terdapat berbagai metode kontrasepsi. Banyak wanita mengalami kesulitan dalam memilih kontrasepsi. Tujuan penelitian ini adalah menentukan hubungan karakteristik ibu dengan pemilihan kontrasepsi. Jenis penelitian yang digunakan bersifat deskriptif analitik dengan metode cross sectional.  Populasi penelitian adalah seluruh pasangan usia subur yang sudah menikah dan masih aktif menjadi akseptor KB.  Sampel diambil dengan menggunakan teknik consecutive sampling berjumlah 96 responden. Variabel dependen pada penelitian ini adalah  umur ibu, jumlah anak dan tingkat pendidikan sedangkan variabel independen adalah pemilihan kontrasepsi. Hasil penelitian didapatkan 29 responden memilih kontrasepsi IUD (30,21%) dan 67 responden memilih kontrasepsi non-IUD (69,79%).  Hasil analisis bivariat menunjukkan tingkat pendidikan memiliki hubungan yang bermakna dengan pemilihan kontrasepsi (p=0,000), sedangkan umur dan jumlah anak tidak memiliki hubungan yang bermakna dengan pemilihan kontrasepsi (p=0,590). Dapat disimpulkan bahwa ada hubungan bermakna antara tingkat pendidikan dengan pemilihan kontrasepsi. Seseorang dengan tingkat pendidikan tinggi cenderung memilih kontrasepsi IUD.Kata kunci: kontrasepsi, IUD, KB AbstractThe government efforts to suppress rate of population growth in Indonesia with doing The Family Planning program (KB) stated in the Medium Term Development Plan in 2004-2009 was increasing use of the long-term contraceptive method. Currently there are various methods of contraceptive. Many women find it difficult to choose contraception.  The objective of this study was to determinet the relationship of mother characteristic to selection contraceptive.Type of this research use descriptive analytic with a cross sectional method. The population was all couples of childbearing age that already married and active to be KB acceptor.  The 96 respondents were taken by using consecutive sampling. The dependent variable of this reaserch were age, number of children and education while the independent variable was selection contraceptive. The result showed 29 respondents use IUD (30.21%) and 67 respondent use non IUD (69.79%). The result of bivariate analysis showed that the education had significant relation to selection contraceptive (p=0.000), but the age and number of children did not have significant relation to selection contraceptive (p=0.590). It can be concluded that there is a significant relation between the education to selection contraceptive. A person with high education levels tend to choose the IUD.Keywords:  contraceptive, IUD, family planning

Abstract

AbstrakPenyakit ginjal merupakan penyebab kematian nonAIDS paling tinggi pada penyandang HIV. Penggunaan regimen anti- retroviral diselidiki memiliki kontribusi terhadap kejadian penyakit ginjal. Disfungsi tubulus ditemukan pada penyandang HIV dengan terapi tenofovir. Pemeriksaan kreatinin digunakan untuk skrining kerusakan ginjal. Tujuan penelitian ini adalah untuk mengetahui perbedaan kadar kreatinin sebelum dan setelah terapi tenofovir. Telah dilakukan penelitian secara kohort retrospektif terhadap penyandang HIV dengan terapi tenofovir di RS dr. M. Djamil Padang periode 2012-2013. Dilakukan pemeriksaan kreatinin serum sebelum dan setelah terapi tenofovir dengan metode Jaffe. Data kadar kreatinin serum didapatkan melalui rekam medik. Hasil perbedaan rata-rata kadar kreatinin serum sebelum dan setelah terapi tenofovir dianalisis menggunakan uji T berpasangan. Sebanyak 18 subyek penelitian dipilih dari jumlah total 652 penyandang HIV rawat inap dan rawat jalan. Sebanyak 196 penyandang HIV memakai terapi tenofovir (30%). Rentang lama pemberian terapi tenofovir adalah 2-57 minggu. Pada hasil penelitian, didapatkan perbedaan rata-rata kadar kreatinin serum yang bermakna sebelum dan setelah terapi tenofovir sebesar 0,7± 0,2 mg/dLdan 0,9 ± 0,5 mg/dL (P<0,05). Dapat disimpulkan bahwa terdapat peningkatan kreatinin serum setelah terapi tenofovir pada penyandang HIV.Kata kunci: HIV, tenofovir, kreatinin serumAbstractKidney disease is the highest nonAIDS related mortality among HIV patients. The use of antiretroviral therapy is investigated to contribute in kidney disease. Tubular dysfunction is found in HIV patients with tenofovir therapy. Creatinine test is used to screen kidney dysfunction. The aim of this study was to determine the mean difference of serum creatinine level before and after tenofovir administration.A cohort retrospective research was carried out in DR..M. Djamil Hospital upon HIV patients with tenofovir within 2012-2013. Serum creatinine test was conducted before and after administration of tenofovir with automatic machine and Jaffe reaction. The result of mean difference of serum creatinine before and after administration of tenofovir is analised by paired T test.Eighteen research subjects is determined from total amount of in and out patient. A total of 196 patients were administered with tenofovir (30%). The range of tenofovir administration was between 2-57 weeks. From the research, obtained a significant mean difference before (0,7± 0,2 mg/dL) and after (0,9 ± 0,5 mg/dL) administration of tenofovir (P<0,05).The conclusion of this study is there is increasing of serum creatinine level before and after administration of tenofovir.Keywords: HIV, tenofovir, serum creatinine

Abstract

Infectious diseases are still a major health issue that requires serious treatment. The covid-19 is now one of the significant infectious diseases worldwide marked by rapid contagion and global spread. The world health organization (who) has defined covid-19 as a global pandemic and the government has set public health emergencies in Indonesia, obliging countermeasures. The covid-19 countermeasures should continue massively with several strategies. In addition to promotive strategies and the application of health protocols, another effective strategy is needed to break off the chain of disease transmission through vaccination efforts. The purpose of the activity is to do the education about covid-19 and the covid-19 vaccination of the people in Kampung Batu Busuak, Kelurahan Limau Manis, Kecamatan Pauh, Padang. The implementation of this activity includes the counseling of covid-19 and the covid-19 vaccination on Saturday, November 13th, 2021. Before counseling, the Questionnaire was issued to assess a public knowledge level on covid-19 and a covid-19 vaccination. From a questionnaire on covid-19 and covid-19 vaccination, the total of people who have already been vaccinated twice is 22 people and 4 with the covid-19 vaccine once. Therefore, still needed counseling about covid-19 and covid-19 vaccination to enhance awareness of the society towards the importance of covid-19 vaccination.

Abstract

Background: Since December 2019, a novel coronavirus called SARSCoV-2 (severe acute respiratory syndrome coronavirus) has caused an international outbreak of respiratory illness described as COVID-19. This study aimed to describe the difference in the survival rate of COVID-19 induced AKI with hemodialysis and COVID-19 in patients with CKD on hemodialysis in Dr. M Djamil General Hospitals. Also, in this review, we provide a comprehensive overview of data on the factors that may be affected by COVID-19 survival rates in patients with COVID-19 induced AKI with hemodialysis and COVID-19 in patients with CKD on hemodialysis. This study was conducted to analyze the survival of COVID-19 with initiated or regular HD patients in Dr. M. Djamil General Hospital, Padang, Indonesia. Methods: This study was conducted from January 2021 to July 2021 in Dr. M. Djamil General Hospital, Padang, West Sumatera, Indonesia. Data for this study was collected through medical records of patients admitted for COVID-19 with CKD in hemodialysis and acute renal failure induced by COVID-19 to show the demographics, comorbidities, and survival rates of the patients who underwent hemodialysis. Results: Factors associated with survival in COVID-19 with hemodialysis were COVID-19 severity and abnormal potassium serum level (Table 3). Moderate COVID-19 patients tend to survive than severe COVID-19 patients (OR 60; 95% CI 16.034 – 224.525). There was no significant difference in survival between initiated and regular HD (p = 0.829). Conclusion: There is no difference in clinical outcome from patients with COVID-19 who initiated hemodialysis or regularly HD to the survival rates.

Abstract

The rapid increase in the number of Coronavirus Disease 2019 (COVID-19) cases, including in Padang, has led the government to designate several hospitals as referrals for COVID-19 patients. The profile of COVID-19 patients is needed to describe the pattern of this disease in Padang. Objectives: To revealed the profile of COVID-19 patients in SPH, Indonesia. Methods: This was a retrospective study. This study was conducted and collected data of COVID-19 patients from March to June 2020. Results: There are 54 patients with mean age of 50.93 (14.6) years old, 43% male and 57% female. The clinical manifestations are cough (74%), fever (37%), sore throat (22%), and fatigue (35%). There are 82% of patients admitted that they have contacted with other COVID-19 patients or have activities in COVID-19 cluster area, and only 7% have traveled to other cities. The mean length of stay is 31.91 (14.87) days. The mean laboratory data: Hemoglobin 13.7 (1.39) gr/dl, WBC 8,037.04 (2,846.27) /mm3, platelet 274,389 (75,053.80) /mm3, and NLR 3.16 (3.37). Chest radiograph results are bronchopneumonia (22.22%), cardiomegaly (7.41%), and normal (70.37%). Conclusion: The profile of COVID-19 patients in SPH is more in female patients, cough is the most clinical manifestation that appeared, the mean laboratory data (hemoglobin, WBC, and platelet) are normal, and normal chest radiography is the most chest radiograph results in these patients.Kata kunci: COVID-19, Padang, profile

Abstract

The COVID-19 pandemic has caused substantial morbidity and mortality worldwide. Older patients, male gender and those with preexisting comorbidities such as chronic kidney disease are reported to be more likely infected with SARS CoV-2 and are at higher risk of severe illness or death. It has been reported a 24 years old male was admitted to the hospital with shortness of breath, coughing, fever and paleness. The history of contact with confirmed COVID-19 cases was unclear. The patient works as a security officer. A history of hypertension is present. Laboratory results showed hemoglobin 7 g/dl, ureum 261 mg/dl, and creatinine 22,9 mg/dl. On the second day of admission, the patient experience increased shortness of breath, decreased consciousness and epistaxis. From the result of the nasopharyngeal swab, the patient tested positive for COVID-19 and was given Oseltamivir 75mg. The patient is prepared for hemodialysis, which was performed in the isolation room. After hemodialysis, the patient's condition improved with decreased shortness of breath and increased of consciousness. The patient comes out from the isolation room and discharges home in good condition. Antiviral therapy in CKD patients with Covid-19 infection requires dose adjustment. Immediate hemodialysis is required in patients with CKD and coexisting COVID-19 infection to improve the patient's condition. Prompt management for patients with CKD and COVID-19 will reduce the risk of mortality.Keywords:  COVID-19, chronic kidney disease, hemodialysis

Abstract

Miliary tuberculosis occurs due to the hematogenous spread of Mycobacterium Tuberculosis from the primary complex.  The use of steroid and cytotoxic drugs increases the incidence of miliary tuberculosis.  Typical manifestations of miliary tuberculosis is snowstorm appearance seen on chest x-ray and evidence of tuberculosis microorganism from the microbiological examination.  It has been reported a 33 years old male patient was admitted to the hospital due to breathlessness, chronic coughing, fever, anorexia, weight loss and night sweating with a "damp shadow sign".  Due to chronic myelogenous leukemia and autoimmune hemolytic anemia, the patient is known under imatinib and steroid therapy.  There was no specific sign found from the physical examination.  Chest x-ray showed snowstorm appearance.  The patient underwent GeneXpert MTB/RIF, with the result low MTB detected.  The patient was treated with a 2(HRZE)/4(HR) tuberculosis drugs regimen.  Imatinib and steroid therapy was discontinued.  2-4 weeks of steroid usage with a daily dose equivalent to prednisone 15 mg increases the risk of activating a latent tuberculosis infection.  Imatinib affects the response of T cells to Mycobacterium, thereby triggering tuberculosis reactivation.  In this case, the diagnosis has been made from typical manifestations of tuberculosis, snowstorm appearance from the chest x-ray, and low MTB detected from GeneXpert MTB/RIF.  It has been recommended to detect latent tuberculosis infection before using steroid and cytotoxic drugs.Keywords:  imatinib, miliary tuberculosis,  steroid

Abstract

Kandidat transplantasi organ berisiko tinggi untuk terjadinya komplikasi pasca operasi, terutama komplikasi paru yaitu infeksi dan non infeksi. Organisme penyebab infeksi pasca transplantasi dapat dibagi atas fase pasca transplantasi, maksimum imunosupresi dan sesudahnya. Infeksi dan mortalitas TB pada pasien transplantasi organ lebih tinggi dibandingkan populasi umum. Komplikasi non infeksi sesuai dengan jenis transplantasi organ. Penilaian risiko komplikasi melalui evaluasi riwayat, pemeriksaan fisik perioperatif, tes fungsi paru, radiografi thorax dan penilaian infeksi TB aktif maupun TB laten dilakukan pada seluruh pasien transplantasi organ. Beberapa skor prediktif yang dapat digunakan yaitu skor Azorullah, ARISCAT dan Gupta.

Abstract

Background: The global rise in electric cigarette (e-cigarette) use has prompted urgent investigation into their health effects. This meta-analysis aims to consolidate evidence regarding the impact of e-cigarette use on lung health. Methods: A systematic search of PubMed, Embase, and Cochrane Library databases was conducted, identifying studies published between 2018 and 2024 that assessed lung function, respiratory symptoms, and lung disease incidence in e-cigarette users. Studies meeting inclusion criteria were subjected to quality assessment and data extraction. Random-effects models were used for pooled analysis, and heterogeneity was assessed. Results: Twenty-three studies, encompassing 12,456 participants, were included. E-cigarette use was associated with a small but significant decrease in forced expiratory volume in 1 second (FEV1) (standardized mean difference [SMD] -0.18, 95% CI -0.26 to -0.10, p<0.001). Increased odds of wheezing (odds ratio [OR] 1.38, 95% CI 1.15 to 1.65, p=0.001) and chronic cough (OR 1.25, 95% CI 1.08 to 1.44, p=0.003) were also observed in e-cigarette users. No significant association was found with chronic obstructive pulmonary disease (COPD) incidence. Conclusion: E-cigarette use appears detrimental to lung function and associated with respiratory symptoms. Further long-term research is imperative to establish definitive conclusions on the risk of COPD and other lung diseases.
