Abstract

Adanya kejadian efek samping obat (ESO) pada pasien kemoterapi dapat memberikan resiko pada penurunan kualitas hidup. Penelitian ini bertujuan untuk mengetahui korelasi tingkat keparahan efek samping obat regimen  leucovorin calcium (folinic acid), fluorouracil, dan oxaliplatin (FOLFOX) dengan kualitas hidup pasien kanker kolorektal. Jenis penelitian yang digunakan adalah observasional dengan rancangan studi cross sectional dan pengambilan data secara prospektif tahun 2020-2021 di 3 rumah sakit yaitu RSUP X, RSUD X dan RSUD Y Cirebon. Data yang diambil berupa kejadian efek samping obat baik jenis dan jumlahnya, tingkat keparahan efek samping serta tingkat kualitas hidup. Instrumen yang digunakan adalah Hartwigh et. al. dan kuesioner EORTC QLQ c-30 versi 3 dalam Bahasa Indonesia. Analisis korelasi dengan Pearson Correlation. Jumlah sampel yang didapatkan sebanyak 43 pasien. Jenis ESO yang paling banyak adalah mual, sedangkan yang paling parah stomatitis dan muntah. Skor kualitas hidup pasien yang mengalami ESO yaitu kualitas hidup  global 66,26, kualitas hidup fungsional 71,98 dan kualitas hidup gejala 31,29. Korelasi antara tingkat keparahan efek samping obat dengan kualitas hidup domain global sebesar -0,421, kualitas hidup domain fungsional sebesar -0,638, dan kualitas hidup domain gejala sebesar 0,722. Tingkat keparahan efek samping obat berkorelasi dengan kualitas hidup pasien. Identifikasi efek samping obat dan tingkat keparahannya sejak dini tentu dapat mencegah penurunan kualitas hidup pasien.

Abstract

Introduction: Colorectal cancer (CRC) is the second deadliest cancer with the second highest mortality rate (9.2%) after lung cancer (11.4%) and breast cancer (6.9%). CRC is more prevalent in developed countries and more prevalent among men, with incidence rates per 100,000 in both sexes at 19.7, 23.6, and 16.3 respectively. Majority of the patients came to the hospital with diagnosis of loco regional and metastatic colorectal cancer. Therefore, information and education regarding CRC are important to the patient and their family to assure optimal screening and management. This study aimed to determine the effect of education on knowledge, attitudes, behavior of colorectal cancer patients. Methods: A descriptive-analytic research study design was conducted, including all patients diagnosed with colorectal cancer coming to Dr. Hasan Sadikin Hospital Bandung, a tertiary referral hospital, in the period August 2021 to August 2022. The pre and post-education questionnaire were used to evaluate the knowledge, attitude, and behaviour of the respondent regarding the colorectal cancer. An informational leaflet with verbal explanation was utilized as educational material. Data was analyzed using SPSS version 24. Results: Fifty-two respondents were recruited and more than half of the respondents (73.1%) showed a high mark in the post-test knowledge of CRC. Furthermore, behavior aspect also showed a dramatic increase and almost all the subjects (98.1%) scored high on behavior aspect. Conclusion: The education provided to CRC patients significantly enhances their knowledge, attitude, and behavior.

Abstract

Non-tunneling hemodialysis catheter is one of the most convenient vascular accesses for hemodialysis in End Stage Kidney Disease (ESRD) patients. However, these catheters have the highest morbidity among all available accesses. Current guidelines recommend that non-tunneled hemodialysis catheters be placed for no more than 2 weeks to prevent bacterial contamination. This study was conducted in Dr. Hasan Sadikin General Hospital, Bandung, Indonesia, throughout January-December 2021. A correlation test with a prospective observational analysis design was applied to find the link between the duration of non-tunneled hemodialysis catheter placement and the incidence of positive bacterial culture on the catheter. Pearson correlation test was used to see the strength of the correlation, with a significance limit of 0.05. This study involved 28 subjects. The most common location for catheter placement was in the jugular vein, and laboratory examinations showed no correlation between leukocyte values and positive bacterial culture results. Hypertension and diabetes mellitus were not significant risk factors (p-value 0.887). At ≤14 days of catheter placement, only 1 (6.7%) of the subjects presented a positive culture result. Most of the positive bacterial culture results were found at a duration of 15-30 days of placement, (n=8; 53.3%). The results of the Pearson test showed a p-value of 0.036 and an r-value of 0.399, indicating a significant result with a strong correlation between the two variables. The duration of placement of hemodialysis catheters has a positive correlation with the incidence of positive bacterial cultures, which can increase the risk of systemic infection associated with hemodialysis catheters.

Abstract

Colorectal cancer (CRC) is the third most common cancer globally and a leading cause of cancer-related mortality. Cyclooxygenase-2 (COX-2) plays a critical role in CRC pathogenesis by promoting inflammation, tumor proliferation, and metastasis. COX-2 inhibitors, such as NSAIDs, have shown potential in reducing CRC progression. However, the relationship between COX-2 expression and clinicopathological features remains controversial. This study aims to assess the correlation between COX-2 expression and clinicopathological characteristics in CRC patients in Indonesia. A prospective observational study was conducted on 81 CRC patients at a tertiary hospital in West Java, Indonesia, from January to June 2024. COX-2 expression was quantified using qPCR from tumor tissue samples. Clinicopathological data, including age, sex, tumor grade, stage, and complications, were collected and analyzed. Among the 81 participants, 81.5% were over 50 years old. Low-grade adenocarcinoma was the most prevalent histopathological type (73%), followed by high-grade adenocarcinoma (11%) and mucinous adenocarcinoma (11%). The majority of cases were in regional or early stages (74%), while 26% were in late stages. Higher COX-2 expression was more frequent in males, rectal tumors, high- grade adenocarcinomas, advanced stages, and metastatic cases. Although no statistically significant association was found, a trend toward increased COX-2 expression in advanced CRC was observed. No significant association was found between COX-2 expression and clinicopathologic characteristics of CRC. However, higher COX-2 expression may be associated with advanced disease. Abstract

BACKGROUND: Locally advanced rectal cancer (LARC) is commonly treated with neoadjuvant chemoradiotherapy (nCRT), but highly variable response limits outcomes and highlights the need for predictive biomarkers. Cyclooxygenase-2 (COX‑2) and human epidermal growth factor receptor 2 (HER‑2) are overexpressed in a subset of colorectal cancers and are mechanistically linked to radioresistance. Both pathways are therapeutically targetable and exhibit molecular crosstalk, suggesting that combined assessment may improve prediction of nCRT response, but their combined predictive value in LARC remains unexplored. Therefore, this study was conducted to evaluate the association between COX‑2 and HER‑2 expression and radiotherapy response in patients with LARC.METHODS: This observational retrospective cohort study included 59 patients with stage II–III rectal adenocarcinoma treated with standardized nCRT. COX-2 and HER-2 expressions on pretreatment biopsies were assessed by immunohistochemistry, and radiologic response 4–8 weeks post nCRT dichotomized into good and poor responses using RECIST 1.1.RESULTS: High COX-2 expression was present in 67.8% of tumors and was associated with poor response (p<0.001; OR=10.08; 95% CI: 2.92–34.78). HER-2 positivity (32.2% of cases) was also associated with poor response (p=0.039; OR=4.28; 95% CI: 1.16–15.79). In multivariate analysis, high COX-2 (adjusted OR=0.110; p=0.002) and HER-2 positivity (adjusted OR=0.197; p=0.049) remained independent predictors of poor response. Tumors with combined COX-2 low/HER-2 negative and COX-2 high/HER 2 positive profiles showed good response rates of 86.7% and 13.3%, respectively, representing a 73.4% absolute difference.CONCLUSION: Since Low COX-2 expression and HER-2 negativity is mostly associated with good radiotherapy response, hence COX-2 and HER-2 might be independent molecular predictors of radiotherapy response in LARC, and combined biomarker profiling provides robust risk stratification that may guide treatment intensification or de escalation strategies.KEYWORDS: COX-2, HER-2, rectal cancer, radiotherapy response, biomarker, personalized medicine
