Abstract

Background: Systemic lupus erythematosus (SLE) is an autoimmune disease that causes chronic inflammation in multiorgan, in which its etiology and pathogenesis are unclear. Curcumin has been found to have a selective cytotoxic effects on various types of cancer cells through induction of apoptosis on the various pathways. This study aimed to demonstrate whether curcumin extract had cytotoxic effects on T lymphocytes autoreactivity of patients with SLE. Methods: This research was an experimental laboratory with post test only controlled group design. Samples were using cell cultures of T lymphocytes from the veins blood of a patient with SLE and normal people. Each of T lymphocytes cell culture consisted of media control (KM), cells control (KS), the positive control with cyclophosphamide (KP1), and a group treated with curcumin extract (KP2). Curcumin extracts and cyclophosphamide were divided into 8 serial doses (50 µg/mL, 25 µg/mL, 12.5 µg/mL, 6.25 µg/mL, 3.125 µg/mL, 1.5625 µg/mL, 0.78125 µg/mL, 0.390625 µg/mL). Cell inhibition was calculated based on a percentage (%) of cell viability. Then, the data was analyzed using probit analysis to determine the dose of IC50 with SPSS for Window Release 17.0. Results: The result of cell inhibition probit analysis (IC50) on T lymphocytes cell culture of patients with SLE using curcumin extract was active (score 3) and T lymphocytes cell culture of patients with SLE using cyclophosphamide was moderately active (score 2).While, T lymphocytes cell culture of normal people using curcumin extract was active (score 3) and T lymphocytes cell culture of normal people using cyclophosphamide was moderately active (score 2). Conclusions: Curcumin extract had cytotoxic effect on T lymphocytes cell culture of SLE. The cytotoxic effect of curcumin extract on T lymphocytes cell culture of patients with SLE was stronger than cyclophosphamide.   Keywords: curcumin extract, cytotoxic, SLE

Abstract

Background: Parkinson’s disease is a neurodegenerative disorder that can significantly impact an individual’s mobility, balance, and overall quality of life. It is characterized by movement disorders, including tremors, rigidity, bradykinesia, and gait disorder. In addition to pharmacological therapy, rehabilitation approaches play an important role in the management of occurring movement disorders. One of the potential rehabilitation methods is virtual reality (VR). Objective: This systematic review aims to determine the impact of VR-based rehabilitation on balance, mobility, motor function, and the quality of life of Parkinson's patients. Methods: Systematic research was conducted in February 2024 using PubMed, Scopus, and ScienceDirect. A combination of MeSH terms “virtual reality,” “Parkinson,” “exergame,” “effectivity,” and “rehabilitation” was used. The articles selected were randomized controlled trials published in the last ten years in English. The quality of articles was assessed using the Cochrane risk-of-bias tool. Analyses were stratified by outcomes: balance, mobility, motor function, and quality of life. Results: Fourteen randomized controlled trials with 637 patients were included. Most trials show that VR-based rehabilitation significantly improves balance compared to conventional rehabilitation. Mobility and motor function improve in the VR group, but its superiority remains controversial. Improved quality of life is observed, but no trials show statistical significance compared to conventional rehabilitation. Conclusion: This systematic review shows that VR-based rehabilitation has the potential to improve balance, mobility, motor function, and quality of life in Parkinson's patients.

Abstract

Purposes: Knowing the description of the characteristics of Post-Vaccination follow-up events in health workers who are A Graduate 'Aisyiyah University Surakarta who received Covid -19 vaccination. Methods: The research design was a descriptive study with a cross-sectional method. The research subjects were 95 alumni of 'Aisyiyah University Surakarta, graduates from 2010-2020 who had received the covid-19 vaccine. This research was conducted virtually through the google-form application by respondents in their respective work areas. This research was conducted in March 2021. The data collection technique used in this study was purposive sampling Results: Characteristics of the age of the majority of respondents based on age were 20-30 years old as many as 75 respondents (78.9%), Gender characteristics of the majority of respondents based on gender were women as many as 85 respondents (89.5%). profession is a nurse as many as 63 respondents (66.3%), Characteristics of previous medical history of the majority of respondents based on past medical history are 86 respondents (90.5%), the characteristics of the occurrence of AEFI events, the majority of respondents are based on the incidence of AEFI after vaccine As many as 85 respondents (89.5%) did not have Covid-19, the characteristics of the types of AEFI symptoms were the majority of respondents based on the incidence of AEFI after the Covid-19 vaccine appeared fever was no fever as many as 85 respondents (89.5%), the majority of respondents were based on incidence. AEFI after the Covid-19 vaccine appeared diarrhea was no diarrhea as many as 93 respondents (97, 9%), the majority of respondents based on the AEFI incidence after the Covid-19 vaccine appeared coughing was no cough as many as 93 respondents (97.9%), the majority of respondents based on the AEFI incident after the Covid-19 vaccine appeared shortness of breath was no shortness of breath as many as 93 of respondents (97.9%), the characteristics of the duration of AEFI symptoms, the majority of respondents, based on the duration of the occurrence of the event, were absent (for those without symptoms) as many as 84 respondents (88.4%). Conclusion: The characteristics of the emergence of AEFI in respondents who received Covid-19 vaccination based on age, sex, profession, past medical history, number of AEFI incidents, types of AEFI symptoms, and duration of AEFI symptoms appeared were minimal.

Abstract

Kanker payudara merupakan masalah kesehatan global yang memerlukan deteksi dini untuk meningkatkan harapan hidup pasien. Tantangan signifikan dalam pengembangan sistem diagnostik adalah data medis yang sering tidak seimbang, di mana kasus kanker seringkali merupakan kelas minoritas. Penelitian ini bertujuan mengembangkan model prediktif akurat untuk deteksi dini kanker payudara, dengan menganalisis kinerja algoritma pohon keputusan dan mengoptimalkan berbagai parameter kuncinya, sembari mengatasi ketidakseimbangan data menggunakan teknik penambahan sampel minoritas sintetis. Eksperimen dilakukan pada dataset kanker payudara dengan memvariasikan status penggunaan teknik penambahan sampel, jumlah tetangga, jumlah pohon, kriteria pemisahan, kedalaman maksimum pohon, strategi pemangkasan, strategi pengambilan keputusan, serta eksekusi paralel. Kinerja model dievaluasi komprehensif menggunakan berbagai metrik seperti akurasi, nilai Kappa, dan kemampuan mendeteksi kelas minoritas. Hasil menunjukkan bahwa penggunaan teknik penambahan sampel secara signifikan meningkatkan identifikasi kasus kanker. Konfigurasi optimal yang melibatkan kriteria pemisahan tertentu dan jumlah pohon yang lebih banyak menghasilkan kinerja diagnostik yang konsisten. Optimalisasi kedalaman pohon dan pemangkasan krusial dalam menghindari ketidaksesuaian model, dan eksekusi paralel mempercepat proses komputasi. Model yang dikembangkan mencapai akurasi 81.20% dan nilai Kappa 0.622. Penelitian ini menegaskan pentingnya optimasi parameter model dan penanganan data tidak seimbang untuk meningkatkan akurasi deteksi dini kanker payudara, mendukung pengembangan alat diagnostik yang lebih andal.

Abstract

Stroke is one of the leading causes of disability and death worldwide. Thrombolytic therapy using intravenous recombinant tissue plasminogen activator (rt-PA) is the recommended treatment. However, functional outcomes vary due to various factors. Objective: The aim of this study was to analyze the factors affecting clinical outcomes of acute ischemic stroke patients treated with intravenous rt-PA. Method: This research was a retrospective study with a total sample of 52 acute ischemic stroke patients who received intravenous rt-PA therapy. The research variable data were collected from medical records, then statistically analyzed and correlated with patient clinical outcomes. Data from medical records were collected based on inclusion and exclusion criteria, then statistically analyzed using STATA 13. Univariate analysis is presented in a list of categories according to the research variables determined by percentage (standard error). Bivariate analysis was tested using simple logistic regression and multivariate analysis using multiple logistic regression. Results: The results of this study showed that patient clinical outcomes are not significantly impacted by risk factors (age, gender, prior stroke history, comorbidities), COVID-19 vaccination, onset and severity of stroke, HCTS onset, dosage, or onset therapy. The side effects of intravenous rt-PA have a significant impact on patient clinical outcomes, including therapy response and discharge condition (p = 0.008, p < 0.05, and p = 0.012, OR 34.073 (95% CI 2.165 – 536.147)), but do not affect the length of hospital stay. Conclusions: In considering the other factors, the study indicates that patients experiencing adverse effects from intravenous rt-PA have worse clinical outcomes.

Abstract

LATAR BELAKANG: Mewujudkan budaya keselamatan pasien adalah tantangan bagi rumah sakit dalam proses memberikan layanan yang berorientasi pada kualitas dan keselamatan pasien. Survei budaya keselamatan merupakan sebuah alat untuk merancang dan mengevaluasi efektivitas program budaya keselamatan pasien yang telah dilakukan. Tren nilai dari data dimensi keselamatan pasien digunakan sebagai dasar untuk perbaikan berkelanjutan. Penelitian ini bertujuan untuk mengetahui nilai budaya keselamatan pasien di Rumah Sakit Umum Pusat Dr. Soeradji Tirtonegoro Klaten (RSST) pada tahun 2022. TUJUAN: Untuk mengetahui gambaran budaya keselamatan pasien di RSUP Dr. Soeradji Tirtonegoro Klaten pada tahun 2022. METODE: Penelitian ini menggunakan analisis deskriptif dengan Metode penelitian deskriptif analitik dengan desain potong lintang (cross sectional) menggunakan kuesioner dari Survey On Patient Safety Culture (SOPS) 2.0. Teknik sampling menggunakan metode stratified random sampling dengan kriteria eksklusi dan inklusi yang telah ditentukan sebelumnya. Pengisian kuesioner dilakukan secara online melalui google form. HASIL: Sebanyak 302 (93%) responden telah mengikuti survei budaya keselamatan pasien. Nilai positif budaya keselamatan pasien sebesar 75,1% yang sudah mengalami peningkatan sebanyak 6,4% jika dibandingkan hasil penelitian pada tahun sebelumnya. Sebanyak 5 dimensi memiliki nilai >75% dengan pembelajaran organisasi dan perbaikan berkelanjutan sebagai dimensi yang memiliki nilai tertinggi. Dimensi yang paling rendah adalah kepegawaian dan kecepatan kerja. Tingkat keselamatan pasien dan pelaporan Insiden Keselamatan Pasien (IKP) masih di bawah nilai dasar yang ada pada Agency for Healthcare Research and Quality (AHRQ). KESIMPULAN: Nilai positif budaya keselamatan pasien di RSST tahun 2022 sudah sesuai dengan standar (75,1%). Nilai positif budaya keselamatan pasien, tingkat keselamatan pasien, dan pelaporan insiden keselamatan pasien masih memerlukan upaya perbaikan. Abstract

Neurogenic bladder is one of the most common complications of spinal cord injury (SCI), leading to impaired voiding function, recurrent urinary tract infections, and reduced quality of life. Determining the location, extent, and severity of the lesion is essentialto tailor management strategies to each patient’s neurological condition and functional status. Treatment includes non-surgical approaches such as intermittent catheterization, neuromodulation, and pharmacological therapy using anticholinergic drugs or 3-adrenergic agonists, which have been shown to reduce detrusor pressure. In cases unresponsive to conservative therapy,surgical interventions such as bladder augmentation or sacral nerve stimulation may be performed to preserve renal function, prevent vesicoureteral reflux, and achieve urinary continence. The prognosis is generally favorable when patients undergo longterm rehabilitation combined with comprehensive education and regular renal function monitoring. Multidisciplinary collaboration among urologists, rehabilitation specialists, and nurses plays a vital role in improving patient quality of life, reducing complications, and ensuring sustainable, effective management.
