Abstract

Introduction: The report of doctors who died with COVID-19 in Indonesia was high and influenced by several factors. This study examined COVID-19 infected doctors and mortality risk factors in East Java. Material and Methods: This observational cohort study was conducted among doctors infected with COVID-19 in East Java during March 2020-September 2021 and collected the COVID-19 infected doctors’ deaths report. Doctors received a questionnaire on the status of COVID-19, history of covid-19 infection, age, sex, type of doctor, type of specialization and resident, pregnancy, underlying disease and nutritional status. All data were described and potential risk factors’ association was analyzed. Results: We reported 2041 doctors infected with COVID-19. Majority of them (52,5%) was male, 162 doctors were more than 59 years old, 162 doctors (7,9%) were died and 41,4% was general practitioner. The odds ratio in each risk factor for mortality was older age doctors 15,468 (95% CI 10,587-22,601), male 6,128 (95% CI 3,906-9,614), general practitioner 2,461 (95% CI 1,769-3,423), doctors with underlying disease 39,842 (95% CI 23,077-68,798), Type II diabetes mellitus 82,504 (95% CI 42,767-159,162), cardiovascular disease 50,152 (95% CI 16,672-150,866), hypertension 19,425 (95% CI 10,528-35,840), Chronic Obstructive Pulmonary Disease 5,52 (95% CI 0,610-49,992), Asthma 2,091 (95% CI 0,626-6,986), Obesity 30.750 (95% CI 15,293-61,828) and Pregnant doctors 43.013 (95% CI 10.986-168.414) (p<0,05). Conclusion: Older age, male, general practitioner, doctors with underlying disease, cardiovascular disease, Type II diabetes mellitus, hypertension, chronic obstructive pulmonary disease, asthma, obesity, pregnant doctors had been mortality risk factors among COVID-19 infected doctors.

Abstract

Increasing incidence of advanced nasopharyngeal carcinoma can reduce life expectancy. So that early detection and prevention of nasopharyngeal risk factors are needed to the community. Knowledge about nasopharyngeal carcinoma can increase public awareness to undergo early examination and treatment. The purpose of this activity is to increase knowledge and skills for doctors in early detection of nasopharyngeal carcinoma in the Madiun Raya area. Educational activities are carried out through seminars and training with lecture, discussion, practice methods, as well as pre-test and post-test. The results of the pre-test and post-test of seminar and training activities showed that there was a statistically significant difference (p < 0.0001). It is hoped that this seminar and training will be able to increase nasopharyngeal awareness in the community and be able to make a positive contribution to efforts to treat nasopharyngeal carcinoma in the wider community.

Abstract

Background: Presbyastasis is multifactorial balance dysfunction that occurs in the elderly person. Presbyastasis can increase the risk of fall, anxiety, and decrease the quality of life. Vestibular rehabilitation therapy (VRT) has been proven effective to overcome balance disorders, and it is safe for the elderly. Several studies had reported the success of VRT for balance disorders. All this time, the accomplishment of VRT was assessed by using balance test, which had a risk of falling in elderly patients. Objective: To find out the outcome of Dizziness Handicap Inventory (DHI) questionnaire score in presbyastasis patients after VRT. Method: Ten presbyastasis patients in age range 60-75 years old who met the study criteria were taken by consecutive sampling. A longitudinal observational (pre and posttest) study by analyzing the DHI questionnaire scores. Assessment was performed twice, before and after VRT. The data was analyzed using paired T test and Wilcoxon signed rank test with outcome p<0.05. Result: The measurement of the emotional subscale DHI (DHI.E) showed the mean score before VRT was 4.00 (1.63), after therapy was 0.00 (0.63), p=0.004. The functional subscale (DHI.F) measurement showed the mean score before VRT was 10.40 (3.98), after therapy was 2.40 (2.07), p 0.00. The mean score of physical subscales (DHI.P) measurement before VRT was 9.00 (4.40), after therapy was 2.00 (1.58), p=0.008. The total DHI score (DHI.T) before VRT was 22.6 (7.67), after VRT was 4.20 (2.2) with p=0.000. Conclusion: There was an improvement in DHI questionnaire score before and after 6 weeks VRT.Keywords: Vestibular rehabilitation therapy (VRT), Dizziness Handicap Inventory (DHI), presbyastasis ABSTRAK Latar belakang: Presbiastasis dapat meningkatkan angka jatuh, kecemasan dan menurunkan kemandirian sehingga menurunkan kualitas hidup pada usia lanjut. Terapi rehabilitasi vestibuler (TRV) merupakan modal terapi yang terbukti dapat mengatasi gangguan keseimbangan, dan aman untuk usia lanjut. Beberapa penelitian melaporkan adanya keberhasilan TRV pada penderita gangguan keseimbangan. Keberhasilan dari TRV selama ini dinilai dengan menggunakan pemeriksaan keseimbangan yang memiliki risiko jatuh pada penderita usia lanjut. Tujuan: Membuktikan adanya perubahan skor kuesioner Dizziness Handicap Inventory (DHI) pada penderita presbiastasis sesudah TRV. Metode: Sepuluh penderita presbiastasis usia 60-75 tahun yang memenuhi kriteria penelitian diambil secara consecutive sampling. Studi observasi longitudinal (pre dan posttest) dengan menghitung dan menganalisis skor kuesioner DHI. Pengukuran dilakukan 2 kali yaitu sebelum TRV dan sesudah TRV. Analisis data dilakukan dengan paired T test dan Wilcoxon signed rank test, dengan hasil p<0,05. Hasil: Pengukuran skor kuesioner DHI subskala Emotional (DHI.E) sebelum TRV mempunyai mean 4,00 (1,63), 6 minggu sesudah TRV didapatkan mean 0,00 (0,63), p=0,004. Hasil subskala Functional (DHI.F) sebelum TRV didapatkan mean 10,40 (3,98), 6 minggu sesudah TRV, mean 2,40 (2,07), p=0,00. Pada subskala Physical (DHI.P) didapatkan mean 9,00 (4,40), dan 6 minggu sesudah TRV didapatkan mean 22,6 (7,67), sesudah TRV didapatkan mean 4,20 (2,2) dengan p=0,000. Abstract

Highlights : This study explored the significance of auditory-verbal therapy as an early intervention for children with speech delay and sensorineural hearing loss, a topic that deserves further research in developing countries such as Indonesia. The findings highlight the significance of age and therapy duration on the language development of children with speech delay and sensorineural hearing loss. AbstractSpeech delay is primarily attributed to sensorineural hearing loss, which may significantly impact a child's language development and social independence. Consistent and periodic auditory-verbal therapy as an early intervention has the potential to positively influence language development, thereby fostering independence in children. The objective of the research was to examine the differences in the development of language and social independence among children who have speech delay and sensorineural hearing loss at the children's age for early intervention and with the duration of auditory-verbal therapy taken into consideration. The research employed an analytical-observational design with a cross-sectional approach and total sampling. The sample comprised 29 children who were undergoing the weekly routine of auditory-verbal therapy at Yayasan Aurica, Surabaya, Indonesia. The tool utilized in this study was the Pre-Screening Developmental Questionnaire. The data underwent bivariate analysis, specifically using the Chi-squared test with a significance level set at a p-value of <0.05. The language development analysis resulted in a p-value of 0.013 for the age variable and a p-value of 0.019 for the therapy duration variable. Meanwhile, the social independence analysis yielded a p-value of 0.229 for the age variable and a p-value of 0.111 for the therapy duration variable. In conclusion, the influence of age on early intervention had a significant difference from that of the duration of auditory-verbal therapy on the language development of children with speech delay and sensorineural hearing loss. Conversely, age and therapy duration did not exert any meaningful difference in terms of their influence on the children's social independence development.

Abstract

Background: Human Immunodeficiency Virus (HIV) remains a major global health concern. In Indonesia, women account for 35.1% of HIV cases. In East Java alone, 1,062 new cases were reported between January and March 2023, with Surabaya recording the highest number at 663 cases. Opportunistic infections frequently affect HIV patients due to weakened immune systems. Tuberculosis is the most prevalent OI among HIV patients in Indonesia, accounting for 62.1% of cases, and is commonly found in women, posing a risk of vertical transmission. This study aims to analyze the factors influencing of opportunistic infections in women living with HIV. Methods: A quantitative approach with an observational analytic design and retrospective method. Used a total sampling technique involving all female HIV patients diagnosed with opportunistic infections at HIV clinic of Dr. Soetomo Hospital from January-December 2023, meet the inclusion criteria. The research was conducted from December 2023-October 2024. The dependent variable is opportunistic infections, while the independent variables are duration of HIV infection, adherence to antiretroviral therapy, CD4 cell count, and viral load. Data were analyzed using univariate and bivariate methods, with the Chi-Square test. Results: The results, duration of HIV infection was not significantly associated with opportunistic infections (p = 0.402). However, ARV adherence (p = 0.003), CD4 count (p < 0.001), and viral load (p = 0.001) were significantly associated with increased risk of opportunistic infections. Conclusion: In conclusion, ARV adherence, CD4 count, and viral load significantly influence the occurrence of opportunistic infections among HIV-positive women at Dr. Soetomo Hospital

Abstract

Nasopharyngeal carcinoma is endemic in Southeast Asia. In Indonesia, the recorded mean prevalence is 6.2/100.000, with 13.000 yearly new Nasopharyngeal carcinoma cases. Observational data suggest that cancer-specific mortality was higher during the COVID-19 pandemic when compared with pre-pandemic levels. This study aims to compare the treatment outcome of nasopharyngeal carcinoma cases before and during the COVID-19 pandemic at Dr Soetomo Hospital in 2017-2022. This study employed a retrospective cross-sectional approach with an observational analytic method. Data is taken secondary from medical records for nasopharyngeal carcinoma patients at Dr. Soetomo Hospital in Surabaya and primary by telephone interview for additional information on the patient’s risk factors. The majority of nasopharyngeal carcinoma patients at Dr. Soetomo Hospital were male, elderly,>50 years old, and referral patients (outside Surabaya). In both eras, patients arrived already in stage 4 (advanced stage). However, before COVID-19, more patients were cured and survived before the COVID-19 period in comparison to during the COVID-19 pandemic period. This may be related to delayed treatment caused by COVID-19 policy on social distancing, healthcare regulation during the pandemic, and immunocompromised status.

Abstract

Latar belakang: Deteksi dini karsinoma nasofaring (KNF) merupakan langkah penting untuk menurunkan angka morbiditas dan mortalitas akibat penyakit ini. Tujuan: Dengan diadakannya program pengabdian masyarakat diharapkan dokter umum di Fasilitas Kesehatan Tingkat Pertama (FKTP) mampu meningkatkan pengetahuan dalam deteksi dini KNF agar pasien dengan gejala awal sedini mungkin memiliki harapan hidup yang lebih tinggi. Metode: Metode kegiatan meliputi seminar interaktif yang disertai pre-test dan post-test, serta pelatihan pemeriksaan fisik THT-KL yang dipandu oleh 10 instruktur ahli. Hasil: Kegiatan ini menunjukkan adanya peningkatan signifikan dalam pengetahuan dan pemahaman peserta terhadap deteksi dini KNF melalui hasil pre-test dan post-test. Seminar ini melibatkan metode ceramah, diskusi, dan kehadiran langsung pasien KNF untuk memperkaya pengalaman peserta. Dampak: Melalui workshop ini diharapkan memberi dampak bagi peserta untuk mampu menerapkan pengetahuan dan keterampilan yang diperoleh dalam praktik sehari-hari di Fasilitas Kesehatan Tingkat Pertama (FKTP), sehingga dapat memberikan kontribusi langsung terhadap deteksi dini KNF.
