Abstract

Diabetes mellitus negatively affects bone tissue and unfavourably impacts bone mineral density (BMD), therfore increasing the probability of fractures through pathological mechanisms. This study aimed to explore the factors associated with BMD and osteoporosis risk in individuals with a diagnosis of type 2 diabetes mellitus (T2DM). An investigation was undertaken involving 49 patients diagnosed with T2DM who fulfilled the specified criteria. These individuals underwent BMD assessment as part of a cross-sectional study. The analysis encompassed both univariate and bivariate approaches, utilizing the Chi-square test (X2) and binary logistic regression methods. A total of 30 participants (61%) have decreased BMD. Among the participants aged 60 years and above, 83.4% exhibited a decreased BMD status (osteopenia and osteoporosis), in contrast to the under 60 years age group, in which 40% displayed decreased BMD status. Older age (>60 years) is a risk factor for decreasing bone density onset of diabetes mellitus (DM) diagnosis, duration of DM, glycemic control, body mass index (BMI), use of thiazolidinediones (TZD) drugs, kidney function, were not associated with lower BMD in T2DM patients.

Abstract

ABSTRACTBackground: The aging process in the elderly can be caused by a decrease in vitamin D precursors and reduced Vitamin D levels due to impaired absorption of Vitamin D. Vitamin D plays a role in cognitive function, acts as an immunomodulator affecting blood glucose levels, promotes muscle atrophy, and affects the intranuclear VDR concentration and gene expression of VDR.Objectives: To determine the effect of vitamin D3 supplementation on Serum 25(OH)D levels, hand grip strength, blood sugar levels, and cognitive function in the elderly.Materials and Methods: This study used a quasi-experimental pre-posttest control group design with a total of 60 subjects included and divided into two groups. The treatment group received vitamin D3 at a dosage of 1000 IU/day for three months, while the control group was given a placebo. Blood serum levels were used to obtained serum 25(OH)D and blood sugar level. Hand grip strength values were measured by a hand grip dynamometer, and cognitive function was assessed using the Mini-Mental State Examination (MMSE) Questionnaire. The data was analyzed using the Wilcoxon test, and the bivariate analysis was performed using the Mann-Whitney test. Results: Most subjects were between 60 and 92 years old and had a vitamin D deficiency. Significant (P<0,05) differences were observed in serum 25(OH)D levels, blood sugar levels, and cognitive function following the intervention. The differences in serum 25(OH)D levels were 8,50±6,17, blood sugar levels were -21,68±25,88, and cognitive function was 2,23±3,05. Additionally, hand grip strength increased after the intervention (mean=0,42±4,66) but was not statistically significant (P=0,633).Conclusion: There is an improvement in cognitive function and a decrease in blood glucose levels, along with an increase in 25(OH)D levels after three months of vitamin D supplementation. However, hand grip strength did not significantly increase after supplementation. Keywords: Vitamin D3 Supplementation; serum 25(OH)D; hand grip strength; blood sugar level; cognitive function; elderly.

Abstract

Diabetic foot ulcers (DFUs) represent a serious complication of diabetes mellitus, associated with significant morbidity and healthcare costs. The progression of DFUs is influenced by systemic and local factors, including haemoglobin levels, leukocyte count, glycated haemoglobin (HbA1c), and serum albumin. This study aims to explore the relationship between these clinical parameters and the severity of DFUs. We enrolled 62 patients with DFUs and classified ulcer severity as mild, moderate, or severe according to the Wagner classification system. Using Spearman’s rank correlation, we found significant associations: lower hemoglobin and albumin levels correlated with more severe ulcers (ρ = -0.34, p= 0.0065; ρ = -0.41, p = 0.00084, respectively), while higher HbA1c and leukocyte counts were associated with increased ulcer severity (ρ = 0.62, p = 0.000; ρ = 0.40, p = 0.0013, respectively). These findings suggest that hematologic and biochemical markers may serve as valuable indicators of DFU progression, potentially guiding clinical decision-making and improving patient outcomes. Further research is needed to elucidate the underlying mechanisms and evaluate targeted interventions for this high-risk population.

Abstract

Film and television play a central role in shaping and representing the cultural identity of society. Through artistic expertise manifested by directors, screenwriters, actors, cinematographers, and producers, cultural construction is presented not only as entertainment but also as a social discourse that reflects values, traditions, and the dynamics of collective identity. This study aims to analyze how artistic expertise contributes to constructing cultural identity representation through film and television mediums in Indonesia. The research methodology used is qualitative, employing content analysis and semiotics approaches on selected Indonesian films and TV series that highlight cultural aspects, such as Losmen Bu Broto and Gadis Kretek. These works were chosen because they feature rich local culture while reflecting the role of culture in the dynamics of Indonesian society. The findings indicate that artistic expertise plays a crucial role in constructing cultural representation, both through narrative, visual elements, music, and actors' performances. The representations produced not only affirm local and national identities but also reflect the process of cultural negotiation with global influences. This study underscores that film and television serve as strategic media for cultural diplomacy and the formation of collective identity awareness in Indonesian society. Through this process, these mediums contribute to the preservation of local culture amidst global cultural flows. The research highlights the importance of film and television as tools for maintaining and constructing cultural identity, where local traditions and values are both preserved and adapted to the changing global landscape.

Abstract

BACKGROUND: Diabetes mellitus (DM) is often associated with obesity, which can lead to chronic inflammation. Sleeve gastrectomy (SG) is a common treatment for obesity. Combining SG with omentoplasty, might help reducing inflammation in pancreatic beta cells. However, there is limited research regarding the combined effects of SG and omentoplasty in type 2 DM (T2DM). This study was conducted to fill this gap by evaluating the impact on body weight, insulin resistance, glucagon expression, and levels of interleukins (IL)-1, IL-6, and IL-10 in obese rats with type II DM.METHODS: An experimental study was conducted on 18 obese Wistar rats with DM that were randomized into: control, T1 (SG), and T2 (SG + omentoplasty). SG involved the surgical removal of a portion of the stomach to reduce its size, while omentoplasty utilized the omentum to target inflammation in pancreatic beta cells. Venous blood samples were taken from subjects one day before and ten days after the intervention to measure the biomedical parameters with various methods. Data was statistically analyzed using paired t-tests for pre-test and post-test differences, and Post Hoc tests or Mann-Whitney tests for hypothesis testing.RESULTS: All rats were confirmed to have obesity and DM according to the Lee index and blood glucose levels. T2 group exhibited a significant decrease in body weight, homeostatic model assessment of insulin resistance (HOMA-IR) values were lower, and glucagon expression, IL-1, IL-6, and IL-10 levels were significantly greater compared to both control and T1 groups.CONCLUSION: The combination of SG and omentoplasty significantly improves inflammation and insulin resistance in obese rats with T2DM.KEYWORDS: diabetes mellitus, sleeve gastrectomy, omentoplasty, inflammation, obesity

Abstract

BACKGROUND: Hyperglycemia causes dyslipidemia in type 2 diabetes mellitus (T2DM). Metformin monotherapy is known to be less effective at improving glycemic status, insulin function, and lipid profiles. Lycopene is a potential antioxidant and has been shown to be hypoglycemic and hypocholesterolemic. However, the effects lycopene and metformin combination are still up for debate. This study was conducted to determine the potential of lycopene in enhancing the ability of metformin to improve glycemic status, insulin resistance, beta cell capacity, and lipid profile of T2DM rats.METHODS: Thirty male Wistar rats were randomly divided into six groups: control (N), T2DM-untreated (D), T2DM + metformin (DM), T2DM + metformin + 10 mg/kgBW lycopene (DMLy-10), T2DM + metformin + 20 mg/kgBW lycopene (DMLy-20), and T2DM + metformin + 40 mg/kgBW lycopene (DMLy-40). The treatment was administered once daily through oral route and lasted for 28 days, before blood samples were collected. Fasting blood glucose (FBG) was assessed by oxidase-peroxidase method, fasting serum insulin and HbA1c were measured using enzyme-linked immunosorbent assay (ELISA), while lipid profile was determined using enzymatic methods. The homeostatic model assessment for insulin resistance (Homa-IR) as well as the homeostatic model evaluation of β-cell function (Homa-B) were then calculated.RESULTS: Fasting serum insulin levels increased significantly (p<10.05) in the DMLy-20 and DMLy-40 groups, but Homa-B or high-density lipoprotein (HDL) did not significantly increase. Additionally, the FBG, HbA1c, Homa-IR, total cholesterol, triglyceride, and low-density lipoprotein levels were not significantly decreased than in the group treated with metformin alone.CONCLUSION: Lycopene can enhance the ability of metformin to improve the glycemic status, insulin resistance, beta-cell capacity, and lipid profile of T2DM rats.KEYWORDS: dyslipidemia, Homa-B, insulin resistance, lycopene, metformin, type 2 diabetes mellitus
