Abstract

Objective: This study aimed to determine the effects of Phaleria macrocarpa’s extract on TNF-α and LC3-II level and their correlation in preeclampsia-induced HUVEC. Method: This study used HUVEC culture as an in vitro model and Phaleria macrocarpa’s extract, widely used as an anti-inflammation and antioxidant.Result: Phaleria macrocarpa’s extract reduce TNF-α level siginificantly at concentration of 7.813μg/mL and at 62.5μg/mL reduce TNF-α level to normal level. There was no significant decrease and  reduction to normal level in LC3-II. TNF-α has a strong positive correlation with LC3-II (r=0.958), that reduced TNF-α level will decrease LC3-II levels, where a decrease in TNF-α level of 1pg/mL will reduce LC3-II levels by 0.413pg/mL.Conclusion: Thus, Phaleria macrocarpa’s extract might be used to overcome endothelial dysfunction and autophagy in preeclampsia.Ekstrak Phaleria macrocarpa Menghambat Otofagi Melalui TNF-α Pada Kultur Sel HUVECAbstrakTujuan: Penelitian ini bertujuan untuk mengetahui efek ekstrak Phaleria macrocarpa pada inflamasi dan otofagi pada sel endotel dengan mengukur kadar TNF-α dan LC3-II pada HUVEC yang diinduksi serum preeklampsia.Metode: Penelitian ini menggunakan kultur HUVEC sebagai model in-vitro yang banyak digunakan untuk memelajari patogenesis preeklampsia. Phaleria macrocarpa (Scheff.) Boerl juga dikenal sebagai Mahkota Dewa secara luas digunakan sebagai anti-inflamasi dan antioksidan Hasil: Hasil menunjukkan ekstrak Phaleria macrocarpa menurunkan kadar TNF-α secara signifikan pada konsentrasi 7.813 μg/mL dan konsentrasi 62.5 μg/mL menurunkan kadar TNF-α ke kadar normal. Tidak terdapat penurunan signifikan rerata kadar LC3-II antara kontrol dan model PE dan dibutuhkan ekstrak Phaleria macrocarpa pada konsentrasi lebih dari 250 μg/mL untuk menurunkan kadar LC3-II pada model preeklamsia ke kadar hamil normal. Kadar TNF-α memiliki korelasi positif yang bermakna dengan LC3-II dengan tingkat korelasi sangat kuat (r = 0.847), dimana penurunan kadar TNF-α sebesar 1 pg/mL akan menurunkan kadar LC3-II sebesar 0.413 pg/mL.Kesimpulan: Dengan demikian, ekstrak Phaleria macrocarpa dapat digunakan untuk mengatasi disfungsi endotel dan otofagi pada preeklampsia.Kata kunci: Phaleria Macrocarpa, Preeklampsia, HUVEC, TNF-α, LC3-II

Abstract

Background: Physiologic changes from the reproductive to non reproductive phase is characterized by a gradual decline in estrogen levels. In menopausal women decline of estrogen levels can causes various complaints, one of which is the vasomotor syndromeAim: This study was conducted to determine estradiol serum levels in menopausal women with and without vasomotor syndrome and estradiol serum levels in menopausal women based on severity degrees (mild, moderate, and severe).Metode: This analytical cross sectional study involved 60 women, 34 women with vasomotor syndrome and 26 women with non vasomotor syndrome. Serum estradiol levels in every groups were checked. Data were statistically analysed using Anova and Spearmen correlation tests (p<0.05; CI 95%).Results : Non vasomotor syndrome estradiol serum mean rank was significantly higher than vasomotor syndrome estradiol serum mean rank (17.5 vs 47,5 pg/mL). Mean estradiol levels of menopausal vasomotor syndrome subjects based on severity degrees for mild, moderate, and severe degrees are 25.80 ± 2.34 pg/ml; 15.58 ± 2.64 pg/ml; and 11,80 pg/ml (P<0.05). Age, menopausal age, and menopausal length was significantly negatively correlated with estradiol levels with r values of -0.859, - 0.830, and -0.880, respectively. Body Mass Index (BMI) was significantly positively correlated with estradiol levels with a r value of 0.699Conclusion: Serum estradiol levels were lower in vasomotor syndrome women than non vasomotor women. There is significantly differed serum estradiol levels based on severity degress. More severe degrees of vasomotor syndrome will make lowerv serum estradiol levels. Subject characteristics were also significantly associated with serum estradiol levels in menopausal women. Key Words : Menopause, vasomotor syndrome, serum estradiol.

Abstract

Introduction :The premenopausal period is characterized as a decline in estrogen levels that consequently result in disturbing symptoms. One noted significant factor in this period is the psychological state, a condition that is closely related to cortisol levels. Stress is known to be a product of the neuroendocrine system. As a product of tbis mechanism, cortisol is frequently used as a biomarker to study stress. Currently, menopausal symptoms are diagnosed based on present complaints, which tends to be subjective, consequently, a biomarker is required to diagnose menopausal disturbances. Method :This analytic, diagnostic, cross sectional study was conducted in the General Hiospital of Haji Adam Malik Medan, and enrolled paramedics working at the out patient clinic from March until April 2015. All subjects were asked to fill a Menopause Rating Scale (MRS), the results of which were then associated with obtained salivary cortisol levels. Results :Salivary cortisol levels was significantly associated with menopausal disturbances with p< 0.01. Salivary cortisol also significantly differed based on each MRS scores with p=0.008. A cut-off value of 9.52 ng/ml was obtained, with specificity and sensitivity values of 85 and 77%, respectively. Conclusion :Salivary cortisol testing as a marker for menopausal disturbances obtained a marker of 9.52 ng/mL with specificity and sensitivity values of 85 and 77%, respectively. Abstract

Tujuan: Untuk mengetahui hubungan kadar Follicle Stimulating Hormone (FSH) dengan keluhan urogenital pada wanita menopauseMetode: Penelitian ini merupakan studi potong lintang. Sampel adalah wanita menopause yang telah berhenti haid minimal 3 tahun. Penelitian dilakukan di Klinik Sari Mutiara Diski. Pada pasien akan dilakukan pemeriksaan kadar FSH dan penilaian keluhan urogenital dengan menggunakan kuisoner Most Bothersome Symtoms (MBS). Data akan dianalisa dengan menggunakan SPSS dan secara statistik dengan menggunakan uji T independent dan Chi-square. Nilai P < 0.05 dianggap signifikan.Hasil: Dari 30 sampel penelitian didapatkan karateristik wanita menopause dengan rerata usia 56.4±2.5 tahun, lama menopause 4.2±2.1 tahun, IMT 26.5±3.3 kg/m2, Paritas 3.1±1.2, dan kadar FSH 69.7±11.4 mIU/mL. Didapatkan perbedaan bermakna rerata kadar FSH pada kelompok dengan keluhan urogenital dan tanpa keluhan keluhan urogenital (P = 0.03). Didapatkan perbedaan bermakna kelompok FSH  60 mIU/mL dengan kelompok FSH > 60 mIU/mL terhadap adanya keluhan urogenital (P < 0.001)Kesimpulan: Kadar FSH > 60 mIU/mL berhubungan dengan adanya keluhan urogenital pada wanita menopause Kata Kunci: Menopause, Follicle Stimulating Hormone, FSH, Keluhan Urogenital
