Abstract

Cardiogenic shock is an acute state of end-organ hypoperfusion following cardiac failure. Usually many children have good compensation if they suffered from cardiogenic shock and sometimes delay diagnosis leads to unfavorable outcome. Comprehensive approach in treatment following investigation about the cause of cardiogenic shock is very important and proper management will prevent complication and mortality.

Abstract

Latar Belakang. Pulse oksimetri (POX) banyak digunakan se­bagai deteksi PJB pada anak. Namun, akurasi pulse oxymetri sebagai pa­ra­meter tunggal kurang memuaskan. Oleh karena itu kombinasi peng­gunaan pulse oxymetri, auksultasi dan klinis dismorfisme fasial di­perkirakan dapat meningkatkan akurasi dan sensitivitas dalam deteksi PJB pada bayi baru lahir. Tujuan dari studi ini adalah untuk menilai sensitivitas, spesifisitas, nilai prediski positif, dan nilai prediksi negative dari kombinasi pulse oxymetri, auskultasi dan dismorfik feature.Metode. Studi potong lintang dilakukan di RSUD Dr Saiful Anwar Malang mulai Februari 2019 hingga Februari 2020. Pasien usia kurang dari 1 bulan yang dirujuk dari berbagai faskes di Malang Raya ke Departemen Anak RSUD Dr. Saiful Anwar Malang dengan curiga PJB. Subyek diukur saturasi oksigen, auksultasi, dan dismorfik feature kemudian dilakukan ekokardiografi untuk melihat adanya kelainan struktural jantung. Data diolah menggunakan Microsoft Excel dan dianalisa menggunakan software SPSS 22. Sensitivitas, spesifitas serta predictive value masing-masing dan kombinasi dihitung.Hasil. Subyek yang memenuhi kriteria penelitian ini berjumlah 274. Tiga belas subyek tidak didapatkan kelainan struktural jantung. Kelainan paling banyak ditemui adalah ASD diikuti dengan VSD. Empat puluh dua subyek memiliki saturasi kurang dari 90%. Murmur didapatkan pada 42 subyek. Dismorfik feature ditemukan pada 6 subyek. sensitivitas dan spesifiktas masing-masing parameter adalah 15,33% dan 84,64% untuk pulse oksimetri, 16,09% dan 100% untuk auskultasi, 2,3% dan 100% untuk dismorfik feature. Kombinasi ketiga metode meningkatkan sensitivitas menjadi 29,12% dan spesifitas menjadi 84,62% dengan PPV 97,44% LR+ sebesar 1,89.Kesimpulan. Penggunaan kombinasi pulse oxymetri, auskultasi dan dismorfik feature dapat meningkatkan sensitivitas diagnosis PJB pada bayi baru lahir di Malang

Abstract

Introduction: Children with disabilities/chronic illnesses often have difficulties in socializing and tend to have a low self-image. Parents, as the main children’s self-image-forming factor, also ex-perience difficulties in dealing with their children. This study aims to increase children’s and parents’ knowledge about self-image and acceptance, hence improving children’s self-image. Methods: A quasi-experimental research study with the intervention of three modules for children and parents was carried out between June and September 2022. Thirty children and their parents from the Community for Empowerment of Parents of Children with Special Needs, the Heart Warrior Community, and the Foundation for De-velopment of Disabled Children were included. Pretest and posttest were measured and statistically tested using the Wilcoxon test. At the end of the interventions, the Rosenberg Self-Esteem Scale, Self-Harm Inventory, Strengths and Difficulties Questionnaire, and Pediatric Quality of Life were measured. Results: There was a sig-nificant increase in knowledge on 3 child modules and 1 parent module. The Rosenberg Self-Esteem Scale score is 17.17 ± 2.618 (good self-image); the Self-Harm Inventory score is 2.18 ± 3.275 (low risk of self-injury); the Strength and Difficulties Questionnaire score is 18.37 ± 5.230 (moderate difficulty); and the Pediatric Qual-ity of Life value is 34.64 ± 12.077 (good quality of life). Conclusion: The self-image strengthening module intervention is effective in increasing children’s and parents’ knowledge. Children with dis-abilities/chronic illnesses have a good self-image, low self-harm tendencies, and relatively good quality of life.
