Abstract

Coronary Heart Disease (CHD) is the leading cause of death globally, including in Indonesia. One of the procedures to treat CHD is Percutaneous Coronary Intervention (PCI), but this action often causes significant anxiety in patients. Unmanaged anxiety can worsen the physical and mental condition of patients. This study aims to analyze the effectiveness of audio-visual education based on the Caring Jean Watson concept in reducing anxiety in pre-PCI patients at Dr. Loekmonohadi Hospital, Kudus. The study design was a quasi-experiment with a "nonequivalent control group design", involving 40 patients divided into two groups: intervention (Watson's audio-visual caring education) and control (conventional education). Anxiety levels were measured using the Hospital Anxiety and Depression Scale (HADS) before and after the intervention. The data obtained before being analyzed was tested using the Shapiro-Wilk test to ensure that the data was normally distributed, then a parametric paired T-test was carried out to see the difference in anxiety between before and after being given education. The results showed a significant decrease in anxiety in the intervention group, where 45% of patients reached normal anxiety levels and only 10% still experienced severe anxiety. In contrast, in the control group, 70% of patients remained at severe anxiety levels. The paired T-test statistical test produced a p-value of 0.000 (p < 0.05 ), indicating a significant difference before and after education. Caring-based audio-visual education has been shown to be effective in reducing pre-PCI patient anxiety.

Abstract

HIV/AIDS is a global health problem that remains a major challenge, particularly regarding quality of life, stigma, and patient independence in long-term care. Jean Watson's Human Caring Theory offers a holistic approach that focuses on therapeutic relationships, respect for human dignity, and emotional and social support, potentially increasing the independence of HIV/AIDS patients in the hospital. This study aims to identify, synthesize, and analyze scientific evidence regarding the application of Jean Watson's Theory of Human Caring to increasing the independence of HIV/AIDS patients in hospitals. This study used a Systematic Review method with PRISMA guidelines. Literature sources were searched through Google Scholar, Semantic Scholar, ProQuest, and ScienceDirect for the years 2015–2025. The keywords used were human caring, Jean Watson, independence, PLWHA, HIV/AIDS, hospital. Of the 1,099 articles found, 8 articles met the inclusion criteria for further analysis. The review showed that the implementation of Human Caring improved communication, motivation, social interaction, medication adherence, and quality of life in HIV/AIDS patients. Factors such as family support, employment status, clinical condition, and psychosocial support were shown to influence patients' levels of independence. The caring approach also reduced stigma, strengthened self-confidence, and encouraged active patient involvement in care. Jean Watson's Theory of Human Caring has been shown to contribute significantly to increasing the independence of HIV/AIDS patients in hospitals through a holistic and multidimensional approach. Future research is recommended to develop a caring-based intervention model with more measurable effectiveness tests using experimental or quasi-experimental designs and incorporating independence as the primary indicator.

Abstract

Surgical Site Infection (SSI) is a post-surgical complication that significantly contributes to increased morbidity, mortality, and hospital costs. The prevalence of SSI in Indonesia is reported to reach 2.3–18.3 % and is an important indicator of service quality. One effective prevention strategy is the implementation of a surgical site bundle, a collection of evidence-based interventions that must be implemented consistently. This article aims to systematically review empirical evidence regarding the effectiveness of implementing bundled surgical sites in reducing the incidence of SSI in hospitals. This study is a systematic review using the PRISMA protocol. Article searches were conducted through Google Scholar, Semantic Scholar, ProQuest, and ScienceDirect with publications spanning 2015–2025, Keywords used Surgical Site Bundle, Surgical Site Infection, compliance, hospital. Of the 1,285 articles found, eight articles met the inclusion criteria for analysis, covering quantitative, qualitative, and program evaluation designs related to the implementation of SSI bundles in hospitals. The review showed that bundle implementation was proven to reduce SSI rates. Studies in the United States reported a 30% reduction in SSIs in colorectal surgery, while in gynecologic oncology the incidence of SSIs decreased from 6% to 1.1%. Studies in Indonesia showed that bundle compliance was negatively associated with SSI rates (r = -0.502; p < 0.05 ) and that its implementation was able to shorten patient lengths of stay. However, the effectiveness of bundles is highly dependent on full compliance by healthcare workers and support from hospital management functions. Implementation of the surgical site bundle is effective in reducing SSI rates in hospitals, but its success is determined by consistent implementation, full adherence to the bundle elements, and management oversight. Efforts to improve education, audits, feedback, and patient engagement need to be strengthened to ensure sustainable implementation.

Abstract

hronic kidney disease (CKD) is a global health problem with increasing prevalence, particularly in end-stage patients requiring renal replacement therapy in the form of hemodialysis. The success of hemodialysis is greatly influenced by patient adherence to the treatment regimen, including dialysis attendance, fluid restriction, diet, and medication use. However, patient adherence remains variable and tends to be low, particularly with regard to diet and fluid restriction. This study aims to identify and analyze the determinant factors that influence the compliance of CKD patients in undergoing hemodialysis in hospital. This study used a systematic review with PRISMA guidelines. A literature search was conducted through electronic databases covering the period 2015–2025. Literature searches were obtained from Google Scholar, Semantic Scholar Proquest, and ScienceDirect . The keywords used were CKD, hemodialysis , determinant factors, adherence, hospital . Of the 1,145 articles obtained, 10 met the inclusion criteria and were subjected to further analysis. The results of the study showed that the dominant factors influencing patient compliance were economic factors (p = 0.009) , patient knowledge and education (p = 0.001) , and family support (p = 0.000) . In addition, the factor Occupation (p = 0.026) , duration of hemodialysis (p = 0.000) , and psychological aspects such as sleep quality (OR = 3.36 ) and depression also significantly contribute to patient compliance. These factors play a multidimensional role and interact with each other in influencing patient compliance. Adherence to hemodialysis in CKD patients is influenced by a combination of economic factors, knowledge, family support, sociodemographic factors, and psychological conditions. Interventions that emphasize financial support, increased health literacy, and active family involvement are crucial for increasing adherence, improving quality of life, and producing better clinical outcomes in hospitalized CKD patients.
