Abstract

Introduction: Choledocholithiasis, the presence of stones in the common bile duct (CBD), is a challenge in the field of digestive surgery due to the wide range of cases with varying levels of difficulty. Until now, the options for diagnostic and therapeutic modalities have been increasingly evolving, ranging from non-invasive to invasive procedures. Therefore, the Indonesian Society of Digestive Surgeons provides recommendations for the management of CBD stone cases in Indonesia.Method: The consensus was developed using the Delphi survey method involving digestive surgery experts from various cities in Indonesia. Several issues related to the diagnosis and management of CBD stones were formulated and answered based on current research, while also considering the opinions of the experts.Result: This consensus consists of two parts: 15 recommendations related to the selection of diagnostic modalities and 10 recommendations related to management options. Recommendations regarding diagnosis include the use of ultrasonography, abdominal CT-scan, and magnetic resonance cholangiopancreatography (MRCP). In addition to standard therapeutic procedures, this consensus also provides recommendations regarding contemporary procedures such as endoscopic retrograde cholangiopancreatography (ERCP), laparoscopic common bile duct exploration (LCBDE), and open common bile duct exploration (OCBDE).Conclusion: These recommendations are aimed to assist digestive surgery experts in providing the best management for CBD stone cases.

Abstract

Colorectal cancer (CRC) is the third most common cancer globally and a leading cause of cancer-related mortality. Cyclooxygenase-2 (COX-2) plays a critical role in CRC pathogenesis by promoting inflammation, tumor proliferation, and metastasis. COX-2 inhibitors, such as NSAIDs, have shown potential in reducing CRC progression. However, the relationship between COX-2 expression and clinicopathological features remains controversial. This study aims to assess the correlation between COX-2 expression and clinicopathological characteristics in CRC patients in Indonesia. A prospective observational study was conducted on 81 CRC patients at a tertiary hospital in West Java, Indonesia, from January to June 2024. COX-2 expression was quantified using qPCR from tumor tissue samples. Clinicopathological data, including age, sex, tumor grade, stage, and complications, were collected and analyzed. Among the 81 participants, 81.5% were over 50 years old. Low-grade adenocarcinoma was the most prevalent histopathological type (73%), followed by high-grade adenocarcinoma (11%) and mucinous adenocarcinoma (11%). The majority of cases were in regional or early stages (74%), while 26% were in late stages. Higher COX-2 expression was more frequent in males, rectal tumors, high- grade adenocarcinomas, advanced stages, and metastatic cases. Although no statistically significant association was found, a trend toward increased COX-2 expression in advanced CRC was observed. No significant association was found between COX-2 expression and clinicopathologic characteristics of CRC. However, higher COX-2 expression may be associated with advanced disease. Abstract

Obstructive jaundice is commonly caused by both benig n and malignant hepatobiliary disorders. Early identification of malignancy remains challenging, especially in settings with limited access to advanced diagnostic modalities. To evaluate the diagnostic accuracy of the Probability of Malignancy (P-MAL) score in predicting malignant causes of obstructive jaundice. A prospective analytic study was conducted at Dr. Hasan Sadikin General Hospital, Bandung, from December 2024 to August 2025. Forty-eight patients with obstructive jaundice were enrolled consecutively. Clinical characteristics and laboratory parameters were collected to calculate the P-MAL score. Final diagnosis was confirmed by histopathological examination. Diagnostic performance was assessed using receiver operating characteristic analysis. Among 49 patients, 15 (31.3%) were diagnosed with malignant obstructive jaundice. Patients with malignancy were significantly older and had higher levels of total bilirubin and alkaline phosphatase. The P-MAL score demonstrated good diagnostic accuracy with an area under the curve of 0.832 (95% CI 0.720–0.945; p < 0.001). A cut-off value ≥0.9994 yielded a sensitivity of 93.3% and specificity of 54.5%. The P-MAL score shows good accuracy for predicting malignancy in obstructive jaundice and may serve as a useful preoperative screening tool.

Abstract

Background: Abdominal cocoon, also termed sclerosing encapsulating peritonitis (SEP) or encapsulating peritoneal sclerosis (EPS), is a rare cause of intestinal obstruction in which the small bowel is encased within a fibrocollagenous membrane, producing the prototypical frozen abdomen. Its surgical outcomes had never been pooled quantitatively. Objective: To quantitatively pool the surgical outcomes of adult abdominal cocoon — peri-operative mortality, morbidity, recurrence and pre-operative diagnostic yield — in patients undergoing operative management. Methods: PubMed, Scopus, the Cochrane Library and Google Scholar were searched for observational studies reporting the operative management and outcomes of adult abdominal cocoon. Ten case series (179 operated patients) were included. As all studies were single-arm, proportions were pooled with the Freeman–Tukey transformation under a DerSimonian–Laird random-effects model; robustness was tested with a logit-normal model, leave-one-out analysis and the Egger test, and quality with the Joanna Briggs Institute checklist. Results: Pooled peri-operative mortality was 4.5% (95% CI 0.2–12.1%; k=7); because zero-event studies influence the double-arcsine transformation, a logit-normal model gave 13.1% and the crude proportion 7.9%, so mortality is best summarised as low-to-moderate. Pooled morbidity was 19.5% (95% CI 5.9–36.9%; k=5), recurrence 3.3% (95% CI 0.0–21.8%; k=3) and pre-operative diagnostic yield 19.1% (95% CI 6.0–36.0%; k=4). Leave-one-out kept mortality between 3.0% and 8.5%. Conclusion: Surgery for adult abdominal cocoon achieved low-to-moderate mortality and acceptable morbidity with infrequent recurrence, supporting membrane excision and adhesiolysis in selected patients, while the low pre-operative diagnostic rate signals a need for greater awareness.

Abstract

BACKGROUND: Colorectal cancer (CRC) is a major global health problem associated with high cancer-related mortality. Profiling of Kirsten rat sarcoma viral oncogene homolog (KRAS) mutation status has become a decisive predictive biomarker in the management of CRC. However, KRAS mutation testing is commonly performed using tumor tissue obtained through invasive biopsy or surgery; therefore, fecal DNA analysis might offer a promising non-invasive alternative. This study was conducted to evaluate the concordance of KRAS mutations between matched tumor tissue and fecal samples and their association with clinicopathological characteristics.METHODS: This cross-sectional study included 94 patients confirmed of CRC. Tumor tissue specimens were obtained during surgical resection or biopsy, while fecal samples were collected pre-operatively. KRAS mutations were analyzed using polymerase chain reaction (PCR) and DNA sequencing, and the associations with clinicopathological variables were statistically evaluated.RESULTS: KRAS mutations were detected in 45.74% of tumor tissue and 26.60% of fecal samples. The overall concordance rate was 65.96%, with a Cohen’s Kappa of 0.291 (95% CI: 0.110–0.472), indicating "fair agreement". Notably, allele-specific concordance was 100% among double-positive cases. The fecal assay demonstrated 41.86% sensitivity, 86.27% specificity, 72.00% positive predictive value (PPV), and 63.77% negative predictive value (NPV). Tumor location (colon versus rectum) was significantly associated with fecal KRAS detection (p=0.028); other variables showed no significant association (p>0.05).CONCLUSION: KRAS mutations were more frequently detected in tissue than in fecal samples, with fair agreement. High allele-specific concordance suggests fecal DNA accurately reflects the tumor's mutational profile. Tumor location significantly influences DNA detectability, supporting fecal-based KRAS testing as a potential non-invasive approach for CRC molecular assessment.KEYWORDS: colorectal neoplasms, feces, KRAS protein, human, mutation, neoplasm tissue

Abstract

The development of health technology must be accompanied by the establishment of robust healthcare laws to safeguard both healthcare professionals and patients from potential risks and losses arising from errors in telesurgery. In light of this, three key issues are formulated in this study: namely the development of telesurgery in the legal framework, the legal construction of telesurgery and legal responsibility in the event of errors in telesurgery. This study is a normative legal analysis examining the regulatory framework governing telesurgery and the legal responsibilities arising from potential errors. A literature review serves as the primary method for collecting both primary and secondary legal sources. The research employs a legislative and conceptual legal approach, with qualitative analysis used to address the identified issues. Legal frameworks for the regulation of telesurgery must be developed at both the international level, particularly in relation to jurisdiction, and the national level, focusing on medical standardization in its implementation. The practice of telerobotic surgery must be regulated by law, derived from policy regulations, and regulated by internal guidelines from surgeons. At a minimum, several legal elements must be addressed, including jurisdiction, ethics committees within surgical organizations, inter-ministerial collaboration, the legal relationship between patients and physicians, pre-operative explanations and informed consent, legal accountability, medical confidentiality, medical team requirements, and information security management systems.  Legal liability for failed surgical procedures may arise in cases of errors, whether intentional or due to negligence.
