Abstract

Tuberculosis (TB) remains a significant global health concern, particularly in Indonesia. Ensuring rational TB treatment is crucial for effective disease control and preventing the emergence of drug resistance. This study aimed to evaluate the rationality of TB treatment among newly diagnosed drug-susceptible pulmonary TB patients in a primary care center in Bandung. A descriptive-analytical study was conducted on 56 patients who met the inclusion and exclusion criteria. Treatment rationality was assessed based on the Indonesian Society of Respirology 2021 and the National Guidelines for Medical Services 2020. While 100% of patients received the correct medication for the right indication and dose, the duration of therapy was suboptimal for most patients (83.93%) due to drug unavailability. This resulted in a lower-than-ideal treatment regimen, potentially compromising treatment outcomes and increasing the risk of drug resistance. These findings highlight the need for improved drug supply management and adherence to treatment guidelines to optimize TB treatment outcomes and reduce the burden of TB in Indonesia.

Abstract

Background: Tuberculosis (TB) is a significant global health challenge in Indonesia. The rational use of anti-TB drugs is essential for treatment success and for preventing drug resistance in patients with TB. Objectives: This study aimed to evaluate the rationality of anti-TB drug use in patients with pulmonary TB at the Cimahi Tengah Public Health Center (Puskemas Cimahi Tengah). Methods: This research employed a descriptive method with a retrospective approach based on observational data from the Tuberculosis Information System (SITB) between January and December 2023. Samples were selected using purposive sampling based on the predefined inclusion and exclusion criteria. Results: The results showed that All patients (100%) received treatment according to the national guidelines and WHO recommendations, including the HRZE regimen (rifampicin, isoniazid, pyrazinamide, and ethambutol). The accuracy of indication, drug selection, dosage, and administration intervals also reached 100%, demonstrating adherence to the clinical standards. Conclusion: Anti-TB drug use at the Cimahi Tengah Public Health Center complies with established treatment standards, ensuring treatment effectiveness and contributing to optimal patient outcomes.

Abstract

Diarrhoea in children is often associated with irrational antibiotic use, even though the cause is not bacterial infection. Excessive antibiotic use can increase antibiotic resistance, a significant problem in healthcare. This study aimed to evaluate the rationality of antibiotic use in paediatric inpatients with diarrhoea at a single hospital in Bandung, including demographic characteristics, types of antibiotics used, and appropriate indication, drug selection, dosage, and administration intervals. This study was observational, using a non-experimental descriptive analysis design. Data collection was conducted retrospectively from January to December 2024. Out of a total population of 229 patient medical records, 54 patient medical records met the inclusion criteria and were used as the study sample. The rationality of drug use refers to the Indonesian Ministry of Health’s guideline. The results showed that 55.6% of patients were male, with toddlers (46.2%) being the largest age group. Ceftriaxone was the most commonly prescribed antibiotic (24%). The rationality of antibiotic use was 100% for indications and drug selection, 94.4% for dosage, and 90.7% for the interval between doses. However, the identified discrepancies in dosage (5.6%) and administration intervals (9.3%) require immediate attention to minimize the risk of therapeutic failure and antibiotic resistance.

Abstract

Type 2 diabetes mellitus (T2DM) with hypertension complications requires rational and standardized pharmacological management to prevent cardiovascular deterioration. This study aimed to evaluate the pattern and level of rationality of antidiabetic drug use in T2DM patients with hypertension complications in the inpatient ward of a hospital in Cimahi City for the period of January 2023 to December 2024. This study utilized a retrospective descriptive-analytical observational design involving 79 medical records that met the inclusion criteria. The assessment of rationality was measured using five parameters based on the Indonesian Ministry of Health guidelines: appropriate indication, drug selection, dosage, administration interval, and duration of therapy. The majority of patients were elderly females (62%). Prescribing patterns indicated that 50.6% of patients received a combination therapy of insulin and oral hypoglycemic drugs (OHDs), 31.7% received OHD monotherapy, and 12.6% received insulin monotherapy, with metformin and glimepiride being the most prescribed OHDs. The evaluation results of the rationality level reached 100% for the parameters of appropriate indication and duration of therapy, 96% for appropriate administration interval, 90% for appropriate dosage, and 70% for appropriate drug selection. The majority of inappropriateness stemmed from the selection of insulin regimens that had not been optimally individualized to the patients' clinical profiles. Overall, the antidiabetic management was classified as rational; however, a multidisciplinary approach and strong coordination between physicians and clinical pharmacists are crucial to optimize therapy individualization and adherence to standard guidelines.
